Comparison of Herbs in Prescription Composition of Consumptive Disease and Internal Injury in Donguibogam Through Network Analysis (네트워크 분석을 통한 동의보감(東醫寶鑑) 내상(內傷)문과 허로(虛勞)문의 처방 구성 본초 비교)

  • Objective: Internal injuries and consumptive disease have different causes, yet they can affect each other. The relationship and combination of prescription drugs in the clinical practice of internal injuries and consumptive disease were analyzed for various diseases of "Donguibogam" through network analysis. Methods: The prescriptions used in consumptive disease and internal injury were established by conducting a full survey on the papers extracted from Donguibogam. The R version 4.0.3 (2020-10-10) and the igraph and arules package were used to perform network analysis and association rule relationship mining analysis in the first and second prescription compositions. Results: The herb frequently used for internal injury was Glycyrrhizae Radix, while the herb combination frequently used was Citri Pericarpium-Glycyrrhizae Radix. For centrality, the main factor was generally Glycyrrhizae Radix. In the case of consumptive disease, the herb most frequently used was Angelicae Gigantis Radix, and the combination most frequently used was Rehmanniae Radix Preparata-Angelicae Gigantis Radix. In terms of centrality, it was Angelicae Gigantis Radix. As a result of the network analysis of herbal prescription frequency, each group was divided into three. Conclusion: The interrelationship between internal injury and consumptive disease prescription drugs may reveal the differences and similarities between internal injury and consumptive disease and may serve as a basis for the development of new drugs or materials that can enhance mutual effectiveness in the treatment of internal injury and consumptive diseases.

  • Objectives: This analysis of prescription used for low back pain in the Yomun(腰門) chapter of ${\ll}$Donguibogam(東醫寶鑑); The Precious Mirror of Oriental Medicine${\gg}$ is designed to be helpful to practical use of clinics. Methods : Proscriptions used for low back pain in the Yomun(腰門) chapter of ${\ll}$Donguibogam(東醫寶鑑)${\gg}$ were classified and analyzed according to the frequency of proscriptions and the characteristics of each herbs in proscriptions(efficacy, used frequency, related organs etc.) Results and conclusions : After analysis, we obtained the following results : 1. The causes of low back pain are mainly eohyeol(瘀血), yangheo(陽虛), punghanseub(風寒濕). In care of low back pain, I suppose more efficiency that if Angelica gigas NAKAI(當歸) Cnidium officinale MAKINO(三芎) Prunus persica BATSCH(桃仁) is added when the cause is eohyeol(瘀血), or if Psoralea corylifolia L.(破古紙), Cinnamomum cassia PRESL(肉桂), Foeniculum vulare MILL(茴香), Eucommia ulmoides OLIV.(杜冲), Citrus unshiu MARKOVICH(陳皮) are added when the cause is yangheo(陽虛), or if Phellodendron amurense RUPR.(黃柏), Notopterygium incisum TING(羌活), Atractylodes Japonica KOIDZ.(蒼朮) are added when the cause is punghanseub(風寒濕).

  • The result were achived from 29 kinds of the medical literature of many generation is like this. 1. Oriental medical expressions of Xerophthalmia are "Baksab(白澁)", "Kunsabhonhwa(乾澁昏花)", "Sinsoojanggo(神水將枯)", "Donginkunkyul(瞳人乾缺)", "Taljung(奪精)", etc. "Baksab(白澁)" is very close to Xerophthalmia. 2. The cause of Xerophthalmia is Wind, Wind-heat Pathogen, Damp-heat of spleen and lung, DefIciency of body fluid, Yin-deficiency of liver and kidney, Liver deficiency syndrome, Deficiency of blood, Fire, Fire of deficiency type, Liver heat, etc. 3. In the frequency of prescription used Xerophthalmia are "Sangbakpi-Tang(桑白皮湯)", "Eunkyosan(銀翹散)", etc as Excess type, "Kikookjihwang-Tang(杞菊地黃湯)", "Samooloja hwan(四物五子丸)", etc as Deficiency type. 4. In the frequency of medical herbs of Xerophthalmia use much Rehmannia root nourishing Yin and clearing away heat and Divaricate Saposhnikovia root(expelling pathogenic wind.

  • After this study: I report the following result from it. 1. Beriberi is defined as sensory or motor disorder of leg with a painful-swelling and pyrexia. If it progress severely, the disease transmits to heart and cause numbness of the lower abdomen, vomiting, anorexia, palpitation, chest distress and confusion. 2. The external etiologic factors of Beriberi are wind-toxic pathogen, dampness, improper diet and climate sickness, the internal etiologic factors are deficiencies of the kidney, both Gi and blood and original vital energy. 3. The treatment of Beriberi is free-going of blockage, because it is a blocked disease. 4. Danggwiy-juntongtang and Ganghwar-dochetang is the most in the order of frequency of use. Mahwang-juagyongtang, Banha-juagyongtang, Daehwang-juagyongtang, Shinbi-juagyongtang are used in sequence. 5. The prescriptions of Beriberi are made up of the herbs that clear heat, for example, Ganghwar, Changchul, Mahwang and Bangpung, and the herbs that reinforces Gi, for example, Insam and Bakchul.

  • As social interest in acne treatment has grown, various external preparations have been developed and studied. So, we investigated the external prescription which can treat acne in medical classics, then reviewed and divided it by dynasty. The data for analysis of Herbal formula was taken from 'medicine Dictionary of traditional Chinese medicine prescriptions(中醫方劑大辭典)'. 31 external medicines were searched, 3 were before song(宋) dynasty, 11 were Song(宋) dynasty, 11 were Yuan(元) dynasty, 7 were Ming(明) dynasty and 7 were Qing(淸) dynasty. The recipe and usage were only making them pill(丸) and paste(膏) using grinded medicinal herbs, then rubbing to face. As the age developed, unique manufacturing process(i.e fermentation method), and usage(i.e time-based usage, herbal medicine extracts for wash) were proposed. The external application with oriental medicine for acne used with Angelicae Gahuricae Radix(n=12), Bletillae Rhizoma(n=8), Syzygii Flos, Saposhnikoviae Radix(n=7) and so on. In particular, Bletillae Rhizoma was searched with high frequency in this study, but it was not included or studied in patent composition. The results of this study will provide basic data for future experiments and clinical studies.

A Study on the Frequencies of Medicinal Herb Mombinations in the Prescriptions of "Wenbingtiaobian(溫病條辨)" ("온병조변(溫病條辨)" 수록(收錄) 처방(處方) 내(內)의 약물(藥物) 조합 빈도(頻度) 연구)

  • Through analyzing the frequencies of medicinal herb combinations in the prescriptions of "Wenbingtiaobian(溫病條辨)", we could understand the characteristics of prescription composition and author's medical thoughts. So we conducted this study that analyze combination of 258 medicinal herbs in 206 prescriptions, and found meaningful combinations of 700. Among these, the most meaningful combinations are the two of 8 herbs. One consists of Glycyrrhiza uralensis Fischer, Platycodon grandiflorum, Lonicera japonica Thunberg, Sojae Semen Praeparatum, Mentha piperascens (Malinv.) Holmes, Forsythia suspensa, Phyllostachys nigra var henonis, Schizonepeta tenuifolia var. japonic, the other consists of Glycyrrhiza uralensis Fischer, Platycodon grandiflorum, Lonicera japonica Thunberg, Mentha piperascens (Malinv.) Holmes, Forsythia suspensa, Arctium lappa, Phyllostachys nigra var henonis, Schizonepeta tenuifolia var. japonica. The former is Yeonkyo-san without Arctium lapp, the letter is Yeonkyo-san without Sojae Semen Praeparatum. It means these combinations are kind of modified Yeonkyo-san which is effective in outthrustting through the exterior with pungent-cool and clearing heat and detoxify. In conclusion, prescriptions of "Wenbingtiaobian(溫病條辨)" involve herbs mostly used to outthrust through the exterior with pungent-cool and clear heat and detoxify. Through this study, we know the author' s one of medical view points on outthrustting through the exterior with pungent-cool and clearing heat and detoxify.

A Study of the Case Record on Dyspnea and Wheezing Asthma Recorded in Xu Ming Yi Lei An ((${\ll}$속명의류안(續名醫類案)${\gg}$에 기재(記載)된 천(喘) 및 효천(哮喘)에 관(關)한 의안(醫案) 연구(硏究))

  • Objectives : Select and analyze the case record of dyspnea and wheezing asthma recorded in Xu Ming Yi Lei An that is the most abundant and wide in contents in existing case records that are systematic, comprehending relatively modern Traditional Chinese Medicine to secure more deep and objective basis of Traditional Chinese Medicine approach for dyspnea and wheezing asthma to analyze and review possibility for clinical application in this study. Methods: The study was conducted with the case records of dyspnea and wheezing asthma in whole Xu Ming Yi Lei An. Pattern identify and classify selected case records and again classified with deficiency syndrome and excess syndrome. Also analyzed prescriptions and herbs used in the case records. Nature of herbs and properties and flavors that were used in the case records were classified and frequency of each nature of herbs were analyzed. Applicable case records were interpreted and suggested prescriptions, pulse feelings, pattern classification were analyzed and described. Results : Among the 5254 case records stated on the complete collection, it is researched that there are 63 case records for the symptom complex of dyspnea as 1.2% of the whole case records, and the case records on the symptom complex of wheezing asthma are 14 as the 0.27% of the total examples. 63 case record examples related with symptom complex of dyspnea were pattern identified and classified. As a result, deficiency syndrome of the Kidney(33 %), deficiency syndrome of the Spleen(26.0%), Wind-Cold(12.3%), phlegm turbidity(12.3%), Heat in the Lung(8.2%), asthenia of the Lung(8.2%) were investigated as above order. 14 case record examples related with wheezing asthma were pattern identified and classified. As a result, phlegm-Heat(26.3%), upper excess and lower deficiency(26.3%), external affections Wind-Cold(15.8%), Dampness-phlegm(10.5%), Lung asthenia(10.5%), Cold phlegm(5.3%), mutual deficiency and detriment of Heart and Kidneys(5.3%) were investigated as above order. Symptom complex of dyspnea has 67.1% of deficiency syndrome, 32.9% of excess syndrome resulting more deficiency syndrome than excess syndrome. Symptom complex of wheezing asthma has 42.1 % of deficiency syndrome and 57.9% of excess syndrome resulting more excess syndrome than deficiency syndrome. In case of symptom complex of dyspnea prescription used in the case record, the order of frequency is as following. Palmijihwang-tang, Bojung-ikgitang, Yungmijihwang-tang, Ijintang, Sojaganggitang, Igongsan. In case of symptom complex of wheezing asthma prescription in the case record, Yungmijihwang-tang, Ohotang, Dodamtang were mostly used. Herbs used in case records of symptom complex of dyspnea are Ginseng Radix, Poria, Glycyrrhizae Radix, Aconiti Iateralis Preparata Radix, Atractylodis Macrocephalae Rhizoma, Dioscoreae Rhizoma, Angelicae Gigantis Radix, Rehmanniae Radix Preparat, Pinelliae Rhizoma, Zingiberis Rhizoma Recens are mostly used. Nature of herb properties used for symptom complex of dyspnea and symptom complex of wheezing asthma are herbs that are warm properties. When the symptom complex of dyspnea and the symptom complex of wheezing asthma were treated. if the patient felt tenderness at Pyesu, doctors conducted pricking blood around the opposite Pyesu or Sipseon acupoint. when the patient didn't have tenderness at Pyesu by soft press, pricking blood was performed both sidees, right and left Pyesu. In case of the treatment of symptom complex of dyspnea and symptom complex of wheezing asthma, when they got treatment, when the symptom complex of disease is severe, a doctor cauterized the opposite Pyesu while the other Pyesu felt tenderness, and decided how the above treatment is performed whether the degree of the symptom compolex of disease is severe or not. In case of the treatment of symptom complex of dyspnea and symptom complex of wheezing asthma, if the person felt tenderness at Pyesu and is caught by the Wind-Cold pathogen, slight acupuncture is treated at relevant Pyesu with Fire needling. When patient with symptom complex of dyspnea and symptom complex of wheezing asthma cannot hawk sputum up from the oral and laryngopharynx, suction method is treated. Conclusion : With this study, actual traditional and clinical pattern identification form and characteristics of symptom complex of dyspnea and symptom complex of wheezing asthma were recognized. Modern case report utilizing in clinical application need to be secured and an incurable disease asthma need to be diagnosed and improvement for treatments have to be searched through other case records.

Analysis of 『Jinguiyaolue』 Prescriptions using Database (데이터베이스를 이용한 『금궤요략』 처방(處方) 분석 연구)

  • Objectives : The aim of this paper is to study the methodology for effectively analyzing the "Jinguiyaolue" prescriptions using database, and to explore possibilities of applying the data construction and query produced in the process to comparative research with other texts in the future. Methods : Using "Xinbianzhongjingquanshu(新編仲景全書)" as original script, the contents of "Jinguiyaolue" were entered into the database, in which one verse would be separated according to content for individual usage. Also, data with medicinal construction and disease pattern information of the previously constructed "Shanghanlun" database designed for comparison with other texts was applied for comparative analysis. Results : For input and analysis, 6 tables and 12 queries were made and used. Formulas were accessible by using herbal combinations, and applications of these formulas could be assembled for comparison. Formulas were also accessible by using disease pattern combinations, and combinations of herbs and disease pattern together were also applicable. In comparison with other texts, examples and frequency of usage of herbs could be relatively accurately compared, while disease patterns could not easily be compared. Conclusions : Herbal combinations, disease pattern combinations could yield related texts and herb/disease pattern combinations of the prescriptions in the "Jinguiyaolue", which shortened time needed for research among formulas in texts. However, standardization research for disease pattern is necessary for a more accurate comparative study that includes disease pattern information.

Study on Application of the Morus Cortex Mainly Used in Herbal Prescription (On the Focus of Prescriptions including Morus Cortex in Donguibogam) (상백피(桑白皮)가 주약(主藥)으로 배오(配伍)된 방제(方劑)의 활용(活用)에 대한 고찰(考察) (동의보감 상백피 가미방 중심으로))

  • This study was investigated to make sure the range of the Morus Cortex's treatment, the nature of disease, the chief virtue of medicine, the pathology and the usage quantity of it in Donguibogam The results were as follows; 1. The frequency of being used the Morus Cortex in the consumption or cough-relative chapter is as high as a ratio of 40.6%. 2. The Morus Cortex is used in a realm of the consumption or cough-relative disease, an otorhinolalyngology, an edema, a skin-relative disease, a bleeding trouble, a thirsty and diabetes melitus, etc. 3. The Morus Cortex is used in a realm of the pathology of the pulmonary heat, the pulmonary noxious vigor, the damage from external winds and colds, fatigue, etc. 4. The Morus Cortex is used in a range of $1g{\sim}12g$ in herbal-prescription. The main using dosage is 4g(about 42%). 5. The Morus Cortex is used with various crude herbs in accordance with the pathogeny. The Morus Cortex has been used to reduce heat from the lungs and soothe asthma, to promote urination and reduce edema from shinnongboncho era. According to the results, I suggest that we use the Morus Cortex in a various pathogenic fields. The Morus Cortex is able to remove not only pathogenic heat from the consumption or cough-relative disease, but also pathogenic otorhinolalyngology, an edema, a skin-relative disease, a bleeding trouble, a thirsty and diabetes melitus, etc.

A Study on the Applications of Ostericum Radix Mainly Blended Prescription in Dongeuibogam (東醫寶鑑 中 羌活이 主藥으로 配伍된 方劑의 活用에 대한 考察)

  • This report describes 90 prescriptions related to the use of Ostericum Radix(OR) blended as the main ingredient in the Dongeuibogam.

    The following conclusions were reached through investigations on the prescriptions that use OR as a key component.

    Prescriptions where OR was used as one of the monarch(drug) utilized for 27 therapeutic purposes in total.

    It was searched that the frequency of prescriptions in the therapeutic purposes were as follows. The chapter of eyes is 22.2%, the wind is 13.4%, the child 11.1%, the cold is 8.9%, the head is 6.7%, the malaria disease 4.5% in the order.

    Prescriptions that utilized OR as the main ingredient were most used for treating diplopia, facial nerve palsy, headache and a fever in the whole body. These were also used for treating 55 different types of diseases.

    The dosage of OR in formulas was ranging from 2pun(about 0.75g) to 2don(about 7.5g). However, 1don(about 3.75g) was most used for clinical application.

    We can find out that according to herbs or prescriptions blended with itself, OR made a variety of functions to eliminate cold with wind, humidity, pain, and convulsions from the body. And also, Bangpungtang was the most useful base prescription which used OR as the main component.
