FACTORS AFFECTING CHILDREN'S DENTAL UTILIZATION: AN APPLICATION OF THE ANDERSEN MODEL (앤더슨 뉴만모형을 이용한 아동의 치과의료이용행태에 영향을 미치는 요인에 관한 연구)

  • The purpose of this study is to provide framework for understanding children's dental utilization. In this paper Andersen-Newman's model is applied to the use of dental visits. This model consists of predisposing, enabling, and need components that describe a person's decision to use dental health services. One thousand, nine hundred seven children and their mothers were selected for the study. The children were fourth grade to sixth grade in elementary schools in Iksan city, Korea. Models are operationalized using stepwise multiple regression analysis and path analysis. The number of independent variables used in the analysis was 39 in total, ie 32 predisposing components, 6 enabling components, and 1 need component. Children's Dental utilization was measured based on the number of visits. The data collected by means of a questionnaire survey. In this study, the amount of variance by the model was 25 percent. Predisposing factors had the greatest effect on utilization. Number of restricted activity days caused by oral disease, having a regular dental care, and susceptibility on oral disease of children were found to have significant major effects on dental utilization of children. Mother's dental visits was most important factor affecting dental utilization of children.

Conditions and Performance of Quality Improvement Activity in Korean Dental Hospitals (치과의료 질 향상활동 현황과 개선활동 만족도)

  • This study aimed to evaluate conditions and performance of quality improvement (QI) activity in Korean dental hospitals. Twenty one representative dental hospitals in Korea were selected by the selection criteria. A questionnaire was implemented to measure awareness, perceived needs, educations, conferences, works, activating/obstructive factors and satisfaction related to QI activities. Satisfaction score range from 0 (lowest) to 5 (highest) among 329 dental hygienists who worked in the selected 21 dental hospitals. Implementation rate of programs on external customer satisfaction was the highest 59.9% in QI activities, and reduction of expenses (59.4%), Improvement in health care quality (58.7), risk management (52.8) were followed, while improvement in work efficiency (46.5) showed the lowest implementation rate. The most influencing factor accelerating QI activities was 'active participation of hospital workers' (54.7%), and 'effort to improve customer satisfaction' (44.7%) followed. The most influencing QI related obstructive factor was 'lack of QI-related manpower and support' (47.1%). A balanced development including QI programs with lower implementation rates is necessary. Encouraging participation of workers may be the most important in developing dental QI activities further.

Trend in Preference for Dental Hygienist Image among Health Care Workers (일부 병원 종사자들의 치과위생사에 대한 이미지 선호 경향)

  • The purpose of this study was to examine preference for dental hygienists. The subjects in this study were 228 employees in general hospitals located in the region of Busan, on whom a survey was conducted in May and June, 2009. The findings of the study were as follows: As for top priority of dental hygienists' duties, providing assistance for dentist treatment ranked first(28.2%), followed by scaling(23.2%) and preventive duties(20.6%). Concerning a favorable image of a dental hygienist, a cheerful look was most preferred(61.4%), followed by a clean-cut look (32.9%), a refined look (3.5%) and an intellectual look (2.2%). Among the subjects, married respondents exhibited a greater liking for a cheerful look, and the gap between them and the unmarried subjects was statistically significant($x^2=8.11$, p<.05). In terms of uniform preference, a two-piece suit with pants was most favored(76.3%). Female respondents showed a greater preference for a two-piece suit with pants, and the gap between them and the male respondents was statistically significant($x^2=41.09$, p<.001). As for major qualifications, professional knowledge was most valued (49.1%), followed by integrity/responsibility(23.2%), friendliness(22.4%), and a great personality/being well-cultivated(5.3%). Employees who were in their 40s and up placed more importance on integrity and responsibility, and the gap between them and the others was statistically significant($x^2=20.62$, p<.01).

Essential services in children's family dentistry program and the role of dental hygienists (아동 치과주치의 프로그램의 필요도와 치과위생사의 역할)

  • Objectives: The aim of this study was to investigate the essential services, importance, interval of examinations, and role of dental hygienists in children's family dentistry program. Methods: A total of 124 participants completed a self-administered questionnaire. The data were analyzed using independent t-test, and ANOVA, and Pearson correlation analysis. Results: All participants stated that oral examination and panoramic radiography are required; children should be educated about brushing, use of oral products, and regular check-ups; and preventive treatments such as molar sealants and prophylaxis should be offered. They stated that light-curing resins and glass ionomer fillings should be offered in treatment services. They stated that examination intervals should be shorter for education and prevention rather than treatment. Dental hygiene students were more likely than dentists and dental hygienists to say that the program was more important. There was a correlation between oral examinations and education and treatment, and between essential services and their importance. Conclusions: Services considered essential and important should be provided first, education and prevention should be provided more frequently than treatment, and their importance should be emphasized not only to dental hygiene students but also to dental hygienists and dentists who are the main providers of services.

Cognition of registered nurse on emergency treatment for oral and maxillofacial injury in Jeju province (제주 지역 간호사의 구강 악안면 영역 손상에 대한 응급 처치 인식도)

  • The purposes of this work were to assess the cognition of the registered nurse(RN) on oral and maxillofacial emergency treatment and to compare cognition of the RN with that of the 119 emergency medical technician(EMT). 450 RNs who were working at each of secondary hospitals in Jeju province had responded to the questionnaire. Independent sample t-test and chi-square test were used to assess the state of RN on dental emergency treatment and to compare RN with EMT. The question 'education time on dentistry in formal education' that marked '0 hour' and '1-3 hours' were 73.3% and 20.0%, respectively. The question 'refresher training class on dentistry' that marked '0 hour' and '1-3 hours' were 92.9% and 6.7%, aggregately 99.6%. The results showed low score in the question 'reduction of temporo-mandibular joint(TMJ)'($1.67{\pm}0.857$), 'fixation of dislocated TMJ'($1.70{\pm}0.853$) and 'post-avulsed tooth treatment'($1.78{\pm}0.774$) by 5-point Likert scale. Likewise, the scores were $2.02{\pm}0.806$ in the question 'treatment of maxillofacial trauma', $2.76{\pm}1.061$ in the question 'emergent care of avulsed tooth', $2.70{\pm}1.095$ in the question 'treatment time of avulsed tooth' and $2.79{\pm}1.056$ in the question 'mouth guard', respectively. Compared to EMT, results of RN showed a statistically lower figure(p<0.05) in all items compared except the question 'medicine control', and the question 'doctor care in emergency room' was borderline(p=0.069). From this study, it is necessary for RN and student of nursing science to be educated on the oral and maxillofacial emergency treatment for the initial management of injuries. Authors suggest further co-study and nation-wide research with nursing care.

A Study on Factors Associated with the Oral Health Promotion Behaviors of College Students (대학생들의 구강건강증진행위와 관련된 요인 연구)

  • The purpose of this study was to delve into the factors associated with the oral health promotion behaviors of college students. The subjects in this study were 453 students who attended three different colleges in Gyeonggi province. After a survey was conducted, the collected data were analyzed with SPSSWIN 12.0 program. The findings of the study were as follows: 1. Concerning the basic oral health care of college students, 54.1 percent had ever visited dental clinics over the last year, and 53.4 percent hadn't received any preventive treatment during the same period of time. 2. As to their self-perception of oral health status, 80.1 percent didn't find themselves to be in good oral health. 3. Regarding the influence of their general characteristics, their self-efficacy was statistically significantly different according to their gender (p = .022). Their control of oral health varied statistically significantly with their major(p = .000), and whether or not they smoked (p = .004) made a statistically significant difference to that as well. Their oral health knowledge differed statistically significantly with their gender (p = .000), major(p = .000) and age group(p = .000), and whether or not they smoked(p = .006) made a statistically significant difference to that as well. 4. As for correlation among the variables, better oral health promotion behaviors were found among those whose self-efficacy was higher, whose control of oral health was better and who had more knowledge on oral health. 5. Preventive treatment, control of oral health and self-efficacy were identified as the variable that affected their oral health promotion behaviors.

Association of Poly (ADP-Ribose) Polymerase 1 Variants with Oral Squamous Cell Carcinoma Susceptibility in a South Indian Population

  • Background: Oral cancers account for approximately 2% of all cancers diagnosed each year; however, the vast majority (80%) of the affected individuals are smokers whose risk of developing a lesion is five to nine times greater than that of non-smokers. Tobacco smoke contains numerous carcinogens that cause DNA damage, including oxidative lesions that are removed effectively by the base-excision repair (BER) pathway, in which poly (ADP-ribose) polymerase 1 (PARP-1), plays key roles. Genetic variations in the genes encoding DNA repair enzymes may alter their functions. Several studies reported mixed effects on the association between PARP-1 variants and the risk of cancer development. Till now no reported studies have investigated the association between PARP-1 variants and oral squamous cell carcinoma (OSCC) risk in an Indian population. Materials and Methods: In the present case control study 100 OSCC patients and 100 matched controls were genotyped using PARP1 single nucleotide peptides (SNP's) rs1136410 and rs3219090 using TaqMan assays. Results: The results indicated significantly higher risk with PARP1 rs1136410 minor allele "C" (OR=1.909; p=0.02942; CI, 1.060-3.439). SNP rs1136410 also showed significantly increased risk in patients with smoking habit at C/C genotype and at minor allele C. Conclusions: The PAPR-1 Ala762Val polymorphism may play a role in progression of OSCC. Larger studies with a greater number of samples are needed to verify these findings.

Effect of Gap between Expected Job and Performed Job on Turnover Intention and Job Satisfaction in Clinical Dental Hygienists (치과위생사의 기대업무와 수행업무 격차가 이직의도와 직무만족도에 미치는 영향)

  • The purpose of this study was to investigate effect of gap between expected job and performed job on job satisfaction and turnover intention in clinical dental hygienists. A total 292 hygienist in dental hospitals and dental offices in nationwide completed a questionnaire on their daily routine job and job which they expected and measured gap between expected job and performed job, such as chairside assisting, preventive care, consultation, reception. The study showed the gap on chairside assisting job among age, career and showed the gap on consultation among age, education, career and showed the gap on reception among age, career. gap on chairside assisting, salary satisfaction, personal day affected job satisfaction and gap on chairside assisting, children, salary satisfaction affected turnover intention. The study showed that salary satisfaction and gap on chairside assisting associated with turnover intention and job satisfaction. Personal day affected job satisfaction but did not affect turnover intention.

The relationship between the numbers of natural teeth and nutritional status of elderly in Korea -based on 2007~2009 national health and nutrition survey data- (한국 노인의 자연치아 수와 영양소 섭취상태와의 관련성 -2007~2009년 국민건강영양조사 자료에 근거하여-)

  • Objectives : The aim of this study was to evaluate the relation between the state of dental health(number of natural teeth) and nutritional status of Korean elderly using Korean Dietary Reference Intakes, which was an objective standard for nutritional intake based on database of Korea National Health and Nutrition Examination Survey, large scale of sample obtained by the government. Methods : Complex sampling procedure was used to analyze the fourth data(2007-2009) of Korea National Health and Nutrition Examination Survey. When preparing planning file, the estimator of variance as a stratification variance (variance name : kstrata), population of survey unit as a cluster variance(variance name : PSU), and previous exam and nutritional related weighted as a weighted were analyzed by considering reproduced survey and nutritional related total weighted. Complex samples chi-square test was used to estimate the relation between number of natural teeth and inadequate intake and relation factors included in the model were analyzed by complex samples logistic regression analysis. Results : The group of edentulous had a higher risk to intake less nutrient, except calcium, riboflavin and vitamin C than recommended level comparing to reference group which had natural teeth more than 20(phosphate : OR=1.763; 95% CI=1.273-2.443, thiamine : OR=1.748; 95% CI=1.276-2.395, protein : OR=1.610; 95% CI=1.213-2.138). Conclusions : The number of teeth in Korean elderly over 65 years old had a relation with nutritional status in this investigation. Especially, intake level of nutrients was different between the edentulous group and the reference group. Therefore, dental health care is needed from young and middle age to keep health dental condition for through whole life as well as old age. Although the dental condition of the aged is not good, it is evitable to educate them about the relation between dental health and nutritional ingestion to take balanced nutrition, we think.

Trends of Dental Caries Prevalence in Children Under 14-Year-Old Using a Health Insurance Database (건강보험 데이터를 이용한 14세 이하 소아청소년의 치아 우식 유병률 경향성)

  • The purpose of this study is to analyze trends in the prevalence of dental caries and demand for dental caries treatment among children under 14 years old using Health Insurance Review and Assessment data. The analysis was conducted using treatment records from a random sample of approximately 1 million pediatric patients from a population that included all children and adolescents for each year from 2011 to 2020. In this study, the number of children diagnosed with K02 dental caries and the number of children receiving dental caries treatment across all ages have increased. However, the number of children aged 10 to 14 who received pulp treatment or extraction has decreased. In the National Survey of Children's Oral Health, the decay-missing-filled teeth index for 5- and 12-year-olds has stagnated or increased slightly, but the percentage of the population with active dental caries has decreased. Accessibility and local environments for dental caries treatment have generally improved compared to the past, but preventive dental care has stagnated over the past decade. Therefore, it is necessary to evaluate the effectiveness of oral health programs implemented in Korea to promote and prevent dental caries among children.
