A Clinical Experience of Dermatitis suspected Chronic Arsenic Intoxication (만성 비소 중독으로 의심되는 피부염 치험 1례)

  • Objectives : To report the patient suspected chronic arsenic intoxication after Korean medicine treatment. Methods : One patient with highly suspected chronic arsenic intoxication was selected. He was suspected to be chronic arsenic intoxication because symptoms and hair heavy metals test. Acupuncture was applied for twice a day and herbal medicine(Pyeonghyeol-eum) was administered in 28 days. Evaluation method applied to xeroderma, pruritus, psoriasis were overall dry skin score(ODS), visual analogue scale(VAS), pruritus grading system(PGS). Results : Pyeonghyeol-eum, acupuncture and the other Korean medicine treatments improved xeroderma, pruritis, psoriasis after 28 days of treatment. ODS decreased from 3 to 2. VAS of Pruritus decreased from 8 to 1. PGS decreased from 15 to 4. Adverse effects were not reported. Conclusions : This study shows that the Korean medicine treatment was effective in impoving symptoms of chronic arsenic intoxication. But the additional study will be conducted for revealing dermatitis caused by chronic arsenic intoxication.

Prognostic Factors of Inpatients with Severe Atopic Dermatitis Treated in a Traditional Korean Medicine Hospital (한방병원에서 입원치료를 시행한 중증 아토피피부염 환자의 예후지표 탐색)

  • Objectives : This study was aimed to find significant prognostic factor of inpatients with severe atopic dermatitis (AD) who treated in a Traditional Korean Medicine (TKM) hospital. Methods : A retrospective review of medical records of 39 patients with severe AD who hospitalized for more than 7days was performed. All patients were treated with acupuncture, herbal decoction and herbal wet wrap dressing. Therapeutic effect was assessed by difference of objective SCORing of Atopic Dermatitis index (${\Delta}OSI$) at admission and at discharge. Patients were divided into two groups: improvement group (${\Delta}OSI{\leq}5$) and no-improvement group (${\Delta}OSI>5$), and clinical characteristics, severity, infectious complication, serum total IgE, eosinophil counts, adherence and concomitant treatment were investigated. Results : There were 7 cases in no-improvement group. There was no significant difference in OSI or infectious complication between the two groups. Also, there was no significant difference in total serum IgE and eosinophil counts, which are known to be related to severity of AD. However, the proportion of adherence group was significantly lower in no-improvement group than improvement group. Conclusions : This study suggest that treatment adherence is a significant prognostic factor for treatment outcome in inpatients with severe atopic dermatitis. If the adherence to treatment is improved, the therapeutic effect of hospitalization would increase in spite of severe AD or infectious complication.

A Case of Atopic Dermatitis Patient with Exudate Using Korean Medicine Treatment and Western Treatment (한양방 치료를 병행한 삼출액을 주소로 하는 아토피피부염 치험 1례)

  • Objectives : This study is clinical report of one atopic dermatitis patient with exudate.Methods : We treated a 16-year old woman patient with acupuncture, pharmacopuncture, herbal medicine, herbal bath, aroma cream therapy and western medicine. The improvement of the patient was evaluated by SCORAD INDEX and photography.Results : The SCORAD Index score of admission day at hospital was 73.5, and the SCORAD INDEX score of discharge day was 11.4. Amount of exudate and the score of oozing were decreased.Consults : The complex treatments of korean medicine and western medicine was effective to improve of atopic dermatitis patient with exudate. And the influence of weather condition to prognosis of atopic dermatitis with exudate was observated. It is necessary to have more observations and case reports.

  • Tracheostomal stenosis after total laryngectomy is a distressing complication which con-tributes significantly to both psychosocial and physical morbidity according to nature and severity in laryngectomee. Sternal stenosis will compromise not only optimal air exchange, crust formation but also the ability to clear tracheobronchial secretion, so pneumonia and atelectasis will develop. Having a number of procedure recommended for correction of such stenosis with limited results. We developed new technique which is based on tracheal advancement flap had been ap-plied to 12 patients, successfully. We think that total or partial tracheal advancement flap technique Is useful for widening the stoma and advantages of this method are following. 1. Simple technique. possible under local anesthesia 2. Healthy tracheal ring facilities width control 3. Less chance of refractory scar stenosis 4. Tracheoesophageal shunt can be constructed after the partial advancement flap.

Ryodoraku pattern classifications of tinnitus patients using cluster analysis (군집분석을 이용한 이명환자의 양도락 패턴 분류)

  • Objective : The aim of this study is to classify the patterns of Ryodoraku using cluster analysis on tinnitus patients. Method : 261 patients with tinnitus and 159 patients with other disease were analyzed. We used cluster analysis with SPSS 20.0 to classify the pattern of Ryodoraku. We searched the cluster characteristics and tried to interpret the specific patterns from previous studies. Result : We divided tinnitus patients to four groups and control patients to five groups. There were some shared patterns between tinnitus group and control group. One group on tinnitus patients showed lower age than other groups. Time, sex, total average did not show any differences among groups. Conclusion : Cluster analysis on tinnitus patients showed the possibility of presenting the specific Ryodoraku patterns. But because of weak Ryodoraku studies, clinical usefulness is unknown. Further study is needed.

The Clinical investigation studies in early stage of intractable peripheral facial paralysis using Needle-Embedding Therapy (매선요법(埋線療法)을 이용한 중증 구안괘사(口眼喎斜) 초기치료에 대한 임상적 고찰)

  • Objectives : This study was performed to investigate the effect of Needle-Embedding Therapy in early stage of intractable peripheral facial paralysis. Methods : 46 patients with severe peripheral facial paralysis were treated with oriental medicine therapy including Needle-Embedding Therapy in their early stage. We evaluated the effect of Needle-Embedding Therapy by House-Brackmann Grading system, decrease of subjective symptoms and satisfaction measurement. Results : After treatment, HB-Scale grade was significantly decreased(p-value<0.001). More than 50% of subjective symptoms were disappeared in 82.61% of patients. 80.48% of the patients was satisfied with the Needle-Embedding Therapy. Conclusion : Needle-Embedding Therapy could be effective to improve symptoms of severe peripheral facial paralysis in early stage. Further studies will be required to identify the beneficial effect of Needle-Embedding Therapy in early stage of peripheral facial paralysis.

  • Objectives : The purpose of this study is to investigate the effect and side effects of steroids on dermatitis and skin barrier through lipid metabolism. And to propose using Herbal medicine to suppress Steroid rebound and prevent side effects. Methods : We reviewed recent studies about the relationship between dermatitis, skin lipid, steroid, and herbal medicine through Google scholar. Results : In various inflammatory skin diseases, the corticosteroid is selected as the primary drug due to its strong anti-inflammatory and immunosuppressive effect. However, long-term use of steroids has a variety of side effects, especially lipid metabolism disruption, which aggravates skin barrier damage underlying various skin diseases and is more susceptible to inflammatory reactions. Conclusions : Herbal medicine is used as a comprehensive approach, and it can be used to reduce the frequency of steroid exposure by protecting against barrier damage by controlling anti-inflammatory, antioxidant, and systemic/sebaceous lipid metabolism and stratum corneum protein differentiation.

A Proposal and Considerations for Treatment Approaches of Psoriasis (건선의 치료 접근법에 대한 고찰 및 제언)

  • Objectives : To investigate treatment approaches of psoriasis, and to provide universal and holistic standards to assist in optimizing patient care and future research. Methods : Review articles of psoriasis regarding pathophysiology, risk factor or treatment were searched from Pubmed (January 2016 to June 2020). Treatment approaches were investigated based on the searched articles. Additional data collecting was done for further discussion by searching Pubmed and Google scholar with keywords relevant to the approaches, and the relevant references of articles retrieved were manually inspected to be included. Results : Modalities to directly regulate the relevant helper T cell or inflammatory cytokines can constitute the treatment approaches of psoriasis. Modalities to treat gastrointestinal tract inflammation, to correct metabolic syndrome and to improve epidermal lipid abnormality via whole body lipid metabolism can also constitute the treatment approaches of psoriasis. Probable adverse effects of long term use of western medication should be addressed carefully, and alleviating the hazards of western medication can be a treatment approach of psoriasis. Conclusion : Treatment of psoriasis should take account of systemic aspects such as gastrointestinal tract and lipid metabolism. Treatment approaches of psoriasis established on the pathophysiological basis can serve as universal standards.

Levels of Soluble Intercelluar Adhesion Molecule-1 and Total Sialic Acid in Serum of Patients with Oral Cavity Cancer and Laryngeal Cancer (진행된 구강암 및 후두암 환장의 혈청 Intercellular Adhesion Molecule-1(ICAM-1) 및 Sialic acid(SA)농도)

  • Adhesion molecules have been implicated in tumor progression. In this study, we aimed to investigate serum soluble intercellular adhesion molecule-1 (ICAM-1) and sialic acid (SA) levels in oral cavity cancer and laryngeal cancer and correlate their levels with cancer progression. Method : The sera from 31 patients with advanced oral cavity cancer (5 at stage III, 10 at stage IV) and advanced laryngeal cancer (1 at stage III, 15 at stage IV) were extracted before treatment. The concentrations of ICAM-1 was measured by Endogen kit (measured absorbance at 490nm) and the concentration of SA was measured by Roche kit (measured absorbance at 550nm). Respectively, gained data was compared with those from a control group (n=12). Result : Mean serum ICAM-1 and SA levels were found to be higher in oral cavity cancer group and laryngeal cancer group than control group. But statistical meaning was at SA (p<0.001, oral cavity cancer and laryngeal cancer versus control). Conclusion : These data reveal that the significant correlations serum SA level in advanced oral cavity cancer and laryngeal cancer. Serum ICAM-1 level was higher at advanced oral cavity cancer and laryngeal cancer than at control group but that was not significant.

  • Damum(痰飮) is a kind of pathologic secretion products that excessive water is accumulated in a part of body. Damhun(痰暈) is the vertigo induved by Damum(痰飮). We experienced two clinical cases treated vertigo by orietal medicine(herbal medication, acupuncture). 33-years old male and 63-years old female patients had Damhun(痰暈), vertigo by Damum(痰飮). We administered Banhabaechulcheoma-tang(Pansiabaichutienma-tang) and Yeonggyechulgam-tang(Lingguishugna-tang) to each Patient, and two Patients improved vertigo and referred symptoms. In other words, this study is that We did not limited peripheral vertigo to vestibular system, made a diagnosis of Damum(痰飮) and treated vertigo by orietal medicine(herbal medication, acupuncture) referred to many medical practitioner of many generations. We suggest that digestive sypmtom(related to Damum(痰飮), abdominal examination and stool frequency are closely associated with vertigo and could be estimated prognosis of vertigo.
