Measuring Thresholds of Serum Lipid Level related to Hypertension and Age Using Receiver Operating Characteristic Curve in Korean Adult

  • PURPOSE: The purpose of this study was to determine the optimal thresholds of serum lipid level related to hypertension according to age in Korean adults. METHODS: In total, 564 adults who visited the health examine center in general hospital were included. The blood pressure and lipid profiles of low density lipoprotein cholesterol(LDL), high density lipoprotein cholesterol (HDL), triglyceride(TG) were measured. Receiver operating characteristic (ROC) curve were used to establish optimal thresholds between blood pressure and lipid profiles. RESULTS: The optimal TG cutoff value were 110.50(mg/dl) in the 18-39 and 81.50(mg/dl) in the 40-59 age group, and optimal cutoff value of LDL were 126.50(mg/dl) in the 40-59 age group and 111.00(mg/dl) in the 60 and over age group. There was a negative correlation between HDL and hypertension, a higher HDL decreased hypertension. The optimal cutoff value of HDL was 49.50(mg/dl) in the 18-39 age group. CONCLUSION: The thresholds of hypertension were lower in LDL and TG with aging. This result indicated that elderly people needs to concern more about their lipid profiles to maintain healthy cardiovascular function.

Postoperative Hemodynamic Changes of VSD with Pulmonary Hypertension (폐고혈압을 동반한 심실중격결손증의 술후 혈류역학 변화)

  • This series compromised 31 patients with pulmonary hypertension of 282 patients of ventricular septal defect(VSD) who underwent operation at the department of Thoracic and Cardiovascular Surgery in Chonnam University Hospital, from January, 1986 to December, 1991. Pulmonary hypertension was noted in 59 of 280 cases of VSD. Of them, 31 cases underwent cardiac catheterization on postoperative 8th to 77th month. Age at operation was ranged from 10 months to 29 years (mean 9.13 years). 17 patients were male and 14 patients were female. Results of follow-up studies were as follows: Cardiothoracic ratio was decreased from 0.59${\pm}$0.04 to 0.54${\pm}$0.03 (p=NS). Postoperative systolic pulmonary arterial pressure (PAPs), mean pulmonary arterial pressure (PAPm), and systolic right ventricular pressure (RVPs) were decreased significantly (p<0.001). And also Rp/Rs was decreased from 0.37${\pm}$0.21 to 0.14${\pm}$0.06 (p<0.02). However, systemic arterial pressure (SAP), right atrial pressure (RAP), and pulmonary capillary wedge pressure (PCWP) were changed insignificantly. There were significant relations of follow-up period with the decrement of PAP(p<0.005). In contrary, ther were no relations between the decrement of PAP and the age at operation. These data suggested that the long-term hemodynamic changes remained to be determined in some of the patients, even though they Were asymptomatic, with pulmonary hypertension.

Relationships of Depression Symptom, Self-Esteem, and Stress to Health-Related Quality of Life in Patients with Hypertension Registered to a Community Health Center (보건소를 이용하는 고혈압 환자의 우울증상, 자아존중감, 스트레스 및 건강관련 삶의 질 관계)

  • Purpose: This study was conducted to examine the relationships of depression symptom, self-esteem, and stress with health-related quality of life(HRQOL) in patients with hypertension registered to a community health center. Methods: This study was a correlational survey using a convenience sampling. A total of 110 patients diagnosed with hypertension were recruited from a health center in Gyeonggi-do. The questionnaires used were the Cardiovascular Disease Specific-HRQOL questionnaire, Center for Epidemiologic Studies Depression, Rosenberg Self-Esteem Scale, and Perceived Stress Scale. The acquired data were analyzed using IBM SPSS version 22.0. Multiple linear regression analysis was performed. Results: Moderate depression symptom(${\beta}=-.368$, p<.001), severe depression symptom (${\beta}=-.450$, p<.001), stress(${\beta}=-.339$, p=.001), and gender(${\beta}=-.148$, p=.049) were significant predictors for the HRQOL. Multiple linear regression showed that 51.8% ($R^2=.518$) of the variance in the HRQOL was explained. Conclusion: Based on these results, development of an intervention or education program, to decrease depression symptoms and stress is recommended. This may improve the HRQOL in patients with hypertension registered to a community health center.

One clinical case of hypertension, pulmonary with hemoptysis (객혈을 동반한 폐동맥고혈압환자에 대한 치험 1예)

  • Objectives : There has been little report on the treatment of pulmonary hypertension on the basis of Oriental medicine. Several weeks ago we had a case of pulmonary hypertension, and found something in the treatment of the disease. A 60-year old man with hemoptysis was admitted to the Kyunghee Oriental Medical Center on April 23, 2002 and discharged May 4, 2002. During that time, we saw good results. Methods : On the oriental medical basis that Chuongsangboha-tang (Qingshangbuxia-tang) has an effect on chronic pulmonary disease, we treated him with that medicine three times a day for 12 days along with Roxithromycin 150mg/tab 1T qd, Digoxin 0.25mg/tab 0.5T qd, and Bambuterol Hel 1T qd. He was also given acupuncture therapy daily with tonifying SP3(太白), L9(太淵) and sedating H8(少府), L10(魚際). Results : The following data was observed from the therapy : hemoptysis volume decreased by 90%, general weakness by 40%m and ABGA results improved. Conclusions : As we managed pulmonary hypertension, we came to the conclusion that the oriental medical therapy can be of assistance to western medical therapy and can mitigate hemoptysis.

Self Care Behavior according to the Risk of Vascular Complications in Elderly Women with Hypertension (고혈압 여성노인의 혈관합병증 위험정도에 따른 자가간호 행위)

  • Purpose: The purpose of this study was to examine self care behaviors according to the risk levels of vascular complications in elderly women with hypertension. Methods: The subjects of this study were 162 women living in the community who had been diagnosed with hypertension in clinics. The data were analysed by the SPSS 10.0 program using descriptive statistics, t-test, and ANOVA with Scheff$\dot{e}$ post-hoc test. Results: The average self care behavior score of the elderly women was 2.79. Total self care behavior was significantly different depending on the risk levels of vascular complications such as systolic blood pressure, diastolic blood pressure, ankle-brachial index, and the framingham point score. Among sub-scores of self care behaviors, exercise management was the poorest performance compared with other self care behaviors. Conclusion: This study proved the differences in self care behaviors according to the risk levels of vascular complications. To decrease the prevalence of vascular complication, it is necessary to develop programs specifically to enhance self care behaviors of elderly women with hypertension.

Activation of the renin-angiotensin system in high fructose-induced metabolic syndrome

  • High fructose intake induces hyperglycemia and hypertension. However, the mechanism by which fructose induces metabolic syndrome is largely unknown. We hypothesized that high fructose intake induces activation of the renin-angiotensin system (RAS), resulting in hypertension and metabolic syndrome. We provided 11-week-old Sprague-Dawley rats with drinking water, with or without 20% fructose, for two weeks. We measured serum renin, angiotensin II (Ang II), and aldosterone (Aldo) using ELISA kits. The expression of RAS genes was determined by quantitative reverse transcription polymerase chain reaction. High fructose intake increased body weight and water retention, regardless of food intake or urine volume. After two weeks, fructose intake induced glucose intolerance and hypertension. High fructose intake increased serum renin, Ang II, triglyceride, and cholesterol levels, but not Aldo levels. High fructose intake increased the expression of angiotensinogen in the liver; angiotensin-converting enzyme in the lungs; and renin, angiotensin II type 1a receptor (AT1aR), and angiotensin II type 1b receptor (AT1bR) in the kidneys. However, expression of AT1aR and AT1bR in the adrenal glands did not increase in rats given fructose. Taken together, these results indicate that high fructose intake induces activation of RAS, resulting in hypertension and metabolic syndrome.

Kami-bang-pung-tong-sung-san is Involved in Regulating Physiological Parameters Associated with Hypertension in Spontaneously Hypertensive Rat

  • KBPT is the fortified prescription of Bang-pung-tong-sung-san(BPTS) by adding Spatholobi Clulis and Salviae Miltiorrzae Radix. BPTS prescription has been utilized in oriental medicine for the treatments of vascular diseases including hypertension, stroke, and arteriosclerosis. Yet, the overall mechanism underlying its activity at the cellular levels remains unknown. Using spontaneously hypertensive rat (SHR) model, we investigated whether the KBPTS has an effect on the pathophysiological parameters related to hypertension. Pretreatment of SHR with KBPTS was found to lower blood pressure and heartbeat rate. Levels of aldosterone. dopamine, and epinephrine were found to be significantly reduced in the serum of KBPTS-treated SHR. Histological examination of adrenal cortex and superior aorta showed that tissues from KBPTS-treated SHR rats were more intact and cleaner compared to saline-treated control. Levels of superoxide dismutase (SOD) protein in adrenal gland, aorta, myocardial tissue, and kidneys were higher in KBPTS-treated animals than control group. The present data suggest that KBPTS may play a role in normalizing cardiovascular function in SHR by controlling hypertension-related blood factors and superoxide stressors.

Effect of Kamicheonmagudeungeum on Hypertension (가미천마구등음이 고혈압 병태모델에 미치는 영향)

  • Kamicheonmagudeungeum(KCGE), a traditional herbal medicine, has been used as a therapeutic agent for the treatments of acute stage of hypertension. We evaluated the effects of KCGE on hypertension induced by DOCA-salt in rats. The systolic blood pressure and pulse rate significantly lowered by oral administration with KCGE. The levels of plasma hormones including dopamine, epinephrine and norepinephrine, and serum electrolyte were also reduced in KCGE treated rats. In addition, the production of reactive oxigen species (ROS) were greatly decreased by the treatment with KCGE in cultured bovine endothelial cells and angiotensine converting enzyme (ACE) activites were also inhibited by KCGE in a dose dependent manner. This study indicated that KCGE is a safe and effective treatment for hypertension.

Effect of Health Services on Workers with Hypertension in Small and Medium Sized Enterprises (중소규모 사업장에서의 고혈압 근로자에 대한 보건관리 효과)

  • The purpose of this study was to evaluate the effect of health services on workers with hypertension in small and medium sized enterprises (20-300 employees). One hundred seventy three workers among 296 workers with hypertension were responded to the questionnaire on general and occupational characteristics, health habits and anti-hypertensive medications. Blood pressure was obtained while seated using a calibrated standard manometer before and after 4 months health services. Statistical analysis was performed using $X^2$-test and logistic regression analysis. The results were as follows ; 1. No significant differences were noted in variables of smoking, glucose intake and sleeping time, while significant differences were showed in variables of exercise time, frequency and amount of drinking and intake of salt, protain, lipid, calcium, vegetable and fruits between groups which was provided and not provided health services. 2. The rate of anti-hypertensive medication was increased and the blood pressure was decreased significantly in group which was provided health services compared with those of group which was not provided. 3. With the logistic regression analysis, significant variables affecting blood pressure change were smoking and anti-hypertensive medication. The results of this study suggested that health services on workers with hypertension in small and medium sized enterprises was effective, and the role of health professionals for health promotion movement in enterprises was important.

Effect of Nutrition Counseling in Hypertension Patients (고혈압환자의 영양교육 효과에 관한 연구)

  • The purpose of this study was to measure the effects of counseling for hypertension patients. the subjects were 44 hypertension patients who visited the general, hospital, located in Daegu, to receive medical treatments. Among 44 hypertension patient, 22 patients received the nutrition counseling three times for 8 weeks and the rest of the patients didn't as a non-counseling group(control group). The lifestyle, food habit, nutrient intakes, anthropometric measurements, and body fat and blood pressure as a main index were analysed before and after the nutrition counseling. The result after the nutrition counseling are as follows: 1) Body mass index(BMI), systolic blood pressure(SBP) and diastolic blood pressure(DBP) were significantly decreased in women(p<0.05). 2) The food habit score and nutrition knowledge score were dramatically increased(p<0.001). 3) The calcium intake was significantly increased in women(p<0.01). And energy, carbohydrate and fat intakes were decreased both men and women but there were no significant differences. The calcium intake as an Index of nutrition quality(INQ) and nutrient adequacy ratio(NAR) were significantly increased 5) Fat and cholesterol intakes in a group whose blood pressure reduced were significantly decreased compared with a group that had the same or higher blood pressure. These results showed that a well-planned nutrition counseling program would reduce the risk of cardiovascular disease.
