Development of Warfarin Talk: A Messenger Chatbot for Patients Taking Warfarin (와파린 복용 환자를 위한 메신저 기반 챗봇 개발)

  • Background: Despite the increased use of direct-acting oral anticoagulants, warfarin is still recommended as first-line therapy in patients with mechanical valves or moderate to severe mitral stenosis. Anticoagulation management services (AMSs) are warranted for patients receiving warfarin therapy due to the complexity of warfarin dosing and large interpatient variability. To overcome limited health care resources, we developed a messenger app-based chatbot that provides information to patients taking warfarin. Methods: We developed "WafarinTalk" as an add-on to the open-source messenger app KakaoTalk. We developed the prototype chatbot after building a database containing seven categories: 1) dosage and indications, 2) drug-drug interactions, 3) drug-food interactions, 4) drug-diet supplement interactions, 5) monitoring, 6) adverse events, and 7) precautions. We then surveyed 30 pharmacists and 10 patients on chatbot reliability and on participant satisfaction. Results: We found that 80% of the pharmacists agreed on the consistency of chatbot responses and 44% agreed on the appropriateness of chatbot. Furthermore, 47% of pharmacists said that they were willing to recommend the chatbot to patients. Of the seven categories, information on drug-food interaction was the most useful; 90% of patients said they were satisfied with the chatbot and 100% of patients said they were willing to use it when they were unable to see a pharmacist. We updated the prototype chatbot with feedback from the survey. Conclusion: This study showed that warfarin-related information could be provided to patients through a messenger application-based chatbot.

A survey of dentists' opinions on the performance of dental hygienists (치과위생사 수행업무에 관한 치과의사의 견해조사)

  • This study examined the opinions of dentists on dental hygienists' practice and attempted to use it as the basis for the revision and modification of the legal system associated with dental hygienists. The subjects of this study were a total 15 dentists, including two from Gyeonggi, two from Gangwon, two from Gyeongsang, two from Jeolla, two from Chungcheong, and two from Seoul, who were surveyed face-to-face. This study revealed that from the perspective of dentists, the most important job of dental hygienists was scaling, and the most difficult job was patient counseling and response. From the perspective of dentists, there is a difference between what the dental hygienists actually perform and what is legal, and there is a high need for change. The inclusion of dental hygienists in the category of medical personnel should be accomplished to cover the practice of dental care support, and dentists opinionated that it is appropriate for dental hygienists to be included in the category of medical personnel because they directly perform oral health-related procedures. A careful review of the dental hygienists' scope of work should be performed to ensure that revisions to the law are made in accordance with the dental hygienists' actual practice. Further, it is necessary to revise the Medical Law and include dental hygienists among the medical personnel and simultaneously revise the scope of work of dental hygienists to reflect the actual dental practice.

Effect of Castor Oil on Cervical Ripening and Labor Induction: a systematic review and meta-analysis

  • Objectives: Post-term pregnancy is a condition associated with increased maternal and fetal complications. Administration of castor oil causes cervical stimulation by increasing the production of prostaglandins. We examined the effects of castor oil on cervical ripening and labor induction through a systematic review and meta-analysis. Methods: The search process was performed to obtain relevant articles from databases including Pubmed, Cochrane library, Scopus, Science direct, SID, Iran Medex, and Google Scholar using the English keywords of cervical ripening, post-term, castor oil, labor induction, Bishop score, and pregnancy considering all possible combinations without time constraints and their Persian equivalents from national databases. Results: A total of eight related articles from the 19 primary studies were extracted and systematically reviewed. According to a cumulative chart, the difference in the post-intervention Bishop score was statistically significant (standard mean difference [SMD]: 1.64, 95% confidence interval [CI]: 1.67-2.11, p = 0.001), indicating an effect of castor oil on increasing the Bishop score. In addition, the difference in labor induction was statistically significant after the intervention (odds ratio: 11.67, 95% CI: 3.34-40.81, p = 0.001), indicating an effect of castor oil on increasing the odds ratio of labor induction (experience of vaginal delivery). Conclusion: This meta-analysis showed that oral administration of castor oil is effective for cervical ripening and labor induction. Midwives should closely monitor pregnant women with prolonged labor and collaborate with obstetricians to employ castor oil as a safe intervention to induce cervical ripening and labor to prevent undue caesarean surgery.

The Effect of Mobile Web-Based COVID-19 Isolation Hospitalization Management Training on Patient Uncertainty, Anxiety and Nursing Education Satisfaction (모바일 웹 기반 코로나19 격리 입원관리 교육이 환자의 불확실성, 불안 및 간호교육 만족도에 미치는 효과)

  • Purpose: This study aims to identify the effects of mobile web-based COVID-19 isolation hospitalization management training on patient's uncertainty, anxiety, and nursing education satisfaction. Methods: Patients hospitalized with COVID-19 isolation rooms from August to October 2021, in Medical Center B which is an infectious disease dedicated hospital located in metropolitan city B were included. The total number of subjects was 142, of which 71 were the experimental group and 71 the control group. As an experimental treatment, a total of 5 minutes and 35 seconds of mobile web-based education were provided to the experimental group three times, including the day of, the first day of, and the second day of hospitalization. Existing training conducted on the control group was provided by the nurses in charge of the ward through oral explanation using printed materials. The data were analysed using the IBM SPSS/WIN 26.0 program. Results: Significant differences were observed between the two groups in pre-and post-assessment of uncertainty scores (t=-22.92, p<.001), anxiety scores (t=-15.03, p<.001) and nursing education satisfaction score (t=11.61, p<.001). Conclusion: As a result of the above, mobile web-based education can be used as an effective educational medium in nursing practice to improve the work efficiency and quality of nursing care and contribute to improving the patient's educational satisfaction.

The Relationship between Tooth Brushing and Hand Washing in Hypertensive Patients amidst COVID-19 Pandemic (COVID-19 상황에서 고혈압 기저질환자의 칫솔질과 손 씻기 실천율과의 관련성)

  • It has been found that the incidence and mortality of COVID-19 are high in patients with underlying chronic diseases. Amid COVID-19 pandemic, tooth brushing is as important as hand washing for health care. The aim of this study was to identify factors related to the rate of tooth brushing and hand washing in hypertensive patients. We analyzed 62,762 hypertensive patients over the age of 40 from the Community Health Survey 2020. The practice rate of hand washing was higher among women, younger age, higher education level, mask wearers when going out, and tooth brushing practitioners after lunch. The hand washing practice rate was higher in hypertensive patients who practiced tooth brushing. In conclusion, there was the relationship between oral hygiene and personal quarantine. It is necessary to prepare sustainable measures to increase the practice rate of infectious disease prevention behaviors for patients with underlying chronic diseases who are susceptible to COVID-19.

Effect of Self-determination Motivation on Job Engagement and the Moderating Effect of Compensation Satisfaction in Dental Hygienists (치과위생사의 자기결정성동기가 직무열의에 미치는 영향 및 보상 만족도의 조절효과)

  • Purpose : The purpose of this study was two-fold: to identify the effect of dental hygienist's self-determination motivation on their job engagement and to analyze the moderating effect of compensation satisfaction that affect the relevance. Methods : Data were collected using a structured self-report questionnaire administered to 260 dental hygienists working in dental hospitals and clinics in Busan, South Korea. These were analyzed using IBM SPSS Statistics version 26.0 and SPSS Process Macro 3.5. A frequency analysis, including the respondents' general characteristics, frequency, percentages, and standard deviations, was performed. A regression analysis was also performed using SPSS Process Macro to verify the moderating effect of compensation satisfaction in the effect of self-determination motivation on job engagement. Results : The self-determination motivation of the dental hygienists had a statistically significant positive effect on their job engagement, which was the dependent variable. Higher intangible compensation satisfaction levels led to a stronger effect of intrinsic motivation but a weaker effect of extrinsic motivation on job engagement. Moreover, higher tangible compensation satisfaction levels strengthened the effect of intrinsic motivation on job engagement. Conclusion : Recently, the demand for oral health care has been increasing, Competition in the dental medical service market warrants, high-quality dental services based on accurate diagnosis and treatment. In this context, dental hygienists' job engagement must be improved. For this purpose, increasing the satisfaction of dental hygienists with self-determination motivation and appropriate compensation is effective. In addition, attention must be paid to the moderating effect of compensation satisfaction on the relationship between intrinsic motivation and job engagement. On the basis of the implications of this study, the results can be used as basic data for improving dental hygienists' welfare system and manpower management.

Antibacterial Effect of Dioscorea Batatas Ethanol Extract Against L. gasseri, S. mutans and P. gingivalis (산약(Disocorea batatas) 에탄올추출물의 L. gasseri, S. mutans, P. gingivalis에 대한 항균능과 성장억제 효과)

  • Purpose : In this study, to prove the antibacterial effect of Disocorea batatas, which is widely used for food, and to confirm the growth inhibitory effect, the antibacterial activity against L. gasseri, S. mutans, and P. gingivalis was verified. Based on this, it is intended to verify the utility as a preventive and therapeutic composition for dental caries and periodontal disease. Methods : RAW 264.7 cells were used to verify the cell survival rate and NO (Nitric Oxide) inhibitory effect on Disocorea batatas ethanol extract (DBEE). In order to verify the antibacterial effect against L. gasseri, S. mutans, and P. gingivalis, concentrations of 125, 250, and 500 ㎎/㎖ of DBEE were used and measured by the disk diffusion method. In order to confirm the growth inhibitory effect, the absorbance was measured at 600 ㎚ at 3, 6, 12, 18, and 24 hours using the liquid medium dilution method, and the growth inhibitory effect was measured compared to the control group. Results : The cell viability for DBEE was 91 % at 50 ㎎/㎖, and there was no cytotoxicity. The NO production inhibitory effect was shown from 10 ㎍/ml, and the higher the concentration, the greater the inhibitory effect. As for the antimicrobial effect using the disk diffusion method, the higher the concentration, the higher the antibacterial effect. At 125 ㎎/㎖ and 250 ㎎/㎖, S. mutans and L. gasseri showed high antimicrobial activity, and at 500 ㎎/㎖, the antibacterial effect was higher in L. gasseri. The growth inhibitory effect in DBEE was concentration-dependent as the higher the concentration, the higher the growth inhibitory effect, and all of them began to show growth inhibitory effects from 6 hours. Conclusion : Considering that it is widely used as an edible and medicinal material, Disocorea batatas has shown the potential to be used as a substance to prevent and alleviate dental caries and periodontal diseases, and it is believed that further research can be applied to oral health care products.

Effect of abutment types and resin cements on the esthetics of implant-supported restorations

  • PURPOSE. The aim of the study was to evaluate the optical properties of new generation (3Y-TZP) monolithic zirconia (MZ) with different abutment types and resin cement shades. MATERIALS AND METHODS. A1/LT MZ specimens were prepared (10 × 12 × 1 mm, N = 30) and divided into 3 groups according to cement shades as transparent (Tr), yellow (Y) and opaque (O). Abutment specimens were obtained from 4 different materials including zirconia (Group Z), hybrid (Group H), titanium (Group T) and anodized yellow titanium (Group AT). MZ and abutment specimens were then cemented. L*, a*, and b* parameters were obtained from MZ, MZ + abutment, and MZ + abutment + cement. ∆E001* (between MZ and MZ + abutment), ∆E002* (between MZ and MZ + abutment + cement) and ∆E003* (between MZ + abutment and MZ + abutment + cement) values were calculated. Statistical analyses included 2-way ANOVA, Bonferroni, and Paired Sample t-Tests (P < .05). RESULTS. Abutment types and resin cements had significant effect on L*, a*, b*, ∆E001*, ∆E002*, and ∆E003* values (P < .001). Without cementation, whereas zirconia abutment resulted in the least discoloration (∆E001* = 0.68), titanium abutment caused the most discoloration (∆E001* = 4.99). The least ∆E002* = 0.68 value was seen using zirconia abutment after cementation with yellow shaded cement. Opaque shaded cement caused the most color change (∆E003* = 5.24). Cement application increased the L* values in all groups. CONCLUSION. The least color change with/without cement was observed in crown configurations created with zirconia abutments. Zirconia and hybrid abutments produced significantly lower ∆E002* and ∆E003* values in combination with yellow shaded cement. The usage of opaque shaded cement in titanium/anodized titanium groups may enable the clinically unacceptable ∆E00* value to reach the acceptable level.

Influence of size-anatomy of the maxillary central incisor on the biomechanical performance of post-and-core restoration with different ferrule heights

  • PURPOSE. The study aims to investigate the influence of the ferrule effect and types of posts on the stress distribution in three morphological types of the maxillary central incisor. MATERIALS AND METHODS. Nine models were created for 3 maxillary central incisor morphology types: "Fat" type - crown 12.5 mm, root 13 mm, and buccolingual cervical diameter 7.5 mm, "Medium" type - crown 11 mm, root 14 mm, and buccolingual cervical diameter 6.5 mm, and "Slim" type - crown 9.5 mm, root 15 mm, and buccolingual cervical diameter 5.5 mm. Each model received an anatomical castable post-and-core or glass-fiber post with resin composite core and three ferrule heights (nonexistent, 1 mm, and 2 mm). Then, a load of 14 N was applied at the cingulum with a 45° slope to the long axis of the tooth. The Maximum Principal Stress and the Minimum Principal Stress were calculated in the root dentin, crown, and core. RESULTS. Higher tensile and compression stress values were observed in root dentin using the metallic post compared to the fiber post, being higher in the slim type maxillary central incisor than in the medium and fat types. Concerning the three anatomical types of maxillary central incisors, the slim type without ferrule height in mm presented the highest tensile stress in the dentin, for both types of metal and fiber posts. CONCLUSION. Post system and tooth morphology were able to modify the biomechanical response of restored endodontically-treated incisors, showing the importance of personalized dental treatment for each case.

A Case study on geriatric dental hygiene and practical education courses based on industry demand (산업체 수요기반의 노인치위생학 및 실습 교육과정 운영 사례 연구)

  • Background: This study was conducted to verify the effectiveness of geriatric dental hygiene education by developing and operating an industrial demand-based curriculum for geriatric dental hygiene. Methods: Wilcoxon signed rank test was performed to verify the before-and-after differences in major competency achievement, geriatric dental hygiene awareness, and class satisfaction according to industrial demandbased field-oriented practical education, and Spearman's correlation analysis was performed to confirm the association between each factor(p<0.05). Results: In the case of major competency achievement, 'communication competence with the older adults' was significantly improved(p=0.031) after conducting industrial demand-based field-oriented practical training. Conclusion: It is believed that the understanding of the older adults and the practical skills for oral care of the older adults can be further developed when the learners are provided with a practical curriculum that can be used in the geriatric industrial field.
