Chordoma is a malignant tumor arising from the primitive notochord involving the axial skeleton. It usually occurs at sacrococcygeal and besisphenoidal area but only rarely does at other vertebral areas, especially at the thoracic vertebrae. It has a slow growth rate and is locally aggressive with an extremely high rate of local recurrence. Either surgery or radiation alone often fails to cure the disease and the local failure is the main cause of treatment failure and death. Overall 5 year survival rate is less than $10\%$. Useful palliation or occasional cure can be obtained by the combination of surgery and radiotherapy. After incomplete resection, the tumor requires radiation dose of 7,000 cGy or more over 6-7 weeks for local control. Tumor regression is slow in response to irradiation and continuation of the regression for several months after completion of RT is not unusual. We report a case of chordoma of the thoracic vertebra, the site of extreme rarity, which showed good local control after partial resection and radiation therapy. He is well and alive without any evidence of recurrence after 13 months of treatment with near complete tumor regression.
As the use of virtual simulation expands, digitally reconstructed radiographs (DRRs), which mimic conventional simulation films, play an increasingly important role as reference images in the verification of treatment fields. The purpose of our study is to develop an algorithm for computation of digitally reconstructed radiographs based on Monte Carlo simulation that take into account almost all possible physical processes by which photons interact with matter. The Monte Carlo simulation based DRRs have the following features. 1) Account has been taken of almost all possible physical processes of interaction of photons with matter, including a detector (film) response. In principle, this is equivalent to X-ray radiography. 2) Arbitrary photon energies (from diagnostic to therapeutic) can be used to produce DRRs. One can even use electrons as the source. 3) It is easy to produce a double exposure, which mimics the double exposure portal image and may have superior visual appeal for treatment field verification, with weighting within the treatment field.
D-(+)-glucose (Glc) was added to the original Fricke polyvinyl alcohol-glutaraldehyde-xylenol orange (FPGX) hydrogel dosimeter system to make a more stable FPGX hydrogel three-dimensional dosimeter in this paper. Polyvinyl alcohol was used as a substrate, which was combined with Fricke solution. Various concentrations of Glc were tested with linear relevant fitting for optimal hydrogel production conditions. The effects of various formulations on the stability and sensitivity of dosimeters were evaluated. The results indicated that D-(+)-Glc, as a free radical scavenger, had a great effect on stabilizing the dose response related to absorbency and reducing the auto-oxidization of ferrous ions. A careful doping with Glc could slow down the color change of the dosimeter before and after radiation without any effect on the sensitivity of the dosimeter.
Despite of mortality and morbidity rates that are higher than other forms of therapy, surgical resection has been mainstay for the treatment of esophageal cancer because of a prompt completeness as well as a high possibility of cure. But a substantial numbers of patients are unsuitable for surgical treatment and those undergoing resection have still poor long term survival rate. With hopes of improving long term survival, we have attempted multimodal approach, composed of preoperative induction chemotherapy utilizing Cisplatin and 5 \ulcornerFU, surgery and postoperative loco-regional radiotherapy for the treatment of esophageal cancer since 1985. During the period of 1967 \ulcorner1985, 27 patients[group A] were treated by surgery only and during recent 5 years 28 patients[group B] by multimodal treatment, Clinical review and comparison between the two groups were as follows: l. Applied surgical procedures were hand-sewn esophagogastrostomy, esophagocologastrostomy and esophagojejunostomy in group A. In group B, only esophagogastrostomy was underwent using stapler mainly. 2. Incidence of peri and postoperative complication showed no remarkable differences between the two groups, but the occurrences of leakage from the anastomotic site were 5[19% ] out of group A and 1[4%] out of group B. 3. The response rate to induction chemotherapy was 36% in group B, 4. The number of local cancer recurrence at the site of anastomosis was 6[22%] out of group A, whereas 2[7%] out of group B. 5. Postoperative I year and 2 year survival were 61%, 15% in group A and 75%, 42% in group B.
The objective of this retrospective study was to investigate prognostic factors associated with survival of patients with extensive stage small cell lung cancer (ES-SCLC). Included were 200 patients admitted to the Liberation Army General Hospital with a diagnosis of ES-SCLC. The demographics of patients, disease characteristics, pre-treatment biochemical parameters and therapeutic plan were assessed or evaluated. Univariate analysis found that second-line chemotherapy, radiotherapy, and no liver metastasis were associated with improved survival. Tumor response to first-line chemotherapy and normal initial hemoglobin levels were also associated with a survival benefit (all P-values ${\leq}$ 0.0369). Multivariate Cox regression analysis indicated that liver metastasis and the total number of all chemotherapy cycles were independent prognostic factors of survival. The morbidity risk in patients with liver metastasis was 2.52-fold higher than that in patients without liver metastasis (hazard ratio (HR)=2.52 (1.69-3.76); P<0.0001). However, one unit increase in the total number of chemotherapy cycles decreased the risk of death by 0.86-fold (HR=0.86 (0.80-0.92); P<0.0001). Absence of liver metastasis and ability of a patient to receive and tolerate multiple lines of chemotherapy were associated with longer survival.
Malignant pleural mesothelioma (MPM), an aggressive malignant tumor of mesothelial origin associated with asbestos exposure, shows a limited response to conventional chemotherapy and radiotherapy. Therefore, the overall survival of MPM patients remains very poor. Progress in the development of therapeutic strategies for MPM has been limited. We recently reported that the calpain inhibitor, calpeptin exerted inhibitory effects on pulmonary fibrosis by inhibiting the proliferation of lung fibroblasts. In the present study, we examined the preventive effects of calpeptin on the cell growth of MPM, the origin of which is mesenchymal cells, similar to lung fibroblasts. Calpeptin inhibited the proliferation of MPM cells, but not mesothelial cells. It also prevented 1) the expression of angiopoietin (Ang)-1 and Tie-2 mRNA in MPM cells, but not mesothelial cells and 2) the Ang-1-induced proliferation of MPM cells through an NF-kB dependent pathway, which may be the mechanism underlying the preventive effects of calpeptin on the growth of MPM cells. These results suggest potential clinical use of calpeptin for the treatment of MPM.
Radiotherapy is one of the major therapies for cancer treatment. p53 acts as a central mediator of the cellular response to stressful stimuli, such as radiation. Recently it has been known that activation of the phosphatidylinositol-3-kinase (PI3K) pathway is associated with radioresistance. In this study, we investigated whether X-irradiation up-regulates PI3K in a p53-dependent manner in human colon cancer cells. In order to study this phenomenon, we have treated p53-wild type and p53-mutant type HCT116 cells with X-ray. Treatment of wild type HCT116 cells with 8 Gy resulted in a marked increase in PI3K (p85), which paralleled an increase in PTEN, a counterpart of PI3K. However, these effects of X-rays in the p53-mutant cells were not observed. These results suggest that the X-irradiation-induced up-regulation of PI3K/PTEN pathway is p53-dependent.
Image-based three dimensional radiation treatment planning(3D RTP) has a potential of generating superior treatment plans. Advances in computer technology and software developments quickly make 3D RTP a feasible choice for routine clinical use. However, it has become clear that an evaluation of a 3D plan is more difficult than a 2D plan. A number of tools have been developed to facilitate the evaluation of 3D RTP both qualitatively and quantitatively. For example, beam's eye view(BEV) is one of the most powerful and time-saving method as a qualitative tools. Dose-volume histogram(DVH) has been proven to be one of the most valuable methods for a quantitative tools. But it has a limitation to evaluate several different plans for biological effects of the tissue and critical organ. Therefore, there is a strong interest in developing quantitative models which would predict the likely biological response of irradiated organs and tissues, such as tumor control probability(TCP) and normal tissue complication probability(NTCP). DVH and NTCP of hepatoma were evaluated for three dimensional conformal radiotherapy(3D CRT). Also, 3D RTP was analysed as a dose optimization based on beam arrangement and beam modulation.
Malignant lymphoma rarely occurs in the larynx and hypopharynx. Few cases of malignant lymphoma in the larynx were reported in Korea. However, malignant lymphoma in the hypopharynx had been not reported in Korea. A 68-year-old woman came to the outpatient clinic with a foreign body sensation in her throat. A round, smooth margin, bright pink-colored mass was confirmed by the laryngoscopy. The patient took neck computer tomography. A small bulging of mucosa was observed, but there was no peripheral infiltration or abnormally enlarged lymph nodes. We did excision using CO2 LASER. She was finally diagnosed with mucosa-associated lymphoid tissue lymphoma (MALT lymphoma). After diagnosis, several image studies confirmed that there was no metastatic lesion. She got only radiotherapy after that and kept a complete response state for over 2 years.
목 적: 근침윤성 방광암으로 진단받고 방광 보존적 치료를 시행받은 환자를 대상으로 종양 반응 정도 및 생존율을 분석하고 이에 영향을 주는 예후 인자를 알아보고자 하였다. 대상 및 방법: 1995년 9월부터 2004년 6월까지 삼성서울병원에서 방광암으로 진단 받고 방광 보존적 치료를 받은 환자 39명 중 편평상피암 환자 1명과 선암 환자 1명을 제외한 37명의 환자를 후향적으로 분석하였다. 성별로는 남녀가 각각 33명, 4명이었으며 연령은 $38{\sim}86$세(중앙값 67세)였다. 조직학적으로 모두 이행상피암이었다. 임상적으로 원발 병소 병기는 T2, T3, T4가 각각 19명, 13명, 5명이었다. 경요도방광절제술(TURB) 시 완전 절제와 부분절제가 각각 17명과 19명에서 시행되었고, 조직 검사만 시행된 경우가 1명 있었다. 항암화학방사선병용요법을 받은 환자가 24명, 유도 항암화학요법 후 방사선 치료를 받은 환자가 8명, 방사선 치료만 받은 환자가 6명이었고 보조항암화학요법을 시행받은 환자가 18명이었다. 방사선 치료는 $10{\sim}15\;MV$ 광자선을 사용하였으며 하루에 $1.8{\sim}2.0\;Gy$로 전골반을 대상으로 $45{\sim}46\;Gy$를 4문 조사한 후 방광에만 국한하여 3차원 입체조형치료로 $10.8{\sim}22.0\;Gy$ (중앙값 19.8 Gy) 추가하여 총 $55.8{\sim}67\;Gy$ (중앙값 64.8 Gy)를 조사하였다. 항암화학치료는 Cisplatin을 기본으로 하여 $2{\sim}6$회(중앙값 3회) 시행하였다. 각 변수 간의 상관 관계 분석은 Chi-Square test를 이용하였다. 생존율은 Kaplan-Meier 법으로 산출하였고 두 군 간의 비교는 Log-rank test 이용하였다. 생존율에 영향을 미치는 요인들의 다변량 분석은 Cox 비례위험 모델을 이용하였다. 유의수준 0.05 미만을 의미있는 것으로 판단하였다. 결 과: 치료 반응 평가는 치료 종료 후 3개월 전후에 시행한 요세포 검사, 방광 내시경 소견 및 복부골반 전산화단층촬영을 종합하여 판단하였는데, 완전 관해, 비완전 관해가 각각 16명(43%), 20명(54%)이었다. 대상 환자의 3년 전체 생존율은 54.7%였고 중앙 생존 기간은 54개월($3{\sim}91$개월)이었다. 3년 무진행 생존율은 37.2%였다. 추적 관찰 중 24명(64.9%)이 재발하였는데 국소 재발이 17명(46%), 원격 전이가 6명(16%), 국소 재발과 원격 전이가 동시에 나타난 경우가 1명(3%) 있었다. 국소 재발의 구제 요법으로 근치적 방광절제술이 시행된 경우는 없었고 TURB만 시행하거나 항암화학치료가 병행되기도 하였다. 치료 후의 반응 정도가 전체 생존율(p=0.006)과 무진행 생존율(p=0.001)에 통계적으로 의미있는 인자였고, TURB의 정도가 전체 생존율(p=0.091)과 무진행 생존율(p=0.081), 방사선 조사선량이 무진행 생존율(p=0.07)에서 통계적 의의에 접근하였다. 결 론: 방광 보존적 치료 시 치료 후 반응 정도가 전체 생존율과 무진행 생존율에 있어 중요한 예후 인자임을 알 수 있었다. 또한, TURB를 가능한 충분히 시행하고 병소 부위에 방사선 조사선량을 높이는 것이 방광보존 치료에서 중요하리라 생각된다. 그리고, 방광보존 치료 후 치료 반응 판정에서 완전 관해가 오지 않은 경우는 수술 등의 적극적인 구제 치료가 필요하겠다. 향후 방광 보존을 위한 항암화학 방사선 병용치료 프로토콜에 따른 임상 경험을 더 쌓아야 할 것으로 생각된다.
