Background: The purpose of this study was to identify factors inhibiting access of people with disability to health check-ups as well as identify pertinent solutions for improvement. Methods: Twenty-three people with disability older than the age of 19 who took respective health check-ups within the last 3 years were selected as participants. For the data collection, the 1:1 intensive interview was used. The data were analyzed by the grounded theory by Corbin and Strauss. Results: The results comprised nine categories, 23 subcategories, and 179 concepts. The central phenomenon was 'failure to obtain check-ups.' Causal conditions were observed as a 'lack of communication method,' 'physical difficulties,' and 'staff unfamiliar with people with disability,' Interventional conditions comprised 'physical accessibility,' 'staffs' competency,' and 'assistant manpower.' The active strategy was included 'to investigate the professional medical institution,' 'to find the medical institution of convenient traffic accessibility,' 'to overcome communication difficulties through equipment,' and 'to overcome linguistic barriers through sufficient communication.' Whereas, 'utilization of ancillary equipment,' 'the education of staffs on people with disability,' 'universal design manual,' and 'customized check-ups' were included in the passive strategy. Such processes arose in the contextual conditions of 'lack of expectations for daily lives' and 'lack of government support.' As a consequence, the subjects participated experienced the 'disadvantages,' 'discrimination,' and 'reduced reliability of the health check-ups.' Conclusion: The subjects who participated in this study emphasized 'staffs familiar with people with disability' and 'systems customized for people with disability' are mandatory to secure complete health check-ups for people with disability.
The purpose of this study is to examine the influence of the family health on the self-esteem and pro-social behaviour of children. The objects were 422 children who were 5th and 6th grade in primary school. The questionnaires were collected between June and July in 2006. T-test, Pearson's correlation and hierarchical multiple regression were used for data analysis. The results are as follows: First, the family health is higher than the average and also the self-esteem and pro-social behaviour of children are higher than the average. Second, the higher family's economic and parent's educational level, the higher family health and self-esteem of children. In addition, Female children have higher pro-social behaviour than male children. Third, family health has positive correlations with self-esteem and pro-social behaviour of children. Furthermore there is a positive correlation between the self-esteem and pro-social behaviour of children. Fourth, the self-esteem and pro-social behaviour of children are strongly influenced by the family health. As a consequence the family health is the principal variable which influences on the self-esteem and pro-social behaviour of children. This study shows that the healthy family support program is necessary in order to build up the positive self-esteem and pro-social behaviour of children.
Objectives : The objective of this paper is to discuss the importance of obtaining informed consent for conducting epidemiological studies and public health activities, based on the Report of the UNESCO's Working Group on Informed Consent. Methods : The Report of the UNESCO's Working Group on Informed Consent was reviewed and discussed in connection with the ethical considerations of public health activities and epidemiological research. Results : It was at the Nuremberg Trial for the German war criminals of the Second World War that the principle of 'consent' was first stated as a consequence of the medical abuses carried out during the War. As a result of the Trial, the Nuremberg Code came out in 1947. Since then, various international declarations or ethical principles on 'informed consent' have been developed and published. These ethical principles on 'informed consent' have mostly to do with the clinical research that involves human subjects, and not with epidemiological studies and public health activities. However, UNESCO recently issued a comprehensive Report on Informed Consent based on the Universal Declaration on Bioethics and Human Rights adopted in 2005, and this included detailed guidelines on informed consent in epidemiological studies and public health activities. Conclusions : Universal Declaration on Bioethics and Human Rights emphasizes the principle of autonomy to protect the human rights of the human subjects involved in any public health activities and epidemiological research. As a practical guideline, obtaining informed consent is strongly recommended.
In recent years a concern of excessive intakes of vitamins and minerals from various sources is increasing, since there has been a marked increase in production and consumption of vitamin and mineral supplements and fortified foods. The purpose of this study was to assess the maximum exposure of vitamins and minerals from various sources including diet, fortified foods, and health functional foods among Koreans. As a result, the highest exposure group of most vitamins and minerals from diet was adults (30 - 49 years of age) according to 2001 Korean National Nutrition and Health Survey Report. Maximum dietary intakes of vitamin A, $B_1,\;B_2$, C, nicotinamide, calcium, phosphorus and iron were 0.5 - 7 times of the RDA for Koreans, 7th ed. Maximum intakes of vitamins and minerals from fortified foods by adults (20 - 59 years of age) were 8 - 760% of the Korean RDA. In addition, maximum exposure of vitamins and minerals from vitamin mineral health functional foods by middle aged people was 35 - 140% of the upper limits (UL: DRI for Koreans). As a consequence, maximum combined intakes of vitamin $B_6$, vitamin C, calcium, iron and zinc from the above sources including diet, fortified foods and vitamin mineral health functional foods were greater than the UL. These results would be applied for determining the safe upper limits of vitamin and mineral of health functional foods.
The Process Safety Management (PSM) by the Law of Industry, Safety and Health has been performed for preventing major accidents of chemical plants since 1996. In terms of preventing chemical accidents more precisely, it is essential to develop a tool for quantitative risk assessment. For this, KOSHA (Korea Occupational Safety and Health Agency) developed an Integrated Risk Management System (IRMS). The system is designed to assimilate data on chemical plant hazards from external database, to integrate these data with location information(topographic and demographic), and to make them user-friendly accessible. The system consists of several main functions: display of five major Korean petrochemical complex layout display of equipment layout with its information utilizing the external database, zonation of the hazard effected area with consequence analyses, the most probable accident scenario generation, accident/incident database and calculation of frequency of accident using equipment reliability database, etc. The highlight of IRMS is to provide the risk contours using GIS(Geographical Information System) technology. IRMS is intended to manage hazardous installation more systematically and effectively, to reduce the number of accident remarkably, further minimizing production loss in the plant. The system is now under application to about 500 PSM sites as well as and emergency authorities in Korea by KOSHA (Korea Occupational Safety and Health Agency)
A researcher examined 158 medical technologists and 140 radiologists who are working at 9 general hospitals in Gyeongsangbuk-do area using structured questionnaire to find out degree of fatigue of health professionals, the primary factors that have effect on them, and actions to control their fatigue on December 1 through December 20, 2008. The average complaining rate of fatigue subjective symptom by syndromes was overall 17.24 points, and medical technologists scored 16.48 points while radiologists scored 18.09 points. There were significant difference in the average fatigue complaining score of both medical technologists and radiologists such as when the younger their age is, when they are single, when the lower their monthly salary is, when the shorter their total working period is, and when the current status is staff at work. As a result of multiple regression analysis which sets fatigue score as dependent variable, there were significant difference on both syndromes such as satisfaction on work, work stress, and sleeping condition. It was the highest on relaxation, which was 76.6%, among the 15 questions on actions to control fatigue, and the next was enough sleep and controlling stress. And the lowest was help from medical professionals, which was 7.3%. It was the highest on enough sleep, which was 1.98 points, and next was relaxation and controlling stress, and the lowest points were help from medical professionals and taking medicine in the effective score of fatigue control action. In consequence of research, it is necessary to develop program and health education to control health professionals' various fatigue such as stress management and sleeping, and it is considered to find out the plan about effective work system.
Objectives : The purpose of this study was to grasp association with dental hygienists' job expectation through correlation between curricular subjects for the department of dental hygiene and job. Methods : It carried out convenience sampling of dental hospitals where are located in Capital Area, and then conducted a self-administered questionnaire survey targeting 274 dental hygienists who are working here. The statistical program was analyzed by using PSAW 18.0. Results : Research subjects' 60.2% experienced turnover. Its reason accounted for 31.7% with the working condition. As a result of examining difference in experience of turnover according to dental hygienists' individual characteristic and working form, the significant difference was shown in age, marriage, annual salary, working institution, and career. In consequence of confirming association between curricular subjects and job, the majority showed high correlation. However, the correlation with oral physiology, clinical dental hygiene, and dental clinics showed low correlation as the insignificant variable. Conclusions : As a result of this study, the high correlation between curricular subjects and job can be considered to reflect dental hygienists' job expectation. On the other hand, the working condition, professional sense, and turnover experience, which are factors related to dental hygienists' job expectation, were indicated to fail to meet job expectation.
The multi-origin of obesity and its associated diseases made it's a complex area of biomedical science research and severe health disorder. From the 1970s to onwards this health problem turned to an epidemic without having any report of declining yet and it created a red alert to the health sector. Meanwhile, many animal models have been developed to study the lethal effect of obesity. In consequence, many drugs, therapies and strategies have already been adopted based on the findings of those animal models. However, many complicated things based on molecular and generic mechanism has not been clarified to the date. Thus, it is important to develop a need based animal model for the better understanding and strategic planning to eliminate/avoid the obesity disorder. Therefore, the present review would unveil the pros and cons of presently established animal models for obesity research. In addition, it would indicate the required turning direction for further obesity and obesity based disease research.
Purpose: The research was conducted to investigate the experience of maternal role attainment of mothers of premature infants admitted in NICU and to conceptualize the phenomena. Method: The grounded theory method was utilized for data collection and analysis. 8 mothers of premature infants were selected and in-depth interview was performed. Paradigm model was utilized for data analysis and presentation. Result: The central category was 'unstable maternal identity'. The properties of the core phenomena was 'ambivalent feeling to baby' 'negative emotion' 'commitment to baby'. The loss of control due to premature delivery was the causal condition. contextual condition was the 'perceived threats' due to severity of the premature infant and uncerainty of the baby's life. The mother's health status, economic status, and familial and social support was recognized as intervening conditions during the process of maternal role attainment. The strategic action/interactions were emotion-focused coping, reappraisal of the situation, problem-focused coping, and information seeking. The consequence was the maternal role attainment with competence and expectation. Conclusion: The process of maternal role attainment was affected by threats due to severity of the baby and intervening factors and interaction strategy. Further research is recommended to develop adequate intervention method during the process of maternal role attainment.
The purpose of this article is to examine the cause of policy non-compliance in the case of pharmaceutical rebates from the perspective of rational choice institutionalism. In Korea, there have been rebates practices between pharmaceutical companies and hospitals since the introduction of the Actual Remuneration System for insured medicine in 1999. The government has chosen the policy means of punishment to eliminate pharmaceutical rebates but the illegal practices are still widespread. Institution in rational choice institutionalism usually reflects the incentives and preferences of actors, and the Actual Remuneration System has resulted in a the lack of procedures to ensure savings on drug expenditures. Pharmaceutical rebates are the product of the institutions which reflect their incentives: the Actual Remuneration System, the current pricing policy for generic drugs, the drug distribution system, and so on. In the end, the problem of the rebates is the consequence of policy non-compliance as actors' rational choice because their incentives lead to opportunistic behaviors. We should therefore understand the incentive structure of policy stakeholders, which is derived from the view of new institutionalism; also, the newly designed Korean drug pricing policy reform must be compatible with the incentive structure.
