전화상담원 도움이 심폐소생술 수행에 미치는 효과 (The Effect of Dispatcher-assistance on Cardiopulmonary Resuscitation Performance)

  • Purpose : The comparative effectiveness of without dispatcher-assisted telephone instruction CPR but received lectured and training(NDCPR) and without received lectured and training CPR but dispatcher-assisted telephone instruction(DCPR) in CPR. Methods : The CPR instruction to 774 students and faculties in universities and colleges. There selected without dispatcher-assisted telephone instruction CPR but received lectured and training(NDCRP) and without received lectured and training CPR but dispatcher-assisted telephone instruction(DCPR). Compare the effectiveness to sills performance of DCPR and NDCPR students and faculties of according to the CPR. Results : 397(51%) students and faculties was DCPR and 377(49%) students and faculties was NDCPR. There was no difference in the compression depth, hand position, adequacy of recoil, volume of ventilation, self-confidence, and willingness to do CPR between the two instructional methods. Conclusion : Under the guide of dispatcher-assisted CPR instruction integrated into mobil phone, novice could perform more effective CPR. This method could be used as a supplement to CPR practice and skill retention.

구급상황관리사에 의한 심폐소생술 안내 실태 연구 (A study on the cardiopulmonary resuscitation by the emergency medical dispatcher)

  • Purpose: The purpose of the study is to check up the status of 119 emergency control centers usage. Therefore, the status of use of 119 emergency control centers and the incidence of pre-hospital cardiac arrest patients were investigated. Methods: The emergency activity daily reports and first aid diaries of 119 emergency control centers from January to December 2018 were reviewed. For more accurate status analysis, Among the first aid guidance received in the emergency rescue standard system, the cardiopulmonary resuscitation guide log was reviewed. Results: In 2018, the total usage of the 119 emergency control centers was 1,358,356 calls, hospital guidance werethe most commom (n=629,676, 46.4%), followed by first aid (n=428,027, 31.5%), disease consultation (n=170,238, 12.5%), medical oversight (n=111,188, 8.2%), and interhospital transfer (n=5,052, 0.4%). Regarding the user number per 1,000 persons, Jeju was the greatest at 48.0, whereas Changwon was the lowest at 13.0. A total number of dispatcher-assisted cardiopulmonary resuscitation was 12.181. The time from report to chest compression were 156.2±80.8 seconds for those with previous cardiopulmonary resuscitation training and 168.0±79.3 seconds for those without such training (p<.05). Conclusion: The ratio of first aid instructions, including dispatcher-assisted cardiopulmonary resuscitation, among total usage of the 119 emergency control centers increased. Therefore, additional efforts are required to improve the quality and expertise of information provided through the 119 emergency control centers.

국제질병분류기준을 이용한 응급의료 전화상담 내용 분석 (An evaluation study of 1339 emergency medical dispatch based on ICD-9)

  • Purpose : To evaluate the emergency medical protocols and assessments through the case reports. Method : We studied retrospectively investigates the emergency calls through the 1339 emergency patients information center in 1997. Results : The emergency calls show an order : abdominal pain(13.5%), unconsciousness(12.0%), traffic accidents(7.4%), bleeding(6.6%), respiration difficulty(5.9%) by emergency medical protocols, and injury and poisoning(22.3%), symptom (51.6%), others (26.1%). Conclusion : The results suggest that a specific training program for emergency medical dispatchers should be established, and emergency medical dispatcher managers must review emergency medical dispatching cases on the basis of emergency medical protocols every year. Emergency medical services, furthermore, will continue to raise the standard for emergency medical dispatching.

  • Purpose: To evaluate the emergency medical protocols and need assessment through the case report Method: The study investigated the emergency call system through the 1339 emergency patients information center in 1997. Results: The emergency calls tend to follow general trends of the type of cases : abdominal pain (13.5%), unconsciousness(12.0%), traffic accidents (7.4%), bleeding (6.6%), respiration difficulty (5.9%). This was all collected through emergency medical protocols injury and poisoning (22.3%), symptom (51.6%), others (26.1%). 재난발생 시 일반응급의료체계에 관한 연구 (A Study for General Emergency Medical Service Systems in Disaster)

  • In Korea, There are many disasters, like the collapse of Sampung department store, the strike of severe typhoon 'Rusa' and the subway tragedy in Taegu, because of global warming, urbanization, high-density and high-rise of buildings. So, the government made 'The Framework Act' on the safe and management of disaster and 'The National Emergency Management Agency' was established. But emergency medical service systems in Korea is not growing so much. The purpose of this research is to give basic data for the development of emergency medical service systems in Disaster by comparing of disaster management systems and emergency medical systems among the nations of the world, analysing emergency medical systems in disaster in Korea and suggesting some improvement methods. The improvment methods are like this ; First, establishing the National Disaster Medical System in Korea, making the good triage by EMT, expansion of EMT's working area, developing protocols and framing of medical director increasing the working force of EMT, broad inner cavity of ambulance for treatment of patientent, supplement of professional equipments, active using of helicopters are needed in prehospital are. Second, equal establishment of emergency medical center and increase of working force of emergency medical team are needed in hospital area. Finally, enforcement of the dispatcher's qualification, smooth communication among EMSS systems and actualization of medical direction through screen are needed in the Telecommunication system.

대형화재에 대비한 응급의료체계에 관한 분석과 개선방안 (A Study on the Analysis and Improvement methods of Emergency Medical Service Systems for Large Scaled Fire)

  • In Korea, Crowds, as well as more complex and larger structures, have been caused more victims in the event of fire. In that situation, EMSS should have done triage the patients according to their severity in the field, treat with standard guidelines as like advanced burn life support and transfer to appropriate facility. But in many cases, they didn't practice like that. The purpose of this study is to give basic data for proper emergency medical services by analysing EMSS of large scaled fire in Korea and suggesting some improvement methods. The improvment methods are like this ; First, developing of protocols about burn patient is needed for EMT and dispatcher. Second, Legal approval for advanced treatment is needed for EMT. Third, Fire-helicoptors are needed in Daejeon and Jeju. And Emergency Medical Helicoptors are needed in EMSS. Forth, more advanced emergency medical centers of burn must be established. Sixth, more pocket-mask and burn sheet are needed for 119 rescuer. Finally, building owners must change inflammable materials as non-inflammable materials and educate fire-prevention and first-aid to employees.

구급출동지령서 개선을 통한 구급대원 현장 도착시간 단축방안 (Improvement Strategies of Arriving Time to the Scene by Enhancing EMTs' Recognition of Triage)

  • The purpose of this investigation is to enhance the survival rate of patients by transporting them to the hospital within the golden hour through the operational improvement of emergency dispatch instruction. To this end, problems and improvements of current operating system were derived by carrying out a survey against paramedics of Incheon city in 2012 and analyzing the current emergency dispatch instruction. This study analyzed the emergency activity daily reports for one year from January 1 through December 31, 2012 and researched the consciousness of 119 emergency medical technician. According to the analysis of the survey, there were no meaningful differences in the on-site arrival times per triage. Therefore, the item of 'Emergency Classification' specified in the emergency dispatch instruction needs to be integrated in the scheme of "triage". Also, the feedbacks of the emergency action log and the emergency dispatch instruction are necessary for 'duty for operation' to review the adequacy to the severity after the end of emergency operation. Finally, the improvement of the system for the continuous communication between the paramedics and the command staff is necessary. This improvements as stated above are expected to contribute to raise survival rate of patients.

일반대학생에 의한 자동제세동기 사용 능력 연구 (A Study on college students ability to use automated external defibrillator(AED))

  • Purpose: The purpose of the study was to compare the ability of college students to use automated external defibrillators (AED) and to provide a basic data for helping laypersons easily apply the AED. Methods: The subjects were randomly assigned to instruction groups. One group was instructed by pictures (group P), one by videos (group M), and one by telephone dispatchers (group D), Without employing cardiopulmonary resuscitation, a single rescuer was asked to apply defibrillation to a mannequin for practice in accordance with the instructions assigned to each group. Results: Regarding the proper pad attachment of the AED, group P followed the instruction $0.31{\pm}0.47times$, group M, $0.81{\pm}0.40times$, and group D $0.69{\pm}0.47times$. Regarding the instruction of taking the hand off a patient for analyzing cardiac rhythm and the instruction of taking the hand off a patient before defibrillation, group P did not follow the instruction; group M followed the instructions $0.50{\pm}0.51times$; and group D followed all instructions at all times. Conclusion: If a dispatcher gives real-time instructions for using an AED to laypersons wtih no experience at the time of AED use, the quality of laypersons' AED use will greatly improve.

제세동이 가능한 심정지 환자를 119구급상황관리센터 상황요원이 예측한 지령 분석 (Analysis of Predicted Instructions about Shockable Cardiac Arrest Patients by Dispatcher at 119 Emergency Situation Management Center)

  • 본 연구는 2010년 1월 1일부터 2011년 12월 31일까지 2년간 119구급대로 이송된 병원 전 제세동 가능한 심정지 환자를 대상으로 연구를 진행하였으며 연구대상자들의 구급활동일지와 구급지령서를 분석하였다. 119구급상황관리센터 상황요원(Dispatcher)이 가장 많이 예측한 지령은 실신으로 74건(33.3%)이었다. 다양하게 예측된 지령 중 실신, 흉통, 전신쇠약, 기타와 같은 지령은 심정지로 예측 불가능한 지령으로 112건(50.5%)을 차지하였으며, 심정지로 예측 가능한 의식장애, 호흡곤란, 무호흡, 심정지, 경련과 같은 지령은 110건(49.5%)을 차지하였다. 이 때 심정지로 예측 가능한 지령에서 목격자의 심폐소생술이 유의하게 높았다. 이와 같이 상황요원은 환자의 의식평가와 호흡평가를 구체적으로 질문하여 심정지를 정확하게 분류해야하며, 이렇게 분류된 심정지 환자는 목격자가 심폐소생술을 할 수 있도록 지도해야 한다. 또한 상황요원에 의해 심정지로 예측 가능한 지령(49.5%)을 받은 구급대원뿐만 아니라 심정지로 예측 불가능한 지령(50.5%)에서도 심정지가 아니라고 정확하게 판단되지 않았다면 현장 구급대원은 심정지 상황에 대비하여 현장출동 준비를 해야 한다.

응급의료전화상담원의 도움에 의해 교육 받지 않은 목격자의 제세동 시행 후 생존한 병원 전 심정지 1례 (A Pre-Hospital Cardiac Arrest Patient Surviving after Dispather-Assisted Defibrillation by an Untrained Witness)

  • 59세의 기왕력 및 가족력에 특이사항이 없는 남자가 가슴통증을 호소하다 의식을 잃고 쓰러지자 이를 목격한 가족이 119에 신고하였다. 응급의료상담원의 도움에 의해 목격자가 심폐소생술을 시행하였고 거주 중인 아파트에 설치된 자동심장충격기를 이용하여 제세동 1회를 시행하였다. 이후 도착한 119구급대에 의해 제세동 2회 실시 후 자발순환회복되어 인근 응급의료센터로 이송되었으며, 저체온 치료 15일 후 대퇴수행분류 1점으로 퇴원하였다. 현재 우리나라의 응급의료전환상담원 도움에 의한 목격자 심폐소생술 및 제세동의 시행은 시작 단계이지만 본 증례를 통해 충분한 효과를 볼 수 있다고 판단되며, 이에 따라 응급의료전화상담원의 적극적인 자동심장충격기 사용 안내와 그에 맞는 체계적인 교육이 필요하다. 특히 훈련받지 않는 목격자의 자동심장충격기의 사용을 도울 수 있고, 119구급대가 현장까지의 반응시간이 지연되는 원거리 지역에서 중요한 의미를 가질 것으로 사료된다. 또한 우리나라 PAD 프로그램의 양적인 보급뿐만 아니라 목격자의 접근성을 높일 수 있는 방안이 될 것이다.
