Bacterial infection of canine atopic dermatitis is largely caused by Staphylococcus intermedius and may be a superficial or deep pyoderma. The Purpose of this study was to identify the major proteins of S. intermedius cell surface components in humoral immune response of atopic dermatitis dog. Sera samples were obtained from dogs with atopic dermatitis and superficial pyoderma referred to the Veterinary Medical Teaching Hospital, Konkuk University. An isolate of S. intermedius from a clinical case of canine atopic dermatitis was cultured in brain heart infusion broth overnight at $37^{\circ}C$ in aerobic conditions on an orbital shaker. Following culture, Staphylococci were harvested by centrifugation, washed in PBS, and resuspended in PBS containing lysostaphin. The soluble components were separated by centrifugation and were collected. The soluble extract of S. intermedius was separated by sodium dodecylsulphate polyacrylamide gel electrophoresis (SDS-PAGE). The proteins were electrophoretically transferred onto nitrocellulose membrane. Western blotting for the specificity of serum IgG antistaphylococcal antibody was performed with anti-dog-IgG and sera obtained from an atopic dermatitis case and a normal dog. The molecular masses of four major proteins of S. intermedius recognized by serum obtained from an atopic dermatitis case were 18, 31, 75, and 110 kDa as determined by Western blot analysis. The present study indicates that most dogs of S. intermedius infection with atopic dermatitis could have a significant humoral immune response to bacterial proteins of the causative organism.
Purpose: This study was performed to investigate the type and frequency of end colostomy-related complications and to identify the risk factors for those complications. Methods: Retrospective analysis of medical records was made in 708 patients who underwent end colostomy in Samsung Medical Center between October 1994 and February 2005. The type was divided into stomal and peristomal complications: stomal complications included bleeding, necrosis, mucocutaneous separation, prolapsed stoma, retraced stoma, stenosis, and hyperplasia; peristomal complications did peristomal varix, peristomal hernia, irritant contact dermatitis, allergic contact dermatitis, maceration, folliculitis, hyperplasia, bacterial infection, candidal infection, malignancy in the peristomal area, mechanical damage and pyoderma gangrenosum. Results: For stomal complications, hyperplasia was most common(9.0%). For peristomal complications, irritant contact dermatitis was developed in 17.4%. Sex and BMI were risk factors for irritant contact dermatitis, hyperplasia, peristomal hernia, flat stoma, and retracted stoma. Conclusion: Teaching for preventing irritant contact dermatitis such as proper pouching and peristomal skin protection, and for comprehensive weight control should be emphasized on self care program for ostomates, while ostomy care nurse should take a careful consideration of preoperative ostomy site marking in female obese patients.
A 4-year-old female American cocker spaniel was hospitalized with 6 months prolonged dermal problems in The Veterinary Medical Teaching Hospital of Seoul National University. Initial noticed signs by owner of this dog were rashes and papules on ear part and they were spreaded whole body. On initial physical examinations, papules, pustules, crusts, and erythema were presented on whole body. Diameters of focal scaly and erythematous alopetic lesions were varied ranging 1 to 4cm. Skin scraping for detecting ectoparasites and wood's light examinations for dermatophyte were negative. Any bacteria were not detected but Malassezia pachydermatis was detected on cytological examination and fungal culture. The dog was treated with Itraconazole (5mg/kg, q12h) and Cephalexin (20mg/kg, q12h) orally and then bathed with 2% chlorhexidine shampoo twice per week for 5 weeks. From 12 weeks after starting therapy, all dermal problems were disappeared although some mild pyoderma were once relapsed. Regular chlorhexidine bathing treatment so far has been continued for preventing recurrence.
An 11-year-old, female Miniature Schnauzer dog was presented with recurrent skin ulcer of the second metatarsal region in the right hindlimb following metatarsal resection. Physical examination revealed an ulcerated and bleeding lesion of the second metatarsal region in the right hindlimb. Impression smears of the ulcerative lesion confirmed numerous degenerated neutrophils and mixed bacterial infection. Initially, the dog was treated with antibiotics and povidone-iodine flushing for the control of deep pyoderma. Because the skin lesion had been deteriorated over time despite of topical and systemic treatments, skin biopsy was performed. Histopathologic examination indicated squamous cell carcinoma based on the features of multiple nests of squamous neoplastic cells and mitotic figures. Although amputation of the right hindlimb was performed, the dog was expired five months later because of tumor metastasis to the lung and the popliteal lymph node. This is the first case report describes malignant digital squamous cell carcinoma in Korea.
Lactobacillus spp. are the bacteria most commonly used as probiotics and it has been proven that they inhibit pathogenic bacterial growth and improve skin repair in humans. This study was conducted to investigate the growth inhibitory effect of Lactobacillus on Staphylococcus pseudintermedius, the most commonly isolated pathogen in canine pyoderma, and whether Lactobacillus could inhibit the adhesion capability of S. pseudintermedius to canine corneocytes. For this study, L. salivarius and S. pseudintermedius were isolated from healthy beagle fecal samples and the skin surface of dogs with skin infection, respectively. S. pseudintermedius was co-cultured with L. salivarius to assess the inhibitory effect. For the adhesion assay, corneocytes were collected from healthy beagle ventral abdominal skin. Both bacterial species attached to corneocytes and were assessed in number. As a result, L. salivarius significantly inhibited the growth of S. pseudintermedius in the culture medium. Moreover, L. salivarius reduced attachment of S. pseudintermedius in the adhesion assay. These results suggest that L. salivarius has an inhibitory effect on S. pseudintermedius and may be effectively used in the topical therapy of canine skin infections.
This study evaluated the efficacy and safety to determine the therapeutic responses of oclacitinib in canine atopic dermatitis (CAD) and identified factors related to the therapeutic response. Overall, 13 dogs with CAD were treated with oclacitinib for 56 days. Owners and veterinarians assessed visual analog scale (VAS) scores of pruritus and dermatitis. The examined dogs were grouped according to treatment success and failure based on changes in the VAS scores. To identify factors related to the therapeutic responses of oclacitinib, signalments (breed, sex, age, and body weight), mean progression time of CAD, mean Owner and Veterinarian VAS scores at day 0, and preexisting infection rate were compared between the two groups. Among the 13 dogs, 7 (53.8%) met the criteria of treatment success. In the success group, the Owner VAS scores were significantly lower from day 6 to 56 compared with the score at day 0 (P < 0.05). Additionally, the Veterinarian VAS scores were significantly decreased at days 14 and 42 compared with the score at day 0 (P < 0.05). There were no changes in hematological indices after the administration of oclacitinib. The most common abnormality reported was otitis externa (30.8%), followed by pyoderma (23.1%), and vomiting (7.7%). Factors related to responses of oclacitinib were not identified. This study demonstrated that oclacitinib was safe and moderately effective in dogs with CAD. This is the first report of the clinical application of oclacitinib in South Korea.
A 10-year-old castrated male Shih-tzu dog presented with a history of generalized demodicosis, refractory to conventional therapy with ivermectin and amitraz for a year. The patient was also diagnosed with concurrent deep pyoderma, Malassezia dermatitis, and otitis externa. Treatment with amoxicillin-clavulanate, antifungal drugs (itraconazole, miconazole), and milbemycin oxime resulted in a good response for 90 days. Approximately 4 months later, the first relapse of demodicosis occurred and the miticidal therapy was changed to ivermectin. Additional diagnostic tests were performed to investigate an underlying cause for the recurrence of demodicosis, and endocrinopathies and allergic dermatitis were excluded based on the results. Although ivermectin therapy was sustained for 440 days, a second relapse occurred and amitraz baths were added to the therapy. Despite this therapy, the demodicosis persisted, and the miticidal therapy was changed to oral fluralaner, which led to rapid resolution. Demodicosis did not recur again before death approximately 920 days after administration of oral fluralaner. This case report describes the complete resolution of refractory demodicosis using oral fluralaner in a dog.

Background: A recent increase in the occurrence of canine skin and soft tissue infections, including otitis externa and pyoderma, caused by antimicrobial-resistant Staphylococcus pseudintermedius and S. schleiferi has become a significant public and veterinary health issues. Objective: We investigated the virulence potentials associated with the occurrence of canine otitis externa in S. pseudintermedius and S. schleiferi. Methods: In this study, the prevalence of genes encoding leukocidins, exfoliative toxins, and staphylococcal enterotoxins (SEs) was investigated using previously characterized S. pseudintermedius (n = 26) and S. schleiferi (n = 19) isolates derived from canine otitis externa. Susceptibility to cathelicidins (K9CATH and PMAP-36) and hydrogen peroxide (H2O2) was also examined in both staphylococcal species. Results: A high prevalence of genes encoding leukocidins (lukS/F-I, lukS1/F1-S, and lukS2/F2-S), exfoliative toxins (siet, expB, and sset), and SEs was identified in both S. pseudintermedius and S. schleiferi isolates. Notably, S. pseudintermedius isolates possessed higher number of SE genes, especially newer SE genes, than S. schleiferi isolates harboring egc clusters. Although no significant differences in susceptibility to K9CATH and H2O2 were observed between the two isolate groups, S. pseudintermedius isolates exhibited enhanced resistance to PMAP-36 compared to S. schleiferi isolates. Conclusions: These findings suggest that high a prevalence of various toxin genes together with enhanced resistance to cathelicidins may contribute to the pathogenicity of S. pseudintermedius and S. schleiferi in canine cutaneous infections.
Staphylococcus intermedius is a common cause of otitis externa, pyoderma, and wound infections in companion animals. Although S. intermedius infections are rare in humans, it is zoonotic, with several case reports describing fatal human infections. Presently, we sought to isolate S. intermedius strains from various sources at animal hospitals nationwide in Korea, examine their antibiotic susceptibilities, and determine the possibility of horizontal transmission between animals and humans. Pulsed-field gel electrophoresis (pFGE) was used to compare the mecA gene in S. intermedius strains from humans, animals, and the environment in animal hospitals. A total of 119 S. intermedius strains were isolated from 529 samples. Using the disk diffusion method, over 90% of the isolates were found to be susceptible to cephalothin, amoxicillin-clavulanic acid, vancomycin, imipenem, nitroflurantoin, and amikacin, whereas 97.5% and 98.3% of the isolates were resistant to penicillin and ampicillin, respectively. Among the 39 S. intermedius strains harboring mecA, similar PFGE patterns were observed between seven isolates from an animal, two isolates from veterinary staff, and the environment in one animal hospital, and single isolates from an animal and a veterinarian at another hospital. This result suggests the possibility of horizontal transmission of S. intermedius containing mecA between humans, animals, and the environment in animal hospitals and also emphasizes on the importance of S. intermedius with mecA as a possible emerging threat to public health.
A 36 year old blindman, engineer was admitted with chief complaints of hemoptysis, recurrent sore throat, pyoderma in genital organ, uveitis and thrombophlebitis for 10 years. Above the chief complaints were remission or exacerbation during hospitalization. Physicalexamination showed that left radial, ulnar & brachial pulse was not palpable. No bruit or murmur was obtained over the mass. Neurologic examination revealed no significant finding.On admission, chest P-A showed hen egg sized round & oval compact hazy density on left upper lung field. Bronchogram revealed no pathological finding and Lt. tomogram showed well define large,ovoid mass density in the superior mediastinum. Fluoroscopy finding showed nonpulsatile on left upper lung field. Pre-op. aortography was not taken, under the impression of lung Ca. rule out .sortie aneurysm, exploratory operation was performed through the 2nd intercostal space, Lt. It was performed that the mass was ascending sortie aneurysm of saccular type. Direct aneurysmectomy with multiple figure of eight suture were done without any prosthetic graft. Post-op. control I.V.C graphy showed completely obstruction sign. Postopcontrol aortography revealed good surgical result. Final, histopathological answered non-specific sortie aneurysm, saccular type. Post-op. courses were uneventful except mild neurologic disturbance with subclavian steal syndrome and associated with both lower leg pitting edema due to inferior vena cava obstruction. After op, 3 month later, discharged to home, with big systemic problem. Behcet`s syndrome reviewed with related literatures. The coexistence of mouth and genital ulceration with hypopyon mentioned by hippocrates and described by various workers in the early part of this century was first defined as a syndrome by Behcet in 1937. In 1937 Behcet described a chronic relapsing triple symptom complex of oral ulceration, genital ulceration, and ocular inflammation. The place of the syndrome as part of a systemic disorder in now clearer, and the under lying pathology appears to be a vasculitis. The disease runs a- chronic course, blindness being the greatest disability and control nervous system involvement a cause of death. Thrombophlebitis is fairly frequent, france et al [1951] giving an incidence of 25% and Dowling [1961] 12%, superficial thrombophlebitis migrans and thrombosis of large veins, including venae cavae [Thomas, 1947: Boolukos 1960] are recorded. Little attention has been paid to arterial involvement. Mishima et al. [1961] described resection cf an aortic aneurysm in a 38 year old man with Behcet`s syndorme. Mounsey in a clinicopathological conference described a case [Brit, med. J., 1966] of ruptured aortic aneurysm in Bechcet`s syndrome treated by aorto-iliac graft. Also, Shikano and Oshima et al [1963] recorded two aneyrysm of smaller arteries. Unfrequently, aortic aneurysm was presumed to be secondary to osteomyelitis of the lumber spine, though the possible association between aortic aneurysm and Behcet`s syndrome was raised. A further case is reported here, in which ascending aortic aneurysm with Behcet`s Ds. appeared to form part of this generalized disease. This is a case report of surgical experience of Behcet`s Ds. with ascending aortic aneurysm which had nearly all the typical clinical features.
