X-LINKED DOMINANT HYPOPHOSPHATAEMIC RICKETS: 13 YEARS FOLLOW-UP STUDY (X 염색체 우성 저인산혈증성 구루병: 13년간의 치험례)

  • X-linked dominant hypophosphataemic rickets(XLHR) is a hereditary disease generally transmitted as an X-linked dominant trait. Oral manifestations include poorly mineralized dentin, enlarged pulp chambers and root canals, early tooth loss. Also, spontaneous dental abscesses could occur in the absence of a history of trauma or caries. This report describes the clinical finding and dental treatment of a child, who came to the hospital for treatment caries but was refered to pediatrics because the child showed clinically short of status, bow-leg and radiographically enlarged pulp chamber and canal, there as diagnosed as XLHR. After the diagnosis, 13 years follow-up and treatment were performed.

Mitophagy stimulation as a novel strategy for the treatment of mitochondrial diseases

  • Mitophagy, the selective degradation of damaged or surplus mitochondria using core autophagy machinery, plays an essential role in maintaining cellular mitochondrial function. Impaired mitophagy is closely linked to various human diseases, including neurodegenerative diseases, cardiovascular diseases, cancers and kidney disease. Defective mitophagy induces the accumulation of damaged mitochondria and thereby results in a decline in cellular survival and tissue function. Accordingly, enhancement of mitophagy has been proposed as a novel strategy for the treatment of human diseases closely linked to mitochondrial dysfunction. Recent studies showing that the stimulation of mitophagy has a therapeutic effect on several disease models highlight the possibility of disease treatment using mitophagy. The development of mitophagy inducers with toxicity and the identification of molecular mechanisms will enable the clinical application of mitophagy-based treatments.

Overview of Risk-Sharing Schemes: Focusing on Anticancer Drugs (위험분담제도에 대한 고찰: 항암제 사례를 중심으로)

  • This article aimed to introduce 'risk sharing' schemes for pharmaceuticals between drug manufacturers and healthcare payer. Published literature review was undertaken to summarize risk sharing concepts and collect information on existing scheme examples in other countries focusing on new anticancer drugs. Risk sharing schemes could be categorized into health outcomes-based and non-outcomes (financial) based ones. Outcome-based schemes could be broken down into performance-linked reimbursement and conditional coverage. Performance-linked reimbursement can be further broken into outcomes guarantee and pattern or process of care and conditional coverage included coverage with evidence development and conditional treatment continuation schemes. Non-outcome based schemes included market share and price volume at population level, and utilization caps and manufacturer funded treatment initiation at patient level. We reviewed the fifteen examples for anticancer drugs that risk sharing agreements in response to the inherent uncertainties and increased costs of eleven anticancer drugs. Of them, eight cases were coverage with evidence development schemes. The anticancer drugs except bevacizumab and cetuximab were all listed on the national health insurance formulary in Korea, with reimbursement criteria defined on the basis of approved indications and administrations. Risk sharing approach may be a useful tool to ensure values for drug expenditure, but there are a number of concerns such as high administration costs, lack of transparency and conflicts of interest, especially for performance-based health outcomes reimbursement schemes.

Dependence of $O_2$ Plasma Treatment of Cross-Linked PVP Insulator on the Electrical Properties of Organic-Inorganic Thin Film Transistors with ZnO Channel Layer

  • The organic-inorganic thin film transistors (OITFTs) with ZnO channel layer and the cross-linked PVP (Poly-4-vinylphenol) gate insulator were fabricated on the patterned ITO gate/glass substrate. ZnO channel layer was deposited by using atomic layer deposition (ALD). In order to improve the electrical properties, $O_2$ plasma treatment onto PVP film was introduced and investigated the effect of the plasma treatments on the electrical properties of the OITFTs. The field effect mobility and sub-threshold slope (SS) values of the OITFT decreased slightly from 0.24 to 0.16 $cm^2/V{\cdot}s$ and from 9.7 to 9.2 V/dec, respectively with increasing RF power from 30 to 50 Watt. The $I_{on/off}$ ratio was about $10^3$ for all samples with $O_2$ plasma treatment.

  • The use of PPARν (peroxisome proliferator activated receptor ν) activators in the treatment of type 2 diabetes is well established due to their ability to lower blood glucose and insulin levels and omprove insulin sensitivity. Thiazolidinedione analog is one of the potential antidiabetic drug that binds and activates PPARν selectively. In an effort to develop novel and effective antidiabetic thiazolidindione analogs, synthesis of tetrahydroquinoline and para-substituted benzene-linked thiazolidinedione analogs were carried out via coupling reaction of the hydrophobic segments with hydroxybenzylthiazolidinedione.

The Effects of Annealing on Resistant Starch Contents of Cross-linked Maize Starches (Annealing 처리가 가교결합 옥수수전분의 저항전분 수율에 미치는 영향)

  • To develop a method for increasing RS level in maize starch, cross-linked resistant starches treated with annealing were prepared. Maize starch and amylomaize VII were used in the study and annealed at $40{\sim}60^{\circ}C$ before cross-linking modification. To compare effect of annealing below gelatinization temperature, starches were heat treated at 70 and $100^{\circ}C$. RS contents were assayed by pancreatin-gravimetric (P/G) method. When maize starch and amylomaize VII were cross-linked at $45^{\circ}C$ and pH 11.0 by slurrying the starch on a solution of STMP(sodium trimetaphosphate), STPP(sodium tripolyphosphate), and sodium sulfate, RS content was 14.7% and 45.3%, respectively. Annealing below gelatinization temperature before cross-linking increased RS contents of prepared cross-linked starches but did not affect the swelling power. Heat treatment above gelatinization temperature increased the swelling power of cross-linked starch prepared from maize starch. The characteristics by X-ray diffractometry and scanning electron microscopy of cross-linked resistant starch were not changed by annealing.

A Literature Review On Postpartum Deep Venous Thrombosis (산후(産後) 심부정맥혈전증(深部靜脈血栓症)에 관한 문헌적(文獻的) 고찰(考察))

  • Objectives : The purpose of this study is to examine the significance of Korean Medical literature on representative symptoms of postpartum deep venous thrombosis (DVT), such as lower extremity edema and pulmonary embolism. Methods : Pattern differentiation, cause, symptoms and treatment related to DVT and pulmonary embolism as written in Korean Medical texts were studied comparatively. Results : In Korean Medical understanding, DVT could be linked to lower extremity edema due to blood stagnation, while pulmonary embolism could be linked to coughing, asthma and nose bleed due to stagnated blood. Conclusions : To prevent relapse and manage its after-effects, Korean Medical treatment for DVT and pulmonary embolism is much in need.

Cellulose-based carbon fibers prepared using electron-beam stabilization

  • Cellulose fibers were stabilized by treatment with an electron-beam (E-beam). The properties of the stabilized fibers were analyzed by scanning electron microscopy, Fourier transform infrared spectroscopy, X-ray photoelectron spectroscopy, and thermogravimetric analysis. The E-beam-stabilized cellulose fibers were carbonized in N2 gas at 800℃ for 1 h, and their carbonization yields were measured. The structure of the cellulose fibers was determined to have changed to hemicellulose and cross-linked cellulose as a result of the E-beam stabilization. The hemicellulose decreased the initial decomposition temperature, and the cross-linked bonds increased the carbonization yield of the cellulose fibers. Increasing the absorbed E-beam dose to 1500 kGy increased the carbonization yield of the cellulose-based carbon fiber by 27.5% upon exposure compared to untreated cellulose fibers.

Dental management of patients with X-linked hypophosphatemia

  • X-linked hypophosphatemia (XLH) is a hereditary metabolic disease caused by the loss of phosphate through the renal tubules into the urine, and an associated decrease in serum calcium and potassium phosphate. Its dental features include spontaneous dental abscesses that occur in the absence of trauma or dental caries. The aim of this case report was to describe the dental problems of XLH patients and to evaluate limitations in their treatment. A 14 year old male and a 38 year old female with XLH were referred to the Department of Conservative Dentistry for endodontic treatment. The dental findings were periapical abscesses without obvious trauma or caries. Conservative endodontic treatment was performed in teeth with pulp necrosis and abscess. In case 1, the treated teeth showed improvements in bone healing, without clinical symptoms. However, in case 2, the implants and the treated tooth showed hypermobility, and the final restoration was therefore postponed. Early diagnosis, periodic examinations, and communication with the patient's pediatrician are important in the dental management of patients with XLH.
