Numerical modeling using the finite element method (FEM) offers crucial insights into the mechanical behavior of prostheses, including stress and strain distribution, load transfer, and stress intensity factors. Analyzing cracking in PMMA surgical cement (polymethylmethacrylate) for total hip prostheses (THP) is essential for understanding the loosening phenomenon, as the rupture of orthopedic cement is a primary cause. By understanding various failure mechanisms, significant advancements in cemented total prostheses can be achieved. This study performed a numerical analysis using a 3D FEM model to evaluate stress levels in different THP models, aiming to model damage in the orthopedic cement used in total hip arthroplasty. Utilizing ABAQUS software, FEM, and XFEM, the damage in three types of THPs-Charnley (CMK3), Osteal (BM3), and THOMPSON was modeled under stumbling loading conditions. XFEM allowed for the consideration of crack propagation between the cement and bone, while the GEARING criterion employed a user-defined field subroutine to model damage parameters. The study's findings can contribute to improving implant fixation techniques and preventing postoperative complications in orthopedic surgery.
The interphalangeal joint (IPJ) of the hallux has received little attention compared with the first metatarsophalangeal joint. But, the hallucal IPJ has several disorders such intra-articular fractures, dorsal dislocation, alignment disorder, and inflammatory or degenerative arthritis. Among these disorders septic arthritis of the IPJ of the hallux is rare. We report a case of sepsis of the hallucal IPJ and adjacent underlying osteomyelitis without neuropathic problem and was performed through infected soft tissue and osseous debridement, temporary antibiotic-impregnated cement spacer, and delayed intercalary allogenic fibular bone graft with K-wire fixation.
PHILOS plate fixation in osteoporotic proximal humerus fracture of old age is well-known for high complication rate, especially metal failure, providing various augmentation techniques, such as calcium phosphate cement, allogenous or autologous bone graft. We report a case of polymethyl methacrylate augmentation to provide appropriate reduction with a significant mechanical support. This can be a treatment option for displaced unstable osteoporotic proximal humerus fracture with marked bony defect.
Gentamicin-loaded bone cement used in total joint arthroplasty is indispensable, as it provides stability by directly binding the surfaces of implants and bones. Depending on multiple factors, including the material of the bone cement used, common complications, such as aseptic loosening, osteolysis, and infection can occur postoperatively. In clinical practice, Doujet bone cement is easy to handle (pre-packed all-in-one system), and has shown low failure rates and non-inferior results compared with similar available products. We conducted a retrospective comparative study to analyze the clinical and radiological results of each bone cement group to establish the safety and usefulness of Doujet bone cement. From July 2020 to July 2022, we enrolled 198 patients in this study after an average follow-up period of 37 months (range, 6-48 months). In 99 patents, Doujet® bone cement (Injecta, Gunpo, Korea) was used for total knee arthroplasty (TKA), while Refobacin® bone cement (Biomet, Warsaw, IN, USA) was used in 99 patients. The average range of motion (ROM) of the knee increased by 2.4° (from 127.0° preoperatively to 129.4° postoperatively) in the Doujet group, and by 0.1° (from 128.7° to 128.8°) in the Refobacin group (P=0.701). The Western Ontario and McMaster Universities (WOMAC) osteoarthritis index scores decreased from 44.1 to 7.8 in the Doujet group, and from 44.2 to 6.3 in the Refobacin group (P=0.162). Complications, such as osteolysis or post-operative wound infection, did not occur in more than two cases in both groups. The WOMAC and ROM of the knee in both groups had no clinical differences. Both Doujet and Refobacin similarly showed low complication rates after TKA.
목적: 골다공증성 척추 압박골절을 가진 환자에서 혈액을 혼합한 시멘트를 사용함으로써 기존 시멘트를 이용하여 경피적 척추 성형술을 시행했을 경우 나타나는 합병증을 줄이는 데 적합한지 알아보고자 한다. 대상 및 방법: 2016년 1월부터 2017년 1월까지 80명을 대상으로 후향적으로 진행되었다. 각 군에서 기존에 사용하던 시멘트 20 g에 혈액을 각각 2, 4, 6 ml를 섞어서 다공성 시멘트를 생성하였다. 지름 2.8 mm, 길이 215 mm의 튜브를 이용하였고 중합온도, 응결 시간, 적절한 통과시간을 측정하여 기존의 시멘트만 사용하던 것과 비교하였다. 방사선적으로 컴퓨터 단층촬영 및 X-ray를 통하여 결과를 평가하여 비교하였다. 결과: 중합온도는 기존의 시멘트(R), 2 ml (B2), 4 ml (B4), 6 ml (B6) 군에서 각각 70.3℃, 55.3℃, 52.7℃, 45.5℃였으며, 응결시간은 960초(R)에서 558초(B2), 533초(B4), 500초(B6)로 감소하는 양상을 보였다. 통과시간은 각각 73초(R)에서 45초(B2), 60초(B4), 78초(B6)를 보였으며, 혈액의 양이 증가할수록 기존의 시멘트 통과시간과 유사하였다. 방사선적 결과 분석에서 추체 재건율과 추체 재함몰률은 4군 간에 유의한 차이가 없었다. 인접 추체 골절은 R군에서 2건, B2, B4 군에서 1건씩 나타났으며, 시멘트 누출률은 기존 시멘트 군에서 약 2배 높게 측정되었다. 결론: 기존의 경피적 척추 성형술 시 본인 자가 혈액을 시멘트와 혼합하여 사용하는 술식은 중합온도의 감소, 응결시간의 단축을 보이며 시멘트 누출 발생률이 적었다. 이러한 물성은 기존의 시멘트만 사용하는 경우보다 합병증을 줄일 수 있는 더 적합한 기계적 특성에 도움일 될 것으로 판단된다.

Periprosthetic fracture after total elbow replacement surgery is a difficult complication to manage, especially when it comes together with implant loosening. If stem revision and internal fixation of the periprosthetic fracture are performed simultaneously, this would be a very challenging procedure. Most of total elbow replacement implants are cemented type. Cement usage at periprosthetic fracture site may interfere healing of fractured site. Authors underwent internal fixation with use of locking plate and cerclage wire for periprosthetic fracture, allogenous fibular strut bone inserted into the humerus intramedullary canal allowing the fractured site to be more stable without cement usage. At 10-month follow-up, the complete union and good clinical outcome was achieved. We present a novel technique for treating periprosthetic fracture with implant loosening after total elbow replacement surgery, using intramedullary allogenous fibula strut bone graft.
Background: The purpose of our study was to investigate short-term outcomes of two-stage reverse total shoulder arthroplasty (RTSA) with an antibiotic-loaded cement spacer for shoulder infection. Methods: Eleven patients with shoulder infection were treated by two-stage RTSA following temporary antibiotic-loaded cement spacer. Of the 11 shoulders, nine had pyogenic arthritis combined with complex conditions such as recurrent infection, extensive osteomyelitis, osteoarthritis, or massive rotator cuff tear and two had periprosthetic joint infection (PJI). The mean follow-up period was 29.9 months (range, 12-48 months) after RTSA. Clinical and radiographic outcomes were evaluated using the visual analog scale (VAS) score for pain, American Shoulder and Elbow Surgeons (ASES) score, subjective shoulder value (SSV), and serial plain radiographs. Results: The mean time from antibiotic-loaded cement spacer to RTSA was 9.2 months (range, 1-35 months). All patients had no clinical and radiographic signs of recurrent infection at final follow-up. The mean final VAS score, ASES score, and SSV were significantly improved from 4.5, 38.6, and 29.1% before RTSA to 1.7, 75.1, and 75.9% at final follow-up, respectively. The mean forward flexion, abduction, external rotation, and internal rotation were improved from 50.0°, 50.9°, 17.7°, and sacrum level before RTSA to 127.3°, 110.0°, 51.8°, and L2 level at final follow-up, respectively. Conclusions: Two-stage RTSA with antibiotic-loaded cement spacer yields satisfactory short-term clinical and radiographic outcomes. In patients with pyogenic arthritis combined with complex conditions or PJI, two-stage RTSA with an antibiotic-loaded cement spacer would be a successful approach to eradicate infection and to improve function with pain relief.

This study examines crack behavior within orthopedic cement utilized in total hip replacements through the finite element method. Its main goal is to compute stress intensity factors (SIF) near the crack tip. The analysis encompasses two load types, static and dynamic, applied to a crack starting from the interface between the cement and bone. Specifically, it investigates SIFs under mixed mode conditions during three activities: normal walking, climbing upstairs, and downstairs. The results highlight that a crack originating from a micro-interface under substantial loading can cause cement damage, leading to prosthetic loosening. Stress intensity factors in modes I, II, and III are influenced by the crack tip's orientation and location in the bone cement, with a 90° orientation yielding notably higher values across all three modes.
We present an unusual case of bone metastases from renal cell carcinoma around orthopedic implants in a 78-year-old female with osteolytic, expansile, highly vascularized, malignant infiltration around suture anchors in the proximal humerus. The patient had undergone arthroscopic rotator cuff repair using suture anchor implants 6 years previously. After diagnosis of bone metastasis, she was successfully treated with metastasectomy and internal fixation using a plate and screws, with cement augmentation. This report is the first to document metastases around a suture anchor in a bone and suggests the vulnerability of suture anchor implants to tumor metastasis.
Purpose: Treatment of diabetic foot infection due to methicillin-resistant Staphylococcus aureus (MRSA) remains challenging. Applying vancomycin-impregnated cement is one of the best methods of treatment. Vancomycin-impregnated cement has been used worldwide; however, to date, there is a limited number of studies regarding its use. We evaluated the duration of antimicrobial activity of vancomycin-impregnated cement stored at room temperature after manufacturing. Materials and Methods: The vancomycin-impregnated cement was manufactured by mixing 1 g of vancomycin with 40 g of polymer and adding 17.90 g of liquid monomer. The cement dough was shaped into flat cylinders with diameter and height of 6 mm and 2 mm, respectively. Another cement of the same shape without mixing vancomycin was prepared as the negative control. All manufactured cements were sterilized with ethylene oxide gas and stored at room temperature. Each cement was placed on Mueller Hinton agar plate lawned with standard MRSA strain. Standard vancomycin disk and gentamicin disk were placed together. After 24 hours, the diameter of inhibition zone was measured, and if the diameter was less than 15 mm, vancomycin-impregnated cement was regarded as a loss of antimicrobial activity. The study was repeated every 2 weeks until vancomycin-impregnated cements lost their antimicrobial activity. Results: Vancomycin-impregnated cement stored for a duration of 16 weeks created a 14 mm inhibition zone, while vancomycin disk created a 15 mm inhibition zone. Vancomycin-impregnated cement stored for a duration of 17 weeks created 7 mm and 9 mm inhibition zones, while vancomycin disk created 16 mm and 15 mm inhibition zones, respectively. Conclusion: We found a decrease of antimicrobial activity in vancomycin-impregnated cements after 16 weeks. After 17 weeks, they showed definite loss of antimicrobial activity. Therefore, we recommend not using vancomycin-impregnated cement spacers that has been stored for more than 16 weeks at room temperature.
