연구배경: ARDS는 다양한 원인으로 초래되는 급성 염증성 폐 질환으로서 인공호흡기 치료 등의 여러 의학적 발전에도 불구하고 사망률이 40~60%로 예후가 좋지 않다. 이러한 환자들의 예후를 예측하는 방법으로 APACHE, SPAPS, MAM 등 방법이 있지만 간편하지가 않아서, ARDS가 염증성 폐질환이란 점에 착안하여서 염증 지표로 흔히 사용하는 ESR, CRP의 ARDS 환자에서 예후 인자로서의 역할을 평가해 보고자 하였다. 방 법: 87명의 ARDS 환자들의 중환자실 입원 당시의 ESR, CRP 결과와 APACHE II score 그리고 추적 검사한 결과를 확인하였다. 또한 대상 환자를 생존한 군과 사망한 군으로 나누어서 생존한 환자군에서 총 입원기간, 중환자실 입원기간, 인공 호흡기 치료 기간과 ESR, CRP, APACHE II score들과의 상관관계 및 ESR, CRP의 변화추이와 사망률과의 상관 관계에 대해서 연구하였다. 결 과: 중환자실 입원 당시의 ESR, CRP로는 ARDS 환자의 사망률을 예측하기가 어렸다. 하지만 CRP는 ARDS로 중환자실 치료 후 생존했던 환자군에서 사망했던 환자군에 비해서 치료 초기에 유의하게 감소하였으며, 중환자실 입원 당시 ESR이 높을수록 ARDS 환자들의 총 입원 기간 및 중환자실 입원기간이 길었다. 결 론: CRP의 초기 변화 및 중환자실 입원 당시의 ESR은 ARDS 환자의 예후를 예측하는데 도움이 될 수 있다.
연생모형(multiple life model)은 보험계약에서 두 명 또는 그 이상의 피보험자들의 사망 또는 생존의 상태에 따른 보험금을 지급하는 보험 상품의 보험료 결정 및 리스크 관리를 위한 모형이다. 본 논문에서는 부부 가입자 중 마지막 사망자가 발생할 때까지 연금이 지급되는 연생보험의 대표적인 상품인 역모기지를 살펴보고자 한다. 역모기지 상품은 만 60세 이상의 고령자가 소유주택을 담보로 거주하면서 일정액을 대출받지만 대출금을 다시 상환하지 않는 금융상품으로 부부 가입의 경우 대출 기간의 확률분포를 적용하기 위해서는 연생모형을 적용해야만 한다. 그러나 현행 역모기지 상품에 있어서 대출한도 및 월지급금 산출 시 연생모형이 적용되지 않고 있으며 우리나라의 경우에는 국민 생명표 상의 여자 사망률을 대출 종료(연금지급종료)확률로 활용하고 있다. 여자의 사망률을 이용하는 이유는 보수적인 관점에서 대출 종료 시점을 예측하기 위해 일반적으로 남자보다 여자가 더 수명이 길다는 점 때문이다. 고령화로 인해 수명이 점점 길어지는 추세이기 때문에 역모기지와 같이 계약기간이 확정되어 있지 않은 보험 상품의 경우 특히 더 계약 종료 시점에 대한 확률분포가 리스크 관리를 위하여 중요하다. 본 논문의 의의는 역모기지의 발행기관 및 보증기관의 적정한 월지급금 지급과 차후 월지급금의 과대지급으로 인한 지급불능을 방지하기 위해 현행 사용하고 있는 모형의 위험률에서 연생 모형으로 변경할 필요성을 실증분석을 통하여 제시한다.
국민연금의 효율적인 운영을 위하여 고령화, 저출산과 같은 사회현상에 대비한 연금 관리를 위한 연구가 요구되고 있다. 본 연구는 유족연금의 발생을 예측하고 유족연금의 발생가능성 정도에 따라 대상자들을 분류하기 위한 통계적 모델을 제안하기 위하여 두 단계의 로지스틱 분석을 실시하였다. 첫 단계의 분석으로부터, 전체 대상자에 대하여 유족연금의 발생에 영향을 주는 주요인의 특성과 국민연금의 종류를 파악하고 이를 대상으로 유족연금의 발생에 대한 로지스틱 회귀모형을 적용하되 대상자를 합리적으로 등급화하기 위한 모델을 제안하고 이를 일반적인 로지스틱모델과 비교하였다. 정확도, 민감도, 특이도와 사후 확률의 분포를 비교하고 K-S통계량을 통하여 등급의 타당성 평가와 리프트 그래프를 통한 모델의 예측력평가를 함으로써 합리적 등급분류를 통한 대상자관리가 가능한 통계적 모델임을 보였다. 예측된 통계적 모델을 적용하여 유족연금 수급유무와 등급별 분류, 등급에 따른 유족연금액 예측을 통하여 효율적인 연금관리 방안을 제안할 수 있다.
Purpose: Nitric oxide (NO) is a vasodilator and inhaled NO (iNO) is used in acute respiratory distress syndrome (ARDS) to improve alveolocapillary gas exchange. The mechanism to improve oxygenation is likely to redistribute blood flow from unventilated areas to ventilated areas. Though improvement of oxygenation, iNO therapy has not been shown to improve mortality and considered as only rescue therapy in severe hypoxemia. We conducted the study to investigate an efficacy of iNO in trauma patients with severe hypoxemia. Methods: We reviewed the trauma patients who underwent iNO therapy retrospectively from 2010 to 2014. Degree of hypoxemia was represented as $PaO_2/FiO_2$ ratio (PFR) and the severity of patient was represented with sequential organ failure assessment (SOFA) score. Patients were divided into the survivor group and non-survivor group according to the 28-day mortality. Results: A total of 20 patients were enrolled. The mortality of 28-day was 40%. There were no significant differences between survivor and non-survivor group in age, sex, severity of injury, PFR and SOFA score. There was significant difference in initiation time of iNO after injury (p=0.047). Maximum combinations of sensitivity and specificity for timing of iNO therapy were observed using cut-off of 3-day after injury with a sensitivity of 88% and specificity of 75%. Conclusion: Though iNO therapy does not influence the mortality, iNO therapy may decrease the mortality caused by respiratory failure in the early phase of trauma.
TETRA Enhanced Data Service (TEDS) 는 기존의 협대역 ETSI TETRA Release 1 표준안을 개선하여 주파수 선택적인 페이딩 채널에서 고속의 패킷 데이터 전송이 가능한 표준 규격이다. TEDS 표준에서 사용하는 M-QAM 변조방식의 성능은 채널 추정 성능에 크게 영향을 받는다. 본 논문에서는 Per-Survivor Processing (PSP) 기법을 사용하여 고속 채널 환경에 적합한 새로운 decision-directed 채널 추정 기법을 제안한다. 제안된 채널 추정기법은 이동 환경에서 기존 파일럿 신호 기반의 채널 추정 기법에 비해 TETRA 수신 성능을 향상시킬 수 있음을 전산 모의 실험을 통해 보인다.
본 논문에서는 고속 TDMA (Time Division Multiple Access) 위성 통신 시스템에 적합한 위상 동기 방식을 제시하고, 추적 성능을 평가하였다. 낮은 SNR에서 작동하고 초기 포착 특성이 우수한 방식을 사용해야 하는 위성통신 시스템에 있어 채널 코딩은 필수적이다. 따라서 코딩에 적합한 위상 동기 방식이 필요하다. 그러나 기존의 DD (Decision Directed)나 NDA (Non-Data-Aided) 방식은 낮은 SNR에서 cyclic slipping 과 Hangup 현상에 민감하기 때문에 코딩 이득을 제대로 얻을 수 없다. 이러한 현상을 보완하기 위해 TD (Tentative Decision) 방식과 PSP (Per Survivor Processing) 방식이 제안되었다. PSP 방식은 TD 방식에 비해 성능이 뛰어나기는 하지만 복잡도가 크다는 단점을 가지고 있다. 따라서 본 논문에서는 채널 상태에 따라 적응적으로 PSP 방식과 TD 방식을 결합하여, 복잡도 문제를 해결하고 PSP 방식의 성능에 근접하는 ARSE (Adaptive Reduced State Estimator) 방식을 제안하였다.
Background: Metalworking fluids (MWFs) are mixtures with inhalation exposures as mists, dusts, and vapors, and dermal exposure in the dispersed and bulk liquid phase. A quantitative risk assessment was performed for exposure to MWF and respiratory disease. Methods: Risks associated with MWF were derived from published studies and NIOSH Health Hazard Evaluations, and lifetime risks were calculated. The outcomes analyzed included adult onset asthma, hypersensitivity pneumonitis, pulmonary function impairment, and reported symptoms. Incidence rates were compiled or estimated, and annual proportional loss of respiratory capacity was derived from cross-sectional assessments. Results: A strong healthy worker survivor effect was present. New-onset asthma and hypersensitivity pneumonitis, at 0.1 mg/㎥ MWF under continuous outbreak conditions, had a lifetime risk of 45%; if the associated microbiological conditions occur with only 5% prevalence, then the lifetime risk would be about 3%. At 0.1 mg/㎥, the estimate of excess lifetime risk of attributable pulmonary impairment was 0.25%, which may have been underestimated by a factor of 5 or more by a strong healthy worker survivor effect. The symptom prevalence associated with respiratory impairment at 0.1 mg/㎥ MWF was estimated to be 5% (published studies) and 21% (Health Hazard Evaluations). Conclusion: Significant risks of impairment and chronic disease occurred at 0.1 mg/㎥ for MWFs in use mostly before 2000. Evolving MWFs contain new ingredients with uncharacterized long-term hazards.
Purpose: Organophosphate insecticide poisoning is common in Korea, but there is no definitive guideline for determining the severity of the poisoning and the predictive factors. Therefore, we evaluated the organophosphate poisoned patients and we divided them into two groups, the survivors and the dead, and the results might be useful for treating organophosphate poisoning patients. Methods: We performed a retrospective analysis of 68 organophosphate poisoned patients who visited the Chosun University Hospital Emergency Medical Center during a 24-month period from January, 2007 to December, 2008. We made a work sheet of the patients' characteristics and the collected data was analyzed and we compared this data between the survivor group and the dead patient group. Results: There were significant differences between the survivor group and the dead patient group for the mean age, the alcohol intake state and the typically expressed signs. The dead patients had lower blood pressure, tachycardia and a lower Glasgo Coma Score (GCS) score than the survivor group. On the arterial blood gas analysis, the dead patients had more severe acidemia and they had lower saturations. Increased serum amylase levels were found in the dead patients. The survivors'initial and follow up serum pseudocholinesterase activity (after 6~8 days) was significantly higher than that of the dead group. The total amount of atropine injected to patient was less in the survivors than that in the dead patients. Conclusion: Old age and expressing the typical intoxication signs, a lower GCS score and blood pressure, showing acidosis on the gas analysis and low serum cholinesterase activity may be useful as poor prognostic indicators for patients with organophosphate poisoning. We suggest that physicians must pay careful attention to the signs and prognostic factors of organophosphate insecticide poisoned patients.
This study was carried out to find out the basic data required to plan and develop Rehabilitation Day Care Program for the stroke survivor's family in Korea. The subjects comprised of 92 stroke survivor's family who discharged from 4 hospitals in Seoul during the past 2 years. The data were collected from August 3, 1998 to September 18, 1998, through interviews with questionnaires about general characteristics, activities of daily living, depression and service need of rehabilitation day care program at the outpatient clinics by trained nursing graduates. Data were analyzed with descriptive analysis, Pearson's correlation analysis, and Stepwise multiple linear regression analysis using SPSS/WIN 10.0 program. The results obtained are as follows; 1. The mean score of the general need of rehabilitation day care program of stroke survivor's family was 3.10(range 1-4). The highest need among the service categories of the rehabilitation day card program was self-care and restorative activities category(3.30), and health services referral category, recreation category, psychosocial activities category in order. The needs of each category are as follows. In the health services referral category, the need for dental examination and medical examination were highest, followed by the need for physical therapy and occupational therapy. In the psychosocial activities category, the need for family counselling was highest. In the self-care and restorative activities category, the need for ROM exercise training was highest, followed by bowel training, and ambulation training. 2. The need of family for rehabilitation day care program service displayed a correlation with the level of education, ADL, and the level of depression, and a reverse correlation with age, illness intrusiveness, depression, knowledge, subject and object burden and relationship with stroke survivors. 3. The stepwise multiple linear regression analysis revealed following results. For the need for rehabilitation day care program service, 22.6% of the variance was initially explained by level of family's knowledge about caring method for stroke survivors, 8.8% was the level of subjective burden and 5.4% was relationship with stroke survivors. In conclusion, above characteristics should be considered to develop stroke survivors' rehabilitation day care program.
