대기오염 노출지표에 의한 도시개발 입지의 평가에 관한 연구 (Evaluation Method of Urban Development Location by APEI (Air Pollution Exposure Index))

  • In this study, relationship between the air pollution of Siheung city and the relative contribution of automobiles to the city's pollution was evaluated for the first time. Then, new air pollution exposure index was developed through simulation. Using the newly developed index, two different urban development scenarios were compared to present a sustainable urban development plan to reduce air pollution from the land utilization point of view. According to the result of this simulation, air quality of the city was found to be affected significantly by human activities. More populated area showed worse level of air quality. Any development in the city resulted in more automobile activity and deterioration of air quality. This simulation result thus explains that a rapid increase of automobiles accompanied by the land development near local roadsides in the city is the major cause of air pollution in Siheung city. In this study, if urban activities are vigorous in an area with high air pollution, people are more likely to be exposed to air pollutant under the bad environmental conditions. On the other hand, if urban activities are less vigorous in an area with high pollution or if urban activities are vigorous in an area with less pollution, the environmental condition was positive. The APEI (Air Pollution Exposure Index) was developed based on these considerations. Scenarios 1 and 2 were compared and analyzed using APEI. In result, scenario 1 is the case in which land is developed and used in an environmentally favorable manner. From this study, it was proved that the impact of air pollution on human health can be minimized with proper land use. The result form the current study can be used as the basic information to solve problems from improper land utilization and air pollution (by road traffic). It also can be utilized to evaluate air pollution level according to land use and road characteristics and to help to choose the best location of land use to comply with the road function and status.

뇌졸중 환자를 위한 일상생활활동 평가도구에 관한 체계적 고찰 (A Systematic Review of Assessment Tools of Activities of Daily Living for Stroke Patients)

  • 목적: 본 연구는 뇌졸중 환자를 위한 일상생활활동 평가도구에 관해 체계적 고찰함으로써 임상 및 연구에서 자료로 활용되고자 하였다. 연구방법: 2001년부터 2016년까지 학회지에 게재된 논문을 검색하기 위하여 Pubmed, Embase, Cochrane, NDSL 데이터베이스를 사용하여"stroke AND activities of daily living AND occupational therapy AND assessment OR evaluation OR measurement"를 검색하여 체계적 고찰을 하였다. 평가도구의 종류와 사용빈도를 분석한 후 이에 대한 경향을 파악하였다. 또한 국제 기능 장애 건강 분류(International Classification of Functioning, Disability and Health; ICF) 모델의 분류 기준에 맞추어 활동과 참여에 대한 평가도구를 분류하였다. 결과: 총 111개의 논문이 최종적으로 분석되었으며, 30개의 평가도구가 확인되었다. 최근 들어 뇌졸중 환자를 대상으로 한 연구 게재편수가 증가함에 따라 평가도구의 종류와 사용빈도가 증가하였다. 가장 많이 사용되는 평가도구는 기능적 독립성 측정(Functional Independence Measure; FIM), 바델 지수(Barthel Index; BI)와 캐나다작업수행측정(Canadian Occupational Performance Measure; COPM)이었다. ICF 모델의 분류 기준에 따라 분석한 결과 대부분의 도구는 활동에 대한 평가도구로 참여에 대한 평가도구 수는 매우 적었다. 결론: 본 연구는 뇌졸중 환자의 일상생활활동 수행능력을 측정하기 위해 사용된 평가도구를 체계적으로 고찰하였다. 연구 결과 FIM, BI와 같은 특정한 도구가 많이 사용되었다. 앞으로는 평가도구의 특성에 맞춰 적절한 평가가 이루어 질 수 있도록 다양한 평가도구를 사용한 연구가 진행되어 근거기반의 작업치료를 뒷받침 해 주어야 한다. 또한 ICF 모델의 분류 기준 중 참여에 대한 평가도구의 개발이 이루어져야 할 것이다.

편측 견부 통증 환자의 안정시 견갑골 자세와 통증수준과의 상관성 연구 (The Relationship Between Resting Scapular Position and Pain Level in Unilateral Shoulder Pain)

  • The purpose of this study was to investigate the most effective and comprehensible method for the assessment of resting scapular position (RSP) and pain level (PL) in unilateral shoulder pain (USP). Fifty volunteers with USP were involved in the study. Resting scapular assessments of the patients' pain sides (PS) and non-pain sides (NPS) were evaluated. The assessment tools for RSP are: 1) sternal notch (SN) to coracoid process (CP) distance 2) 3rd thoracic spinous process (T3S) to posterolateral angle of acromion (PLA) distance 3) scapular index 4) 8th thoracic spinous process (T8S) to inferior angle of scapular (IAS) distance 5) supine measurement of pectoralis minor (PM) distance 6) standing PM distance 7) PM index (PMI) and 8) PM pain. The paired t-test was used to compare PS and NPS in RSP. Pearson correlation analysis was used to confer a relationship between the PL and RSP. The results of this study indicated that: 1) all the variables between the PS and NPS for RSP were statistically significant(p<.05) and 2) the PMI showed the strongest relationship in the correlation analysis between RSP and PL(p<.05, r=.37). Therefore, it can be concluded that there is a relationship between PMI and PL and it is suggested that an assessment tool using PMI to diagnose shoulder pain would be clinically effective.

논과 밭토양의 질 평가 방법 (Soil Quality Assessment Method of Paddy and Upland)

  • 우리나라 논과 밭토양의 질의 현황에 대한 평가방법을 개발하고자 2000년부터 2003년까지 실시한 농업환경변동조사사업의 결과를 이용하여 세부지표 즉 토양의 주요특성의 선정, 선정된 각 지표의 점수화 또는 등급화와 정량화 방법과 각 세부지표를 종합하여 한개의 토양의 질 지표로 만드는 방법을 검토하였다 한편, 본 시안을 이용하여 전국수준에서 우리나라 논 밭 시설원예지 토양의 질 현황에 대한 평가를 시도한 결과 논토양은 매우 높음, 밭토양은 높음, 그리고 시설원예지 토양은 보통으로 나타났다.

경기도내 유통 과실류의 잔류농약 위해평가(2006~2010) (Risk assessment of pesticide residues in fruits collected in Gyeonggi-do, Korea from 2006 to 2010)

  • 2006년부터 2010년까지 경기도내에서 유통되는 과실류 33품목에 대하여 잔류농약 모니터링을 수행하였다. 총 2,558건 중 431건(16.8%)에서 잔류농약이 검출되었으며, 12건(0.5%)에서 잔류허용기준(MRLs)을 초과하였다. 연간 검출률은 6.9~19.4%, 부적합률은 0.3~0.9%였다. 23품목에서 62종의 잔류농약이 검출되었고, 4품목에서 8종의 농약이 부적합 발생하였다. Chlorpyrifos의 검출빈도가 가장 높았다. 과실류 중 감귤류가 검출률과 부적합률 모두 가장 높았다. 검출빈도는 유기인계 농약이(35%) 가장 많았고, 용도별로는 살충제가(57%) 가장 많았다. 결정론적 만성식이위해평가 결과 위해도(%ADI)는 전국민 하한치와 상한치가 각각 0.0000~0.7526과 0.0000~1.3237이었고, 섭취자군에서는 각각 0.0006~9.7801과 0.0058~15.9528로 나타나 전국민과 섭취자군 모두에서 안전한 수준으로 나타났다.

서울시 지역주민의 인구사회적 특성과 정신건강상태에 따른 건강관련 삶의 질 (Health-related Quality of Life Assessment by Socio-Demographic Characteristics and Mental Health among Seoul Citizens)

  • Objectives: This study was conducted to estimate the health-related quality of life using EQ5-D by socio-demographic characteristics and mental health among Seoul citizens for developing health promotion programs. Methods: The study subjects using cluster-stratified sampling method were 1234 adults over 19 years old from 17 dong, S-Ku in Seoul City. A cross-sectional study with face-to-face interview was used to collect data. A questionnaire measuring socio-demographic variables, Symptom Checklist-90-Revised (SCL-90-R) and Euroqol EQ-5D instrument. The Difference on the EQ-5D index among groups were tested with Mann-Whitney U test and Kruskal Wallis test. Results: The mean EQ-5D index for all subjects was 0.946(${\pm}0.105$). The EQ-5D index were significantly different by sex, age, marital status, eduction level and income. The EQ5-D indexes among clinical mental health groups were the lowest scores and significantly different from other groups. Conclusion: The findings suggest that the future intervention of health promotion programs should be carefully designed and tailored by socio-demographic variables. Especially, the mental health programs need to be developed.

Environmental Distribution of Air Pollutants and Environmental Risk Assessment in Regional Scale

  • We measured the concentrations of air pollutants at several residential sites, roadside sites and industrial sites in Iwate Prefecture, Japan. And the concentration distributions of air pollutants were estimated by atmospheric dispersion model using air emissions data. Based on those results, we calculated environmental risk of air pollutants emitted in Iwate Prefecture. As a result, it was found that the surround of factories with high emissions and highly toxic chemicals and the roadsides were high risk area, benzo(a)pyrene, formaldehyde and ozone exceeded the $10^{-5}$ risk level. Moreover, we tried to use "Loss of life expectancy: LLE" for an index to explain those risk to general public intelligibly. The total LLE of the carcinogenic chemicals was about 8.6 hours. Moreover, LLE of ozone was about 9.2 hours. Ozone has a big influence compared with the carcinogenic chemicals.

영양상담을 위한 전산화 프로그램 개발연구 (Development of computer programs for Nutrition Counseling)

  • The purpose of this study is to develop a computer system with data file and computerized programs for nutrition counseling. In this research, a 16 / XT personal computer (word : COBOL) compatible with IBM-PC/XT was used. Computer system developed for this study was as follows: Data files(food composition list, food exchange list, nutrition management comment, special diet therapy) were used for analysis the nutritional status and the ntrition education comment. (1) Programs for the nutritional status assessment 1) General information a) Name, age, sex, higher, weight, activity, disease and special diet b) Ideal body weight and Obesity assessment(Kaup index and Broca index) c) Rest and athletics status d) Biochemical data comparision with standard 2) Food Intakes 3) Nutrient Intakes a) Comparison of the amounts intaked with the recommended dietary allowances for present weight. b) Comparison of the energy composition rate intaked with the recommended dietary allowances for present weight. b) Comparison of the energy composition rate intaked with the recommended for present weight and ideal weight. c) Nutrient analysis by each meal and snack. 4) Food intakes from each food group and comparison with recommended 5) Special nutrient analysis. (2) Programs for the nutrition education based on nutritional status assessment. 1) Suggestion of number of food exchange group 2) Nutritional assessment and advise comments 3) Nutritional management comments 4) Special diet therapy In the study, the nutritioal status and nutrition education comments are based on individual data from nutrition counseling.

재해분석을 통한 배전선로 활선작업 공종별 위험지수 평가 (The Assessment of the Risk Index of Live-line Works on Distribution Line by the Accident Analysis)

  • The live-line works are very dangerous because of direct contacts with the distribution line or neighboring contacts. So the purpose of this study is to identify the risk factor by accident occurrence form and accident case analysis, and to suggest the quantified risk index by risk occurrence frequency and risk strength analysis. And the risk index assessment is researched by accident cases analysis on work type. Accident cases of transmission distribution line are researched based on data of the Ministry of Employment and Labor in the last ten-year period (2000~2009). In results of this paper, high risk isn't always a priority of safety measures. Risk occurrence frequency and risk strength have to be considered according to detail work types, work methods and conditions of field work. And safety management measures must be planned according to risk occurrence frequency and risk strength.

Assessment of Ki-67 for Predicting Effective Prognosis in Breast Cancer Subtypes

  • Ki-67 has been widely performed and become an important biomarker in worldwide clinics, but the standard cut off value of Ki-67 index in breast cancer is still controversy. The objective study was to understand the Ki-67 in breast cancer subtypes and to investigate relative risk of breast cancer subtypes according to Ki-67 cut off value in Korean breast cancer. Immunohistochemical staining (IHC) for estrogen receptor (ER), progesterone receptor (PR), human epidermal growth factor receptor 2 (HER2), and Ki-67 index was examined from 123 breast cancer patients. Ki-67 index was significantly overexpressed in PR, ER, and HER2 hormone negative groups. Ki-67 index in Triple negative and HER2 subtypes was shown significantly higher than that in Luminal A and Luminal B subtype. Then, we compared the relative risk of each subtype according to 14% and 20% Ki-67 cut off value, which were applied in most clinics. Especially, 20% Ki-67 cut off value in HER2 and Triple negative subtypes was shown 8.41 fold and 2.83 fold higher relative risk than this in Luminal A subtype. Moreover, Ki-67 index in HER2 2+ or 3+ status showed significantly overexpressed than this in HER2 1+ status. At the 20% Ki-67 cut off value, HER2 1+ or 2+ status and 3+ status showed significant difference. Therefore, the 20% Ki-67 cut off value will be useful as a precise prognostic management and helpful for interpreting diverse outcomes of other subtypes in breast cancer patients.
