A Comparative Study of Predictive Factors for Passing the National Physical Therapy Examination using Logistic Regression Analysis and Decision Tree Analysis

  • Objective: The purpose of this study is to use logistic regression and decision tree analysis to identify the factors that affect the success or failurein the national physical therapy examination; and to build and compare predictive models. Design: Secondary data analysis study Methods: We analyzed 76,727 subjects from the physical therapy national examination data provided by the Korea Health Personnel Licensing Examination Institute. The target variable was pass or fail, and the input variables were gender, age, graduation status, and examination area. Frequency analysis, chi-square test, binary logistic regression, and decision tree analysis were performed on the data. Results: In the logistic regression analysis, subjects in their 20s (Odds ratio, OR=1, reference), expected to graduate (OR=13.616, p<0.001) and from the examination area of Jeju-do (OR=3.135, p<0.001), had a high probability of passing. In the decision tree, the predictive factors for passing result had the greatest influence in the order of graduation status (x2=12366.843, p<0.001) and examination area (x2=312.446, p<0.001). Logistic regression analysis showed a specificity of 39.6% and sensitivity of 95.5%; while decision tree analysis showed a specificity of 45.8% and sensitivity of 94.7%. In classification accuracy, logistic regression and decision tree analysis showed 87.6% and 88.0% prediction, respectively. Conclusions: Both logistic regression and decision tree analysis were adequate to explain the predictive model. Additionally, whether actual test takers passed the national physical therapy examination could be determined, by applying the constructed prediction model and prediction rate.

An FGI Study on the Adaptation of Beginning Health Teachers During the COVID-19 Era (코로나-19 유행 시기 신규 보건교사의 교직 적응에 대한 FGI연구)

  • In this study, three health teachers assigned to J-do were analyzed through the FGI research method to find out the difficulties, resolution process, and implications of new health teachers in adapting to teaching during COVID-19. As a result of the study, first, there were difficulties in relationships with parents, especially in the course of numerous calls and daily school status reports in the course of handling infectious diseases. After getting used to the COVID-19 work to some extent, beginning health teachers were burdened with having to think about health room work alone, difficulties in health classes for various students, and the burden of showing expertise as the only medical personnel in emergency situations. Second, new health teachers continued to share and communicate related information through online health teacher groups and YouTube to solve the difficulties of teaching. Third, this teaching adaptation process suggests that specific training to provide practical help to new health teachers is needed at the education office level, and it is necessary to support infrastructure such as class contents related to health classes.

A Method for Region-Specific Anomaly Detection on Patch-wise Segmented PA Chest Radiograph (PA 흉부 X-선 영상 패치 분할에 의한 지역 특수성 이상 탐지 방법)

  • Recently, attention to the pandemic situation represented by COVID-19 emerged problems caused by unexpected shortage of medical personnel. In this paper, we present a method for diagnosing the presence or absence of lesional sign on PA chest X-ray images as computer vision solution to support diagnosis tasks. Method for visual anomaly detection based on feature modeling can be also applied to X-ray images. With extracting feature vectors from PA chest X-ray images and divide to patch unit, region-specific abnormality can be detected. As preliminary experiment, we created simulation data set containing multiple objects and present results of the comparative experiments in this paper. We present method to improve both efficiency and performance of the process through hard masking of patch features to aligned images. By summing up regional specificity and global anomaly detection results, it shows improved performance by 0.069 AUROC compared to previous studies. By aggregating region-specific and global anomaly detection results, it shows improved performance by 0.069 AUROC compared to our last study.

Evaluating the Efficacy of a Formalin Alternative in Fixing Pathological Tissues for Histological and Molecular Diagnoses

  • Formaldehyde use is associated with serious health risks, which can affect medical personnel and technicians. Therefore, we investigated the efficacy of an alternative fixative, with respect to two types of formalin fixatives, by hematoxylin and eosin (H&E) staining, periodic acid Schiff (PAS) staining, immunohistochemical (IHC) staining, and RNA extraction. For H&E staining, the circular nucleus was stained dark blue by the basic dye hematoxylin and the cytoplasm was stained red by the acid dye eosin in all three fixative samples. No difference was found in the Duksan General Science (DGS), Sigma-Aldrich, and Core-Fix fixative samples (Corebiotech) used to fix kidney tissue, after PAS staining. IHC staining showed that CD4 was significantly increased in the lippolysaccharide (LPS)-treated group compared to the control group (vehicle), confirming the changes in specific molecules. The quantity and quality of RNA from tissues fixed in the three types of fixatives were evaluated. The average concentration of RNA was 106 ng/µL and average purity at A 260/280 ratio was 1.7~2.0, regardless of fixative used. For quality of protein, glyceraldehyde 3-phosphate dehydrogenase (GAPDH) protein was confirmed by Western blotting. In conclusion, Core-Fix can be used as a fixative for pathological tissues, in histological and molecular diagnoses.

A qualitative study on perceptions and status of oral muscle strength training for older adults among dental medical personnel - Focus group interviews - (치과의료인력의 노인 구강근력 강화훈련 관련 인식 및 실태에 관한 질적 연구 - 초점집단면접 적용 -)

  • Objectives: This study aimed to investigate the perceptions regarding oral muscle strength training for elder people among dental professionals. Methods: The study participants were selected using non-probability sampling methods, as dentists and dental hygienists with more than 3 years of work experience at dental institutions. A total of 15 participants were selected, including 6 dentists, 4 clinical dental hygienists, and 5 public dental hygienists. Interviews were conducted in June and July 2022, and two focus group interviews were conducted for each group. The first round was face-to-face and the second round was conducted through an online video conference. Results: Through focus group interviews, five factors were obtained; lack of awareness, value of training, factors necessary for implementation, performance status, and obstructive factors. It was found that most study participants had a negative perception regarding the application of oral muscle strength training for elder people due to the lack of information and education on the subject. However, the benefit of oral muscle strength training was positively recognized; adequate educational material, appropriate compensation, adequate time, and availability of patients will be necessary for proper training. Conclusions: It is necessary to improve the perceptions regarding the importance of strengthening oral muscles for the elder people among dentists and dental hygienists. In addition, high-quality educational material that can be easily comprehended and practiced should be developed for effective training.

Sasang Constitution may act as a Risk Factor for Hypertension and Pre-hypertension (고혈압 및 전기고혈압 위험요인으로서의 사상체질)

  • Objectives The purpose of this study was to reveal that Sasang constitution(SC) was associated with hypertension and pre-hypertension and could be a risk factor. Methods We introduced this study to educational personnel in D university in Daejeon, and 275 subjects joined this study. The SC classification was conducted with KS 15 questionnaire. The subjected measured the blood pressure with Jawon medical device automatically after 10 minute rest. The hypertension and pre-hypertension was classified by the guide of the Seventh Report of the Joint National Committee on Prevention, Detection, Evaluation, and Treatment of High Blood Pressure. The frequency analysis and T-test was used in general characteristics, and chi-square test was also used between SC and pre-hypertension and hypertension. Logistic regression was used to calculate the odds ratios (ORs) and 95% confidence interval (95% CI) for pre-hypertension and hypertension. Results The number of Taeeumin(TE), Soeumin(SE), and Soyangin(SY) was 142, 71, and 61 respectively. There was significantly different in systolic and diastolic blood pressure among SC types(p<.001). The distribution of the normal group, pre-hypertension and hypertension group by SC types was significantly different (p<.001). The ORs of TE was significantly increased (ORs 4.039, 95% CI=2.019-8.082 in pre-hypertension and ORs 4.235, 95% CI=1.581-11.348 in hypertension) compared with SE(p<.001), and after adjusting gender and smoking habit, it was still significantly different(p<.001). Conclusions It is possible that SC, especially TE could be a risk factor both pre-hypertension and hypertension.

Enhancing the Autonomy of Physical Therapy in Korea and Its Significance for the National Healthcare System: Facing the Challenges of a Super-aging Society

  • Most advanced countries that are members of the World Physiotherapy have established a 4-year education system or specialized graduate school system for physical therapists based on national standards. They have also expanded their laws and systems to provide physical therapists with the autonomy and independence to offer services in their clinics. However, compared with developed countries in North America and Europe, there are issues with the autonomy and independence of physical therapists in Korea related to national regulations. Social status and recognition of the profession are also lagging. Korea is expected to become a super-aged society by 2025. To reduce the financial burden of healthcare and welfare on the government, it is necessary to extend the time spent by older adults on independent activities and minimize their time spent using medical services. To achieve this goal and maximize the active life of older adults, a plan to efficiently use licensed physical therapists in the country should be prepared. Korea should increase the license utilization rate of physical therapists to reduce waste at the national level and increase the professional hope of the younger generations of physical therapists. To create a healthcare policy focusing on the use of physical therapy personnel, similar to that in advanced countries, it is necessary to unify educational systems and produce excellent physical therapists. Providing professional autonomy can help physical therapists develop a sense of job satisfaction. Outstanding talent will choose physical therapy as a profession if they can see hope for their future careers, and if physical therapy services in Korea are similar to those delivered in advanced countries, physical therapy in Korea can develop into a healthcare service that people desire.

Activation of Health Care Big Data (헬스케어 분야에서의 빅데이터 활용 활성화 방안)

  • With the explosive increase in data, the 'big data era' has arrived, focusing on deriving new values and insights through data. With the development of data analysis technology, the importance of data analysis and utilization in the field of diagnosis and treatment as well as prevention is expanding, while the use of big data is emerging in the healthcare field. Moreover, as the three data-related laws (Personal Information Protection Act, Information and Communication Network Act, and Credit Information Act) were passed in January 2020, it became possible to use a wide range of big data through pseudonym information. However, the use of healthcare big data is still struggling due to various policies and regulations, inconsistent data quality, and the absence of specialized personnel. Therefore, in this study, examines the current state of use of big data in the healthcare field, and analyzes the challenges, overseas cases, plans, and expected effects for activation of healthcare big data.

The Education System for Cytotechnologists and Efforts to Improve Professionalism in the United States of America (미국 세포병리사 양성 교육제도와 전문성 향상을 위한 노력)

  • The path to becoming a cytotechnologist (also known as a cytologist) in the United States of America (USA) requires a minimum of a bachelor's degree and the completion of a training program certified by the Commission on Accreditation of Allied Health Education Programs. Aspirants are recognized as cytologists after passing a test administered by the American Society for Clinical Pathology Board of Certification on completion of a recognized cytotechnology training program. Cytotechnology training programs are currently provided by 18 universities and hospitals. Six of them provide master's level education, unifying existing programs to the master's level, which is necessitated due to a shortage of pathologists and the rapid development of the health care system. The improvement and unification of the curriculum, qualification renewal system, and the effort to change the title of qualified personnel from cytotechnologist to cytologist are seen as measures that would strengthen the capabilities of cytotechnologists as well as assist in keeping pace with the rapidly changing medical system. Knowledge about the educational courses available for cytotechnologists and the efforts to strengthen their expertise in the USA would be helpful to professors who guide students preparing for overseas employment and seeking professional improvement in Korea.

Analysis of Case Characteristics of Occupational Diseases and Epidemiological Investigation of Healthcare Workers (최근 10년간(2010~2019) 의료기관 종사자의 업무상질병 및 역학조사 사례 특성 분석)

  • Purpose: We reviewed the cases of occupational diseases that occurred in healthcare workers from 2010 to 2019 and the cases of epidemiological investigations conducted by the Occupational Safety and Health Research Institute for them. Methods: In this study, gender, age, working period, occupational characteristics, and disease characteristics by occupation, which are general characteristics of healthcare workers approved for occupational diseases. In addition, 32 cases of epidemiological investigation of healthcare workers conducted by the Institute for Occupational Safety and Health for the past 10 years (2010~2019) were analyzed. Results: The gender, age, working period, occupational characteristics, and disease characteristics by occupation of medical workers recognized as occupational diseases were all statistically significant (p<.001). In the epidemiological investigation, ionizing radiation( 50.0%) and stress (33.3%) were recognized as disease-related factors, and even in cases of disapproval, ionizing radiation (60.0%) and shift work (25.5%) were designated as related factors. And it was statistically significant (p=.008). Conclusion: The current study identified the types of diseases, various causes, and general characteristics and occupational characteristics that occur frequently in healthcare workers. Therefore, if this study is used as basic information to set the priority and direction of disease prevention project for healthcare workers, it is judged that it will be helpful in preventing industrial accidents.
