GIS 공간분석기법을 이용한 서울시 노인주간보호시설의 접근성 연구 (Measuring Accessibility of Day Care Centers for the Elderly in Seoul Using GIS Spatial Analysis Techniques)

  • 본 연구는 GIS 공간분석기법을 활용하여 서울특별시의 각 동별 노인주간보호시설의 접근성 지수를 산출, 비교 분석하여 향후 시설의 신설 또는 확장을 할 경우에 운영의 공간적 효율성과 형평성을 향상시킬 수 있는 입지 후보지는 어디인지를 파악해 보는데 목적을 두었다. 연구 결과, 전반적으로 서울시 노인주간보호시설의 분포가 시설을 이용하리라 추정되는 노인인구의 분포를 반영하고 있지 못했고, 특히 강서구와 송파구 등 서울의 외곽경계지역에 해당하는 동들의 접근성이 가장 낮은 것으로 나타났다. 이러한 연구 결과는 노인주간보호시설의 운영정책에 있어서 다음과 같은 함의를 가진다. 첫째, 시설이용 노인들의 복지시설에의 접근성 향상이라는 대전제 하에서는 현재 노인주간보호시설로 부터 상대적으로 거리가 먼 지역에서의 시설 설치가 요구된다. 둘째, 예산의 제약 등으로 추가적인 시설의 설치 없이 기존시설의 정원을 늘리는 정책을 전개할 경우 근접성이 높게 나타난 시설들의 규모를 우선적으로 확장함으로써 보다 많은 수의 노인들이 좀 더 용이하고 빈번하게 이용할 수 있도록 하는 것이 바람직하다. 마지막으로 접근성이 중시되는 다른 노인재가시설들과 사회복지시설의 설치와 확대, 평가에 있어서도 본 접근성 분석 기법을 적극 활용함으로써, 이동에 제약이 많은 복지시설 이용자들의 복지권을 보장하는 한편 사회복지 자원 투자의 당위성을 확보할 필요가 있다.

고령자 만성질환 및 건강관리 모니터링에 관한 연구: 웨어러블 기기의 효과 (A Study on Monitoring of Health Care and Chronic Diseases for the elderly: the Effect of Wearable Device)

  • 본 연구는 고령자의 웨어러블 기기 사용 및 사용자 경험에 관한 것이다. 웨어러블 기기 사용을 통해, 고령자는 건강 데이터를 실시간으로 모니터링할 수 있다. 결과적으로, 그들의 건강 관리, 운동 등에 관한 동기가 개선되고 향상될 수 있다. 전세계에서, 고령화는 각국에서 가장 중요한 문제들 중 하나가 되고 있다. 인구 고령화에서 오는 사회적이고 경제적인 부담은 한국을 비롯해 세계와 세계경제의 지속가능성에 심각한 도전을 제기한다. 웨어러블 기기를 포함해 정보통신기술은 고령자 건강 관리를 향상하는데 도움을 줄 수 있다. 2018년 현재, 웨어러블 기기의 보급 대수는 1900만 대 이상이다. 그러한 기기들 중 대부분은 건강 데이터 모니터링과 관련되어 있다. 본 실험에서 저자들은 피험자의 실시간 건강 데이터를 웨어러블 기기를 통해 측정한다. 실험 첫 날, 피험자의 평균 허리둘레 수치는 36.29 인치를 기록했다. 실험 마지막 날(실험 시작으로부터 31일째), 평균 허리둘레 수치는 35.51 인치로 감소했다. 실험 시장 당시, 허리둘레 수치의 표준 편차는 1.93이다. 실험 마지막 날, 표준 편차는 2.24로 확대되었다. 회귀분석에서 실험 일수가 길어질수록, 평균 허리둘레 수치는 감소하는 경향을 보인다. 이 트렌드는 유의미하다(t=2.719, p<0.05). 그 결과는 피험자의 건강 관리 동기가 향상된다는 것을 의미한다고 볼 수 있다. 웨어러블 기기는 고령자의 건강 관리 및 운동에 대한 동기를 증가시킬 수 있다. 결과적으로, 그들의 건강은 개선될 수 있다.

중환자실에서의 K-DRG 분류와 개두술환자군의 간호시간과 간호원가연구 (A Study on the Classification of ICU Patients by K-DRG and the Nursing Care Hours and Costs of Craniotomy Patients)

  • This dissertation classifies sample patients by a measure of K-DRG to identify the most frequent group. and investigates the differences in the dependency of nursing by patient classification system in the SICU of Seoul National University Hospital in Korea. It also calculates the mean nursing care hours and costs per craniotomy patient, who is shown to be the most frequent patient group. The results of the research can be used as basic data for the development of relevant nursing cost system in the future. The results of the research are as follows: 1. Using data from 97 sample patients, as many as 26 groups are identified when the patients are classified by K-DRG. KDRG-001 (craniotomy) is found to be the most frequent group(43.30%). 2. The result from patient classification system grouping in craniotomy patients shows homogeneity in terms of dependency of nursing with 35 patients in the 4th group, 145 patients(74.36%) are in the 5th group. and 15 patients are in the 6th group among the total 195 sample patients. 3. The direct nursing care hours for the 4th, 5th, and 6th patient classification system groups are found to be 381 minuites. 483 minuites, and 519 minuites, respectively, which shows that the nursing care hours increases as the dependency of nursing is intensified. The indirect nursing care hours are found to be 454 minuites(7.57 hours). The total mean nursing care hours, which is the sum of the direct nursing care hours(467 min.: 7.78 hours) and the indirect nursing care hours (454 min.: 7.57 hours), is 921 minuites(15.35 hours) per patient a day. 4. The nursing care cost is calculated to be 123,297 won per patient a day. Considering the average duration in the ICU, we can find the total nursing care cost is 610,318 won.

간접간호중재의 수행 및 위임에 관한 분석 (Identification and delegation of indirect care interventions)

  • The purpose of this research was fourfold: (a) to identify the use rate of the indirect care interventions performed by nurses, (b) to estimate the time to perform each intervention, (c) to identify the indirect care interventions to be delegated to others, and (d) to determine the level of provider preparation needed to delegate indirect care interventions. The sample consisted of 199 nurses working in three hospitals. The Indirect Care Survey developed by the Iowa Intervention Project team was used for data collection. The instrument was translated to Korean and validated by nurse experts. Each of the 26 indirect care interventions were used several times a day. Four interventions (i.e.. Documentation, Shift Report, Specimen Management, and Transport) were performed several times a day by 50% or more of the nurses. The most frequently used intervention was Documentation, followed by the interventions Shift report. Environmental Management, Transport, and Examination Assistance. The least used intervention was Quality Monitoring, followed by the interventions Order Transcription, Referral, Health Care Information Exchange, Multidisciplinary Care Conference, and Product Evaluation. The intervention taking the most time to per-form was Technology Management (155.3 minutes), followed by the interventions Documentation, (122.2 minutes), Delegation (84.4 minutes), Supply management (83.4 minutes), and Preceptor: Student (79.9 minutes), Overall, the nurses reported that they would not delegate to others the majority of the interventions. More than 50% of the nurses would not delegate 21 interventions. Shift Report would not be delegated by 95% of the nurses and Documentation would not be delegated by 92% of the nurses. Caregiver Support would be delegated by 68% of the nurses to family. Three interventions (i.e.. Environmental Management, Examination Assistance, and Transport) would be delegated by more than 50% of the nurses to Nursing Assistant. This study will contributes to determining costs of nursing services and enhancing quality of nursing care. Replication study will be needed with large sample.

성동구내 어린이집 어린이의 수면 및 식생활 실태와 성장과의 관련성에 대한 보고 (A Study on the Relations between Growth and the Current Conditions of the Day-care Center Children's Sleeping Patterns and Diet patterns in Seongdong-district)

  • Objectives : The purpose of this study is to examine sleeping patterns and diet patterns of the children in day-care centers in Seongdong-district and to find out the relationships between those two patterns and children growth. Methods : 686 day-care center kids participated in the examination. We measured their heights and weights. We also conducted a questionnaire survey (of them) on their sleeping patterns and diet patterns. Based on the findings, we have analyzed correlations of the factors. Results : 1. We have interviewed 686 children in day-care centers in Seongdong-district. Substantial survey was conducted to 672(14 kids didn't go through regular medical checkups). Their age ranged from 4 months to 120 months and weights covered from 6.20 kg to 50.80 kg with the average of 16.97kg. Their height ranges were from 70.4cm to 140.6cm and the average was 100.76cm. 2. The survey results showed that birth-height affects children's current height. The results from the group of age over 25months and the group covering 2 to 6 years indicated that birth-height affected the current height as well. However, the result from the age group over 37 months showed that birth-height didn't relate to the current height. 3. Results from questionnaires on sleeping patterns indicate that children sleeping longer are taller than the other. Results of other questionnaires showed no substantial relations to height. 4. Results from questionnaires on diet patterns, cold food eating habit affects growth. Conclusions : According to the results above, sleeping patterns substantially affects the growth. In order to find out, however, the specific relations between them, further research would be necessary.

서울 시내 변두리 저소득지역 유아원 어린이의 영양실태조사 (Nutrition survey of young children in a day care center in the low income area of seoul)

  • A nutrition survey of 128 perschool children, 4 to 6 years old, in the Saemaul Youlgok day care center, located in one uplane, overpolulated, poor area of the city of Seoul, sponsered by the Korean Red Cross Society, was undertaken between August 21 and 31, 1984, to investigate dietary and nutritional status. RESULTS: Mean values of height, weight, arm circumference, girth of chest, and head circumference ranged from 95 to 100% of KIST mean. However, 4.8~14.2% of the subjects were assessed as undernourished in terms of 'weight for height'. Mean hemoglobin vcalue was 12.1±0.7gm%, and mean hematocrit value was 38.5±2.2%; 4.1% of the subjects were proven to be anemic according to the criterion of hemoglobin established by the ICNND. Mean urinary urea nitrogen/creatinine ratio was 11.4±3.8. Regarding mean energy and nutrient intake, the following percentages had intakes below two-thirds of the 1985 RDAs: 28.8% of children, for protein; 48.8% of children, for iron; and 28.8% of children, for calcium. About 25% of children had vitamin A intake below one-third of the recommended allowance. Carbohydrate provided 71% of total energy intake; protein accounted for 13%; fat provided 16%. Energy intake was divided among breakfast, lunch, dinner and snacks in a percentage ration of 19:26:21:34. The lunch, and snacks which were served at the day care center provided 36.5~79.1% of daily energy and nutrient intake. The survey clarified that the day care feding largely supplemented the inadequate dietary intakes of these young children at home. According to factor analysis of energy and nutrient intake, there were two factor groups : the first factor group, for energy, protein, carbohydrate, iron and niacin, mainly provided by grains; and the second factor group, for calcium, vitamin A, thamin, riboflavin and ascorbic acid, provided by vegetables, fruits, milk and milk products. The survey also revealed significant correlation coefficients between family factors such as family income, educational level of parents, and number of children; and dietary or nutritional status of the subjects.

지불용의접근법을 이용한 간호서비스의 가격결정 (Determination of Nursing Price using Willingness to Pay)

  • It will become more and more popular to use the long-term care facilities and home health care services with the chronic disease increasing. It depends on how much the consumers would pay and purchase the services. They might get more benefits from that kind of services than from ordinary hospitalization. So far, the study of determining the medical service price has focused most often on the efforts from the providers' view. But it must be reasonable to include the consumers' value for the service. This study was performed to assess WTP(Willingness to Pay) for home health care service in order to apply to the determination of nursing price in a reasonable manner. In this study, respondents were asked if they would pay for the service's intangible benefits under the four different types(open-ended minimum WTP, open-ended maximum WTP, bidding WTP, referendum WTP). The contingent valuation method is a potentially useful tool in understanding how people value the benefits of the service. As a result, average open-ended minimum WTP was W16,015 per day among 65 respondents. Average open-ended maximum WTP was W29,154 per day among 65 respondents. Average bidding WTP was W26,300 per day among 65 respondents. Average referendum WTP was W22,200 per day among 70 respondents. The results of regression analyses were also consistent with theoretical prediction, e.g., increasing WTP with consumers' value for the service, state of patients, and household income. This study demonstrated that it was more reasonable to consider the consumers' value in determining the services' price. In addition, a further study is needed to test the validity of this CV method and to determine a proper nursing price based on the consumers' view.

주야간보호시설 이용 노인의 일상생활수행능력과 노인 및 주부양자의 간호요구도 조사 (Activities of Daily Living of the Elderly Using Day and Night Care Facilities and the Nursing Needs of the Elderly and their Caregivers)

  • Purpose: This study aimed to identify the activities of daily living of the elderly in day and night care facilities and the nursing needs of the elderly and caregivers. Methods: The subjects of this study were 90 elderly people and their caregivers, who lived in B and G cities used day and night care facilities. The collected data were analyzed using frequency analysis, descriptive statistics, independent t-test, and Pearson's correlation coefficient using the SPSS 25.0 program. Results: Caregivers have a higher awareness of nursing needs than the elderly. Both the elderly and their caregivers had the highest informational nursing requirements. There was a positive correlation between the activities of daily living of the elderly and physical nursing needs of caregivers, and there was also a positive correlation among the nursing needs in each area. Conclusion: These results suggest that day and night care facilities should provide customized nursing services by assessing the activities of daily living of the elderly and the nursing needs of the elderly and caregivers by area before establishing a nursing plan. This will enable them to provide effective nursing services for elderly people. In addition, we propose research to develop a nursing information program for the elderly and caregivers.

어린이집 교사들의 영양지식과 영양교육에 대한 인식 조사연구 (A Survey of Teachers' Recognition on Nutrition Knowledge and Nutrition Education at Day-care Centers)

  • This study was aimed at knowing the recognition of teachers' nutrition knowledge and nutrition education at day-care centers in Seong-nam. Teachers were all female, most of whom were in the 20, and their careers were less than 3 years. And $76.2\%$ of the teachers graduated from high school and junior college. The score of the nutrition knowledge was average $14.3\pm2.2$ (out of 20). It shows that they are lack of general information of nutrition. As the teachers had higher education, they scored higher nutrition knowledge levels (p < 0.01). It also says that there was meaningful interrelationship between the nutrition knowledge and the number of children they had (p < 0.05). $93\%$ of the teachers said that nutrition education should be given when children are three years old, at least and most of the teachers thought that nutrition education is necessary and should be taught in a separate course. $50.4\%$ of the teachers thought that nutrition education for early childhood should be taught by a nutritionist, but $35.2\%$ of them thought that the teachers themselves should be in charge of it. About $60\%$ of the teachers thought that 'Cooking Activity for Early Childhood' course should be established and it is desirable that the nutrition education should be in the area of cooking activity for childhood at day-care centers. The teachers thought that dietary habit is the most important subject in nutrition education and they got more information from the internet rather than in the class related to nutrition. 부양형태에 따른 주부양자의 부양부담의 차이 및 영향 변수에 관한 연구 -재가, 주간보호시설, 시설거주 노인의 주부양자를 대상으로- (A Study on Differences in the Caregiving Burden of Primary Caregivers by Type of Caregiving -Focused on Caregivers in Home Stay, Day Care Center, and Nursing Home Situations-)

  • This study investigates differences by type of caregiving using data on three groups of caregivers, 177 from home stay situations, 189 from day care centers, and 138 from nursing homes. First, the result shows that characteristics of both caregivers and elders differ by type of caregiving. Second, caregivers in the home stay situation have the highest caregiving burden of the three groups. Third, from examination of the related variables, the caregiving burden of home stay caregivers is affected by family income, caregivers' health, type of job, and whether or not the elder has symptoms of dementia. Also, the study reveals that emotional services for elders reduce the aggravation of family relations and economic burden, but that instrumental services highly increase economic burden. It reveals that caregivers of elders in day care centers, especially those who are in bad health, are more likely to experience feelings of constriction, aggravation of family relations and economic burden. On the other hand, caregivers who receive more emotional services have better experiences in family relations, including relations with the elder. In case of the elders of nursing homes, if the main caregiver is a daughter-in-law, aggravation of family relations is higher than if the main caregiver is a spouse. Finally, the caregiver's burden is affected by their own health and income, and by whether the elder has symptoms of dementia or stroke.
