Factors associated with Respiratory Usage of Manufacturing Workers Based on the Reasoned Action Theory (이성적 행동이론에 의한 제조업 근로자들의 호흡보호구착용에 영향을 주는 요인분석)

  • Objectives : The purpose of this study is to analyze the factors associated with respirator usage on the basis of reasoned action theory. Methods : The data were collected from August 1 to September 30 1999, and study subjects consisted of 303 workers who were employed in the manufacturing industries. A self-administered questionnaire was used to measure the attitude, subjective norm and related factors. Results : In bivariate analysis, the variables related to protector usage were prevention of occupational disease, protection of toxic material, disturbance during working, troublesome of usage, proper exchange of protector. Multivariate logit analysis was used to estimate factors associated with respirator usage. Significant predictors that are related to respirator usage were attitude toward the behavior, size of industry and proper exchange of protector. Conclusions : The results suggest that it is strongly required to focus on attitude toward the behavior in order to improve workers' usage of respirator.

Retrospective study on the airway obstruction aspects of computed tomography and lateral cephalometry and the correlation of polysomnography in obstructive sleep apnea patients

  • Objectives: Lateral cephalometry, computed tomography (CT) and full-night polysomnography were used to examine the correlation of obstructive sleep apnea (OSA) severity. Materials and Methods: A total of 29 patients (5 females, 24 males) diagnosed with OSA were evaluated by lateral cephalometry, CT and full-night polysomnography. Lateral cephalometry was performed in the closed and open mouth states. The radiographic and polysomnography measurements of the patients with OSA were evaluated statistically to determine the association with OSA severity. Results: A significant relationship was observed between the increased respiratory disturbance index and closing lateral cephalometry. With mouth opening, the airway space narrowed and the OSA worsened. Lateral cephalometry revealed OSA patients to have an inferiorly positioned hyoid bone, longer-than-normal soft palate and narrowing airway space. As OSA was severe, the airway shape was ovoid in the CT horizontal view. Conclusion: Polysomnography and the radiographic parameter can be used for diagnosing OSA.

  • The study has been carried out to investigate of the Allergic Rhinitis by referring to 87 literatures. The results were as follows; 1. In oriental medical science, Allergic Rhinitis is belong to the category of the 'BiGu'(鼻구) 'GuChe'(구체). The 'Gu'(구) of the BiGu means watery rhinorrhea, the 'Che'(체) of the GuChe means sneezing. 2. The cause of a disease summarize the weak of Lung, Spleen and Kidney, and invasion into the nasal cavity of PoongHan etc a wrong air. Sometimes the pathologial change appear PoongHan changeHwa(風寒化火), HwaYeol is hidden in the inside(火熱內伏). The contributing factors are found a season(spring, winter), an abnormal weather(運氣 : 少陰 少陽 陽明 司天, 歲金不及), an emotional stress, an external wound of the harmful air, a food allergens and fatigue, a contact of substances, a sunlight etc. 3. Predominant symptoms are watery rhinorrhea, sneezing and nasal obstruction. Sometimes accompanic symptoms are nasal bleeding, mucopurulent rhinorrhea, olfactory disturbance, nasal polyp, rhinolalia clausa, respiratory disfunction etc. 4. The treatment-methodes is as follows, OnBoPaeJang GeoPoongSanHan(溫補肺臟 祛風散寒), GeonBilkGi(健脾益氣), BoSinNabGi (補腎納氣). The treatmentherbs is as follows, OnBoJiLuDan GaGam(溫補止流丹 加減), OkByeongPoongSan plus ChangIJaSan GaGam(玉屛風散合 蒼耳子散 加減), BoJungIkGiTang GaGam plus SoCheongLyongTang(補中益氣湯加減 配合 小靑龍湯), SinGiHwan GaGam(腎氣丸加減), GaeJiTang(桂枝湯) etc. 5. The external treatment is as follows, JeokBi(滴鼻), ChuiBi(吹鼻), SaekBi(塞鼻), stick and herbs-injection on the acupuncture-point, pressure ear acupuncture-point, herbs-pillow etc. 6. The acupuncture-moxa treatment is as follows, the methodes of cure apply TongJoGyeongGi(通調經氣), SanTongBiGui(宣通鼻竅) etc. Predominent acupuncture-points are YoungHyang(迎香), InDang(印堂), BiTong(鼻通), SangSeong(上星), HabGok(合谷) and so on. As mentioned above, from now on, it's need to the oriental medical scientific study of the Immunity and Allergy and to the external treatment's application for the ascent of the treatment-effect of the allergic disease.

  • Obstructive sleep apnea syndrome(OSAS) is most frequently diagnosed in the sleep laboratories and its severity is objectively estimated. In terms of treatment, the most prevalent method is the continuous positive airway pressure(CPAP) application as of now. However, in Korea, CPAP is still not sufficiently known and it is attributable to the fact that sleep study facilities have not been popularized. In this article, the authors present their own experience with CPAP in nine subjects with OSAS. In this study, CPAP was found to decrease stage 1 sleep and to increase stage 2 sleep, with increasing mean oxygen saturation and decreasing lowest oxygen saturation during nocturnal sleep. Also, it tended to increase sleep continuity and generally to improve sleep architecture. Rebound slow wave and/or REM sleep stages during CPAP were also noted in 8 out of 9 cases. The authors suggest that CPAP should be considered as the primary mode of treatment for patients with obstructive sleep apnea syndrome and related educational programs for physicians should be developed and provided by sleep specialists.

A Study for the Parent's Recognition of the Oriental Medical Treatment and the Expectation of Children's Growth (소아의 성장과 한방치료에 대한 부모들의 인식조사)

  • Objectives: The purpose of this study is to evaluate the parents' awareness on the oriental medical treatment and their expectation on children's growth. Methods: The survey was conducted on 78 Health Kids Fair visitors, and 87 children's height and weight were measured in this study. Results: 1. Compare to other treatment, 62.82% of the parents responded that herbal medicine is relatively effective in treating weakness, followed by 'Allergic disease'(46.15%) 'Growth disturbance'(26.92%) 'Obesity'(26.92%) 'Respiratory disease'(26.92%) 'Digestive disease'(19.23%) 'Precocious puberty'(8.97%) 'Neurologic & psychologic disease'(6.41%) 'Urogenital disease'(3.85%). 2. Parents recognize that 'Oriental medicine have an effect on children's height mostly'(25.64%) 'Oriental medicine have an effect on children's height partially'(64.10%) 'Oriental medicine have no effect on children's height'(10.26%) 25.64% of the parents responded that herbal medicine would be helpful in increasing height, 64.10% of the parents said they would be helpful to the certain extent, and 10.26% said they would not play any roles. 3. Expected average weight, height, and BMI score for the boys were 71.8kg, 179.6cm and 22.10. For the girls, however, they were 53.4kg, 168.7, and 18.74. 4. Survey on parents' awareness on benefits of different treatments for challenged growth, Herbal medicine'(48.72%) 'Acupuncture'(7.69%), 'Moxibustion'(3.85%), 'Electronic acupuncture and Aqua acupuncture'(1.28%), 'Massage on acupuncture point'(19.23%), 'Consultation of eating habits'(61.54%), 'Consultation of exercise'(47.44%) were measured. Conclusions: Considering the collected results, we realized that the parents' expected height on their children was, in fact, higher than the standard height. In addition, for treatments for their children's growth improvement, parents expected that 'Herbal madicine' 'Massage on acupuncture point' 'Consultation of eating habits' 'Consultation of exercise would be beneficial.

Survey on the Actual Conditions of Patients in Semyung University Attached Oriental Medical Hospital Night Clinic. (세명대학교 부속한방병원 야간 진료실 내원환자의 실태분석)

  • This study on the visits of the oriental medical night clinic of patients, was made to be used as reference data by examining and analyzing statistically the many actual conditions of patients who had been visited in Semyung University Attached Oriental Medical Hospital during the period from April, 1998 to March, 1999. The purpose of this survey was to understand the characteristics of patients, to evaluate the roles of Night Clinic of oriental medical hospital, and contribute to the systemic and efficient management of night clinic service. The results obtained were as follows: I. Distribution of sex: male 53% (421 cases), female 47% (379 cases) 2. The age distribution of patients showed the highest in under ten, followed by the thirties and fifties, forties and twenties in order. 3. The monthly distribution of patients showed the highest in October 1998, followed by May 1998 and February 1999 in order. 4. The daily distribution of patients showed the highest in Sunday, followed by Saturday, Monday, Friday and Tuesday in order. 5. The regional distribution revealed the highest in Jecheon with 76% of all patients, followed by Danyang in order. 6. The distribution of arrival time showed the highest in 7:00 pm~0:00 am (27%), followed by 5:30 pm~7:00 pm (23%) in order. 7. The admission rate in patients was 17%. Among them direct visiting rate was 85%. Circulatory systemic disease was the highest. 8. The highest incidences by disease were of motor system with 48% (394cases), followed by diseases in circulatory system with 19% (l48cases). The majority of the patients 67% was connected with two diseases. 9. The chief complaint of pediatric diseases was febrile seizure with 64% (32cases), digestive disease was abdominal pain with 44% (90cases), circulatory diseases was motor disturbance with 43% (83cases), motor system disease was leg pain with 37% (l19cases), respiratory disease was fever with 46% ( 41 cases). 10. In the treatment method, a major portion of treatment methods was acupuncture treatment with 32% (373cases), followed by acupuncture & herbal medicine treatment with 23% (275cases). The most commonly used herbal medicines were Hangsapyunguisan(l6%) and Ojeoksan(l4%).

An Electrocardiographic Study on Tetrodotoxin Intoxicated Rabbits (Tetrodotoxin 중독가토(中毒家兎)의 심전도학적(心電圖學的) 연구(硏究))

  • Tetrodotoxin (TTX) is the purified active principle responsible for tetrodon (Puffer-fish) poisoning which has long been known in the Orient. The pharmacological actions of TTX have been rather extensively investigated. Two of the most prominent effects of intravenousely administered TTX are severe hypotension and respiratory paralysis resulting from its depressant actions on tissues. This depressant actions of TTX in turn result from the selective inhibition of sodium-carrying mechanism which is essential to generation of the action potential. TTX differs from local anesthetics in that it does not affect potassium conductance. Although the mechanism of the hypotensive action of TTX remains a subject of controversy, most investigator agree that TTX-induced hypotension is caused by alteration in the blood vessels rather than the heart. Not only the study on the effects of TTX on cardiac function is meager but the results of reported works are often contradictory. The present study was undertaken to investigate the effect of TTX on the electrocardiogram of the rabbit and to compare them with well known electrocardiographical characteristics found in digitalis and quinidine intoxicated animals. The results obtained from the present study are summarized as follows. 1. No changes were found in P-R interval and QRS duration after i.v. administration of $1.0\;{\mu}g/kg\;to\;1.5\;{\mu}g/kg$ TTX to the animals. It is obvious that there were no conduction disturbance between atria and ventricles as well as in the ventricular tissue. 2. In $1.0\;{\mu}g/kg$ TTX group, S-T interval and T-P segment were not changed whereas marked changes were observed in $1.5\;{\mu}g/kg$ TTX group. 3. The first and second degree A-V blocks appeared in the $2.0\;{\mu}g/kg$ TTX group. 4. TTX differs from digitalis and quinidine in that it does not cause S-T interval depression and T-wave inversion. In contrast with digitalis, TTX caused Q-T interval prolongation.

A CASE REPORT OF CONGENITAL EPULIS IN THE FETUS (태생 36주에 발견한 선천성 에푸리스(congenital epulis)의 치험례)

  • The congenital epulis of newborn is a rare benign tumor that occurs on the gingiva of the anterior alveolar ridge of the jaws, also known as a congenital granular cell tumor. Females are affected 8 - 10 times more often than males. The typical location is the alveolar ridge of the maxilla near the canine but the mandibular region can also be involved. The lesion may be sessile or pedunculated with red or normal color. The size of tumor varies from several millimeters to a few centimeters in diameter. Surgical excision is generally indicated and no recurrences have been noticed. Spontaneous regression is rare. It is histologically similar to the granular cell tumor, although pseudoepitheliomatous hyperplasia does not occur in the former lesion. Thus sheets of large, closely packed cells showing fine, granular, eosinophilic cytoplasm comprise the tumor mass. This is a case report of congenital epulis in the fetus. The lesion was detected in ultrasonogram, its size is 3cm in diameter. The infant showed nursing disturbance and respiratory obstruction after birth, so the tumor was surgically excised under local anesthesia.

Sleep Monitoring by Contactless in daily life based on Mobile Sensing (모바일 센싱 기반의 일상생활에서 비접촉에 의한 수면 모니터링)

  • In our daily life, quality of sleeping is closely related to happiness index. Whether or not people perceive sleep disturbance as a chronic disease, people complain of many difficulties, and in their daily life, they often experience difficulty breathing during sleep. It is very important to automatically recognize breathing-related disorders during a sleep, but it is very difficult in reality. To solve this problem, this paper proposes a mobile-based non-contact sleeping monitoring for health management at home. Respiratory signals during the sleep are collected by using the sound sensor of the smartphone, the characteristics of the signals are extracted, and the frequency, amplitude, respiration rate, and pattern of respiration are analyzed. Although mobile health does not solve all problems, it aims at early detection and continuous management of individual health conditions, and shows the possibility of monitoring physiological data such as respiration during the sleep without additional sensors with a smartphone in the bedroom of an ordinary home.

  • Objectives: Nasal continuous positive airway pressure (CPAP) corrected elevated blood pressure (BP) in some studies of obstructive sleep apnea syndrome (OSAS) but not in others. Such inconsistent results in previous studies might be due to differences in factors influencing the effects of CPAP on BP. The factors referred to include BP monitoring techniques, the characteristics of subjects, and method of CPAP application. Therefore, we evaluated the effects of one night CPAP application on BP and heart rate (HR) reactivity using non-invasive beat-to-beat BP measurement in normotensive and hypertensive subjects with OSAS. Methods: Finger arterial BP and oxygen saturation monitoring with nocturnal polysomnography were performed on 10 OSAS patients (mean age $52.2{\pm}12.4\;years$; 9 males, 1 female; respiratory disturbance index (RDI)>5) for one baseline night and another CPAP night. Beat-to-beat measurement of BP and HR was done with finger arterial BP monitor ($Finapres^{(R)}$) and mean arterial oxygen saturation ($SaO_2$) was also measured at 2-second intervals for both nights. We compared the mean values of cardiovascular and respiratory variables between baseline and CPAP nights using Wilcoxon signed ranks test. Delta ($\Delta$) BP, defined as the subtracted value of CPAP night BP from baseline night BP, was correlated with age, body mass index (BMI), baseline night values of BP, BP variability, HR, HR variability, mean $SaO_2$ and respiratory disturbance index (RDI), and CPAP night values of TWT% (total wake time%) and CPAP pressure, using Spearman's correlation. Results: 1) Although increase of mean $SaO_2$ (p<.01) and decrease of RDI (p<.01) were observed on the CPAP night, there were no significant differences in other variables between two nights. 2) However, delta BP tended to increase or decease depending on BP values of the baseline night and age. Delta systolic BP and baseline systolic BP showed a significant positive correlation (p<.01), but delta diastolic BP and baseline diastolic BP did not show a significant correlation except for a positive correlation in wake stage (p<.01). Delta diastolic BP and age showed a significant negative correlation (p<.05) during all stages except for REM stage, but delta systolic BP and age did not. 3) Delta systolic and diastolic BPs did not significantly correlate with other factors, such as BMI, baseline night values of BP variability, HR, HR variability, mean SaO2 and RDI, and CPAP night values of TWT% and CPAP pressure, except for a positive correlation of delta diastolic pressure and TWT% of CPAP night (p<.01). Conclusions: We observed that systolic BP and diastolic BP tended to decrease, increase or remain still in accordance with the systolic BP level of baseline night and aging. We suggest that BP reactivity by CPAP be dealt with as a complex phenomenon rather than a simple undifferentiated BP decrease.
