Relationship between the quality of life of the caregiver and motor function of children with cerebral palsy

  • Objective: The purpose of this study was to evaluate the relationship between quality of life (QoL) of the caregiver and disease severity with motor function in children with cerebral palsy (CP). Design: Cross-sectional study. Methods: Research data were collected in the Rehabilitation Clinic of Daegu University. The Gross Motor Function Measure (GMFM-88) and the functional independence measure (FIM) were used for assessment by three occupational therapists, and the 36-item short form health survey (SF-36) were applied to the caregivers. One hundred six caregivers of under 18 years who were diagnosed with CP completed a survey and interview. The caregivers' QoL was evaluated using Medical Outcomes Study SF-36. The children's motor function was scored using GMFM-88 in five dimensions: lying and rolling; sitting; crawling and kneeling; standing and walking, running and jumping and CP's FIM scores. Results: Out of the 8 domains of the Medical Outcomes Study SF-36, the "physical functioning", "physical role functioning", "mental health", and "bodily pain" domains were significantly correlated to "total" percentage scores of the GMFM-88 (p<0.05). In addition, the "mental health" domain was correlated to each subdomain of the GMFM-88, which includes, "lying and rolling", and "crawling and kneeling". Similarly, of Medical Outcomes Study SF-36, "physical functioning", "bodily pain", and "mental health" domains were significantly correlated with "transfer" and "locomotion" of FIM scores (p<0.05). Conclusions: This study showed that the QoL of the caregivers were well correlated with the motor function of children with CP. It is also important to support not only physical health but also psychological health of caregivers of children with CP, especially those with severe motor function.

뇌성마비 아동의 능력, 수행능력, 수행 간 상관관계 (The Correlation between Motor Capacity, Capability, and Performance in Children with Cerebral Palsy)

  • 본 연구의 목적은 뇌성마비 아동의 운동활동에 대한 3가지 개념(능력, 수행능력, 수행)을 구분하고 능력(표준화된 환경에서 할 수 있는 것), 수행능력(일상 환경에서 할 수 있는 것), 수행(일상환경에서 하는 것) 사이의 관련성을 평가하기 위한 것이다. 연구는 평균연령 6.6세의 뇌성마비 아동 40명(남자 19명, 여자 21명)의 평가 기록을 이용한 단면조사 분석으로 수행되었다. 대운동기능분류체계(GMFCS)에 따른 중증도는 1단계 13%, 2단계 10%, 3단계 43%, 4단계 33%, 4단계 3%였다. 운동활동 능력은 대동작기능평가(GMFM)를 이용하여 측정되었다. 수행능력과 수행은 소아장애평가척도(PEDI)의 2가지 척도(기능적 기술, 보호자 도움)를 사용하였다. 연구결과 능력과 수행 사이의 상관관계는 0.811(p<.05)이었고, 수행능력과 수행 사이의 상관관계는 0.845(p<.05)로 가장 높았다. 능력과 수행 사이의 상관관계는 0.711(p<.05)로 가장 낮았다. 뇌성마비 아동의 운동에 대한 능력과 수행능력은 일상생활에서의 수행을 완전히 반영하지 못한다. 수행에는 배경요인(특히, 가족기능과 같은 사회적 요인)이 영향을 미치기 때문이다. 따라서 본 연구는 뇌성마비 아동의 능력, 수행능력, 수행에 대해 명확하게 구분하여 평가하는 것이 필요하다고 제안한다.

Effect of Home Activity Support Program on Daily Living Performance Skills in Children with Cerebral Palsy and their Parents

  • Objective: This study attempted to find out if it changes the child's ability to perform daily life activities when visiting the familiar environment and daily living space of children with cerebral palsy and conducting a home activity support program for children and parents. Design: Randomized Controlled Trial Methods: Among 22 children aged 3 to 12 years old, they were assigned to the intervention group and control group. Of these, 12 boys and 10 girls participated in the study. Gross motor function measure and upper extremity function evaluation were used to measure the physical function of children with cerebral palsy, and self-care skills, mobility and social functions were evaluated in the pediatric evaluation of disability inventory. In addition, a parenting sense of competence was used to find out the efficacy of parents in raising children. After the pre-evaluation, basic rehabilitation treatment and intervention programs were applied to the intervention group, and only basic rehabilitation treatment was performed to the control group, and post-evaluation was performed 8 weeks later. Results: As a result of the study, among the items that measured the gross motor function, upper limb function, and daily life performance ability of the intervention group in the difference between the intervention group and the control group, statistically improved in personal processing and movement (p<0.05). In addition, the parenting sense of competence children in the intervention group was statistically significant (p<0.05). Conclusions: The home activity support program will help strengthen the ability of cerebral palsy children to perform daily life as a way to set mutually agreed goals with their families or children and achieve them in a familiar environment.

정신지체 아동의 보행능력 향상을 위한 보봐스 접근법 : 개별실험연구 (The Bobath Approach for Walking Improvement on Child with Mental Retardation)

  • Purpose : In this case report, we demonstrated the improvement of gait ability on the child who has mental retardation with incomplete gait pattern. Methods : The subject was a 4 years old boy with mental retardation. We applied the Bobath approach to the subject. Treatments included to facilitate trunk alignment and stability, and to train weight bearing and shifting, to facilitate pelvis posterior-anterior movement, and to train walk especially stance phase and assist up-down stairs locomotion in environment similar to actual daily life. It was performed 24 sessions for 12 weeks. Results : With this treatment, he could accomplish dynamic standing stability and he could independent walk at the out door after 12 weeks. In gross motor function measure(GMFM), total motor function was improved to 85.6% from 75.7%. Conclusion : The gait ability of child with mental retardation was improved by using the bobath approach.

뇌성마비 아동의 기능적 수준 분류 체계의 유용성 (Utility of Function Classification System in Children with Cerebral Palsy)

  • 이 연구는 뇌성마비 아동의 기능적 수준 분류 체계의 유용성을 알아보기 위해 실시되었다. 이를 위해 2008년 9월부터 2010년 8월까지의 기간 동안 뇌성마비 아동 217명을 대상으로 대동작 기능 분류체계(GMFCS), 손 기능 분류 체계(MACS), 일상생활 활동을 측정하고 도구 간의 관계를 알아보았다. 그 결과, 대동작 기능 분류체계와 일상 생활 활동은 모든 하위 영역 및 총점과 유의한 상관이 있는 것으로 나타났다(p<.05). 손 기능 분류체계와 일상생활 활동은 이상운동형 아동에서 일상생활 하위 영역 중 의사소통과 상관이 유의하지 않은 것을 제외하고 모든 하위 영역 및 총점과 유의한 상관이 있는 것으로 나타났다(p<.05). GMFCS와 MACS의 관계를 알아본 결과, 가장 많은 분포를 나타낸 것은 GMFCS의 경우 1수준(20.3%)과 5수준(40.6%)이었고, MACS의 경우는 2수준(48.8%)과 5수준(16.6%)이었다. 결론적으로, 뇌성마비 아동의 기능적 수준 분류 체계인 GMFCS와 MACS는 임상적으로 유용한 평가 체계로 사용될 수 있을 것으로 생각된다.

발달지연 아동의 인지발달과 운동발달의 양태분석 (Characteristic Analysis of Cognitive and Motor Development in Children With Developmental Delay)

  • The purpose of this study was to investigate the characteristics of cognitive development and motor development in children with developmental delays. Subjects were 43 children with a mean corrected age of 19.19 month (SD=7.48). The Gross Motor Function Measure (GMFM) and the Bayley Scale of Infant Development-second edition (BSID-II) were administered to the total children. Statistical analysis was done by paired t-test and Pearson correlation, using SPSS software. The results were as follows: 1) Most of the children with developmental delays showed statistical delays on the mental scale rather than on the motor scale. 2) The correlation coefficients of cognitive performance and motor performance were ranged between r=.38 and r=.83 in children with developmental delays. 3) Lower motor performance showed significant association with lower cognitive score. The findings suggest that cognitive and motor performance in children with developmental delays was significantly correlated.

취학 전 유아의 학교준비도 검사 개발 연구 (A Study on the Development of the School Readiness Inventory for Preschoolers)

  • The purpose of this study is to conceptualize the definition of school readiness specific to the context of the Republic of Korea. The school readiness test can be used as basic data to enable early childhood educational institutes to determine the general readiness of preschoolers and provide support to fit individual characteristics. For this purpose, six stages of the school readiness development process were considered, that is, draft development, primary preliminary survey, primary expert verification, secondary expert verification, secondary preliminary survey, and main survey. In the main survey, tests were conducted using the school readiness final draft targeting 344 preschoolers living in Seoul and Gyeonggi-do. The item discrimination power analysis and factor analysis for the examination area and question area were also conducted. Consequently, a test tool was developed on the basic of the results obtained, consisting of 129 items in the five areas of knowledge, function, adaptation, tendency, and attitude. The knowledge area was divided into general, linguistic, and mathematical knowledge, and the function area into fine motor function, basic living ability, expressive function, and gross motor function. The adaptation area was classified into adaptation activity, separation anxiety, maladaptive behavior, and environment adaptation; the tendency area into learning tendency; others understanding, and individual tendency, and the attitude area into rule compliance and life habits.

물리치료사의 관점에서 뇌성마비 아동과 청소년을 위한 ICF-Core Set을 기반으로 한 접근법의 효과: 단일 사례 연구 (An ICF-Core Sets for Children and Youth With Cerebral Palsy Based Approach From a Physical Therapist Perspective: A Single Case Study)

  • Background: The International Classification of Functioning, Disability, and Health-core set (ICF-core set) for children and youth (CY) with cerebral palsy (CP) provides a useful conceptual framework and a guide for health care planning and measuring the changes brought by interventions across a multitude of dimensions from body functions to personal activities, social participation, and environmental factors for them. Objects: This single case study was reported to illustrate the use of a goal directed approach in applying the ICF-core set for CY with CP from a physical therapist perspective. Methods: An eleven year old boy with spastic CP, Gross Motor Function Classification System (GMFCS) level V, and his mother participated in an evaluation of his functioning state. The intervention goal was set through an interview using the ICF-core set, Canadian Occupational Performance Measure (COPM) and Goal Attainment Scale (GAS). Physical therapy was carried out on an outpatient basis using a goal directed approach for 30 min, 1 time/week during 12 weeks and the boy's gross motor function was assessed using the Gross Motor Function Measure (GMFM)-66 version (item set 2) before and after the intervention. Results: As measured by the boy's mother, the COPM score showed a meaningful clinical change (performance=mean 3.5, satisfaction=mean 2.5) and the T-score of GAS changed 34.4 after the goal directed approach. The GMFM-66 (item set 2) score changed from 31.8 to 38.7 and evaluation using the ICF-core set displayed improvement in 6 items of activity level between before and after the intervention. Conclusion: The ICF-core set for CY with CP is useful for understanding the overall functioning of CY with this condition and provides an opportunity to share and integrate information and opinions from different disciplines. We consider it as a useful tool in the universal language for the therapy and education of CY with CP.

경직형 뇌성마비 아동의 손 기능 분류 체계와 기능적 수행도 평가 간의 상관 (Correlation between Manual Ability Oassification System and Functional Evaluation in Children With Spastic Cerebral Palsy)

  • 이 연구는 뇌성마비 아동의 손 기능을 평가하기 위해 개발된 손 기능 분류 체계(Manual Ability Classification System: MACS)를 이용하여 경직형 뇌성마비 아동의 손 기능을 평가하고 손 기능과 기능적 수행도 평가인 일상생활활동평가(Functional Independence Measure of Children: WeeFIM), 대운동 기능 분류체계(Gross Motor Function Classification System: GMFCS)의 관계를 알아보고, 이를 통해 뇌성마비 아동의 손 기능 평가 체계로써의 MACS에 대한 기초의 자료를 제공하는데 그 목적이 있었다. 경직형 뇌성마비 아동 60명을 대상으로 아동의 손 기능과 대동작 기능, 일상생활활동을 평가하였다. 평가한 자료를 바탕으로, MACS와 WeeFIM 및 GMFCS와의 관계와 손 기능 수준에 따른 일상생활활동의 차이를 알아보았다. 그 결과, MACS와 GMFCS의 상관은 유의수준.05에서 유의한 것으로 나타났으며, 그 정도는 r =.659이었다. WeeFIM과의 상관을 알아본 결과에서도 총점과의 상관은 유의수준.05에서 유의한 것으로 나타났으며, 그 정도는 r = -.576이었다. 손 기능 분류 체계로 평가한 수준에 따른 일상생활 활용의 차이를 알아본 결과 유의한 차이가 있는 것으로 나타났다(p <.05). 뇌성마비 아동의 손 기능 평가 도구로써 MACS는 임상적으로 유용한 평가 체계로 사용될 수 있음을 알 수 있다.

부모교육 참여집단과 비참여집단 자폐아동의 행동병리 및 발달기능에 대한 비교연구 (CHANGES OF PSYCHOPATHOLOGY AND DEVELOPMENT IN AUTISTIC CHILDREN THROUGH PARENT EDUCATION PROGRAM)

  • 본 연구는 부모가 부모교육에 참여한 자폐아동 집단과 비참여집단 자폐아동의 행동병리 및 발달기능을 비교해 보고자 총 30명의 자폐아동을 대상으로 Schopler의 PEP를 이용하여 관찰 측정하였는데 결과 부모교육 참여군 자폐아동이 교육후 정동, 언어의 행동영역에서 보다 적절한 행동이 늘어났고 지각, 소근육운동, 대근육운동, $눈{\cdot}손협응,$ 인지수행, 인지언어 등 여러 발달기능영역에서 유의한 증진을 보였으며 또한 비참여군 자폐아동보다 모방, 대근육운동, $눈{\cdot}손협응에서$ 유의하게 높은 발달변화를 보였다. 물론 부모교육 참여군 자폐아동이나 비참여군 자폐아동 모두가 행동병리 및 발달기능에서 성숙에 의한 변화를 보이기는 했으나 참여군 자폐아동이 더 여러 영역에서 발달변화를 보였고 두 집단간에 유의하게 높은 차이를 보이는 영역이 있다는 것은 부모교육 프로그램이 자폐아동에 대한 이해를 돕고 일상속에서의 모방학습, 신체적 접촉이나 대근육운동 등을 이용한 감각통합기능 증진, 언어발달, 적절한 행동의 증진에 효과적이었다고 말할 수 있다. 앞으로 더욱 구체적이고 적극적인 교육방법을 도입함으로써 보다 많은 적절한 행동의 증진과 발달을 가져올 수 있을 것이다.
