The Study of Prescription Behaviors of Practicing Pharmacists with Simulated Patients of Arthritis (관절염 증상의 모의 환자를 이용한 약사의 처방 행태 연구)

  • Objectives : In Korea, pharmacists can dispense medicines without doctor's prescription. This causes the high proportion of pharmaceutical expenditures. The study shows the prescribing behaviors of practicing pharmacists with the simulated patient of arthritis. We select the arthritis as a subject of simulation, because the arthritis is one of the major health problems and the abuse of cortico-steroids is usual in treatment of arthritis patients. Methods : Twenty drug stores among the 320 drug stores in a district, Seoul, Korea were randomly selected. One of the researchers visited the drug stores and received the medicines from the pharmacists after explaining standardized scenario of arthritis. The simulated patient recorded the practice behaviors of pharmacists. Results: The mean number of prescribed drugs are four and half. Among the twenty pharmacists, the nineteen prescribed non-steroidal anti-inflammatory drugs and the seven(35%) prescribed the cortico-steroids. The antacids were prescribed by the fourteen(70%) pharmacists. The five(25%) pharmacists only recommended the simulated patients to visit the medical doctors, and the three(15%) performed physical examination to the simulated patients. The three pharmacists(15%) asked the past history of the drug adverse effects and no pharmacist explained the adverse effects of prescribed medicines. Conclusions : The research shows that the cortico-steroids are frequently prescribed and the pharmacists commonly do not give the explanations of the prescribed medicines to the arthritis patients.

Electromyographic Observation of Silent Period of the Masseter Muscle (교근의 휴지기에 관한 근전도학적 관찰)

  • The author has studied the duration of silent period of the masseter muscle using electromyography in order to obtain the information that is necessary to diagnose the patients with TMJ dysfunction syndrome objectively. The 55 normal subjects and the 40 patients with TMJ dysfunction syndrome were selected for this study. The results were as follows : Duration of silent period of the masseter muscles in normal subjects were 24.4 msec in males, 24.0 msec in females, and mean of $24.2\pm3.19$ msec. Duration of silent periods of the masseter muscles in patients with TMJ dysfunction syndrome were 39.4 msec in males, 41.3 msec in females, and mean of $40.5\pm7.01$ msec. There was no significant difference of the duration of silent periods between in the right and the left side of the masseter muscles, and between in the males and the females (p>0.05). Duration of silent periods of the masseter muscles in patients with TMJ dysfunction syndrome were significantly more prolonged than those in normal subjects (p<0.01). Borderline of silent periods of the masseter muscles between in normal and patients with TMJ dysfunction syndrome was 31.5 msec.

  • Purpose D.I.T.I. can be used to diagnose the dysmenorrhea and prove the Oriental Medical Theory and the Sasang Constitutional Medicine concerning Dysmenorrhea. According to the Sasang Constitutional Medicine, we chose 39 dysmenorrhea patients (Soeumin 26 Patients, Taeumin 13 Patients) and expremented D.I.T.I. to find difference of chilliness and fever in the upper part of body and the lower part of body. Methods We expremented 39 dysmenorrhea patients chosena expremental group from Apr 1. 2002 to May 30, 2002. We diagnosed the constitution by Q.S.C.C. and selected six acu-points CV-17, CV-12, CV-4, G-21, B-15 and B-23. To appraise difference of temperature according to constitution we compared skin temperature of front to back. Result Mean Absolute Value of ${\Delta}T$ in body (front and back) shows significant difference. Totally the temperature of front is higher than the temperature of back. the temperature of Soeumin is higher than the temperature of Taeumin in whole. Conciusion Especially the upper abdomen area(CV-17) shows high temperature in 39 dysmenorrhea patients, the lower abdomen area (CV-4) of Soeumin shows lower temperature relatively.

  • Purpose: We aim to research the difference of abdominal temperature in dysmenorrhea patients comparing with that of normal person. Metholds: We selected the 49 dismenorrhea patients as study group and 54 normal persons as control group, excluding the patients who have dermatitis, ared over 70 or under 10. and have extreme obesity. We compared the 4 points(Rt and Lt Gimun(LR14) and Guirae(ST29)) in thd abdomen of both group, check the temperature of those points with DITI(Dorex, DTI-16UTI) to compare the temperature difference with each other. For statistics, we use independent T-test, SPSS 8.0 for windows. Results: The temperature difference between Rt and Lt Gimun which are in upper abdomen shows statistically significant result in study group. Conclusion: The dysmenorrhea patients show larger temperature difference between Rt and Lt points in upper abdomen than normal persons. In lower abdomen, the temperature between control and study group shows little difference.

  • This study was done to identify the effectiveness of a proper positioning on musculoskeletal complication on patient with Stroke by using a quasi-experimental study. A total of 18 patients were selected as a subject from June 1st to October 31th 1998 who had been hospitalized at Intensive Care Units in K medical center. A experimental group consists of 8 patients who were given proper positioning every two hours. A control group consists of 10 patients who were given traditional positioning. The collected data were analyzed by SPSS windows including $X^2$-test (Fisher's exact method), Wilcoxon Rank Sum test and McNemar test. The result of this study was summerized as follows : 1) The experimental group with proper positioning has shown lower shoulder pain score, dorsiflexion contracture score, Shoulder subluxation score and higher Range of Motion and than the control group. 2) The experimental group with proper positioning has shown lower muscle pain score and edema score than the control group, but it was not statistically significant.Therefore, proper positioning could be applied as an independent nursing intervent ion for patients on Stroke in order to facilitate rehabilitation.

Oral Analgesics for Patients with Cancer Pain (암성 통증에 대한 약물 요법)

  • We investigated the effect of oral analgesics for patients with cancer pain, retrospectively. One hundred forty six cases were randomly selected during the period between 1986 to 1990. These patients were treated with Tramadol (n=31), Tylenol III(Acetaminophen plus codein phosphate, n=44) or MS-Contin (n=71). partial to complete pain relief as achieved accord to each durg group at 67.8, 68.2 and 76.1% respectively. Minimal side effects were noted in a few patients. Our results show encouraging possibilities for pain control in cancer patients. However, futher studies should be done to find more effective cancer pain management by oral analgesics.

  • The purpose of this study is to develop the speaker recognition system which can select one of operating modes in FES for general paralysis patients. As spiral injury by traffic accident, industrial disaster, or stroke has been increased, the development of FES(Functional Electrical Stimulator) system is urgent to prevent paralysis and atrophy, and to assist the patients walking. For these patients we developed FES system(1). To operate this system one of several operating modes must be selected. As this can not be done by general paralysis patients, an attempt has been tried in this study to select the mode by speaker recognition system. RSC-300 of sensory co. has been chosen as a speaker recognition chip, and PIC16F84 is adapted to interface RSC-300 and FES system.

Clinical Implication of Mechanical Insufflation-Exsufflation Method in Patients with Duchenne muscular dystrophy (뒤시엔느 근이영양증 환자에게 기계적 기침보조기법 적용의 임상적 의의)

  • Purpose : The aim of this study was to clarify the lung capacity, maximal insufflation capacity, and peak cough flow when a mechanical insufflation-exsufflation(MIE) method was used to increase Duchenne muscular dystrophy patients' lung function. Methods : The subjects of the study were 21 patients with Duchenne muscular dystrophy. They were randomly selected from patients within the boundaries of the selection criteria, and divided into two groups; The subject group(n=11) used the mechanical insufflation-exsufflation method with traditional therapeutic exercise. The control group(n=10) used only traditional therapeutic exercise. Results :The results indicated that maximal insufflation capacity, unassisted peak cough flow and assisted peak cough flow significantly increased in the subject group(p<.05). By contrast, in the control group, the results didn't indicate the significant differences from the variable. There were significant differences in maximal insufflation capacity and assisted peak cough flow between the subject group and the control group before and after the application of the mechanical insufflation-exsufflation method. Conclusion : A mechanical insufflation-exsufflation method has positive effects on the improvements of cough functions and that of pulmonary functions such as lung volume, lung elasticity in patients with Duchenne muscular dystrophy.

The Effects of Nutritional Education Program on Nausea and Vomiting, Anorexia, Food Intake, and Nutritional Status of GI Cancer Patients Undergoing Chemotherapy (영양교육프로그램이 항암화학요법을 받는 소화기암환자의 오심구토, 식욕부진, 식이섭취 및 영양상태에 미치는 효과)

  • Purpose: The purpose of this study was to identify the effects of nutritional education program (NEP) on nausea and vomiting, anorexia, food intake, and nutritional status among GI cancer patients undergoing chemotherapy. Methods: A nonequivalent, non-synchronized posttest design was used. Study subjects were conveniently selected 30 patients (15 subjects for each group) who were undergoing chemotherapy. The subjects of the experimental group received NEP which consisted of 2 educations and 1 counselling while they were hospitalized, and telephone counselling with nutritional supports after discharge for four times. Results: The score of anorexia was lower and the amount of food intake was higher in the experimental group than those of the control group. However, there were no significant differences in levels of nausea and vomiting, weight, hemoglobin, albumin, and total protein level between the experimental and the control group. Conclusion: The findings demonstrate that the NEP was effective in alleviating anorexia and improving subjective food intake of GI cancer patients undergoing chemotherapy. These findings suggest that NEP can be a useful nursing intervention for preventing nutritional disorders for patients with GI cancer after chemotherapy.

A clinical literature review and research-trends analysis of bee venom pharmacopuncture for cancer patients (암 환자에 대한 봉독 약침요법의 임상문헌 고찰 및 연구동향 분석)

  • Objectives: This review aims to investigate clinical studies related to bee venom pharmacopuncture for cancer patients and to analyze the research trend for further study. Methods: We searched for clinical studies using bee venom pharmacopuncture therapy on patients with cancer through the electronic databases including Pubmed, Cochrane library, OASIS, KISS, NDSL, and KMBASE. There was no restriction on language and publication date, and after selection/exclusion process, the study design, target disease, intervention details including acupoints, treatment frequency and period, outcomes, study results and adverse events were extracted. Results: Thirteen clinical studies were finally selected. There were a randomized controlled trial RCT about the effect of sweet bee venom pharmacopuncture on cancer-related pain, and three case series about chemotherapy-induced peripheral neuropathy. In case reports, there were nine studies about oligodendroglioma, plexiform neurofibroma, breast cancer, prostate cancer, lung cancer, urachal adenocarcinoma, malignant melanoma, and atypical squamous cells of undetermined significance. The bee venom therapy affected the improvement of outcomes such as symptoms, quality of life, tumor response, and lab findings. Conclusions: The present study found that bee venom therapy is applicable to the treatment of cancer patients, and showed some effect on various symptoms. However, due to insufficient number and quality of studies, well designed and high-quality clinical trials are necessary to confirm the effectiveness and safety of bee venom pharmacopuncture therapy in patients with cancer.
