Stellate Ganglion Blocks for Treatment of Impotence (발기 부전증 환자의 치료에 성상신경절 차단 1예 보고)

  • Impairment of erectile function, although normally a benign disorder, has a profound impact on the well-being of many men. Penile erection is produced by a complex series of events involving cognition, peripheral sensory input, central and peripheral autonomic nervous events and hormonal vascular events. The multiplicity of potential determinants identified suggest that impotence does not inexorably accompany age, but it is an arugmented significantly modifiable paraaging phenomena. A 53 year old male patients developed sudden impotence. The patients did not respond to intracavernous injection of papaverine. This patient was successfuly treated with 4 repeated stellate ganglion blocks.

Management of a Retained Bullet in the Corpora Cavernosa after a Civilian Gunshot Injury: A Rare Scenario

  • A 24-year-old man presented to King Saud Medical City emergency department with a retained bullet in his penis following a civilian exchange of gunfire. After an initial assessment, the patient was taken to the operating room. Penile exploration was performed, the bullet was extracted successfully, and the corpora cavernosa were repaired properly. A 6-week follow-up showed full healing with preservation of erectile function. Immediate surgical intervention is mandatory as the primary treatment for penile gunshot injury to ensure proper anatomical and functional recovery.

A study of Sexual Function, Sleep Quality and Depression in Male Patients with Liver Cirrhosis (남성 간경변 환자의 성기능, 수면의 질, 우울에 관한 연구)

  • This study is a descriptive study to investigate the relationship between sexual function, sleep quality, and depression in male cirrhosis patients. Data were collected from a structured questionnaire from August 16, 2017 to January 30, 2018. Data were collected by t-test, ANOVA, Scheffe test, Pearson correlation. AS a result, the lower the sexual function of male cirrhosis patients, the lower the quality of sleep (r=-.232, p =.005) and the higher the depression score (r=-.381, p<.001) The lower the score, the higher the score of depression (r=.561, p<.001). Sexual function, quality of sleep and depression were found to be statistically significant. The results of this study can be used as basic data for the development of nursing intervention for male cirrhosis patients.

The Effects of a Sex Education Program in Middle Aged Men with Diabetes and Their Spouses (중년남성 당뇨병 환자와 배우자의 성교육 프로그램 효과 연구)

  • Purpose: The purpose of this study was to evaluate the effects of a sex education program for middle aged men patients with diabetes and their spouses. Methods: The research design for this study was a non-equivalent control group quasi-experimental study. The subjects were 23 diabetic patients and their spouses with half of the subjects exposed to a structured sex education program. Sexual Beliefs and Information Questionnaire, Dyadic Sexual Communication Scale, Sexual Frequency Scale, Derogatis Sexual Function Inventory and International Index of Erectile Function-5 were used for data collection. Results: In the experimental group, sexual knowledge of diabetic patients and sexual communication with their spouses were improved after two weeks following the program. Also, in the experimental group, sexual knowledge, sexual communication, and sexual frequency in both diabetic patients and their spouses were improved after six weeks following the program. In the experimental group, sexual satisfaction of diabetic patients was improved after six weeks following sex education program, however their spouses in the experimental group showed no significant differences than those of the comparison group. Conclusion: The results of this study suggest that sex education program for both diabetic patients and their spouses could be utilized in public health centers, hospitals, and clinics.

Effect of Korean Red Ginseng on Sexual Dysfunction and Serum Lipid Level In Old Aged Men (남성 갱년기 성기능 장애에서 홍삼이 성기능과 혈중 지질농도에 미치는 효과)

  • To investigate the effect of Korean red ginseng (KRG) on male sexual function and serum lipid level, the results of KRG treatments were compared to placebo group. A total of 35 patients with psychogenic impotence were assigned to medication group with KRG (n=28) and placebo (n=7). KRG and placebo were given to each group for 2 months. Changes in symptoms such as frequency of coitus, morning erection, penile rigidity and tumescence were significantly higher in the group receiving KRG than placebo group. The overall therapeutic efficacies on erectile function were 67% for KRG group and 28% for placebo group (p <0.05). In the group treated with 9 tablets of KRG, there was tendency of better efficacy as compared to the group treated with 6 tablets of KRG. The level of serum high density lipid- cholesterol (HDL-cholesterol) was significantly elevated (p < 0.05) while the other serum lipid such as total cholesterol, tiglyceride and low density lipid-cholesterol (LDL-cholesterol) , were not changed after administration of KRG. This effect was observed significantly in the group in which KRG exerted its effect on sexual function(p < 0.05). From the above results, the administration of Korean red ginseng has shown to have superior of facts as compared to the placebo. The tendency of good effect was increased when a large amount of KRG was administered. The mechanism of KRG in improving sexual function would be the result of elevating the level of geum HDL-cholesterol in impotent patients. The effect of KRG was produced when the level of cholesterol was high before starting KRG medication. Therefore, the effect of KRG could be observed more intensively when the patients have high serum cholesterol level.

Study on Clinical Diseases of Blood Stasis Pattern (어혈증(瘀血證)의 임상 질환 범위에 대한 고찰)

  • Objectives : This article is a study on to which categories of modern diseases blood stasis patterns are assigned and the meaning of blood stasis interpreted with perspectives of Korean Medicine and modern medicine. Methods : We reviewed "Neijing", "Shanghanlun", "Yilingaicuo", "Xuezhenglun" and other books and modern clinical papers related with blood stasis. Results : 1. Blood stasis patterns are related with disorders of hemorrheology, hemodynamics, platelet function, microcirculation, microelements and endothelial damage. 2. From the types of syndrome differentiation, diverse diseases classified in type of qi deficiency with blood stasis and type of blood stasis due to qi stagnation are reported, which reflects qi and blood are closely connected. And many diseases are classified in type of kidney deficiency with blood stasis, which has something in common with chronic diseases can achieve effect from treatment considering blood stasis. 3. Diseases related with kidney involve menopausal disorder, mazoplasia, prostatitis, erectile dysfunction, chronic nephritis, renal calculus, osteoporosis and bursitis. Diseases related with heart involve coronary artery disease, arrhythmia and cerebral thrombosis. Diseases related with spleen involve gastritis, colonitis and digestive organ ulcer. Diseases related with liver involve hepatitis, hyperthyroidism and stroke. Diseases related with lung involve neurodermatitis, bronchitis and paranasal sinusitis. Conclusions : Blood stasis pattern which is one of the areas to draw medicine's attention has broad clinical application.

Effect of Serotonin Uptake Inhibitors on Serotonin Metabolism in the Hypothalamus of Freely Moving Rats

  • Tricyclic antidepressant clomipramine or selective serotonin reuptake inhibitors (SSRIs) have been commonly used for the treatment of premature ejaculation. In the present study, we analyzed the concentrations of serotonin and 5-hydroxyindoleacetic acid (5-HIAA) in the medial preoptic area (MPOA) of the hypothalamus by awakening animal microdialysis following administration of clomipramine and various SSRIs. We then compared the serotonin metabolism and clinical effects of clomipramine and SSRIs on premature ejaculation. Basal extracellular serotonin level in the MPOA was higher than other brain regions and it was significantly increased by clomipramine and the SSRIs. The rank order of the concentration of serotonin at the MPOA was clomipramine, sertraline, paroxetine and fluoxetine and the concentrations of 5-HIAA was vice versa. The changes in serotonin concentration at the MPOA appeared closely associated with the clinical effects of these drugs on premature ejaculation. These results suggest that the serotonergic neuronal activity in the MPOA may have an selective inhibitory influence on ejaculation, and the effects of clomipramine and SSRIs on erectile function are mainly mediated by MPOA of the hypothalamus.

Cumulative Effects of Constituents from the Mushroom Calvatia nipponica on the Contractility of Penile Corpus Cavernosum Smooth Muscle

  • Calvatia nipponica, a puffball mushroom (Agaricaceae), is thought to be an aphrodisiac, as this mushroom is traditionally known to improve sexual function in males. As part of the systematic study to determine the bioactive secondary metabolites from C. nipponica responsible for aphrodisiac effects, chemical analysis of methanol (MeOH) extracts of the fruiting bodies of C. nipponica resulted in the isolation of two major compounds: N,N-dimethyl-anthranilic acid (1) and (7Z,10Z)-7,10-octadecadienoic acid methyl ester (2). Compounds 1 and 2 were evaluated for cumulative dose-dependent relaxation responses to precontracted penile corpus smooth muscle (PCCSM). Results show that compounds 1 and 2 exhibited a maximum relaxation effect of 20.33 ± 2.18% and 24.63 ± 3.60%, respectively. These findings indicate that compounds 1 and 2, major components of C. nipponica, could potentially be used to treat erectile dysfunction, functioning as natural aphrodisiacs.

Study on Clinical Diseases of Yang Deficiency Pattern (양허증(陽虛證)의 임상적 질환 범위에 대한 고찰)

  • Yang deficiency pattern is a representative syndrome differentiation. This article is a study on to which categories of modern diseases yang deficiency is assigned by reference to modern clinical papers and the meaning of yang deficiency interpreted with a perspective of Korean Medicine and a modern perspective. Yang deficiency, yang qi deficiency, lack of yang qi and yang qi debilitation are the words found in "Nei Ching" and yang qi can be interpreted as something to warm, drive and arouse. Zhangzhongjing considered recovery or loss of Yang as the key to life in "Shanghanlun". Danxi proposed "Yang being liable to hyperactivity, Yin being insufficient" and emphasized pathological ministerial fire of Yang exuberance rather than physiological ministerial fire of Yang deficiency. Zhangjingyue proposed "Yang not being in excess, Yin being often deficient" and understood growth and decline of yin qi are all led by yang qi and put emphasis on true yin in addition to yang qi. Diseases of yang deficiency pattern are related with decline of metabolic level, hypofunction of internal secretion, disorder of immune function, disorder of automatic nerve system, sympathetic nerve inhibition, metabolic disorder of microelements, increase of cGMP, change of microcirculation, low speed of blood stream, kidney malfunction. Diseases related with kidney are sterility, polycystic ovary syndrome, spinal stenosis, edema, renal failure, IgA nephropathy, erectile dysfunction, nephritis, prostatitis, benign prostatic hyperplasia, decrease of adrenal cortical hormone by nephrotic syndrome, myelodysplastic syndrome. Disease related with heart are heart failure, arrhythmia, cardiomyopathy, atherosclerosis heart disease, hypertension, hyperlipidemia, pulmonary heart disease. Diseases related with spleen are irritable bowel syndrome, ulcerative colitis. Diseases related with liver are hypothyroidism, liver cirrhosis ascites, hepatitis B, chronic hepatitis, hepatic diabetes. Diseases related with lung are allergic rhinitis, cough variant asthma, bronchial asthma, pulmonary emphysema. And diabetes mellitus, metabolic syndrome, aplastic anemia, headache, encephalatrophy, Alzheimer's disease are also related with yang deficiency.

Restoration of Cavernous Veno-Occlusive Function through Chronic Administration of a Jun-Amino Terminal Kinase Inhibitor and a LIM-Kinase 2 Inhibitor by Suppressing Cavernous Apoptosis and Fibrosis in a Rat Model of Cavernous Nerve Injury: A Comparison with a Phosphodiesterase Type 5 Inhibitor

  • Purpose: To determine if chronic administration of Jun-amino terminal kinase (JNK)-inhibitors and LIM-kinase 2 (LIMK2)-inhibitors from the immediate post-injury period in a rat model of cavernous-nerve-crush-injury could normalize cavernousveno-occlusive-function, and to compare it with phosphodiesterase type 5 (PDE5)-inhibitors. Materials and Methods: A total of 75 12-week-old male Sprague-Dawley-rats were randomized into five groups: sham-surgery (S), cavernous-nerve-crush-injury (I), cavernous-nerve-crush-injury treated with 10.0 mg/kg LIMK2-inhibitor (L) or 10.0 mg/kg JNK-inhibitor and 10.0 mg/kg LIMK2-inhibitor (J+L) or 20.0 mg/kg udenafil (P) for five-weeks. Five-weeks after surgery, dynamic-infusion-cavernosometry, histological-studies, caspase-3-activity-assay, and Western-blot were investigated. Results: Group-I had lower papaverine-response, higher maintenance-rate and higher drop-rate, compared to Group-S. Group-L, Group-J+L and Group-P showed improvement in the three dynamic-infusion-cavernosometry parameters. The papaverine-response and drop-rate in Group-J+L and Group-P recovered to sham-control level, but those in Group-L did not. Regarding apoptosis, Group-I had decreased content of α-smooth-muscle-actin, increased caspase-3 activity and increased cJun-phosphorylation. The cJun-phosphorylation improved only in Group-J+L. The α-smooth-muscle-actin content and caspase-3-activity in Group-J+L and Group-P improved, but those in Group-L were not. Regarding fibrosis, Group-I had decreased smooth muscle (SM)/collagen-ratio, increased protein-expression of fibronectin, and increased Cofilin-phosphorylation. Cofilin-phosphorylation was normalized in Group-L and Group-J+L, but not in Group-P. SM/collagen-ratio and proteinexpression of fibronectin in Group-L, Group-J+L and Group-P improved. Conclusions: Our data indicate that chronic inhibition of JNK and LIMK2 can restore cavernous-veno-occlusive-function by suppressing cavernous-apoptosis and cavernous-fibrosis, comparable to the results by PDE5-inhibitors. Chronic inhibition of JNK and LIMK2 might be a potential mechanism-specific targeted therapy for cavernous-veno-occlusive-dysfunction induced by cavernous nerve-injury.
