Background and Objectives: Cardiff Acne Disability Index(CADI) is one tool used internationally to measure the quality of life of acne sufferers. There, however, is a necessity of developing Korean version of CADI, as the questionnaires of the original one are written in English, making it hard to apply for Korean patients. So as a first step, we conducted a cross-cultural translation of CADI into Korean and verification of face validity. Methods: After properly translating CADI questionnaires into Korean up to guidelines for cross-cultural adaptation of health related quality-of-life measures, we conducted a survey with 122 undergraduates to get face validity, using the translated questionnaires. Results: About the translated CADI questionnaires, 86 out of 107 undergraduates replied that they had no difficulty understanding them, while 21 offered ideas about ambiguous expressions of them. Upon further examination of two oriental doctors, two sentences were additionally modified in the translated version. Conclusions: Firstly, we created the Korean version of CADI, one of the most effective methods in the world to measure acne sufferers' quality of life, by properly translated the original version into Korean. Then we conducted a survey for face validity with the translated questionnaires and gathered opinions from those questioned. After going through some examining and correcting procedures based on the opinions, we finalized the Korean version of CADI. It will also require a follow-up verification process to prove credibility and validity of the final version of Korean CADI.
Many immune down-regulatory molecules have been isolated from parasites, including cystatin (cystain protease inhibitor). In a previous study, we isolated and characterized Type I cystatin (CsStefin-1) of the liver fluke, Clonorchis sinensis. To investigate whether the CsStefin-1 might be a new host immune modulator, we induced intestinal inflammation in mice by dextran sodium sulfate (DSS) and treated them with recombinant CsStefin-1 (rCsStefin-1). The disease activity index (DAI) increased in DSS only-treated mice. In contrast, the DAI value was significantly reduced in rCsStefin-1-treated mice than DSS only-treated mice. In addition, the colon length of DSS only-treated mice was shorter than that of rCsStefin-1 treated mice. The secretion levels of IFN-${\gamma}$ and TNF-${\alpha}$ in the spleen and mesenteric lymph nodes (MLNs) were significantly increased by DSS treatment, but the level of TNF-${\alpha}$ in MLNs was significantly decreased by rCsStefin-1 treatment. IL-10 production in both spleen and MLNs was significantly increased, and IL-$10^+F4/80^+$ macrophage cells were significantly increased in the spleen and MLNs of rCsStefin-1 treated mice after DSS treatment. In conclusion, rCsStefin-1 could reduce the intestinal inflammation occurring after DSS treatment, these effects might be related with recruitment of IL-10 secreting macrophages.
Maternal zinc deficiency is relatively common worldwide, but its consequences for pregnancy outcome are not established. The purpose of this study was to examine the effect of zinc status in the second trimester on pregnancy outcome. Subjects were 248 pregnant Korean women (25-28 wk gestation). Cord blood was collected from subgroup of 69 women and pregnancy outcome data were obtained from 185 babies. Anthropometry measurement, dietary intake, and biochemical characteristics of pregnancy and cord serum substances were measured. The subjects were divided into quartiles on the basis of maternal serum Zn concentration; $ZnQ_1$ (< 25 percentile), $ZnQ_2$ ($26\~50$ percentile), $ZnQ_3$ ($51\~75$ percentile), and $ZnQ_4$ ($76\~100$ percentile). Zn groups were compared in terms of various maternal factors, concentrations of cord serum substances, and pregnancy outcome. Maternal serum Zn concentration was $118.4\;{\pm}\;35.5\;{\mu}g/dl$ as mean. Intake of Zn was lower than Korean RDA. The rate of Zn deficiency among all subjects was $8.5\%$. Maternal serum Zn levels belonged to normal range. Cord serum Zn level was about $154.7\%$ of maternal serum level. Intakes of energy, calcium, iron, folic acid, and riboflavin did not meet the Korean RDA for pregnant women by gestational age. The mean birth weight of neonates is 3083 $\pm$ 697 g, of whom $9.1\%$ were of low birth weight (< 2,500 g). Maternal serum Zn level was positively correlated with pre-pregnancy weight, pre-pregnancy body mass index, and vitamin C intake (p < 0.05). $ZnQ_1$ group had significantly lower maternal serum iron concentration and higher cord serum cholesterol than those in other groups. Maternal serum Zn level, cord serum Zn level, and dietary Zn intake were no related to the pregnancy outcome. The birth weight had a correlation with the maternal hemoglobin and albumin concentration. In conclusion, at this study, we could not find the association with maternal Zn status in 2nd trimester and pregnancy outcome.

Background: Obesity is related to several comorbidities and mortality, but its relationship with acute kidney injury (AKI) and long-term mortality remain undetermined in patients undergoing coronary artery bypass grafting. Methods: Data from 3,018 patients (age ${\geq}18$ years) who underwent coronary artery bypass graft surgery from two tertiary referral centers were retrospectively reviewed between 2004 and 2015. Obesity was defined using the body mass index, according to the World Health Organization's recommendation. The odds and hazard ratios in post-surgical, AKI, and all-cause mortality were calculated after adjustment for multiple covariates. Patients were followed for $90{\pm}40.9$ months (maximum: 13 years). Results: Among the cohort, 37.4%, 2.4%, 21.1%, 35.1%, and 4.0% of patients were classified as normal weight, underweight, overweight-at-risk, obese I, and obese II, respectively. Post-surgical AKI developed in 799 patients (26.5%). Patients in the obese groups (overweight-at-risk to obese II) had a higher risk of AKI than did those in the normal-weight group. During the follow-up period, 787 patients (26.1%) died. Underweight patients had a higher risk of mortality than did normal-weight patients, whereas overweight-at-risk, obese I, and obese II patients showed better survival rates. Conclusion: After coronary artery bypass graft surgery, obese patients encountered a high risk of AKI, and underweight patients exhibited a low chance of survival. Awareness of both obese and underweight statuses should be raised in these patients.
This study is to investigate the effects of demographic characteristics and lifestyles on obesity in the community. The nurse measured the obesity level of 143 residents living in one apartment in Seoul, Korea. Demographics and lifestyles were analyzed by frequency and percentage and obesity level was analyzed by logistic regression analysis. In particular, as the age increases, the probability of obesity increases by 1.109 times. These results show that the development and application of community health promotion programs for obesity are important. Through this study, we discovered that obesity is significantly related to the demographic characteristics and lifestyles of the individual. And also, it is expected that these findings will be used as the basic information to plan a tailored obesity program taking into account local community environment and personal characteristics.
Electromechanical impedance (EMI) based structural health monitoring is performed by measuring the variation in the impedance due to the structural local damage. The impedance signals are acquired from the piezoelectric patches that are bonded on the structural surface. The impedance variation, which is directly related to the mechanical properties of the structure, indicates the presence of local structural damage. Two traditional EMI-based damage detection methods are based on calculating the difference between the measured impedance signals in the frequency domain from the baseline and the current structures. In this paper, a new structural damage detection approach by analyzing the time domain impedance responses is proposed. The measured time domain responses from the piezoelectric transducers will be used for analysis. With the use of the Time Frequency Autoregressive Moving Average (TFARMA) model, a damage index based on Singular Value Decomposition (SVD) is defined to identify the existence of the structural local damage. Experimental studies on a space steel truss bridge model in the laboratory are conducted to verify the proposed approach. Four piezoelectric transducers are attached at different locations and excited by a sweep-frequency signal. The impedance responses at different locations are analyzed with TFARMA model to investigate the effectiveness and performance of the proposed approach. The results demonstrate that the proposed approach is very sensitive and robust in detecting the bolt damage in the gusset plates of steel truss bridges.
BACKGROUND/OBJECTIVES: Several previous studies have investigated whether regular walnut consumption positively changes heart-health-related parameters. The aim of this study was to investigate the effects of daily walnut intake on metabolic syndrome (MetS) status and other metabolic parameters among subjects with MetS. SUBJECTS/METHODS: This study was a two-arm, randomized, controlled crossover study with 16 weeks of each intervention (45 g of walnuts or iso-caloric white bread) with a 6 week washout period between interventions. Korean adults with MetS (n = 119) were randomly assigned to one of two sequences; 84 subjects completed the trial. At each clinic visit (at 0, 16, 22, and 38 weeks), MetS components, metabolic parameters including lipid profile, hemoglobin A1c (HbA1c), adiponectin, leptin, and apolipoprotein B, as well as anthropometric and bioimpedance data were obtained. RESULTS: Daily walnut consumption for 16 weeks improved MetS status, resulting in 28.6%-52.8% reversion rates for individual MetS components and 51.2% of participants with MetS at baseline reverted to a normal status after the walnut intervention. Significant improvements after walnut intake, compared to control intervention, in high-density lipoprotein cholesterol (HDL-C) (P = 0.028), fasting glucose (P = 0.013), HbA1c (P = 0.021), and adiponectin (P = 0.019) were observed after adjustment for gender, age, body mass index, and sequence using a linear mixed model. CONCLUSION: A dietary supplement of 45 g of walnuts for 16 weeks favorably changed MetS status by increasing the concentration of HDL-C and decreasing fasting glucose level. Furthermore, consuming walnuts on a daily basis changed HbA1c and circulating adiponectin levels among the subjects with MetS. This trial is registered at ClinicalTrials.gov as NCT03267901.
Background: In this study, we analyze the performance of the Acute Physiology and Chronic Health Evaluation (APACHE) II, APACHE IV, Simplified Acute Physiology Score (SAPS) 3, and Mortality Probability Model $(MPM)_0$ III in order to determine which system best implements data related to the severity of medical intensive care unit (ICU) patients. Methods: The present study was a retrospective investigation analyzing the discrimination and calibration of APACHE II, APACHE IV, SAPS 3, and $MPM_0$ III when used to evaluate medical ICU patients. Data were collected for 788 patients admitted to the ICU from January 1, 2015 to December 31, 2015. All patients were aged 18 years or older with ICU stays of at least 24 hours. The discrimination abilities of the three systems were evaluated using c-statistics, while calibration was evaluated by the Hosmer-Lemeshow test. A severity correction model was created using logistics regression analysis. Results: For the APACHE IV, SAPS 3, $MPM_0$ III, and APACHE II systems, the area under the receiver operating characteristic curves was 0.745 for APACHE IV, resulting in the highest discrimination among all four scoring systems. The value was 0.729 for APACHE II, 0.700 for SAP 3, and 0.670 for $MPM_0$ III. All severity scoring systems showed good calibrations: APACHE II (chi-square, 12.540; P=0.129), APACHE IV (chi-square, 6.959; P=0.541), SAPS 3 (chi-square, 9.290; P=0.318), and $MPM_0$ III (chi-square, 11.128; P=0.133). Conclusions: APACHE IV provided the best discrimination and calibration abilities and was useful for quality assessment and predicting mortality in medical ICU patients.
Purpose: To explore the influence of S100 calcium binding protein A4 (S100A4) knockout (KO) on methionine-choline-deficient (MCD) diet-induced non-alcoholic fatty liver disease (NAFLD) in mice. Materials and Methods: S100A4 KO mice (n=20) and their wild-type (WT) counterparts (n=20) were randomly divided into KO/MCD, Ko/methionine-choline-sufficient (MCS), WT/MCD, and WT/MCS groups. After 8 weeks of feeding, blood lipid and liver function-related indexes were measured. HE, Oil Red O, and Masson stainings were used to observe the changes of liver histopathology. Additionally, expressions of S100A4 and proinflammatory and profibrogenic cytokines were detected by qRT-PCR and Western blot, while hepatocyte apoptosis was revealed by TUNEL staining. Results: Serum levels of aminotransferase, aspartate aminotransferase, triglyceride, and total cholesterol in mice were increased after 8-week MCD feeding, and hepatocytes performed varying balloon-like changes with increased inflammatory cell infiltration and collagen fibers; however, these effects were improved in mice of KO/MCD group. Meanwhile, total NAFLD activity scores and fibrosis were lower compared to WT+MCD group. Compared to WT/MCS group, S100A4 expression in liver tissue of WT/MCD group was enhanced. The expression of proinflammatory ($TGF-{\alpha}$, $IL-1{\beta}$, IL-6) and profibrogenic cytokines ($TGF-{\beta}1$, COL1A1, ${\alpha}-SMA$) in MCD-induced NAFLD mice were increased, as well as apoptotic index (AI). For MCD group, the expressions of proinflammatory and profibrogenic cytokines and AI in KO mice were lower than those of WT mice. Conclusion: S100A4 was detected to be upregulated in NAFLD, while S100A4 KO alleviated liver fibrosis and inflammation, in addition to inhibiting hepatocyte apoptosis.
Purpose: Malnutrition in children with cerebral palsy (CP) is a significant factor affecting their adequate growth and development. This study aimed at conducting surveys and evaluating the dietary intake of children with CP according to their BMI classification and to thereby highlight the dietary factors affecting the nutritional status of these children. Methods: A total of 16 children were enrolled between the age of four and twelve. These subjects were further classified into three groups, namely underweight, normal and obese, with 6, 8, and 2 children in each group, respectively. The general characteristics, motor disturbances, body composition, feeding problems, eating habits, nutritional intake, dietary variety, and food frequency for children with CP were evaluated. Results: It was observed that motor disturbances tended to increase in underweight children with CP. A significant decrease (p < 0.05) in disturbances related to oral feeding was observed with an increase in obesity. The pattern of eating habits revealed that subjects in the underweight group consumed unbalanced meals, while those in the obese group tended to consume larger meals at a faster pace. The feeding disturbance data revealed that those in the underweight group could not prepare their meals while the obese group had the problem of overeating and consuming an unbalanced diet (p < 0.05). Conclusion: It is necessary for both children with CP, who have a high degree of disability, and their caregivers to take lessons on adequate nutrient intake to prevent malnutrition. Moreover, it is necessary for the caregivers and children with CP having a low degree of disability to take lessons on providing and consuming a balanced diet and to focus on the intake of sufficient calcium in order to prevent obesity.
