Statistical Study of Conservative Treated Herniated Intervertebral Lumbar Disc (요추 추간판탈출증 환자의 보존적 치료에 대한 통계적 연구)

  • Objectives : The object is to study a clinical effect regarding the conservative treatment of the patient who was diagnosed as the herniated Intervertebral lumbar disc(HIVD) and hospitalized in the oriental medical hospital and clinical study about the type of the HIVD. Methods. The clinical studies were done on 65 cases (hospitalized in Dept. of Oriental Rehabilitation Medicine, jaseng on oriental medical hospital from June 2001 to May 2002) who complained of low back pain or sciatica and diagnosed as HIVD on CT(computerized tomography) or MRI(magnetic resonance imaging). They were given both oriental conservative treatment and exercise during the hospitalization period. We investigated and observed 20 items about sex, age, weight, height, occupation, smoking. obesity, duration of disease, clinical symptom and treatment duration in basis of medical recording which was drawn up at patient hospitalization. We classified HIVD with 4 types(Bulging. Protruded. Extruded, Sequestered) and decided the case which simultaneously had 2 overs with Mixed type. The treatment evaluation standard was classified with 4 branches; Excellent. Good, Fair and Poor. Results & Conclusions : The treatment result of over 'fair' is bulging type 85%, protruded type 92.7%, extruded type 100% and the mixed type was 85.1 %. The effect was good in order of bulging, mixed, protruded and extruded type. It appeared most plentifully with 30 people in L4-5 and L5-S 1 disc herniation type.

Clinical and Radiological Outcomes of Anterior Cervical Interbody Fusion Using Hydroxyapatite Spacer

  • Objective : This is retrospective study of clinical and radiological outcomes of anterior cervical fusion using Bongros-$HA^{TM}$ (BioAlpha, Seongnam, Korea) which is a type of synthetic hydroxyapatite (HA) spacer to evaluate the efficacy in its clinical application and usefulness as a reliable alternative to autograft bone. Methods : Twenty-nine patients were enrolled in this study and 40 segments were involved. All patients were performed anterior cervical interbody fusion using HA spacer and plating system. Indications for surgery were radiculopathy caused by soft-disc herniation or spondylosis in 18 patients, spondylotic myelopathy in 1 patient, and spinal trauma in 10 patients. Cervical spine radiographs were obtained on postoperative 1day, 1week, and then at 1, 2, 6, and 12 months in all patients to evaluate intervertebral disc height, and the degrees of lordosis. Cervical computed tomography was done at postoperative 12 month in all patients to confirm the fusion status. The mean period of clinical follow-up was 17 months. Results : Complete interbody fusion was achieved in 100% of patients. Preoperative kyphotic deformities were corrected in all cases after surgery. Intervertebral disc height was well maintained during follow up period. There were no cases of graft extrusion, graft deterioration and graft fracture. Conclusion : HA spacer is very efficient in achieving cervical fusion, maintaining intervertebral disc height, and restoring lordosis. When combined with the placement of a cervical plate, immediate stability can be achieved and graft related complication can be prevented.

Hemilaminectomy for Herniated Discs at the Cervicothoracic Junction in a Beagle Dog (편측추궁절제술을 이용한 비글견의 경흉연접부 추간판 탈출증의 치료)

  • A 7-year-old, female Beagle dog presented with a primary complaint of severe neck pain and paretic gait. A lesion was suspected between C6 and T2 on the basis of neurological examinations. Magnetic resonance imaging of the cervicothoracic spine revealed herniation of disc material from the C7-T1 intervertebral space resulting in marked spinal cord compression. Hemilaminectomy was performed to treat the intervertebral disc disease at C7-T1. Treatment was effective and the clinical condition of the dog was remarkably improved. This result suggests that hemilaminectomy is an effective option for surgical treatment of intervertebral disc disease at the cervicothoracic junction in dogs.

Assesment of Bee-venom Acupuncture Effect on Herniated Disc Patients by Rating Scale (요추간판탈출증 환자 22례에 대한 봉독약침 치료의 요통 평가지수를 이용한 임상보고)

  • Herniation of intervertevral disc(HIVD) is the most common disease causing low back pain. Acupuncture, electroacupuncture and moxibustion has been used for treatment of HIVD. Bee venom (BV) also has been traditionally applied to relieve pain. Recently, it has been reported that BV is effective in treating HIVD. But more objective methods were required to assess the efficacy of BV on HlVD. So we assessed of BV-acupuncture effect on herniated disc patients by use of rating scale for low back pain. We researched 22 patients who were treated at Department of Acupuncture & Moxibustion, Kyung Hee Medical Center from January 1999 to June 1999. The patients were diagnosed by MRI as having a herniated disc. The results of treatment using rating scale for low back pain in HIVD showed that $43.77{\pm}14.39$ point of pre-treatment score and $84.5{\pm}3.97$ point post-treatment score. The improvement index showed $0.43{\pm}0.15$. Especially, clinical score of neurologic deficits showed most highest improvement index of $0.7{\pm}0.45$. Assessment of BV-acupuncture Effect by MRI finding were showed that protruded disc and extruded disc got more higher improvement index than bulging disc. Among 22 patients, satisfactory assessment of BV-acupuncture effect showed that 15(68.2%) were excellent, and 7(31.8%) were good. The result suggest that BV-acupuncture is good method for treatment of HIVD.

Clinical Implication of Prostaglandin E2 Content in Lumbar Disc Disease (추간판내 Prostaglandin E2 양의 임상적 의의)

  • Objective : A prospective biochemical assay of prostaglandin $E_2$ content in symptomatic lumbar disc materials was done in order to clarify the pathogenesis of lumbar radiculopathy. Patients and Methods : Forty-eight disc specimens were purified by a standard solid-phase extraction procedure and analyzed by an enzymelinked immunosorbent assay for prostaglandin $E_2$. Clinical and anatomic correlations were evaluated with analysis of variance and t-test. Results : Acute herniated lumbar disc diseases tended to be associated with a higher prostaglandin $E_2$ content than degenerative lumbar disc disease. Sequestered discs tended to be associated with a higher prostaglandin $E_2$ content than extruded discs, which also showed higher prostaglandin $E_2$ content than protruded ones. A sciatica and positive straight leg raising test appeared to be associated with a higher prostaglandin $E_2$ content than a negative test. Conclusion : This result suggests that the level of prostaglandin $E_2$ would be correlated with clinical symptom and sign in the inflammatory process of lumbar disc herniation.

The Comparative analysis of X-ray film on Lumbar Scoliosis and HNP by Barge Method (Barge 방식에 의한 요추부 척추측만증과 추간판 탈출증의 X-ray 비교분석)

  • Objective: The purpose of this study is to evaluate the disc block subluxation for lumbar scoliosis and herniation of nucleus pulpous (HNP). Methods: We used AP & Lateral view X-ray for patients. And we measured of disc wedge angle, vertebral body rotation to evaluate the typical and atypical disc block subluxation. Results: On the analysis of the lateral view X-ray, 4th lumbar intervertebral disc angle (I.V.D angle) showed $4^{\circ}$, $5^{\circ}$, $3^{\circ}$, $0^{\circ}$ in the cases. On the 4th lumbar analysis of the AP view X-ray, lumbar scoliosis showed right disc wedge angle was $11^{\circ}$ (case 1), $17^{\circ}$ (case 2) and left vertebral body rotation was 13mm, 6mm. Lumbar HNP showed left disc wedge angle was $5^{\circ}$ (case 3), $4^{\circ}$ (case 4) and left vertebral body rotation was 2mm, 4mm. Conclusions: Disc block subluxation has been in lumbar scoliosis, but not been in lumbar HNP.

Suggestion of Flexion Distraction Technique in Lumbar Herniated Intervertebral Disc(HIVD) (요추추간판탈출증에서의 굴곡신연기법에 대한 제언(提言))

  • Objectives : This study aimed to introduce a new traction therapy and proposes that a new Chuna manual therapy technique could be implemented. Methods : The patient is placed in the prone position and their ankle is fixed to the fixing table. Next, clinicians contact on the vertebral spinal process above herniated disc region with the tissue pulled head part of scaphoid bone and the lower part of the pelvis is pulled horizontally to the foot. Conclusions : It was recently identified that traction while maintaining lumbar lordosis is more effective for lumbar disc herniation and is associated with fewer side effects. Chuna manual therapy also uses a technique of lumbar flexion-distraction manipulation to treat HIVD. further research is required.

  • Intervertebral disc herniation is frequently encountered in radiological practice. Sequestered disc herniation occurs when the disc material undergoes degeneration and completely loses continuity with the parent nucleus pulposus. Sequestered discs can reside within and outside the spinal canal, exerting a mass effect on adjacent structures, compressing nerve pathways, and eliciting a range of clinical symptoms. In particular, sequestered discs within the dura cannot be identified without durotomy. Therefore, precise preoperative localization is crucial for surgical planning. On MRI, the signal intensity of the sequestered disc may vary due to independent degeneration processes. Additionally, most sequestered disc fragments show varying degrees of peripheral enhancement depending on the degree of angiogenesis and granulation around the isolated tissue. In this article, we review various imaging findings and the location of the sequestered disc to provide patients with an accurate diagnosis and appropriate treatment direction.

A Case Report on a Patient with Acute Herniated Lumbar Disc due to Coughing Treated with Megadose Pharmacopuncture and Combined Korean Medicine (기침으로 급성 악화된 요추 추간판 파열 환자의 대용량 약침 및 복합 한방치료 호전사례 1례)

  • Objective: The purpose of this study was to report the radiological and symptomatic changes in a patient with acute herniated lumbar disc due to coughing, treated with megadose pharmacopuncture and combined Korean medicine. Methods: MRI examination diagnosed an acute lumbar disc herniation by coughing. We performed a combination treatment of megadose pharmacopuncture and integrated Korean medicine. NRS, ODI, EQ-5D, SLR test, and big toe extension test were checked on admission, after two weeks, on discharge, and after four months to confirm symptom improvements. Results: In the acute stage of lumbar intervertebral disc rupture, megadose pharmacopuncture combined with Korean medicine treatment showed improvement in NRS, ODI, EQ-5D, SLRT, and big toe extension tests. In addition, absorption of the herniated intervertebral discs was observed through an MRI scan on discharge. Conclusions: For patients with herniated lumbar disc acutely deteriorated by coughing, a combination of megadose pharmacopuncture with Korean medicine treatment may be a solution.
