A Study on the Need of Clinical Nurses about the System of 12-Hour Work Shift (이부교대(二部交代) 근무제도(勤務制度)에 대한 임상간호사(臨床看護師)의 요구도(要求度)에 관한 연구(硏究))

  • The 12-hour work shift can be influenced on the nurse's job satisfaction, effective personnel administration, and quality of patient care. The purpose of this study was to explore the perception on the 12-hour work shift by nurses. A total of 516 nurses were selected by convenience sampling from six general hospitals in Seoul. Based on the literature review, the questionnaire on the need of the 12-hour work shift was designed by the investigator. The data were analyzed using descriptive statistics, ANOVA and Duncan's method as post-hoc test. The results of this study are as follows ; 1. The need of 12-hour work shift The mean needs of 12-hour work shift in the nursing practice, psychological, physical, socioeconomic, and environmental perspectives were 3.05, 2.72, 2.66, 3.08, and 4.22. 2. The need of 12-hour work shift by demographic data For the nursing practice, there was a statistically significant relationship between marital status(p=.021), satisfaction on the 8-hour work shift(p=.038), the perception on the 12-hour work shift(p=.001) and the need of 12-hour work shift. For the psychological perspectives, there was a statistically significant relationship between marital status(p=.016), the perception on the 12-hour work shift(p=.0001), and the need of 12-hour work shift. For the physical perspectives, there was a statistically significant relationship between age(p=.002), marital status(p=.005), the number of children(p=.005), the duration of job career(p=.014), the job position(p=.002), the work shift(p=.030), the perception on the 12-hour work shift(p=.0001) and the need of 12-hour work shift. For the socioeconomic perspectives, there was a statistically significant relationship between the job position(0=.002), the work shift(p=.006), the perception of the 12-hour work shift(p=.002) and the need of 12-hour work shift. For the environmental perspectives, there was a statisitically significant relationship between the traffic method(p=.05), the duration of the job career(p=.026), the perception on the 12-hour work shift(p=.309) and the need of 12-hour work shift. 3. The need of 12-hour work shift by the demographic data There was a statistically significant difference between marital status(p=.014), the number of children(p=.038), the job position(p=.007), the work shift(p=.037), and the perception on the 12-hour work shift(p=.0001) for the need of 12-hour work shift.

  • To elucidate the effect of ginseng butanol fraction on streptozotocin-induced hyperglycemia, ginseng butanol fraction was administered before and after injection of streptozotocin(50mg/kg, i.v.), and glucose, insulin, and cholesterol levels in serum were determined at 96 hours after streptozotocin injection. Serum glucose, insulin levels were significantly decreased by administration of ginseng butanol fraction (100mg/kg, p.o.) at 7 hour and 7, 4, 1, hour(three times) before streptozotocin injection. The glucose levels were significantly decresed by administration of ginseng butanol fraction at 1 hour (100mg/kg) after strcptozotocin injection, and also serum glucose levels in groups of continuous administration of ginseng butanal fraction(100mg/kg) for 3 days after streptozotocin injection were markedly decreased than in group of single dose of ginseng butanol fraction. Ginseng butanol fraction has the protective and relieving action against streptozotocin-induced hyperglycemia.

  • To evaluate effects of Sam so on Pye Tang(SSOPT) Water Extract plasma cortisol concentration and plasma $CO_2$ in the Rabbit. The results obtained were as follows. 1. Intravenous administration of SSOPT water extract at the dose of 0.5ml/kg remarkably increased plasma cortisol concentration on 1 hour. 2. Intravenous adminitration of SSOPT Water extract at the does of 1.0ml/kg significantly decreased plasma $CO_2$ on 1 hour. 3. Intravenous adminitration of SSOPT Water extract at the does of 0.5ml/kg remarkable decreased plasma $Na^+$ from 1 to 4 hour and 1.0ml/kg significantly decreased plasma $Na^+$ on 1 hour. 4. Intravenous administration of SSOPT Water extract at the does of 1.0ml/kg remarkably increased plasma $k^+$ from 1 to 3 hour. 5. Intravenous administration of SSOPT water extract at the does of 0.5ml/kg significantly decreased plasma $CI^-$ on 3, 4 hours. and 1.0ml/kg remarkably increased plasma $CI^-$ on 4 hour. These results suggest that therapeutic action of SSOPT water extract for athma may be reated with the increment of plasma cortisol concentration and the decrease plsma $CO_2.$.

  • The present study was carried out to investigate the effect of water extract and 70% ethanol extract from Gastrodiae Rhizoma on cardiovascular activities and plasma levels of catecholamines in unanaesthetized spontaneously hypertensive rats. The depressor response in SHR was observed during three to six hour period after an oral administration of water extract from Gastrodiae Rhizoma(GR). There was a statistically significant correlation between the magnitude of the depressor response induced by an oral administration of water extract from GR and the initial control blood pressure level. The increase in blood pressure induced by norepinephrine was less in Wistar rat treated with GR water extract than those without GR extract. No significant change in heart rate was observed in SHR receiving either water extract or ethanol extract from GR. Associated with the depressor response, there was a concomitant reduction in plasma levels of norepinephrine in SHR at 4 hour after an oral administration of water extract from GR. Plasma levels of norepinephrine and epinephrine were decreased slightly at 2 hour after an oral administration of ethanol extract from GR. These results suggest that the depressor effect of water extract from GR may be due, in part, to a decreased sympathoadrenal activity.

A meteorological factor analysis for high rice production in South Korea

  • Rice yield of South Korea in 2015 was the highest of the last 30 years. It is important issue to establish food policy whether the historically highest yield in 2015 can be continued or just one-off event. Therefore, it is necessary to understand whether such a high yield as 2015 will be reoccurred. The aim of this study was to find out what climatic factor affect rice yield and how often these climatic factor could occur. For this study, the yield monitoring data from National Institute of Crop Science, Rural Development Administration and the meteorological data provided by Korea Meteorological Administration are used to identify the weather conditions could cause high yield, and how often these conditions occurred in the past. Our results indicated that such as high yield as 2015 could occur only when the mean sunshine hours of July and the mean sunshine hours from the end of August to early September was more than 5.1 hours and 6 hours, respectively. Mean sunshine hour of July may be related to grain number. The mean sunshine hour from the end of August to early September was presumed to relate to grain filling ratio. The relationship between monthly mean temperature and yield or yield component was not clear in this study. In this study, any cycle of high weather condition was not found. Therefore, the probability of high yield weather condition was expressed by frequency. The frequency of the sunshine hour, could make high yield, were 8/35 (23%) over the past 35 years. And the frequency of two years consecutive sunshine hour condition, which could cause high yield, was 1/35 (2.9%). The frequency of recurrence of sunshine hour making high yield within the next 5 years or 10 years after high yield weather condition were 4/35 (11.4%). After all, the high yield as much as yield of 2015 could not be one-off event. But it was not also consecutive event.

  • Cinmetacin, one of the candidate of NSAID of arylacetate group was developed into a prodrug SJ-151 with butendiol group to minimize its gastrointestinal side effects. We studied its excretion and distribution after single oral administration in rats. Male rats were orally administered with 30, 60, 80 or 120mg/kg of SJ-151 and their urine and stool were collected at 0, 6, 12, 24 and 48 hour after administration. To evaluate its tissue distribution, 120mg/kg of SJ-151 was orally given and samples of blood, liver, kidney and brain were taken at 0.5, 1, 2, 4, 8, 24, and 48 hour of administration. As results, less than 0.1% of administered SJ-151 was detected in 48 hour collected urine as its metabolite cinmetacin. 33-50% of administered SJ-151 was observed in 48 hour collected stool as SJ-151. 3-7% of excreted SJ-151 was observed in 48 hour collected stool as cinmetacin. SJ-151 and cinmetacin were not detected in the brain regardless of dosage. SJ-151 was detected neither in kidney nor in liver. Only cinmetacin was observed in both organs with kidney concentrations higher than liver throughout the observation period. On the whole, organ concentration of cinmetacin fluctuated through 0.1-1.5 times that of plasma. As no reports on the metabolism of SJ-151 or cinmetacin in specific organs has been published yet, any detailed explanation of these results needs further study and the plasma concentration profile of rats showed remarkable interspecies difference with dogs.

Impact of 12-hour Shifts on Job Satisfaction, Quality of Life, Hospital Incident Reporting, and Overtime Hours in a Pediatric Intensive Care Unit (일 소아 중환자실에서의 12시간 교대근무가 간호사의 직무만족도, 삶의 질, 안전사건 보고 및 시간 외 근무에 미치는 영향)

  • Purpose: The aim of this study was to compare job satisfaction, quality of life (QOL), incident report rate and overtime hours for 12-hour shifts and for 8-hour shifts in a pediatric intensive care unit (PICU). Methods: A descriptive survey was conducted with a convenience sample of 36 staff nurses from a PICU in a regional hospital in Korea. Data were collected using self-administrated questionnaires regarding job satisfaction and QOL at 6 months before and after the beginning of 12-hour shifts. Incident report rate and overtime hours for both 12-hour and 8-hour shifts were compared. Comparisons were made using $x^2$-test, paired t-test and Mann-Whitney U test. Results: After 12-hour shifts were initiated, job satisfaction significantly increased (t=3.93, p<.001) and QOL was higher for nurses on 12-hour shifts compared to 8-hour (t=7.83, p<.001). There was no statistically significant change in incident report rate ($x^2=0.15$, p=.720). The overtimes decreased from $36.3{\pm}34.7$ to $17.3{\pm}34.9$ minutes (Z=-8.91, p<.001). Conclusion: These results provide evidence that 12-hour shifts can be an effective ways of scheduling for staff nurses to increase job satisfaction and quality of life without increasing patient safety incidents or prolonged overtime work hours.

Effects of Hwang Keum Jag Yag Tang Water Extract on the Plasma Cortisol Concentration and in the Rabbit and on the Analgesic Effect in the Mouse (黃芩芍藥湯 煎湯液이 血漿Cortisol 및 鎭痛에 미치는 影響)

  • In order to investigate the effects of Hwang Keum Jag Yag Tang (HJT) water extract on the plasma cortisol concentration in the rabbit and on the analgesic effect in the mouse were administered. The results were summarized as follow: 1. Intravenous administration of HJT water extract at the dose of 0.2ml/kg remarkably increased plasma cortisol concentration after two hour. 2. Intravenous administration of HJT water extract at the dose of 0.4ml/kg significantly increased plasma cortisol concentration after one hour. 3. Intravenous administration of HJT water extract at the dose of 0.2ml/kg, all the experimental period, significantly increased plasma sodium concentration. 4. Intravenous administration of HJT water extract at the dose of 0.4m1/kg significantly decreased plasma potassium concentration. 5. Intravenous administration of HJT water extract at the dose of 0.2ml/kg and 0.4ml/kg, all the experimental period, significantly decreased plasma calcium concentration. 6. The analgesic effect of HJT water extract showed inhibitory effect more than at 0.1ml/20g. According to the results, Hwang Keum Jag Yag Tang water extract remarkably increased plasma cortisol concentration and showed analgesic effect.

Effects of Quercetin and Mannitol on Erythropoietin Levels in Rats Following Acute Severe Traumatic Brain Injury

  • Objective : The aim of this study to investigate the normal values of erythropoietin (EPO) and neuroprotective effects of quercetin and mannitol on EPO and hematocrit levels after acute severe traumatic brain injury (TBI) in rat model. Methods : A weight-drop impact acceleration model of TBI was used on 40 male Wistar rats. The animals were divided into sham (group I), TBI (group II), TBI+quercetin (50 mg/kg intravenously) (group III), and TBI+mannitol (1 mg/kg intravenously) (group IV) groups. The malondialdehyde, glutathione peroxidase, catalase, EPO, and hematocrit levels were measured 1 and 4 hour after injury. Two-way repeated measures analysis of variance and Tukey's test were used for statistical analysis. Results : The malondialdehyde levels decreased significantly after administration of quercetin and mannitol compared with those in group II. Catalase and glutathione peroxidase levels increased significantly in groups III and IV. Serum EPO levels decreased significantly after mannitol but not after quercetin administration. Serum hematocrit levels did not change significantly after quercetin and mannitol administration 1 hour after trauma. However, mannitol administration decreased serum hematocrit levels significantly after 4 hour. Conclusion : This study suggests that quercetin may be a good alternative treatment for TBI, as it did not decrease the EPO levels.

Toxic Effects of Polygalae Radix on Rat Kidney

  • The renal toxicity of the extract of Polygalae Radix was investigated in rats. Rats were treated with 3.5 mg/Kg of the extract, i.p., for 7 days. Changes in consumatory behavior, 24 hour-urine and the activities of urinary enzymes were determined during the administration of the extract. Significant decrease in body weight and food consumption and increase in 24 hour-urine volume were observed during the administration. However, the quantity of total creatinine in urine was decreased significantly. Those indicate that subacute treatment with the extract might induce diuresis and the ditiresis might be due to the decrease in water reabsorption. In the activities of urinary enzymes, the activities of alanine aminopejotidase (AAP) and gamma-glutamyl transpeptidase (GGT) were increased 4.3 and 3.5 times and then returned to the control. The activity of N-acetyl-${\beta}$-D-glucosaminidase (NAG) was increased 7.2 times and then decreased slowly. But, it was significantly higher than that of the control evea after the last administration. The activity of factate dehydrogenase (LDH) was increased continuozlsly during the treatment. It showed 32 times higher than the control. These results suggested that the extract of Polygalae Radix had toxic effect on kidney. Furthermore, the result suggested that the subacute administration of the extract induced resistance against the toxicity of Polygalae Radix.
