Purpose: Sensory changes in the upper limb are complications of a mastectomy with immediate breast reconstruction with the treatment of breast cancer. The purpose of this study is to clarify whether immediate breast reconstruction worsens the sensory changes. Methods: From March 2004 to December 2005, 20 patients who had a mastectomy with immediate breast reconstruction(reconstruction group) were compared with 23 patients who had a mastectomy alone(control group). All patients had stage I or II breast cancer. The sensory changes were assessed in a blind manner by one examiner that used light touch sensation, static two-point discrimination, pain, vibration, hot and cold temperature perception. The sensory changes were identified along the sensory dermatome for diagnosing the damaged nerves. The following factors and their relationship with the sensory changes were analyzed : age, complications, and the mastectomy method. Results: There was no statistical difference in the static two-point discrimination, pain, vibration, hot and cold temperature perception between the two groups. However, the ability to recognize light touch was significantly better(p=0.045) in the reconstruction group than in the control group. The main site of sensory change was the proximal and medial portion of the upper limb in both groups. At these sites, the mean value of Semmes-Weinstein monofilament was $1.01g/mm^2$(reconstruction group 0.82, control group 1.17) and 2-point discrimination was 51.74(converted to perfect score of 100; reconstruction group 42.50, control group 59.78). The total rate of early complications was found to be significantly lower(p=0.006) in the reconstruction group than in the control group. Conclusion: These findings suggest that an immediate breast reconstructive procedure following a mastectomy is as safe as or safer than a mastectomy alone with respect to postoperative sensory changes of the ipsilateral upper limb.
티베트 고원에서 살고 있는 티베트인 또는 장족은 오랜 역사와 자연환경에 순응한 자신만의 독특한 생활양식을 가지고 있어 독자적인 의복문화 형태를 지닌다. 복식 중에 색채, 문양, 도안의 사용에서 종교적인 의미를 나타낼 뿐 아니라, 복식을 통해 자신의 생활의 희망과 자연을 존중한 마음을 강하게 표현하고 있다. 티베트 민족 복식의 색채 상징성에 관한 고찰을 통하여 티베트족 문화의 특수성을 이해하고자 장족 복식에 사용된 색채를 분석하고 그들만의 색채의 의미를 고찰하였다. 장족의 색채인식, 상징성을 색의 사용 예시와 출현빈도를 중심으로 살펴보았다. 연구방법으로는 복식사진의 색채 수집 및 분석을 실시하였는데 I.R.I HUE-TONE 시스템에서 매칭 색상과 색조를 찾는 방식이었다. 장족 복식도서의 칼라사진과 청해 장족문화박물관의 장족 복식을 직접 촬영한 사진, 인터넷 박물관의 복식사진 등 총 96장의 사진이었다. 결과는 첫째, 세상을 구성한다고 생각하는 가장 주요한 요소를 5가지 색 장오색에 연결하여 각 색마다 자연적 의미를 부여하는데, 빨간색은 불, 파란색은 하늘, 흰색은 구름과 흰 눈, 초록색은 초원, 노란색은 대지를 각기 상징한다. 둘째, 홍과 녹, 흑과 백, 홍과 남, 황과 자색 등 강렬한 색채조화를 이루도록 대담한 색채를 사용하고 있는 것이 특징이다. 셋째, 티베트인들의 특유한 심미관을 표현하는데 색의 일반적인 감성을 대부분 따르지만 그들만의 감성이 들어있는 색상이 존재하였다.
목적: 주관절의 후외측 동통과 탄발음을 유발할 수 있는 주관절의 활막추벽 9예에 대한 관절경적 경험과 해부학적 특징을 보고하고자 하였다. 대상 및 방법: 1999년 5월부터 2004년 1월까지 주관절 관절경을 받은 43명의 환자 중 활막추벽으로 진단되었거나 제거 받은 12개월 이상 추시가 가능했던 8명의 환자 9예를 대상으로 조사하였다. 양측 성을 보인 1명 2예를 포함한 남자가 8예 여자가 1예이었고 평균 연령은 29세 이었다. 모든 환자는 수술 전까지 최소 6개월의 보존적 치료를 받았으며 술 전 활막 추벽이라고 진단했던 경우가 6예, 외상과 염으로 진단했거나 진단이 처음부터 불분명하여 수술 중에 발견했던 경우가 3예이었다. 수술 직 후 관절경 소견으로 추벽의 형태를 분류하였고 최종 추시시점에서 통증 및 탄발음 소실 여부와 함께 Visual Analogue Scale (VAS)의 변화와 환자의 만족도를 조사였다. 결과: 관절경 소견상 전 예에서 외측 활막 추벽과 함께 전 외측에서 국한된 국소적 활막염의 소견이 관찰되었으며 7예에서 요골 두의 연골 연화증 또는 미란 소견을 보였다. 그러나 소두변화는 1예에서만 보였다. 2예를 제외한 7예에서 수술 직후부터 통증과 탄발음이 소실 또는 완화되었으며(VAS<1) 최종 추시 상에의 통증지수는 평균 6.8 에서 0.9로 감소하였다. 그 중 미약한 탄발음이 지속되었던 1예는 불완전한 제거로 판단하여 재수술을 시행하였다. 결론: 주관절의 활막 추벽은 주관절의 잠김 또는 외측 동통을 일으키는 주 진단범주에 포함시켜야할 것이며 특히 오랫동안 지속된 저항성 외 상과염이라면 활막 추벽을 고려해 봐야 할 것이다. 관절경을 통한 추벽의 제거는 매우 안전하고 효과적이었으며 단기 추시에서 매우 우수한 결과를 보였다.
Background & Purpose : Acupuncture with aromatherapy (AA) is a branch of complementary or alternative therapy recently growing in use. As the use of aromatherapy itself has grown so rapidly in recent years, studies about its safety have been accumulating. However, safety of AA has not been studied clinically and the treatment method which has the needles inserted after spreading essential oils exposes the human body more directly to components than aromatherapy. Therefore, we examined the safety of AA on patients treated thereby and physicians practising it. Methods : We observed all inpatients treated with AA from November 21, 2005 to March 31, 2006, in Kyung Hee Oriental Medical Center. During treatment, we monitored all of the newly developed signs from the patients to assess the adverse effects of AA. The patients' and physicians' general characteristics (gender, age, present illness, and treatment days) were obtained from medical records and analyzed to assess the clinical safety of AA in detail. Results : There were 440 patients treated with AA; clinical adverse effects appeared in 3 of them, which can be assessed as 0.7%. The major symptom was chest discomfort and choking originated from smell aversion condition (2 patients), and the other adverse effect was sensation of itching and reddening. However, none of them seemed to have direct relation with AA. A total of eight physicians applied AA and there were no symptoms related to it among them. Conclusions : We suggest that our results will contribute to confirming the safety of AA by offering clinical evidence.
The goats raised in the barn are usually fed on fresh grass. As dry forage can be stored for long periods in large amounts, dry forage feeding makes it possible to feed large numbers of goats in barns. This review explains the physiological factors involved in suppressing dry forage intake and the cause of drinking following dry forage feeding. Ruminants consume an enormous amount of dry forage in a short time. Eating rates of dry forage rapidly decreased in the first 40 min of feeding and subsequently declined gradually to low states in the remaining time of the feeding period. Saliva in large-type goats is secreted in large volume during the first hour after the commencement of dry forage feeding. It was elucidated that the marked suppression of dry forage intake during the first hour was caused by a feeding-induced hypovolemia and the loss of $NaHCO_3$ due to excessive salivation during the initial stages of dry forage feeding. On the other hand, it was indicated that the marked decrease in feed intake observed in the second hour of the 2 h feeding period was related to ruminal distension caused by the feed consumed and the copious amount of saliva secreted during dry forage feeding. In addition, results indicate that the marked decreases in dry forage intake after 40 min of feeding are caused by increases in plasma osmolality and subsequent thirst sensations produced by dry forage feeding. After 40 min of the 2 h dry forage feeding period, the feed salt content is absorbed into the rumen and plasma osmolality increases. The combined effects of ruminal distension and increased plasma osmolality accounted for 77.6% of the suppression of dry forage intake 40 min after the start of dry forage feeding. The results indicate that ruminal distension and increased plasma osmolality are the main physiological factors in suppression of dry forage intake in large-type goats. There was very little drinking behavior observed during the first hour of the 2 h feeding period most water consumption occurring in the second hour. The cause of this thirst sensation during the second hour of dry forage feeding period was not hypovolemia brought about by excessive salivation, but rather increases in plasma osmolality due to the ruminal absorption of salt from the consumed feed. This suggests the water intake following dry forage feeding is determined by the level of salt content in the feed.
연구목적: 통증환자의 인지적 특정은 통증에 대한 지각이나 치료에 대한 반응에서 중요한 역할을 한다. 지각된 통제소재와 같은 인지적 특정이 통증환자의 정서장애나 대처방식과 어떤 관련이 있는지 알아보고자 하였다. 방법: 통증클리닉을 내원하여 치료를 받고 있는 환자 96명을 대상으로 지각된 통제소재를 알아보는 질문지와 미네소타 다면적 인성검사 (MMPI), 대처방식질문지, 주관적으로 통증의 정도와 시간, 괴로움정도, 통제정도를 평정하도록 하여 이 자료를 토대로 분석하였다. 결과: 자료분석결과 환자가 지각하는 통제소재가 내부에 있다고 믿을수록 통증에 대한 통제감이나 통증을 감소시키려는 노력을 했을 때 감소정도가 높은 것으로 보고하였다. 반면 외부통제감이나 우연에 의한 통제감이 높을수록 지각된 통증정도나 통증경험시간, 일상생활 장애정도가 높았다. 또한 내부통제감은 MMPI척도중 자아방어관련척도와 유의한 정적상관을 보였으며, 우울, 불안, 정서억압과 같은 척도와는 부적상관을 보였다. 대처방식 중에서 자기진술 및 감각의 재해석과 같은 방식은 내부통제감를 유의한 정도로 설명하였고 외부통제감과 우연에 의한 통제는 재앙화에 의한 설명변량이 유의한 수준에 이르렀다. 결론: 통증은 비록 신체적 장애이지만 주관적 통증지각이나 일상생활기능정도, 정서적 어려움에는 지각된 통제감과 같은 인지적 요인이 영향을 미친다. 따라서 환자가 통증에 효율적으로 대처하도록 하기 위해서는 내부통제감을 증가시키고 보다 효율적인 대처방식을 사용하도록 인지들 수정해수는 것이 필요하다.
평형능력은 보행에 영향을 미치고, 보행속도는 노인의 건강한 척도를 말해주는 기준이 된다. 이 연구의 목적은 체성감각계 지원이 여성 노인의 보행 및 평형감각에 미치는 영향을 보고자 하였다. 평소 지팡이를 사용하지 않고, 규칙적으로 운동을 하지 않으며, 이비인후과적으로 이상이 없는 노인으로 평균 연령이 75세 여성 노인 61명을 실험군 31명과 통제군 30명으로 분류하였다. 종속변인으로는 2.44m왕복걷기, 10m 일반걷기 및 빠르게 걷기, 6분 걷기를 실시하였다. 평형감각 검사로는 뉴로컴사의 EquiTest를 이용하여 분석하였다. 연구 결과 2.44m 왕복걷기와 10m 일반걷기에서는 유의한 차이가 없었으나 10m 빠르게 걷기, 6분걷기에서는 유의한 효과를 보이는 것으로 나타났다. 평형감각에서는 조건 2, 3, 4, 6에서 유의한 효과를 보이는 것으로 나타났다. 결론적으로 체성감각계 지원이 노인 여성들의 보행 및 평형유지 능력에 유의한 효과를 보이는 것으로 나타나 지팡이의 적극적 사용이 권장된다 할 수 있겠다.
회화에서 프레임의 미학을 연구하는 것보다 애니메이션 화면구성에 대한 프레임을 분석하는 것은 더욱 더 중요한 의미를 지니고 있다. 단편 애니메이션작가 조르주 슈피츠게벨의 이미지 형상은 캐릭터를 지우는 흔적을 통하여 작가 신체의 촉각적 감각으로 형상화된 구체적인 표현이라고 말할 수 있다. 이는 들뢰즈가 언급한 형상지우기에 의한 흔적의 변형(메타모포시스 Metamorphosis)과 비유된다. 애니메이션에서 살아 숨쉬는 캐릭터는 들뢰즈가 기술한 형상과 같이 실사영상과는 구별된다. 애니메이션은 프레임 안에서 움직이는 캐릭터의 형상과 큰 움직임이 없는 배경으로 나눠질 수 있는데, 움직이지 않는 배경의 구성은 들뢰즈가 언급한 아플라와 유사한 성격을 갖는다. 이는 실사영상을 프레임 단위로 기록하는 영화와는 다르게 애니메이션은 실제와는 다른 경계인 아플라를 배경으로 표현하고 있으며, 캐릭터는 최소한의 구상성을 갖고 대상에 대한 형상의 일그러짐을 통하여 새로운 형상으로 재탄생되고 있다. 다음은 프레임의 시간지속에 대한 문제이다. 슈피츠게벨의 작품은 프레임과 프레임사이의 상관관계에 의하여 의미가 생성된다. 그의 작품에서는 프레임과 프레임 간의 구성에 따른 형상 묘사 자체가 프레임으로 구성된다. 이러한 과정을 통하여 슈비츠게벨의 작품은 각 프레임 사이에 연결되어 있는 개별 프레임이 새롭게 생성되는 대상의 형상을 묘사하고 있다. 이는 대상에 대한 지시체의 근본이 적용되지 않는 현대영화에서 극히 적게 찾아 볼 수 있다고 들뢰즈는 언급한다. 바꿔 말하면 슈비츠게벨의 작품은 현대영화의 특징을 갖춘 애니메이션으로 프레임과 프레임 사이의 형상을 묘사하고 있다. 이는 들뢰즈가 언급한 대상세계의 법칙과 논리에서 벗어나 이루어지는 크리스털적 묘사임을 알 수 있다.
Transient receptor potential vanilloid 3 (TRPV3) is a non-selective cation channel with modest permeability to calcium ions. It is involved in intracellular calcium signaling and is therefore important in processes such as thermal sensation, skin barrier formation, and wound healing. TRPV3 was initially proposed as a warm temperature sensor. It is activated by synthetic small-molecule chemicals and plant-derived natural compounds such as camphor and eugenol. Schisandra chinensis (Turcz.) Baill (SC) has diverse pharmacological properties including antiallergic, anti-inflammatory, and wound healing activities. It is extensively used as an oriental herbal medicine for the treatment of various diseases. In this study, we investigated whether SC fruit extracts and seed oil, as well as four compounds isolated from the fruit can activate the TRPV3 channel. By performing whole-cell patch clamp recording in HEK293T cells overexpressing TRPV3, we found that the methanolic extract of SC fruit has an agonistic effect on the TRPV3 channel. Furthermore, electrophysiological analysis revealed that ${\gamma}$-schisandrin, one of the isolated compounds, activated TRPV3 at a concentration of $30{\mu}M$. In addition, ${\gamma}$-schisandrin (${\sim}100{\mu}M$) increased cytoplasmic $Ca^{2+}$ concentrations by approximately 20% in response to TRPV3 activation. This is the first report to indicate that SC extract and ${\gamma}$-schisandrin can modulate the TRPV3 channel. This report also suggests a mechanism by which ${\gamma}$-schisandrin acts as a therapeutic agent against TRPV3-related diseases.
인간의 손은 현실의 사물과 인터랙션하는 가장 직접적이고 자연스러운 인터페이스라고 할 수 있으나, 가상 환경에서는 인간 손 관절체의 높은 자유도와 관련 인터페이스 장치의 한계로 말미암아 가상현실 애플리케이션에 활발히 도입되고 있지 못한 상황이다. 특히, 가상 조립 시뮬레이션은 제품 개발 단계에서 디지털 목업의 검증을 위한 프로세스로서 가상현실 애플리케이션 중 도전적인 주제라고 할 수 있다. 하지만, 가상의 객체를 파지(grasp)하기 어렵고 지속적으로 세밀하게 조작 할 수 없는 점 등은 핸드 햅틱 인터랙션이 가상 조립 등의 가상현실 애플리케이션에 적용 시 커다란 장벽으로 인식되고 있다. 본 논문에서는 핸드 햅틱 인터랙션을 두 관점에서 분석하여 각 단계별로 단계적 절차를 수행하는 방법을 제안하고자 한다. 인간이 사물을 쥐고 조작할 때, 손 안에서 사물이 빠져나가지 않도록 사물의 외형에 따라 손의 힘의 균형을 조절하는 견고함과 일단 사물을 쥐게 되면 손 안에서 인간의 의도를 반영한 정밀한 사물 조작이 가능한 세밀성이 가능한 핸드 햅틱 인터랙션을 제안하고자 한다. 제안된 알고리즘은 초기 grasp를 용이하게 하기 위해 품질평가척도를 통해 grasp의 견고함을 확보하고, 세밀한 조작성을 확보하기 위해 물리 기반의 시뮬레이션을 수행한다. 제안된 방법의 효율성을 평가하기 위하여 서로 다른 디스플레이 환경-모노, 입체 디스플레이-에서 실험을 수행하였다. 그리고 2-way ANOVA 테스트를 통하여 본 연구에서 제안한 충돌 전 grasp 단계와 충돌 후 조작 단계로 구분된 알고리즘이 앞서 언급한 두 관점을 모두 만족함을 보였다. 마지막으로, 제안된 인터랙션 방법을 이용하여 복잡한 그래픽 모델에 관한 가상 조립 시뮬레이션에 적용된 실 사례를 보였다.

제 5 장 계약 해지 및 이용 제한
제 15 조 (계약 해지)

제 16 조 (서비스 이용제한)
① 당 사이트는 회원이 서비스 이용내용에 있어서 본 약관 제 11조 내용을 위반하거나, 다음 각 호에 해당하는
경우 서비스 이용을 제한할 수 있습니다.
- 2년 이상 서비스를 이용한 적이 없는 경우
- 기타 정상적인 서비스 운영에 방해가 될 경우
② 상기 이용제한 규정에 따라 서비스를 이용하는 회원에게 서비스 이용에 대하여 별도 공지 없이 서비스 이용의
일시정지, 이용계약 해지 할 수 있습니다.
제 17 조 (전자우편주소 수집 금지)
회원은 전자우편주소 추출기 등을 이용하여 전자우편주소를 수집 또는 제3자에게 제공할 수 없습니다.
제 6 장 손해배상 및 기타사항
제 18 조 (손해배상)
당 사이트는 무료로 제공되는 서비스와 관련하여 회원에게 어떠한 손해가 발생하더라도 당 사이트가 고의 또는 과실로 인한 손해발생을 제외하고는 이에 대하여 책임을 부담하지 아니합니다.
제 19 조 (관할 법원)
서비스 이용으로 발생한 분쟁에 대해 소송이 제기되는 경우 민사 소송법상의 관할 법원에 제기합니다.

1. (시행일) 이 약관은 2016년 9월 5일부터 적용되며, 종전 약관은 본 약관으로 대체되며, 개정된 약관의 적용일 이전 가입자도 개정된 약관의 적용을 받습니다.
