The Refinement Project of Health Insurance Relative Value Scales: Results and Limits (건강보험 상대가치 개정 연구의 성과와 한계)

  • Relative value scales introduced in 2001 remarkably improved health insurance fee schedule, but current relative value scales have many problems. In the beginning the government intended to introduce 'resource based relative value scales(RBRVSs)' like USA, but political adjustment of RBRVS studied in 19.17 weakened the relationship between relative value scale and resource consumption. So unbalance of health insurance fees are existing till now. Also relative value was not divided to physician work and practice expense, and malpractice fee was not divided separately. To correct the unbalance of current relative value scales, the refinement project of health insurance relative value scales started in 2003. The project team divided relative value scales into three components, which are physician work, practice expense, malpractice fee. Physician work was studied by professional organizations like Korean medical association. To develop the practice expense relative value, project team organized clinical practice expert panels(CPEPs) composed of physicians, nurses, and medical technicians. CPEPs constructed direct expense data like labor costs, material costs, equipment costs about each medical procedures. The practice expense relative values of medical procedures were developed by the allocation of the institution level direct & indirect costs according to CPEPs direct costs. Institution level direct & indirect costs were collected in 21 hospitals, 98 medical clinics, 53 dental clinics, 78 oriental clinics, and 46 pharmacies. The malpractice fee relative values were developed through the survey of malpractice related costs of hospitals, clinics, pharmacies. Putting together three components of relative values in one scale, the final relative values were made. The final relative values were calculated under budget neutrality by medical departments, that is, total relative value score of a department was same before and after the revision. but malpractice fee relative value scores were added to total scores of relative values. So total score of a department was increased by the malpractice fee relative value score of that department This project failed in making 'resource based' relative value scales in the true sense of the word, because the total relative value scores of medical departments were fixed. However the project team constructed the objective basis of relative value scale like physician's work, direct practice expense, malpractice fee. So step by step making process of the basis, the fixation of total scores by the departments will be resolved and the resource based relative value scale will be introduced in true sense.

A Rating Method for the Estimation of the Additional Overhead Expenses incurred by Schedule Extension in Public Construction Projects (공공건설공사의 공기연장에 따른 추가간접비 산출을 위한 요율방식 제안)

  • In domestic public construction, disputes are increasing due to differences among stakeholders regarding contract price adjustment. In particular, the actual cost-plus fee for overhead costs due to the schedule extension cannot be agreed upon at the administrative phase, and most of them seek judicial judgment. Thus, this study aims to propose a 'sufficiently satisfactory' alternative to reach an agreement before disputes in order to minimize disputes related to the calculation of additional overhead costs. To this end, this study proposes three alternatives based on the rate method. Firstly, when calculating additional overhead costs, it is not calculated as an actual cost-plus-fee method, but as a rate compared to direct labor costs among net direct costs. Secondly, the calculated indirect labor costs are compensated for up to the legal maximum of legal limit costs such as general management costs, profits and so on. Thirdly, it reflects overhead costs increased or decreased due to change orders. Risks were analyzed by collecting expert opinions on the proposed methods and applying actual cases. Finally, as a result of investigating the level of consensus for each stakeholder, it was confirmed that all stakeholders could agree regardless of the size of the company. The result of this study is expected to as a useful tool among stakeholders in the construction fields that can be able to easily agreed upon.

Factors Associated With Obstructive Pattern Spirometry In Shipyard Workers (조선소 근로자들의 폐쇄성 폐기능 저하와 관련된 요인)

  • Objectives: Alongside smoking, occupational exposure is an important risk factor for chronic obstructive pulmonary disease. The purpose of this study was to evaluate factors associated with occupational pulmonary function decline that can be used to create guidelines for the health management of shipyard workers Materials: This study analyzed spirometry from 10,597 male shipbuilding workers. Functional decline in spirometry was defined as FEV1/FVC <70% and logistic regression for work duration and occupational hazard exposure was performed Results: Among the subjects, 4.2% showed an obstructive pattern in pulmonary function. The odds ratios for hazard exposure were 1.67(indirect) and 3.54(direct), and for work duration 1.97(10-18 years), 2.29(19-27), and 5.02(28+). After adjusting for smoking and work-related factors, the odds ratios for work durations of over 10 years were 1.73(10-18 years), 1.99(19-27), and 4.09(28+), but for hazards exposure was 1.71(direct) alone after adjustment. Conclusions: Occupational COPD is insidious and chronic, and thus long-term hazard exposed(especially over 10 years) shipyard workers with functional decline in spirometry need to prevent and manage COPD. This study is important for establishing guidelines to manage hazard exposure among shipyard workers and prevent COPD.

The Variation Factors of Severity-Adjusted Length of Stay in CABG (관상동맥우회술 시행환자의 중증도 보정 재원일수 변이에 관한 연구)

  • Our study was carried out to analyze the variation factors of severity-adjusted length of stay(LOS) in coronary artery bypass graft(CABG). The subjects were 932 CABG inpatients of the Korean National Hospital Discharge In-depth Injury Survey from 2004 through 2008. The data were analyzed using $x^2$ test and the severity-adjusted model was developed using data mining technique. The results of the study were as follows: male(71.1%), older than 61 years of age(61.6%), more than 500 beds(92.8%) and admitting via ambulatory care(70.0%) appeared to have higher rate than otherwise. In-hospital mortality of CABG inpatients was 2.8%. In addition, 46.4% of the patients received their care in other residence. The angina pectoris(45.6%) was found to be the highest in principle diagnosis, followed by chronic ischemic heart disease(36.9%) and acute myocardial infarction(12.0%). We developed severity-adjusted LOS model using the variables such as gender, age and comorbidity. Comparison of adjusted values in predicted LOS revealed that there were significant variations in LOS by location of hospital, bed size, and whether patients received the care in their residences. The variations of LOS can be explained as the indirect indicator for quality variation of medical process. It is suggested that the severity-adjusted LOS model developed in this study should be utilized as a useful method for benchmarking in hospital and it is necessary that national standard clinical practice guideline should be developed.

Association between Mental Health with Oral Health Behaviors in Korean Adolescents (우리나라 청소년의 정신건강이 구강보건행태에 미치는 영향)

  • Objectives: The purpose of the study was to investigate the effect of stress, depressive mood and suicide ideation with oral health behavior in Korean adolescents. Methods: The subjects were 57,904 adolescents who were selected from the web-based survey of the 10th(2014) Korean Youth Risk Behavior of Korean Center for Disease Control. Data were analyzed by SPSS 18.0. A web-based self-reported questionnaire was completed by 72,060 middle and high school students. The subjects consisted of 28,269 man students(48.8%) and 29,635 women students(51.2%) from 500 middle schools and 400 high schools. Results: Multiple logistic regression analysis after adjustment to demographic characteristics and stress, depressive mood and suicide ideation by man and women of middle, high school students the subjects whose have had different oral health behaviors. low level of stress, do not think about suicide ideation, they were increased their oral brushing frequency of yesterday($$3{\geq_-}$$) with brushing after lunch(p<0.05). alternatively subjects whose do not have depressive mood, they were decreased their oral health behaviors(p<0.05). Conclusions: According to gender by middle and high school students are mental health of the youth, which has direct and indirect effects on physical health, is closely associated with their oral health according to oral health behaviors.

The Digital Model Generation for the Human Body Measurement (인체 계측을 위한 수치모형생성)

  • The size and shape of the human body has been an important factor not only in anthropology but in integration of designs aimed for daily use. Although the three-dimensional measurements have been proposed and obtained introducing new methods, there still remains many problems concerning how to analyze and evaluate the results. In this study, we have got the internal and external orientation factors, image coordinates from the ac-quired photographs, have calaulated the object coordinates through Bundle Adjustment, have matched multi-images. have drawn the object in the computer, and smoothly handled the surface of the body model using the Au-to-CAD system or graphic processing utility then we could generate the digital model of human body. The. results show that once digital modeling of the body is generated. the information which a designer needs can be acquired and the adequate calculations for many application fields can be done easily and many geometrical properties could be extracted. Therefore we propose the possibility of indirect human body measurement through digital model generation.

  • This study attempts to find out the characteristics of digital currency and currency transformation through the analytical descriptions of the literature. In the early days of the emergence of new currency, market-oriented autonomous monetary adjustment was made along with various attempts by the private sector, and then government-centered central currency management and coordination were made for the national monopoly of profits and power. Digital currency can be seen as the emergence of a new form of money that will bring about paradigm changes. CBDC can be divided into direct and indirect types. CBDC is expected to require a strategic approach by the government or firm as it will bring about changes in the ecosystem of related industries. Libra is a stablecoin designed to minimize price fluctuations, and if it succeeds in commercializing it, it is expected to bring about revolutionary changes in the financial industry around the world.

THE ACCURACY OF DIGORA SYSTEM, AN INDIRECT DIGITAL RADIOGRAPHIC SYSTEM, IN DETERMINING THE WORKING LENGTH (근관장 측정시 구내 디지털 방사선 시스템인 Digora System의 정확도에 관한 연구)

  • An accurate working length is an essential factor in the success of endodontic treatment. There are several methods for determining working length; electronic apex locator, tactile sense by clinician, and radiography. Among these methods, the most commonly used method is radiography. But this method requires excessive radiation, long developing time, and many equipments. In additon, it could give an image distortion and two dimensional image. To improve these disadvantages, an intraoral digital radiographic system (Digora$^{(R)}$) which use an imaging plate instead of a film, was developed. The purpose of this investigation was to compare Digora imaging with conventional radiography in determining the working length. Maxillary first or second molars of human dried skull were used. Files were inserted into canals at randomly selected lengths, from 2mm short of the radiographic apex to 2rnm beyond. Radiographs and Digora images(Digora positive and Digora negative) were evaluated to determine the adjustment needed to place the file 0.5mm from the radiographic apex. The results were as follows ; 1. There was no significant difference in accuracy between those evaluated in ${\pm}0.5mm$ and those accurately evaluated in the 3 images. 2. When comparing the accuracy of each image without distinguishing the 3 images, in the group accurately determined within ${\pm}0.5mm$, the mesiobuccal group showed significantly higher accuracy compared to the palatal group(p<0.05).

  • this study aims to analyze causality on the effect of job satisfaction on vocational adaptation of ex-prisoners through empirical study and to verify the moderated effect of self-esteem and the Moderated Mediation effect of the job stress. According to the analysis, job satisfaction have a static effect on Vocational Adjustment and the self-esteem have a mediated effect in this process. also, the indirect effects of job satisfaction on vocational adaptation by self-esteem are moderated by job stress. advanced researchs study that job-related emotions affect vocational adaptation and maintenance, but psychological factors affecting vocational adaptation is not dealt with in depth, and what factors actually moderate this process is not explored in detail. also, becausea there are few studies related to the job of ex-prisoner, this study carried out full-fledged exploration and verification of it.

Effects of Perceived Stigma on Life Satisfaction and Self-Esteem of the Mental Illness (지각된 낙인이 정신장애인의 삶의 만족과 자아존중감에 미치는 영향)

  • According to modified labeling theory, because persons with mental illness internalize the stigma of the public, they anticipate discrimination or rejection by others. Such reactions may have negative effects on both psychological and social functioning. We propose that perceived stigma are central to understanding the poor adjustment of mental patients. This study was conducted by the survey with 377 mental patients in order to discover the pathways by which perceived stigma affected life satisfaction and self-esteem. The major findings of the study were : 1) Perceived stigma has a direct effect on life satisfaction as well as indirect effect through patients' experience of discrimination. 2) Perceived stigma has no direct effect but indirectly affected self-esteem through patients' experience of discrimination and internal attribution of discriminatory situations. Based on these findings, consumer-initiated anti-stigma campaign were discussed.
