제 3대구치와 인접한 제 2대구치의 치주상태에 대한 고찰 (A study on the periodontal status of second molar adjacent third molar)

  • The purpose of this study was to determine the relationship between the third molar and periodontal status of the adjacent second molar. Fifty patients who had four maxillary and mandibular second molars were consecutively selected for the study subjects. The subjects provided a total of 200 molars, i. e., 100 maxillary and 100 mandibular molars, and classified the groups as follows; third molars that are normally erupted are control group, that are impacted are test 1 group, that are simply extracted are test 2 group, that are surgically extracted are test 3 group. Probing depth, plaque index, gingival index and mobility were measured. The results were as follows. 1. In mesial probing depth, there was no significantly difference. In distal probing depth, there was a significantly difference between control group and test 1 & 3 group in maxilla and between control & test 2 group and test 1& 3 group in mandible(p<0.05). 2. In buccal probing depth, there was a significantly difference between test 2 group and test 3 group in mandible. In lingual probing depth, there was a significantly difference between control group and test 1 & 3 group in mandible(p<0.05). 3. In plaque index, there was a significantly difference between test 1 group and test 2 group in maxilla, between test 1 group and control & test 2 group in mandible(p<0.05). 4. In gingival index, there was a significantly difference between control group and test 1 & 3 group in mandible. In mobility, there was no significantly difference(p<0.05). As a result of this study, the second molars adjacent to the third molars that are impacted or surgically extracted had poor prognosis, so impacted third molars should be extracted in early time and the second molars are actively treated for periodontal health.

  • As the dental arch is the curve connecting the cusp tip of tooth, the dental arch form, composing of the occlusion, is one the important factors of occlusal reconstruction. Many studies about the horizontal dental arch form have been reported, but until now, it is unclear to infer the position of the teeth in dental arch form, to evaluate the effect of the horizontal dental arch form on chewing movement. The purpose of this study is to make objective criteria to infer the position of the teeth in dental arch. In this study, 100 subjects with individdual normal occlusion were evaluated. By multiple regression analysis on the basis of the relation of the canine and the first molar, the positions of teeth in dental arch were inferred. According to buccolingual relationship of maxillary to mandibular posterior teeth, the dental arch forms were classified into five groups, i, e. the normal group, the group which the maxillary second molar positions buccal side, the group which the maxillary premolars position buccal side, the group which the maxillary premolar position lingual side. From the results, objective criterial to infer the positons of the first premolar, the second premolar, the second molar in dental arch were made.

Risk factors for external root resorption of maxillary second molars associated with third molars

  • Purpose: This study aimed to evaluate the diagnostic performance of panoramic images compared to cone-beam computed tomography (CBCT) imaging for maxillary third molar (M3)-associated external root resorption (ERR), and to identify the risk factors of ERR on panoramic images. Materials and Methods: The study population was composed of all patients who underwent panoramic imaging at Dankook University Dental Hospital from May to October 2019. In total, 397 cases of maxillary M3s in 247 patients(147 men and 100 women) were included. The diagnostic accuracy of ERR in panoramic images compared to CBCT images was evaluated using the chi-square test. To identify risk factors for ERR, dental records and panoramic findings were evaluated by logistic regression analysis. Results: The diagnostic accuracy of ERR on panoramic images was 0.79 compared to CBCT images (P<0.05). Superimposition of M3s onto second molars (M2) was associated with an approximately 33 times higher risk of ERR than separated M3s(P<0.05). Impacted M3s showed a 5 times higher risk of ERR than erupted M3s(P<0.05). Conclusion: ERR related to M3s is a common clinical condition, and superimposition of M3 onto M2 on panoramic images was the most important risk factor for ERR. It seemed that CBCT examinations for maxillary M3s might be indicated for ERR diagnosis especially if panoramic radiographs show superimposition of M3 onto M2. Impaction itself was also a risk factor, and it should be carefully examined.

Bilateral supernumerary maxillary fourth and fifth molars: A clinical case report and literature review

  • Supernumerary teeth that are present in the molar region may be evident based on crowding and impaction, but most cases are asymptomatic and discovered as incidental findings during routine radiological examinations. This article reports the case of a 29-year-old woman who presented with a severe feeling of pressure in the region of the maxillary third molars that had been increasing in intensity for weeks. A clinical examination revealed crowding of the maxillary anterior teeth despite the completion of orthodontic treatment and an erupted third molar with localized gingivitis in the second quadrant. A radiographic examination revealed bilateral supernumerary maxillary fourth and fifth molars, so cone-beam computed tomography was performed to locate the supernumerary teeth precisely for a preoperative diagnosis and comprehensive treatment planning. This report presents the radiological and surgical case management of a rare case of bilateral supernumerary molars and reviews the literature regarding epidemiology and treatment options.

The clinical assessment of aggressive periodontitis patients

  • Purpose: Few epidemiologic studies have investigated aggressive periodontitis in Koreans, but such studies of disease prevalence and other clinical characteristics would be invaluable in providing proper treatment. The aim of this study was to assess the prevalence of aggressive periodontitis and to measure the extent of associated periodontal breakdown. Methods: The study population consisted of 1,692 patients who visited the Department of Periodontology, Wonkwang Daejeon Dental Hospital from January to December, 2010. Clinical parameters (probing depth, gingival recession, periodontal attachment loss) were measured by a single examiner, and radiographic examination was performed at the baseline. Results: Twenty-eight (1.65%) patients showed clinical features of aggressive periodontitis, of which 27 patients exhibited the generalized form, and 1 exhibited the localized form. There was no significant difference between the percentage of male and female patients. The probing pocket depth of the maxillary first molar was deeper than that of the other teeth and gingival recession was also the most serious at the maxillary first molar. The periodontal attachment loss was the highest at the maxillary first molar. The average number of missing teeth was 1.29 per subject. Loss of the second molar was prominent. Conclusions: Within the limitations of this study, the periodontal breakdown evaluated by attachment loss was found to be most severe at the first molars of aggressive periodontitis patients. However, further large scale multicenter studies are necessary to access more precise data, including prevalence.

상악 구개부 치은에 발생한 편평상피세포암의 증례보고 (Squamous Cell Carcinoma In Palatal Gingiva. A Case Report)

  • Squamous cell carcinoma is the most common malignant tumor of the gingiva. Oral cancer accounts for 5 percent of all malignant tumors in the body, and 6 percent of these occur in the gingiva. We present one case of squamous cell carcinoma of the gingiva in 23-year-old female. She complained of gingival swelling and pain during mastication between left maxillary first molar and second molar. Since she had received scaling 2 years ago, food impaction occurred frequently in this site and an ulcerative lesion recurred several times. When she was referred from local clinic, she had a large, irregularly ulcerated lesion of palatal gingiva between left maxillary first molar and second molar, accompanying induration in center and slight elevation around ulcerative margin. The etiology could not be defined but we could diagnose by careful history taking and excisional biopsy of the recurrent ulcerative lesion. In addition, computed tomography and nuclear medicine imaging were undertaken. As a result, we finally diagnosed as moderately differentiated squamous cell carcinoma without metastases. It is concluded that if ulcerations do not respond to therapy in the usual manner, it should be biopsed and histologically evaluated for the definitive diagnosis and treatment.

악교정 수술을 요하는 골격성 3급 부정교합 환자의 상악 기저골 형태에 관한 연구 (A STUDY ON MAXILLARY BASAL BONE MORPHOLOGY IN SKELETAL CLASS III MALOCCLUSION REQUIRING ORTHOGNATHIC SURGERY)

  • 본 연구는 골격성 3급 부정교합 환자의 악교정 수술시 나타날 수 있는 중안모의 부조화에 대해 영향을 미치는 하부 경조직 특히 상악 기저골 형태에 관해 연구해 보고자 단국대학교 치과대학 부속치과병원 교정과에 내원하여 술전 교정치료 및 술후 교정치료 중인 성인 환자 남성 15명, 여성 14명, 총 29명과 대조군으로 동 교정과에서 소장하고 있는 성인 정상 교합자의 자료 중 남성 13명, 여성 11명, 총 24명을 대상으로 측모 두부 방사선사진과 상악 석고모형을 이용하여 두 군간의 상악 기저골에 대한 형태적 차이를 연구해 본 결과 다음과 같은 결론를 얻었다. 1. 골격성 3급 부정교합군의 상악 기저골 형태에 대한 성별간의 비교 결과, 여성 보다 남성에서 제1소구치부 폭경, 제1대구치부 폭경과 oblique canine height, oblique molar height 그리고 상악 기저골 둘레 길이가 더 크게 나타났다. 2. 정상교합군의 상악 기저골 형태에 대한 성별간의 비교 결과, 여성 보다 남성에서 견치부 폭경, 제1, 제2소구치부폭경, 제1대구치부 폭경 그리고 oblique canine height, oblique molar height가 더 크게 나타났다. 3. 골격성 3급 부정교합군과 정상교합군 중 남성간의 상악 기저골 형태 비교에서 정상교합군의 견치부 폭경과 견치부 장경, 제1대구치부 장경, oblique canine height, oblique molar height가 3급 부정교합군 보다 더 크게 나타났다. 4. 골격성 3급 부정교합군과 정상교합군 중 여성간의 상악 기저골 형태 비교에서 정상교합군의 견치부 장경, 제1대구치부 장경, oblique canine height 그리고 oblique molar height 와 상악 기저골 둘레길이가 3급 부정교합군 보다 더 크게 나타났다.

Computed tomographic analysis of maxillary sinus anatomy relevant to sinus lift procedures in edentulous ridges in Taiwanese patients

  • Purpose: To analyze the maxillary sinus anatomy over edentulous ridges in the bilateral posterior maxillary area in Taiwanese patients using cone-beam computed tomography (CBCT). Methods: In total, 101 anatomical sites from 61 patients, including 32 premolar and 69 molar regions, were analyzed using CBCT. Measurements were made of the width and height of edentulous ridges, the thickness of the lateral wall of the maxillary sinus, and the presence of a sinus septum and the posterior superior alveolar artery (PSAA). A statistical analysis of the measurements was performed, and correlations among the measurements were assessed. Results: The average ridge width was $10.26{\pm}3.16mm$, with a significantly greater ridge width in the second molar region than in the premolar region. The mean residual ridge height was $8.55{\pm}4.09mm$, and ridge height showed an opposite trend from ridge width for the premolar and molar regions. A sinus septum was present at 5.9% of the sites, and the PSAA was observed in 24.5%. The average thickness of the lateral wall of the maxillary sinus was $2.08{\pm}0.94mm$, with no significant difference between the tooth position and lateral wall thickness. Conclusions: This study presents the anatomical features of the maxillary sinus, which should be considered in sinus lift procedures for implant placement, in the Taiwanese population. The use of CBCT is recommended to avoid intraoperative complications.

CT사진을 이용한 한국인의 C형 치근관의 분포에 대한 연구 (The distribution of C-shaped canal system in Korean population with CT image)

  • Purpose: To investigate the incidence and prevalence of C-shaped root canal using computed tomographic images of head and neck in Korean population. Materials and Methods : Regardless of each examination purpose, randomly selected 268 examples which have serial axial computed tomographic images with 8 normal molars in maxilla and mandible were selected and investigated. Defined C-shaped root canal and we could get 82 proper image examples in view of this definition. These were detected and investigated of incidence and prevalence of C-shaped root canal. Results: C-shaped root canals were found in 82 examples among 268 (30.6%) and 147 teeth. They were only found on molar area, and the highest incidence was 37.41% of left mandibular second molars, and the lowest was 2.04% of right maxillary second molars. On prevalence of tooth position, mandibular second molar was the highest, 65% of C-shaped root canal teeth and maxillary second molar was 6% the lowest. Conclusion: 31% of randomly selected CT images no considering check-up reason have C-shaped root canals. The C-shaped root canals of mandibular second molar are found most frequently and they are also detectable on maxilla. On Korean population, C-shaped root canals are one of conditions that have to concern with dental treatment for it appears at a high prevalence relatively. 제1유구치의 조기 상실로 인한 공간 변화에 대한 3차원적 분석 (THREE-DIMENSIONAL SPACE CHANGES AFTER PREMATURE LOSS OF THE PRIMARY FIRST MOLAR: A LONGITUDINAL STUDY)

  • 본 연구는 제1유구치의 조기 상실 시공간 변화에 대한 분석을 위하여 3-Dimensional Laser Scanner를 이용하여 그 유용성을 평가하고자 하는 예비논문이다. 6세-8세 사이의 제1대구치가 맹출하여 확실한 교합 관계를 이루는 혼합치 열기 어린이 중 편측 제1유구치의 조기 발치가 필요한 6명의 발치 전 후의 모형을 이용하여 다음과 같은 결과를 얻었다. 1. 유구치 공간변화는 상악은 실험군과 대조군 모두 현저한 감소를 보이지 않았고, 하악은 실험군에서 3명 중 2명에서 공간감소가 관찰되었다. 2. 치열궁 너비, 치열궁 둘레는 초기모형과 최종모형에서 비슷한 값을 보였다. 3. 협설측 경사도 변화는 유견치에서는 상하악 모두 특별한 경향을 보이지 않았으며, 제2유구치는 상악은 실험군과 대조군이 유사하였고, 하악에서는 3명중 2명에서 실험군이 더 큰 설측경사도 변화를 보였다. 제1대구치는 하악에서 실험군이 더 큰 설측경사도 변화가 관찰되었다. 4. 근원심 경사도 변화 측정에서 유견치는 상하악 모두 특별한 경향이 관찰되지 않았고 제2 유구치의 경우 하악에서 실험군이 더 큰 근심경사도 증가가 보였다. 제1대구치는 상악에서 실험군, 대조군 모두 원심 경사도가 증가되었다.
