A Study on Subjective Symptoms of a Oral Malodor in Korean Adults (한국 성인의 주관적 구취 자각증상에 대한 연구)

  • The purpose of this study was to investigate the actual conditions of the subjective oral malodor perception of the survey subjects. Through the subjective questionnaire method over about 3 months from June to August in 2007, the data was collected targeting 275 adults. Statistical methods used were Chi-square test, Fisher's exact test and One-way ANOVA. And, the following results were obtained. 1. When having seen the ratio of those who were conscious of oral malodor, the adults, who responded with saying of feeling oral malodor at present and at ordinary times, accounted for 68.4% and 79.3%, respectively. And, the adults, who responded with saying of not feeling, were indicated to be 31.6%(at present) and 20.7%(at ordinary times), respectively. 2. Almost 83.5% of total people answered that they had most severe oral malodor at the time of awakening and 51.9% of subjects answered tongue contributed to the their oral malodor. 3. About the type of oral malodor, although 37.2% of the group thought the type of oral malodor was a food smell and fetid smell was second general burning smell occurred the time of hunger(22.5%). 4. In the answers of hoping for treatment of oral malodor, 66.9% of the people showed positive response to dental treatment for controlling their oral malodor. Especially 7.3% of subjects showed very big desire for treatment. 5. In the level of oral malodor that is felt now according to the time of a meal, the case of within 1 hour after having a meal was responded to be not felt the oral malodor with 44.7%. And, there was response with saying of not feeling oral malodor in 37.6% as for 1-2 hours, 16.3% as for 2-3 hours, 14.8% as for 3-4 hours, and 23.7% as for more than 4 hours. Thus, the level of oral malodor according to the time of a meal showed the statistically significant difference(p<0.01). Through the results of this study, it could be known about whether or not being conscious of oral malodor in general people, the main cause and region for oral malodor, the importance and practicing method of tongue brush through literature. Accordingly, there is necessity of further researching and comparing whether or not being conscious of oral malodor by diversely surveying relevancy with factors of causing oral malodor based on the objective oral malodor measurement value. The future diverse researches are thought to be necessarily performed in order to prevent or reduce oral malodor.

Characteristics of Intravenous Midazolam Sedation with Nitrous Oxide in Pediatric Dental Treatment (소아환자에서 midazolam 정주 및 아산화질소 흡입진정법 하 치과치료의 특성)

  • Midazolam is a short-acting benzodiazepine that is widely used in pediatric dental sedation. However, its clinical effectiveness as an intravenous sedative agent in children has not been widely documented. A retrospective study was conducted to evaluate the efficacy and safety of intravenous midazolam and nitrous oxide inhalation sedation in pediatric dental treatment. The subjects were 115 patients (118 cases) who received dental treatment under intravenous midazolam and nitrous oxide inhalation sedation. Demographic factors, general health status, sedation time, midazolam and nitrous oxide dosage, and success rate of sedation were evaluated from electronic medical records. Behavioral management was the main reason of choosing sedation. Mean duration of sedation was 56.7 minutes for surgical treatment, and 74.4 minutes for restorative treatment. The initial dosage of intravenous midazolam was 0.051 ± 0.019 mg/kg. In 34 cases (28.8%), additional midazolam of 0.036 ± 0.057 mg/kg was delivered during the treatment. The concentration of nitrous oxide was maintained between 40% and 50%. The success rate of sedation was 99% (n = 117). In 1 case, laryngospasm occurred and the patient was reversed with benzodiazepine antagonist, flumazenil. Intravenous midazolam sedation with nitrous oxide was shown to be clinically effective for the dental treatment in children, if administered by trained personnel and patients are carefully selected in accordance with guidelines.

Shisha Smoking and Associated Factors among Medical Students in Malaysia

  • Background: The aim of the study was to determine the prevalence of shisha smoking and associated factors among medical students in Malaysia. Materials and Methods: A cross-sectional study was conducted at the Management and Science University from December 2011 until March 2012. The questionnaire consisted of five sections including socio-demographic, social environment, knowledge about shisha, psychosocial factors, and personal shisha smoking behavior. Obtained data were analyzed using Statistical Package for the Social Sciences (SPSS 13). T-test was used to determine the relationships between shisha smoking and socio-demographic characteristic. Results: A total number of 300 medical students participated in this study. Mean age was $22.5{\pm}2.5$ years. The majority were female, Malay, single, from urban areas (67%, 54%, 97%, 73%; respectively). The prevalence of shisha smoking among medical students was found to be 20%. The study revealed that many students believed that shisha does not contains nicotine, carbon monoxide, does not lead to lung cancer, dental problems and does not lead to cardiovascular diseases (25%, 20.7%, 22.3%, 29%, 26.7%; respectively). Age and sex were found to be significantly associated with smoking shisha status among medical students (p=0.029, p<0.001; respectively). Furthermore, having parents, siblings and friends smokers of shisha were found to be significantly associated with shisha smoking status (p<0.001, p<0.001, p<0.001; respectively). Furthermore, family problems, problems with friends, financial problems and university life were found to significantly associated with shisha smoking status among medical students (p<0.001, p=0.002, p<0.001, p=0.002; respectively). Conclusions: There is a high prevalence of shisha smoking and a poor knowledge about its impact on health among medical students. More attention is needed to focus on medical education in this regard. The policies that are currently employed in order to reduce the cigarettes smoking should be applied to shisha smoking and shisha products.

EFFECT OF PH AND STORAGE TIME ON THE ELUTION OF RESIDUAL MONOMERS FROM POLYMERIZED COMPOSITE RESINS (산도변화와 침지시간이 광중합 복합레진의 잔류단량체 유출에 미치는 영향)

  • The purpose of this study was to determine whether pH and time has any influence on the degradation behavior of composite restoration by analyzing the leached monomers of dental composites qualitatively and quantitatively after storage in acetate buffer solution as a function of time using high performance liquid chromatography (HPLC) / mass spectrometer. Materials and Methods:Three commercial composite restorative resin materials (Z-250, Heliomolar and Aeliteflo) with different matrix structure and filler composition were studied. Thirty specimens (7mm $diameter{\times}2mm$ thick) of each material were prepared. The cured materials were stored in acetate buffer solution at different pH (4, 7) for 1, 7 and 45days. As a reference, samples of unpolymerized composite materials of each product were treated with methanol (10mg/ml). Identification of the various compounds was achieved by comparison of their mass spectra with those of reference compound, with literature data. and by their fragmentation patterns. Data were analysed statistically using ANOVA and Duncan's test. Results:1. Amounts of leached TEGDMA in Aeliteflo were significantly larger than those of UDMA in Z-250 and Heliomolar at experimental conditions of different storage time and pH variation (p<0.001). 2. As to comparison of the amounts of leached monomers per sorage time, amounts of leached TEGDMA in Aeliteflo and UDMA in Z-250 and Heliomolar were increased in the pH 4 solution more significantly than in the pH 7 solution after 1day, 7days and 45days, respectively (p<0.001). 3. In total amounts of all the leached monomers with storage times, the overall amounts of pH 4 extracts were larger than those of pH 7 extracts for all resin groups, but there was no significant difference (p>0.05).

Effect of worsening family economy due to COVID-19 on gingival bleeding and pain in Korean adolescents (우리나라 청소년의 COVID-19로 인한 가정경제악화가 치은 출혈 및 통증에 미치는 영향)

  • Objectives: This study aimed to determine the effect of COVID-19-induced household economic deterioration on gingival pain and bleeding. Methods: Data from the 16th Adolescent Health Behavior Survey 2020 were used. A total of 57,925 adolescents were included in this cross-sectional study. We evaluated the worsening of the COVID-19-induced household economic situation, gingival pain, and bleeding. All surveys were assessed in a subjective non-face-to-face online interview. Data were analyzed using descriptive statistics, chi-squared tests, and logistic regression. Results: The overall prevalence of gingival pain and bleeding symptoms in the study population was 19.2%. People with COVID-19 were more likely to suffer from gingival pain and gingival bleeding than those who did not have economic deterioration due to COVID-19. There was no economic deterioration due to COVID-19 (aOR=1.048, 95% CI=1.034-1.227) and worsened very much (aOR=1.358, 95% CI=1.164-1.585). Conclusions: There were more cases of gingival pain and gingival bleeding, which are early symptoms of periodontal disease, in patients with COVID-19 compared to cases without deterioration of the household economic situation due to COVID-19. It is necessary to measure and study gingival bleeding objectively rather than relying on self-reports.

The Convergence Influence of excessive smartphone use on attention deficit, learning environment, and academic procrastination in health college students (보건계열 대학생의 스마트폰 과다사용이 주의력결핍, 학습환경, 학업지연행동에 미치는 융합적 영향)

  • The purpose of the study is to investigate the convergence influence of excessive smartphone use on attention deficit, learning environment, and academic procrastination in health college students. A self-reported questionnaire was completed by 255 college students in Busan drom March 6 to June 12, 2017. The degree of smartphone overuse, lack of attention, learning environment, and academic procrastination according to smartphone use characteristics showed significant effects on the time spent on smartphones per day, awareness of smartphone addiction, and personal use of smartphones during class time (p<0.001). It was shown that smartphone overuse was positively correlated with attention deficit (r=0.870, p<0.01), learning environment (r=0.812, p<0.01), academic procrastination (r=0.772, p<0.01), and attention deficit showed a positive relationship with learning environment (r=0.918, p<0.01) and academic procrastination (r=0.798, p<0.01) Learning environment was positively correlated with academic procrastination (r=0.777, p<0.01). The influence factors of smartphone overuse were attention deficit (p<0.001), followed by academic delay behavior (p<0.01). It is necessary to establish a healthy learning environment through prevention and proper use of smartphone.

TREATMENT OF DENTOFACIAL DEFORMITY PATIENT WITH CEREBRAL PALSY (뇌성 마비를 동반한 악안면 기형 환자의 치험례)

  • Cerebral palsy(CP) is one of the most common motor disease, due to brain injury during fetal and neonatal development which results in neuromotor paralysis and associated neuromuscular symptoms. Features of CP include motor disability due to the lack of muscle control, often accompanied by sensory disorders, mental retardation, speech disorders, hearing loss, epilepsy, behavior disorders, etc. There are increasing chances of treatment of dental patients with cerebral palsy, as the occurrence of CP is increasing with the decrease in infant mortality and an increase in immature birth and premature birth and also, there is a trend to pursue of higher quality of life. Reports on the relationship between CP and maxillofacial deformity are uncommon, but it is well known that the unbalance and discontrol of the facial muscles, lip, tongue and the jaws leads to malocclusion and temporomandibular joint disorders, and statistics show that class 2 relationship of the jaws and open bite is frequently reported. However, it is difficult to perform maxillofacial deformity treatment, which consists of orthodontic treatment, maxillofacial surgery and muscle adaptation training, due to difficulties in communication and problems of muscle adaptation caused by difficulties in motor control which leads to a high recurrence rate. This case report is to trearment of maxillofacial deformity in CP patient. A 26 year old female patient came to the department with the chief complaint of prognathism of the mandible and facial asymmetry. According to the past medical history, she was diagnosed as cerebral palsy 1 week after birth, classified as GMFC, classII accompanied with left side torticollis. The patient's intelligence was moderate, and there were no serious problems in communication. For two years time, the patient underwent lingual frenectomy, pre-operation orthodontic treatment and then bimaxillary orthognathic surgery to treat mandibular prognathism and facial asymmetry followed by rehabilitatory exercise of facial muscle. After 6 months of follow up, there was a good result. This is to report to the typical signs and symptoms of DFD in CP patient and the limitation of the usual method of the treatment of DFD in CP patient with literature review.

The Characteristics and Medical Utilization of Migrant Workers (외국인 노동자의 특성과 의료이용 실태)

  • This study deals with the current medical utilization for migrant workers and the characteristics of them. The purpose of this study is to provide the basic information to establish proper medical policy. For the study self-made questionnaire was used, which was answered by 453 migrant workers working in the area of manufacturing and non-technical work in 10 cities like Seoul, Inchon, Namyangju, Sungnam, Kwangju, Pyungchon, Kunpo, Kimpo, Masuk in Kyungki-do and Chunan in Chungchungnam-do. Besides, 303 medical records of those who had visited free medical check-up center were analyzed. The period of accumulating data is 6 months, from November 1st, 1996 to April 30th, 1997. The characteristics of migrant workers and current medical utilization are analyzed by percentage and the relation between characteristics and current medical utilization were analyzed using ${\chi}^2$-test, t-test, ANOVA. The finding of this study was as follows : 1) The number of nationality was 16. The first majority was Philippians as 32.0%. Among 16 nationalities Southeastern and Northern Asians were 48.9%, Southwestern Asian was 46.5%, the rest was 7.3%. Men were 81.0%, those who are aged from 26 to 30 were 39.0%, Graduatee from high school 92.7%, Christians 56.3%, unmarried 55.4% and salary from 600,000 Won to 800,000 Won 53.8% averaging monthly payment 669,810 Won. As for their residence, those who resided over 3 years were 31.9% and the illegal residence reached 77.4%. As for Korean language, those who speak in middle level were 5.6%. 2) As for kind of work and circumstances, manufacturing was 81.1%, 4 off-days per month 72.2% and 9-10 working hours per day 42.1%. As for accommodation, residence in fabric was 62.6% and one or two members as roommate 40.2%. 3) The characteristics of health behavior showed that 89.4% of migrant workers had 3 meals, 70.9% of them did not drink alcohol, 73.5% of them did not smoke. 4) As a characteristic of health status, 71.8% of them perceived of their health. 76.1% thought that they had no illness before coming Korea. Among them who recognized their illness, those who had problem in circulatory system was 35.3%, respiratory system ENT 19.1% and nervous system 19.1%.66.2% of those having illness had already had sickness when coming to Korea. 5) During last one month, 79.2% of them were known as ones having no illness. Among the sick, those who had problem in circulatory system was 31.6%, nervous system 23.7% and respiratory system 21.1%. 60.3% of the sick were not cured at that time. 6) Sorting the symptom of those who visited free medical check up, dental care was 24.2%, orthopedic 14.0% and digestive system 13.8%. Teethache was 34.4%, stomach problem 11.6%, upper respiratory inflammation 10.2% and back pain 5.9%. Averagely they visited free medical check up 1-2 times. According to symptom, epilepsy 25.5 times, heart and vascular disease 9 times, constipation 2.8%, neurosis 2.38 times and stomach problem 2.34 times. 7) The most frequently visited medical service by migrant workers was hospital. The most mentioned reason was good healing as 36.3%. The medical service satisfied migrant workers mostly was hospital as 64.3%. The reason of satisfaction was also good healing as 45.9%. 8) 77.2% of respondents did not spend money for medical check. Average monthly medical cost was 25,100 Won, 3.7% of income. Those who had no medical security was 73.4%. In their case, 67.7% got discount from hospital or support from working place and religious organization. 9) As for the difference of medical utilization according for the characteristics of migrant workers, legal workers and no-Korean speaker used hospital more frequently. 10) Those who were satisfied most of all with the service of hospital were female workers, hinduists and buddhists, legal workers or manufacture workers. 11) Christians, those who have 3 meals or recognize themselves as healthy ones mostly had no illness. As a result, the most of migrant workers in Korea are from Asia. They are good educated but are working in manufacturing and illegal. Their average income is under 700,000 Won which in not enough for medical cost. They have no medical security and medical fee is supported by religious organization or discounted. Considering these facts the medical policy by government is to be established.

DMFT Index, Periodontal Index and Oral Hygiene Status in Diabetic Patients (당뇨병 환자에서 우식경험영구치지수, 치주질환지수 및 구강위생 상태)

  • Background: This study examined the oral health condition of diabetic patients and evaluated the oral health behavior through a questionnaires survey. In addition, this study provide primary informations for developing a dental health program for diabetic patients. Materials and Methods: There were fifty patients with diabetes mellitus or glucose tolerance (controlled by a community health center at Dalsung-Gun in Daegu City and fifty nondiabetic age, gender and location matched control subjects. Data were collected by a direct oral examination and by questionnaires from 100 subjects in July, 2004. Results: The DMFT (Decay, Missing, Filling Teeth) mean values in diabetics were significantly higher than that in non-diabetics (p<0.05). The decay teeth index and Filling teeth index was similar in the two groups, but the mean Missing teeth index was significantly higher in the diabetics than in non-diabetics. The PI (Periodontal Index) mean values were significantly higher in the diabetics than in the non-diabetics (p<0.01). There was a significant difference between the diabetics and non diabetics aged in their 50s and 60s, but there was no difference with those aged in their 70s. The oral hygiene status was similar in the two groups. The average number of remaining teeth was significantly lower in the diabetics ($16.1{\pm}10.35$) than in the non-diabetics ($20.04{\pm}8.70$) (p<0.05). Conclusion: In the diabetic patients, the M component of the DMFT index was found to be higher than the controls. Diabetics lose their teeth mainly due to periodontal disease, which is supported by the increased PI mean values.

SEDATION EVALUATION USING BIS INDEX ASSESSMENT WITH AND WITHOUT THE ADDED SUBMUCOSAL MIDAZOLAM (점막하 Midazolam의 병용투여 시 BIS 분석을 이용한 진정 평가)

  • The aim of this study was to examine the difference of the depth of sedation using the Bispectral index assessment with and without the added submucosal Midazolam to oral Chloral hydrate and Hydroxyzine for pediatric patients. Twenty seven sedation cases were performed in this study Selection criteria included good health(ASA I), 2 to 6 years of age, the need for sedation to receive dental treatment including anesthesia, and restorative procedure over at least two teeth. Patients were randomly classified into one group taking oral Chloral hydrate(60 mg/kg) and Hydroxyzine(1 mg/kg) and the other group recieving Chloral hydrate(60 mg/kg), Hydroxyzine(1 mg/kg) and submucosal Midazolam(0.1 mg/kg). Nitrous Oxide(50%) was used for both group during sedation. Patients were monitored using a pulse oximeter and a Bispectral monitor. A behavior scale was rated as quiet(Q), crying(C), movement(M), or struggling(S) every 2 minutes watching a recorded videotape. Analysis showed a significant difference in mean Bispectral index and SD during sedation across two groups(P<0.001). The group of patients injected with submucosal Midazolam in addition to oral Chloral hydrate and Hydroxyzine showed a lower mean Bispectral index and a narrower SD. PR and SpO2 for both groups remained within the normal values. Submucosal Midazolam improved the sedation quality by deepening sedation depth without compromising safety and enabled the sedation pattern to be kept more stable.
