Purpose: The purpose of this study was to test the reliability and validity of an instrument to measure suffering of family of patients with terminal cancer. Methods: This research was a methodological study processed as follows: 1) The preliminary 32 items were developed based a conceptual framework of suffering for patients with cancer. 2) The content was validated by an expert panel (n=24) and the family of patients with terminal cancers (n=33). 3) The instrument was validated by survey (n=92). Finally, 29 items were selected in developing final version of the scale. Results: Seven factors were extracted through factor analysis: 'economical distress', 'loss of meaning in life', 'feeling of social isolation', 'emotional distress', 'burnout', 'guilty feeling', and 'physical discomfort'. These factors explained 66.94% of the total variance. Cronbach's alpha and Guttman Split-half coefficient of the 29 items were .93 and .92. respectively. Conclusion: This scale identified as a valid and reliable tool with a high degree of reliability and validity. It can be effectively utilized to assess the degree of suffering of family in palliative care settings.
Objectives : This study sought to identify the predisposing factors which influence the adolescent oral health enhancement behavior by analyzing controlling effects depending on the parents' oral health care behaviors in the relationship between the predisposing factors and adolescent behavior improvements which enhance oral health. Methods : A structured, self-administered questionnaire was given from July 6 through July 24, 2016. The collected data were analyzed with the SPSS 18.0 program. Results : Parents' oral health behaviors in terms of discipline and guidance showed a controlling effect in terms of importance (${\beta}$=.116) and benefits (${\beta}$=.133). In addition, the analysis showed that the parents' oral health care behavior had a controlling effect in terms of benefits (${\beta}$=.164) and susceptibility (${\beta}$=-.116). Conclusions : From the results of this study, the development of materials and education courses to lead to behavior changes are thought to be necessary in order to enhance the importance and benefits of the factors of oral health beliefs and to reduce psychological discomfort. Moreover, the role of parents with regard to desirable habits and beliefs to maintain oral health in their children is essential.
The purpose of this research was to understand the dysmenorrhea experience of women. To do this work, we asked 9 women a lot of questions about dysmenorrhea. The interviews were carried out from JUL 1, 1998 through JUL 30, 1998. They were audio-recorded and analyzed using Van kaam's Phenomenological method. Results were as follows. This study examined the effect of ergonomic heel rest that was designed for drivers who have physical handicap in the low leg muscles or have to drive prolonged hours with frequent foot pedaling. An experiment was designed to test the ergonomic heel rest with traditional foot pedal. Forty subjects participated in the experiment. Electromyography(EMG) was used to monitor the muscle activity and fatigue of right leg, and Electro-goniometer was used to measure the ranges of motions of the knee and ankle. A simulator of driver's seat was built for the experiment and the heel rest was installed on it. In order to examine the low muscle activity and range of motion, subjects used the foot pedal for 15 minutes repetitively for each experimental condition. Another 15 minutes test without the heel rest was also performed for comparison. The Root Mean Square(RMS) and Mean Power Frequency(MPF) Shift were used to quantify the level of muscle activity and local muscle fatigue. In results, statistically significant decreases of muscle activity and fatigue were found in all the low leg muscles. The range of motion of the knee and ankle joint also decreased when the heel rest was used. The mechanism of the heel rest effect was discussed in this study. This type of heel rest can be applied to real driving situation after ensuring the safety, or overcoming the psychological discomfort possibly due to unfamiliarity.
With the recent development of sleep medicine, insomnia has been perceived as a disease from a simple symptom. As there are various causes in a disease, proper treatment according to each cause is necessary for a more effective treatment In general, insomnia is classified into five categrories of physical, physiological, psychological, psychiatric and pharmacological aspects. However, such categorizations are often insufficient in treating insomnia. Furthermore understanding of the developmental mechanisms of insomnia is required. The function of sleep is developed and maintained through the balance of the reciprocal forces of sleep and arousal. These forces are contantly regulated by what is called a circadian rhythm. Sleep is induced by this rhythm which is affected by factors such as awakening time in the morning, amount of intellectual function, amount and time length of physical exercise and sunlight Insomnia could develop when this rhythm is delayed and leads to a "forbidden zone" which is a very difficult period for inducing sleep about two to four hours before the routine bedtime. Whereas sleep gradually develops in line with the circardian rhythm, arousal can occur very abruptly by any cause triggered by emotional discomfort or anxiety. Such characteristic and emotional factors as perfectionism, separation anxiety, secondary gain, insecurity, and negative cognition may provoke the inner anxiety and fear for insomnia, which can lead acute insomnia to a chronic one. As chronic insomnia is developed by multiple causes and factors, integrated approaches through analysis of above mentioned factors will be more effective in the treatment of insomnia than a simple administration of hypnotics.
In orthodontic field, although lots of new materials have been developed and many mechanics intro duced, we can face the case patient cooperation still remain a problem to solve. So, factors related to the compliance of 254 adolescent orthodontic patients using intraoral elastic or extraoral orthopedic appliances were under investigation. The study subjects were 11 to 18 years old and from 5 exclusive orthodontitc clinics in seoul. The subjects were asked to fill a questionnaire and compliances were evaluated by their assigned orthodontists. The questionnaire was consisted of 63 questions, and they represent 13 factors-7 psychological & 6 nonpsychological fators. The collected data were analyzed using ANOVA test between the compliance group and the factors. 1. The compliances were evenly distributed in both sex and age groups. 2. As a whole, it was found that the Attitude towards appliances ffactor affected the degree of compliance. 3. Besides that, in younger(11-12) age group, Pain and discomfort associated with treatment factor was also found to be related to the degree of compliance. 4. On the contrary, in older(16-18) age group, the degree of compliance was influenced by the factor of Achievement motivation, Role expectation, parental relationship.
Purpose: This study investigated the quality of life, mental health and subjective health status of community dwelling cancer patients. Methods: Subjects were 212 out-patients with cancer at Kangwon University Hospital. The study was conducted from July 10 to August 14, 2008. Quality of life as determined by the Euro Quality of life-5 Dimensions (EQ-5D) was measured using Korea Centers for Disease Control and Prevention(KCDC). Mental health and subjective health status were measured using KCDC. Results: EQ-5D determined degrees of difficulty were 25.5%(mobility), 25.2%(anxiety or depression), 23.6%(pain or discomfort), 20.3%(activities of daily living), and 13.7%(self care). An analysis of the association between several factors and mental health showed that the statistically significant factors were age, gender, medical coverage, and private insurance. An analysis of the association between several factors and subjective health status showed that the statistically significant factor was occupation. Conclusion: Cancer patients suffer from significant psychological distress. Programs that address mental and physical health would be beneficial.
UL-syndrome(鬱證) is a syndrome of emotional depression and various physical symptoms generally due to chronic psychological stress, without constitutional abnormality. These are cases of female patients suffering from headache, chest discomfort, heart burning, insomnia, dyspepsia, anorexia, fatigue and some other minor symptoms due to chronic stress. We diagnosed these cases as UL-syndrome and treated them with oriental medicine, specifically Gamisoyosan(加味逍遙散). This paper describes the symptoms that the patients suffered, the treatment used, and identifies stages in the development of UL-syndrome, and suggests a stage in which Gamisoyosan is efficacious.
Psychogenic vomiting (七情吐) is the disease of vomiting due to seven emotions that joy, anger, melancholy, anxiety, grief, fear and terror being the response of the mind to the environmental stimuli. We experienced a 21year-old man who had a psychogenic vomiting due to maladjustment in a military life, and whose condition was improved through oriental medical treatment. We treated the patient with Herbal medications and Giungoroen (至言高論)-wise saying and lofty opinion). Giungoroen is psychological therapy that promotes patient's recognition of disease and will to cure it through conversation. After being treated, the patient showed that symptoms (vomiting, nausea, abdominal discomfort, insomnia, a depressed mood, a feeling of uneasiness) was improved considerably. Tills result suggests that oriental medical treatment bas good effect on psychogenic vomiting due to adjustment disorder.
Purpose: Male hypertrophic nipples can lead to psychological distress and physical discomfort. The authors present a new technique of male nipple reduction and describe its advantages Methods: The neonipple is designed to reduce diameter and height of nipple while preserving the subdermal plexus. After the central wedge excision, additional four triangular section of nipple skin is excised. One of the remaining two flaps is amputated partially and both flaps were approximated using 4 - 0 PDS and 6 - 0 Nylon sutures. Results: Between December of 2007 and January of 2009, 52 nipple reductions were performed in 30 male patients(mean = 29.5 years). Postoperative recovery was rapid and few complications were encountered. The mean diameter of the hypertrophic nipple was $9.1{\pm}2.5mm$(range, 7 to 15 mm). The mean diameter of the neonipple was $5.0{\pm}0.7mm$(range, 4 to 6 mm), with an average reduction of $3.8{\pm}0.6mm$(range, 2 to 11 mm). At follow-up, the neonipple had a natural appearance, with less projection and an inconspicuous scar. Conclusion: The wedge and triangular skin excision and partial amputation are easy to perform and yields consistent results. This technique decreases both the diameter and height of any size nipple and can be modified to meet patient preferences.
