The Meaning of Pain in Labor (분만 동통의 의미)

  • From of old, labor has been accompanied by pain and much effort has been mode to eliminate or diminish the amounts of pain during labor. Little concern has yet been given to the subjective meaning of pain in labor. Recently, rates of cesarean section in Korea and in some other nations have increased rapidly and some investigators are reporting negative reactions such as anger, disappointment and feeling of loss due to lack of control over labor and its pain. These findings are thought to suggest that control of labor and its pain gives some meaning to the laboring woman. Thus the investigators sought to discover the meaning of pain during labor for Korean women. Specific objectives of this study were to explore the meaning of pain in labor to the mothers, their reactions to the experience of labor add their preference for delivery method. The subjects of this study were 95 mothers who delivered their babies in hospital from September 989 to May 1990 : 45 gave birth by vaginal delivery, and 50 by cesarean section. Data were gathered through direct interviews by the investigators, and questions were focused on five areas i.e. mothers' feelings about delivery and their babies, their feelings about of having more children, the most difficult aspect about this labor and delivery, and what they thought the differences were between vaginal delivery and cesarean section. After interview, mothers' answers were summarized, and classified according to the degree of positive or negative attitude. To ascertain the difference in meaning of labor pains and reaction to delivery experience between mothers delivered vaginally and by cesarean section a Median test was done using an SAS. Results were as follows. 1. More mothers who had delivered vaginally realized that they “have became a mother” than those who had a cesarean section(X$^2$=8.409, df=3, p=0.038). 2. Immediate reaction to their delivery experience was more positive for mothers who had a cesarean section. 3. These Korean mothers expressed preference for vaginal delivery. Suggestions for further research on the meaning for mothers of their experience of labor, and on the meaning of pain for mothers who have a cesarean section were made.

Childcare Teachers' Perception of their Director's Servant Leadership and the Mediating Role of the Internal-External Control Relationship on Childcare Teachers' Emotional Labor (보육교사가 지각한 원장의 서번트리더십과 보육교사의 정서노동의 관계에서 내-외통제성의 매개효과)

  • The purpose of this study was to explore mediating effects of internal-external control between directors' servant leadership and teachers' emotional labor. The subjects of the study were 425 childcare teachers in Seoul, Gyeonggi province and Incheon in Korea. The statistics program used to analyze the data was SPSS(version +18.0). The statistics methods applied were the Pearson correlation coefficient, simple linear regression, and multiple linear regression. The major results of this study were summarized as follows: First, emotional labor and internal control caused by the servant leadership style were found to be positively correlated. Second, servant leadership affected the ability of the teachers to act naturally. These effects were mediated in part by the amount of internal-external control that was shown. Third, servant leadership affected the internal behaviors of the teachers. These effects were mediated in part by amount of the internal-external control that was shown.

Effects of Doula-type-delivery Nursing Care on Plasma $\beta$-Endorphin, Serum Cortisol related to Delivery Stress during Labor, and Postpartum Status Anxiety of Primipara - Clinical trial - (Doula식 분만간호가 초산부의 분만중 분만스트레스로 인한 혈장 베타엔돌핀, 혈청 코티졸 및 분만후 상태불안에 미치는 효과 - 임상 실험연구 -)

  • This study was designed to verify the effect of Doula-type-delivery nursing care on plasma $\beta$-endorphin, serum cortisol, related to delivery stress during labor, and postpartum anxiety of primipara by a quasi experiment(nonequivalent control group pretest-posttest design), from December, 1999 to August, 2000. The subjects of this experiment consisted of sixty eight primipara, with single gestation, full term, uncomplicated pregnancies, thirty three for the experimental group and thirty five for the control group. Their mean age was 26.1 years for the experimental group and 25.5 years for the control group. Their mean gestation period was 39.7 weeks for the experimental group and 40.1 weeks for the control group. As treatment, Doula-type-delivery nursing care was given for the experimental group. Data assessed plasma $\beta$-endorphin, serum cortisol during labor, and anxiety during postpartum. Plasma $\beta$-endorphin, serum cortisol were measured in the latent phase before treatment(pre-test) and the transition phase after treatment(posttest). Also, anxiety was measured in the latent phase before treatment(pre-test) and 24 hours postpartum after treatment(posttest). Data was analyzed by t-test, $x^2$-test, Repeated measures ANOVA with SAS Program. The results of this study were as follows; 1. Plasma $\beta$-endorphin was significantly elevated in the experimental group who were cared for with Doula-type-delivery nursing care during labor(P=.0463). 2. No significant group effects were found, but significant time effects were found for serum cortisol. 3. The postpartum anxiety of the experimental group was significantly lower than the control group(P=.0110). In conclusion, these findings indicate that Doula-type-delivery nursing care during labor could be effective in increasing maternal plasma $\beta$-endorphin and decreasing postpartum anxiety. Doula-type-delivery nursing care during labor could be applied as an effective nursing treatment for primipara.

Effect of Family-Participated Delivery in a Labor Delivery Room on the Childbirth of Primiparas (가족분만실에서의 가족분만이 초산모의 분만에 미치는 효과)

  • The purpose of the present study is to consider its effect on the childbirth of a woman. This is a quasi-experimental study with nonequivalent control group post-test design. The subjects of this study are 60 primiparas (30 in the control, and another 30 in the experimental group) who have had a regular prenatal care from February 5 to March 20, 2002, in an outpatient obstetrics and gynecology of S university medical center located in Seoul. The result is as follows: 1. The hours of labor pains in the entire delivery period: the average hours are 7 hr. 9 min. in the experimental group, and 10 hr. 39 min. in the control group. The hours of labor pains are shorter in the woman with a family delivery experience in LDR. The difference is statistically significant (t=-3.34, p=.001). 2. The degree of pains in the entire delivery period: the average degree is 7.38 in the experimental group, and 7.68 in the control group. The degree of labor pains are lower in the woman with a family delivery experience in LDR. But, the difference is statistically insignificant (t=-0.86, p=.396). 3. The perceptions of the delivery experience: the average score of the perception is 73.63 in the experimental group, and 63.57 in the control group. The women with a family delivery experience in LDR have more positive perception of the delivery procedure, and, the difference is statistically significant (t=4.65, p=.000). In summary of the above result, a family-participated delivery in LDR is proved to be an effective nursing intervention that shortens the hours spent in the delivery procedure and promotes positive perceptions of the delivery experience.

Effects of Aromatherapy on Labor Pain & Perception of Childbirth Experience (아로마테라피가 분만동통 및 분만경험지각에 미치는 효과)

  • Purpose: The purposes of this study were to verify the effects of aromatherapy on labor pain and perception of the childbirth experience. Methods: This study was a nonequivalent control group pretest-posttest design. The subjects of this experiment consisted of forty eight primiparas without problems during the gestation period. Twenty four primiparas in the experimental group were given general obstetric nursing care with aromatherapy every two hours. Twenty four primiparas in the control group were given general obstetric nursing care only. Data was collected for labor pain measured by a labor pain expression scale, uterine contraction activity measured by Montevideo units in the latent phase, active phase, and transition phase and the perception of childbirth experience 24hours after birth. Data was analyzed by t-test, and repeated measures of ANOVA with an SPSS program. Results: No significant group effects were found, but significant time effects were found for labor pain, and uterine contraction activity. There was no significant difference in postpartum mothers' perception about childbirth. Conclusion: In this study, effects of aromtherapy decreasing labor pain expression, and increasing the perception of childbirth was not found.

Effects of treatment with San-Yin-Jian(SP-6) acupressure for labor women on labor pain, length time for delivery and anxiety - A clinical trial pilot study - (산부의 분만통증, 분만소요시간과 불안에 미치는 삼음교 지압의 효과 - 임상 실험 예비연구 -)

  • The study examined the effects of San-Yin-Jiao(SP-6) acupressure treatment on labor pain, length of delivery and anxiety for women in the labor. The effects of using SP-6 acupressure were evaluated by comparing two groups, a SP-6 acupressure group (22) and a control group (17), for a total of 39 women in labor who had a normal vaginal delivery. Data were collected using a structured questionnaire which included general characteristics, a subjective labor pain scale, measurement of duration of delivery time and a subjective anxiety scale. Data were collected before treatment (pre) and after treatment (post). The results of this study are summarized as follows : 1. The post-scores for total labor pain increased over the pre-scores but the difference between the two groups was not statistically significant(p=0.219). Wilcoxon signed rank test of the difference in pre-post labor pain scores for the SP-6 acupressure group was not statistically significant (p=0.081) but the increase for the control group was statistically significant (p=0.001). 2. The length of time for the delivery in the group which had the SP-6 acupressure was shorter (143.91${\pm}$67.77) than the control group (197.94${\pm}$89.64). The difference between the two groups was statistically significant (p=0.028). 3. The post-scores for anxiety increased over the pre-scores but the difference between the two groups was not statistically significant (p=0.426). The scores of SP-6 acupressure group did not show a significant increase by the Wilcoxon signed rank test (p=0.194) but in the control group showed a significant increase (p=0.008).This study showed that SP-6 acupressure was effective in relation to labor pain, length of time for delivery and anxiety for labor women. But it is necessary to replicate the study with a larger number of participants to generalize the results.

Effects of San-Yin-Jiao(SP6) Acupressure on Labor Pain, Delivery Time in Women during Labor (삼음교(SP6)지압이 산부의 분만통증, 분만소요시간에 미치는 효과)

  • Purpose: The study was done to examine the effects of San- Yin-Jiao(SP6) acupressure treatment on subjective labor pain, length of delivery time in women during labor. Method: The study design was a randomized controlled clinical trial study using a double-blinded method. Data were collected using a structured questionnaire, a subjective labor pain scale and measurement of delivery time. The experimental group(n=29) was received SP6 acupressure and control group(n=29), SP6 touch for the duration of each uterine contraction, during 30 minutes after 3cm dilatation of cervical os. Result: The subjective labor pain scores was significantly different between the two groups(p=0.042). The total length of delivery time in the group which had the SP6 acupressure was shorter than SP6 touch group (p=0.036). Conclusion: These findings showed that SP6 acupressure was effective related to labor pain, length of time for delivery. SP6 acupressure during labor could be applied as an effective nursing treatment.

  • The role of the garment and textile industry is particularly important in the economies of many countries in the context of international goods trade. There is no denying that the garment and textile industry contributes significantly to the economic growth in the global economy. The study seeks to investigate the relationship between control variables and Vietnamese consumers' intention to buy Chinese garment products. While previous research has found some control variables influencing consumers' intention to buy products, little research has been done about the influence of control variables on consumers' intention to buy Chinese garment products, in developing countries like Vietnam. In particular, the textile industry plays an important role in export, but outsourcing is accounting for a high proportion of trade, hence, it is necessary to increase innovation to increase consumers' intention to buy domestic garment products. The data is collected from a survey of 406 Vietnamese consumers' in Hanoi city and Ho Chi Minh City. The methodology includes a mixed-method, i.e. qualitative method and quantitative method. The quantitative method applies SPSS analysis to measure the control variables' influence on Vietnamese consumers' intention to buy Chinese garment products. The results identify 1 control variable that impacts Vietnamese consumers' intention to buy Chinese garment products, which is domicile.

Effects of Paters' Duola Touch during Labor on the Paternal Attachment and Role Confidence to Neonate and Couple Attachment (아버지의 듀라터치 참여 분만이 부성애착, 부성역할 자신감, 부부애착에 미치는 영향)

  • Purpose: The purpose of this study was to identify the effects of paters' doula touch during labor on paternal attachment and role confidence to neonate and couple attachment. Methods: The participants were 60 couples of laboring woman and her husband who were hospitalized from May 2010 to december 2010. Among 60 couples, 30 couples were assigned to an experimental group and the other 30 couples to a control group. All of them had any other complications of pregnancy. During one month, the paters' duola touch was applied to the experimental group of 32~42 gestation once a week and be in labor. Data were analyzed using SPSS/WIN 14.0 Win program. Results: The scores for role confidence to neonate and couple attachment in the experimental group were significantly higher than those for the control group. The paternal attachment score of the experimental group was higher than that of the control group, but it was not significant. Conclusion: From these findings, it is concluded that paters'doula touch during pregnancy and labor can be helpful to improve attachment to the neonate and his/her spouse.

Effects of Music Therapy on Stress of Preterm Labor and Uterine Contraction in Pregnant Women with Preterm Labor (음악요법이 조기진통 임부의 조기진통 스트레스 및 자궁수축에 미치는 영향)

  • Purpose: The purpose of this study was to test effects of music therapy on stress due to preterm labor and uterine contraction in pregnant women with preterm labor. Methods: An experimental research design was used. Participants were 35 pregnant women with preterm labor who were between 20 to 37 weeks of pregnancy: control group (n=18) received only tocolytic drugs, while experimental group (n=17) received additional music therapy. In the experimental group, Traumerei was applied before Non-Stress Test (NST) from the second day to fifth day after admission as music therapy. Results: There was a statistically significant difference in stress due to preterm labor (z=-3.368, p<.001) between the two groups. Conclusion: The music therapy is an effective method for reducing the stress of pregnant women with preterm labor.
