Long-Term Effects of Adenotonsillectomy on Growth and Symptoms in Childhood (소아 성장 및 편도 비대 증상에 미치는 편도절제술의 장기 효과)

  • Background and Objectives This study was performed to evaluate the long-term effect of adenotonsillectomy on childhood growth by examining preoperative growth status and presenting symptoms. Subjects and Method One hundred and four patients who underwent adenotonsillectomy from January to December 2009 were enrolled in this study. Clinical data were collected from medical record reviews and through the administered questionnaire. We investigated symptoms and growth changes during 5 years following the surgery. Results The mean age of patients was $6.0{\pm}1.94$ years (range, 3-10), with the male to female ratio of 62:42. Pre-operative symptoms were significantly improved after the surgery. The mean pre-operative height and weight percentiles were $53.6{\pm}27.4$ and $59.6{\pm}29.2$ at initial evaluation, and $67.1{\pm}26.4$ and $59.6{\pm}28.6$ at 5 years post adenotonsillectomy (p<0.001, p=0.989), respectively. An increase in height percentile was more prominent in patients whose pre-operative height percentile was less than 50 compared to those with a percentile of 50 or more (p<0.001). Weight percentile was significantly increased in those with the pre-operative weight percentile of less than 50 and decreased in patients with a percentile of 50 or more. Conclusion Adenotonsillectomy has a positive effect on height growth in children with adenotonsillar hypertrophy, especially in patients whose height percentile is less than 50. Weight gain also can be expected in preoperative low-weight children.

The Retrospective Study for the Effect of Traditional Korean Medicine on Patients with Olfactory Disorder after Failure of Steroid Treatment (스테로이드 치료에 반응하지 않은 후각장애 환자에 대한 한방치료의 효과)

  • Objectives : The purpose of this study was to analyze the effects of traditional korean medicine treatment on patients with olfactory disorder who did not respond to steroid treatment. Methods : A retrospective chart review was conducted on 16 patients who visited the outpatient clinic with olfactory disorder for over 1 month. The patients were treated with Herbal medicine, acupuncture, electronic moxibustion, intranasal pharmacopuncture, and olfactory training. The treatment effect was evaluated by KVSS I test. Results : There was no significant difference in KVSS I test score according to sex, age, duration of treatment, number of treatment, the days after onset. There was no significant difference in the KVSS I test scores before and after the treatment for the cases of sinonasal disease, head trauma, and idiopathic cases. However, there was a significant difference in cases of post-viral olfactory disorder. Conclusion : Traditional korean medicine may be effective for patients with post-viral olfactory disorder that did not show improvement in steroid treatment.

A Review of the Latest Discussions on the Mechanism of Itching - A Study on Recent Research Trend and the Relationship between Gastrointestinal Tract and Itching (가려움증의 기전에 대한 최신 논의 고찰 - 최근의 연구 동향과 위장관계와의 연관성)

  • Objectives : The purpose of this study is to review the latest discussions on the mechanism of Itching. Methods : Articles that reviewed the mechanism of itching were searched from Pubmed (January 2016 to June 2021). In addition, review articles discussing the gastrointestinal tract and the mechanisms of pruritus were searched seperately. Results : The articles are classified into three categories. These categories are the classification according to the passage of time (acute, chronic), the immune factors involved (inflammatory, non-inflammatory), and the neurophysiological mechanism (pruritoceptive itching, neuropathic itching, neurogenic itching, psychogenic itching). In each category, the articles over the past five years are summarized and reviewed. Also, how the status of the gastrointestinal tract is reflected in itching was discussed in terms of leaky gut syndrome, neuro/gastrointestinal peptides, and gut microbiota. Conclusions : This review introduces the recently discussed mechanism of itching, and in particular, examines how the gastrointestinal tract is related to skin itching. Based on these considerations, it is expected that more diverse therapeutic approaches can be explored in the future.

Investigation of Effective Korean Herbal Medicine for Psoriasis - Focusing on Lipid Metabolism - (건선에 효과적인 한약 처방 탐색 - 지질 대사를 중심으로)

  • Objectives : The purpose of this study is to investigate the possibility of using herbal medicine for the management of psoriasis focusing on lipid metabolism. Methods : We reviewed studies about pathophysiology, and medical treatment of psoriasis, the relationship between psoriasis and metabolic syndrome and lipid metabolism, and herbal medicine on Pubmed and Google scholar. Results : Psoriasis is a chronic multi-organ inflammatory disease not limited to skin, and steroids, immuno-suppressants, and biological agents are used. It is known that psoriasis and metabolic syndrome act as mutual risk factors, and lipid metabolism are involved in psoriasis. The effects of various single herbal preparations and complex herbal extract, decoction on improving lipid metabolism have been consistently reported, and there was an improvement of psoriatic skin lesions and improvement of blood lipid levels through herbal medicine. Conclusions : Herbal medicine research in psoriasis has focused on the anti-inflammatory effect and the suppression effect of certain immune mediators. However, considering that psoriasis is affected by lipid metabolism and side effects of Western medicines, the use of herbal medicines for the purpose of controlling lipid metabolism in psoriasis is useful in aspects of reducing side effects of concurrent Western medicine, improving the severity of psoriasis, and managing metabolic risk factors.

3 Cases of Systemic Atopic Dermatitis Patients Improved by Korean Medicine Hospitalization Care (입원 치료로 호전된 전신 아토피 피부염 한방치험 3례)

  • Objectives : This study aims to report 3 cases of systemic atopic dermatitis improved by Korean medicine hospitalization care. Methods : Unlike outpatients, inpatients were supervised their lifestyle including dietary habits by physicians every morning and daily treated with acupuncture, herbal medicine including Bangpungtongseongsan-gagam, and Hwangryunhaedok-tang wet dressing therapy during all week. For dressing preparation, Hwangryunhaedok-tang was used to skin lesion except face. Mixed decoction of Lonicerae Flos and Forsythiae Fructus was used to facial lesion. Hwangryunhaedok-tang pharmacopuncture was injected to 2 patients. Western oral medicine and injection such as antihistamine or corticosteroids were prescribed by physician depending on each patient's condition. Symptoms were assessed with SCORAD index, IGA, and taking photos. Results : All patients showed the decrease in SCORAD index, IGA, skin lesion amelioration, and patient-subjective improvement. As time passes, dependence on western treatment also decreased and patients were able to endure pruritus without antihistamine injection during hospitalization. Conclusions : This study suggests that Korean medicine hospitalization care including acupuncture, herbal medicine, external therapy is effective for improving atopic dermatitis through decrease SCORAD index, IGA and ameliorating skin lesion.

A Case Study of Wound Dressing Treatment in a Patient with Diabetic Foot Ulcer (당뇨병성 족부 궤양 환자의 창상 드레싱 치료 증례)

  • Objectives : The purpose of this study is to report on the treatment of wound dressing in patients with diabetic foot ulcers in the dermatology clinic of Korean medicine. Methods : First, sufficient marginal resection of the ulcer and necrotic tissue on the foot of a patient with diabetic peripheral neuropathy was performed. Inflammation was treated with Anti-inflammatory pharmacopuncture solution, and dressings were applied for one month using Cornu Cervi Parvum pharmacopuncture solution, and Haeboo ointment for granulation tissue formation and skin regeneration. Results : After 10 days of the treatment, the formation of new granulation tissue was observed in the necrotic tissue area, and the extent of the lesion decreased sharply from the 14th day. On the 40th day, epithelialization progressed so that the treatment was terminated, and complete keratinization of the site was observed at follow-up 3 weeks after the end of the treatment. Conclusions : In the treatment of wound dressings in diabetic foot ulcer patients, marginal resection treatment with the application of pharmacopuncture solution and herbal ointment showed good effect.

The Analysis on Trend of Articles about Retina and Optic Nerve Disease in Journal of Korean Medicine (국내 한의학 학술지에 게재된 망막과 시신경질환 관련 논문들의 경향성 분석)

  • Objectives : This study was designed to examine and analyze the recent trend of Korean medicine research on retinal and optic nerve diseases, which is increasing. Methods : This study examines papers related to diseases occurring in the retina and optic nerve that were published in Korean journals of Korean medicine, and analyzes the results of research so far by classifying them by year, journal, disease type and type of paper. Results : 1. Since it was first published in 1995, a total of 17 articles have been published until 2018, with 2 articles(11.1%) each in 1997, 2014 and 2018, and 1 article(5.6%) each in other years. 2. The number of searched journals was 17 paper; 4 review articles, 1 original articles, 12 case reports. 3. Distribution of journals; the percentage of Journal of Korean Medicine Ophthalmology and Otolaryngology and Dermatology ranked the highest(41.2%). 4. Classification of 12 case reports into three categories; 7 retinal diseases, 3 optic nerve diseases, 2 other diseases. Conclusions : Currently, researches on retinal and optic nerve involvement in the Korean medicine journals have been conducted mainly through case reports. In the future, more clinical research and case reports are necessary to give practical application to patients.

A Case of Chronic Cough in a Child Misdiagnosed as a Tic Disorder (틱 장애로 오인된 소아청소년의 만성 기침 1례)

  • Objectives : This study aimed to report improvement of patient with chronic cough treated with Korean medical treatment. Methods : A 12-year-old male patient who began coughing at the end of October 2023 misdiagnosed by a tic disorder and was prescribed oral medication. However, the cough worsened, persisting throughout the day without respite, accompanied by sleep disturbances (desire to sleep continuously) and gait disturbance (only able to walk with support). The patient visited the clinic on December 15, 2023, and was re-diagnosed with chronic cough. And the patient received Korean medical treatment, including herbal medicine(Ryokankyomishinge-tang, Ryokankyomishingenin-tang, Sogunjung-tang), and acupuncture. The severity of chronic cough was evaluated by VAS before and after administration of each treatment term. Results : The patient's symptoms improved and VAS changed 10 to 0. Conclusions : This case study showed an effectiveness of Korean medical treatment on chronic cough.

A Systemic Review and Meta-Analysis of Herbal Medicine Treatment for Acute Pharyngitis : Based on Chinese Articles (급성인두염의 한약 치료에 대한 체계적 문헌 고찰 및 메타분석 : 중국 논문을 중심으로)

  • Objectives : The purpose of this study is to investigate the effectiveness and safety of herbal medicine treatment for acute pharyngitis. Methods : 2 databases(Wangfang Med Online, CNKI) were used to search Randomized Controlled Trials(RCTs) that used herbal medicine treatment for acute pharyngitis, which were published from January 2007 until April 2024. Results : 9 RCTs were selected in this study, and all studies compared herbal medicine alone treatment with western medicine alone treatment. The most frequently used medicine was Toxin-removing medicinal(淸熱解毒藥). Platycodonis Radix(桔梗) was the most frequently used herb. Meta-analysis of nine studies showed that the Total Effective Rate(TER) of treatment group was statistically higher than that of control group(RR:1.12, 95% CI:1.06 to 1.19, p<0.00001, I2=22%). Meta-analysis of 3 studies showed that the total symptom score of treatment group statistically improved than that of control group(MD: -1.00, 95% CI: -1.30 to -0.71, p<0.00001, I2=0%). Mild adverse events such as anorexia were identified in 4 RCTs, but no serious adverse events were reported. Conclusion : Using herbal medicine alone can be more effective and safe in improving acute pharyngitis than using western medicine. But further well-designed studies are needed because heterogeneity between studies existed and the quality of the reports needs to be improved.

A Case of Olfactory Disorder Treated with Mahwanggachul-tang (마황가출탕(麻黃加朮湯)으로 호전된 후각장애 1례)

  • Objectives : The purpose of this study is to report the case of olfactory disorder treated with Mahwanggachul-tang. Methods : A 58-year-old female patient began to smell the odor of swimming pool water during daily life at the October 2023. Symptoms get worse over time, all smells in daily life felt like bad odors from around January 2024. A brain CT scan was performed to identify other causative diseases, but no abnormal findings were diagnosed. The patient visited the clinic on April 1, 2024, and the patient received Korean medical treatment, including herbal medicine(Mahwanggachul-tang) and acupuncture. The severity of olfactory disorder was evaluated by NRS before and after administration of each treatment term. Results : The patient's symptoms improved and NRS changed 10 to 0. Conclusions : This case study showed an effectiveness of Mahwanggachul-tang treated on Olfactory disorder.
