Reattachment of Fractured Teeth : Case Report

  • Maxillary anterior crown fractures are a common form of injury that mainly affects children and adolescents. The position of maxillary incisors and their eruptive pattern carries a significant risk for trauma. In the pre-adhesive era, fractured teeth needed to be restored either with pin-retained inlays or cast restorations that sacrificed healthy tooth structure and were a challenge for dentists to match with adjacent teeth. But. recently the development of adhesive dentistry has allowed dentists to use the patient's own fragment to restore the fractured tooth. The purpose of this paper is to present 2 cases of reattachment of fractured teeth which had different fracture mode, and to evaluate prognosis of cases.

Study on the Design Program for Non-standard Plastic Gear (플라스틱 기어용 비표준 기어 설계프로그램의 개발)

  • Currently, plastic gear is widely used as parts of office equipment and industrial machines, because plastic substance has an advantage of light weight and possible to operate in oil-fewer conditions. However, under cyclic loadings, their occurred repetitive deformation due to weak tensile strength and bending stress rather than metal gear. Furthermore, they have a problem of attrition and breakage owing to frictional heat. For solving these problems, when plastic gear's opponents are metal gear, we should design that plastic gear's tooth be thick and metal gear's tooth be thin. In this research, we developed the program which developing tooth profile of non-standard gears automatically and calculating over-pin diameter for inspection after making gear.

Dislodgement resistance of modified resin-bonded fixed partial dentures utilizing tooth undercuts: an in vitro study

  • STATEMENT OF PROBLEM. Over the years, resin-bonded fixed partial dentures (RBFPDs) have gone through substantial development and refinement. Several studies examined the biomechanics of tooth preparation and framework design in relation to the success rate of RBFPDs and considered retention and resistance form essential for increase of clinical retention. However, these criteria required preparations to be more invasive, which violates not only the original intentions of the RBFPD, but may also have an adverse effect on retention due to loss of enamel, an important factor in bonding. PURPOSE. The object of this in vitro study was to compare the dislodgement resistance of the new types of RBFPDs, the conventional three-unit fixed partial denture, and conventional design of RBFPD (Maryland bridge). MATERIAL AND METHODS. Fifty resin mandibular left second premolars and second molars were prepared on dentiforms, according to the RBFPD design. After model fabrication (five group, n = 10), prostheses were fabricated and cemented with zinc phosphate cement. After cementation, the specimens were subjected to tensile loading at a cross head speed of 4 mm/min in a universal testing machine. The separation load was recorded and analyzed statistically using one-way analysis of variance followed by Duncan's multiple range test. RESULTS. Group V, the pin-retained RBFPDs, had the highest mean dislodgement resistance, whereas specimens of group II, the conventional RBFPDs, exhibited a significantly lower mean dislodgement resistance compared to the other 4 groups (P <.05). There were no significant differences between group I, III, and IV in terms of dislodgement resistance (P>.05). Group V had the highest mean MPa (N/$mm^2$) (P <.05). There was no significant difference between groups I, II, III and IV (P > .05). CONCLUSION. Within the limits of the design of this in vitro study, it was concluded that: 1. The modified RBFPDs which utilizes the original tooth undercuts and requires no tooth preparation, compared with the conventional design of RBFPDs, has significantly high dislodgement resistance (P < .05). 2. The modified RBFPDs which utilizes the original tooth undercuts and requires minimal tooth preparation, compared with the conventional FPDs, has significantly no difference in retention and dislodgement resistance)(P>.05). 3. The pin-retained FPDs showed a high dislodgement resistance compared to the conventional three-unit FPDs (P<.05).

  • Operating test of power-transmission plastic spur gears were performed inspecting both characteristics of friction-wear and endurance, and suggesting endurance improvement method that either drills internal holes of tooth or inserts metallic pin in the internal hole of tooth and verifying this newly-provided method. In case of acetal gears, amount of friction-wear is observed to increase by development of plastic deformation and increase of tooth stiffness due to brittle material property of acetal. To the contrary, in case of nylon gears, suggested method is shown to drop down the tooth temperature for about 3∼10$^{\circ}C$ than original gear, thus amount of wear is reduced by over 30% and operating lift prolonged by more than 200%. Hence, suggested method is proved to be practically applicable to the plastic gears made by soft polymers such as Nylon.

  • EHD(elastohydrodynamic) lubrication theories are applied to analyse the contact characteristies between the circular are profiled teeth of the plano-wheel gear and the cylindrical pin type teeth of the inner gear for the planocentric gears unit, For improving lubrication characteristic and contact frictional interference between the teeth, a new design method of optimising continuous meshing position is introduced, and the new tooth profile which is modified as a rounded curvature of the edge of circular arc is also suggested. The results of mathematical computation from conventional and the modified gear unit are compared, respectively.

  • In this review article, in vivo and in vitro wear test methods which measure the wear of tooth and restorative material were discribed. In vivo test, the criteria of each test were reviewed. The merits and limitations were also commented. In vitro tests, three dimensional scanning methods which scan the occlusal tooth surface three dimensionally were described. Profilometer method, Pin on disk method, laser scanner method were reviewed and the limitations of each test were commented. Additionally, the structer of the three dimensional scanner which has been developed in Zurich university and has been reputated as the most accurate one was described. This study was partly supported by 1997 post-doctoral foreign study program.

  • This paper discusses process design of cold forging for inner tooth part, drum clutch. In respect to high productivity, low material consumption and low piece production costs, Metal forming has more merits than machining process. Net shape forming is huh technology which satisfies merits of metal forming and achieves high accuracy. Recently, net shape forming method widely applied because of high productivity, low material consumption and low piece production costs using press. In this study, the method which accuracy of drum clutch, automatic transmission pin, can be improved is discussed. First, process variables for process design of drum clutch are selected, and then process design is accomplished using forming analysis method. from forming analysis, forming load, stress, unfiling part is obtained. and comparing these results, optimal process design can be determined.

A STUDY OF THE CRAZING OF TOOTH STRUCTURE ASSOCIATED WITH PLACEMENT OF PINS (Pin장착시(裝着時)에 치질귀열(齒質龜裂)에 미치는 영향(影響)에 관(關)한 연구(硏究))

  • The purpose of this study was to determine the extent of tooth structure crazing when pinhole were drilled in dentin at various distances from the dentionenamel junction, and when pins were inserted by hand wrench and Auto-klutch handpiece. Sixty freshly extracted teeth were embedded in blocks of cold-curing acrylic resin to a level 1mm apical to the cementoenamel junction, the crown of the teeth were removed to a level 1.5mm coronal to the cementoenamel junction. After being sectioned, the cut surface of the teeth were smoothed, and they were divided into 4 groups. Group I : 4 pinholes were drilled with self-threading Minim spiral drill and pins were inserted with hand wrench in fifteen teeth. Group II : 4 pinholes were drilled with self-threading Minim spiral drill and pins were inserted with Auto-klutch handpiece in fifteen teeth. Group III : 4 pinholes were drilled with self-threading Regular spiral drill and pins were inserted with hand wrench in fifteen teeth. Group IV : 4 pinholes were drilled with self-threading Regular spiral drill and pins were inserted with Auto-klutch handpiece in fifteen teeth. The cut surface were then examined microscopically for the presence or absence of craze line. The results were as follows: 1. The self-threading Minim pin groups (Group I & II) produced less craze lines than did the self-threading Regular pin groups (Group III & IV). 2. In the self-threading Minim pin groups, there was more craze lines with hand wrench (Group I) than with Auto-klutch handpiece (Group 11). 3. In the self-threading Regular pin groups, there was more craze lines with hand wrench (Group III) than with Auto-klutch handpiece (Group IV). 4. Placement of the pinhole at 0.0mm from the dentionenamel junction produced more craze lines than placed at 0.5mm, 1.0mm (p<0.05), however there was no statistically significant between the 0.5mm and 1.0mm distance.

Causes of failures of long-term used double crown denture and new rehabilitation with dental implant and tooth combined denture using remaining teeth and implants (장기간 사용한 이중관 의치의 실패원인 및 자연치와 임플란트를 이용한 이중관 의치 수복증례)

  • Hybrid telescopic double crown have ever been good treatment option for patient with periodontally unfavorable few remaining teeth for successful prognosis. Tooth and implant combined telescopic double crown can be used for improving retention and support for denture with strategically placed implants on edentulous ridge. In this case, an 55-year-old female patient had chronic periodontitis with few remaining teeth on maxilla and fixed partial prosthodontics on mandible. Treatment of hybrid telescopic double crown with friction pin lasted 9.5 years only with natural teeth. After 9.5 years, additional implants was installed due to fracture of cast-posted abutment teeth. After implantation, tooth and implant combined double crown had fabricated. Through re-treatment, no complications of new denture has been found during 2 years follow up. This case presents fair prognosis of tooth and implant combined double crown denture in periodontally unfavorable condition.

The effect of metal artifacts on the identification of vertical root fractures using different fields of view in cone-beam computed tomography

  • Purpose: The aim of this study was to investigate the effects of metal artifacts on the accurate diagnosis of root fractures using cone-beam computed tomography (CBCT) images with large and small/limited fields of view (FOVs). Materials and Methods: Forty extracted molar and premolar teeth were collected. Access canals were made in all teeth using a rotary system. In half of the teeth, fractures were created by the application of mild pressure with a hammer. The teeth were then randomly put into a wax rim on an acryl base designed in the shape of a mandible. CBCT scans were obtained using a Newtom 5G system with FOVs of $18cm{\times}16cm$ and $6cm{\times}6cm$. A metal pin was then placed into each tooth, and CBCT imaging was again performed using the same fields of view. All scans were evaluated by two oral and maxillofacial radiologists. The specificity, sensitivity, positive predictive value, negative predictive value, and likelihood ratios (positive and negative) were calculated. Result: The maximum levels of sensitivity and specificity (100% and 100%, respectively) were observed in small-volume CBCT scans of teeth without pins. The highest negative predictive value was found in the small-volume group without pins, whereas the positive predictive value was 100% in all groups except the large-volume group with pins. Conclusion: The specificity of CBCT decreased with the presence of a pin in the large-volume group, but not in the small-volume group.
