Evaluating Performance of Vietnamese Public Hospitals Based on Balanced Scorecard

  • The study evaluates the performance of public hospitals in Vietnam by applying the Balanced Scorecard (BSC). The authors first review the literature to find the research gap of performance in public hospitals. Then, we built Likert questionnaires to collect data from more than 200 managers of public hospitals in the Northwestern provinces of Vietnam. The research uses correlation regression to evaluate the performance based on the influence of factors in the BSC model, including Strategic planning, Internal process, Finance, Mission, Customer, and Employee learning and growth. The results show that the performance of public hospitals in the research sample is influenced by the factors in the BSC model in descending order based on the regression coefficient as follows: Internal process, Finance, Mission, Strategic planning, Customer, Employee learning and growth. Based on the quantitative research findings, we continue by conducting some deep interviews with specialty to propose intensive recommendations about how to implement Mission, Internal process, Financial policies, etc. to managers in public hospitals with an aim to improve the performance of public hospitals in the Northwestern mountainous region of Vietnam. The lessons could be applied for other public hospitals in Vietnam and other jurisdictions that have similar conditions.

중환자간호사의 경관영양 공급에 대한 인식과 지식 및 수행 (Critical Care Nurses' Perception, Knowledge, and Practices of Enteral Nutrition)

  • Purpose: The purpose of this study was to assess critical care nurses' perception, knowledge, and nursing practices regarding enteral nutrition. Methods: A descriptive study was conducted with 187 nurse participants who worked in one of the eight medical and surgical intensive care units (ICUs) from four hospitals in Korea. Data were collected using a self-administered questionnaire. Results: Although critical care nurses' perception toward enteral nutrition was high, knowledge was relatively low. The overall perception and knowledge of the nurses did not differ significantly between medical ICU and surgical ICU nurses. Perception of their own knowledge, in particular, 'nutritional goal' was lower for medical ICU nurses compared to surgical ICU nurses. Nurses also had limited knowledge about the significance of enteral nutrition, confirmation of feeding tube location, and nutritional requirements for ICU patients. They inadequately performed the following: changing the feeding tube every 24 hours, inspecting nostrils daily, and adjusting feeding schedule if feeding was stopped. Conclusion: Our results indicate that ICU nurses need up-to-date information about enteral nutrition. Based on the improved perception and knowledge, nursing practice activities with regard to enteral nutrition should be emphasized to enable nurses to provide optimal nutrition for ICU patients.

간호사의 중동호흡기증후군 감염 실태와 감염관리 인식 (Infection Control of Hospital Nurses: Cases of Middle East Respiratory Syndrome)

  • Purpose: The 2015 Korean Middle East Respiratory Syndrome Coronavirus (MERS-CoV) outbreaks resulted in 186 cases, with 8% (15 persons) of these being nurses. This study aimed to examine MERS-CoV infection status of clinical nurses and to evaluate perception for infection control. Methods: We investigated the MERS-CoV infection status of nurses using MERS-CoV press release data. We examined and analysed perception for Infection control of 121 nurses of the three MERS intensive therapeutic hospitals in July 2015. Results: One to six nurses per hospital in total 8 health care facilities were infected with MERS-CoV. They mainly had short clinical careers and were unaware of infection possibility. The personal and organizational infection control levels that nurses perceive were low and the relationship between two levels was statistically significant. Conclusion: For promoting health protection and infectious disease management competency of nurses, it is necessary to prepare institutional system for controlling infectious disease.

신전운동이 루푸스 환자의 우울, 통증 및 피로에 미치는 영향 (Effect of Stretching Exercise on Depression, Pain, and Fatigue in Patients with Systemic Lupus Erythematosus)

  • Objective: The purpose of this study was to determine the effect of stretching exercise on depression, pain, and fatigue in patients with systemic lupus erythematosus(SLE). Methods: The study was designed as a non-equivalent control group pre-posttest quasi-experimental design. The patients with SLE who signed the consent form were conveniently assigned into two groups(11 experimental subjects and 10 control subjects). The subjects in the experimental group were participated in 6 weeks stretching exercise program for one and a half hours per session twice a week. The data were analyzed by using a SAS-pc+ 8.01 program. Results: 'The experimental group would have less depression scores than the control group' was supported (Z=2.2, p=.025). 'The experimental group would have less pain scores than the control group' was supported (Z=3.4, p=.001). 'The experimental group would have less fatigue scores than the control group' was supported (Z=-2.0, p=.041). With this study, we identified the stretching exercise program was effective on depression, pain, and fatigue of SLE patients. Conclusions: From above results, 6 weeks stretching exercise program could be an effective nursing intervention to reduce depression, pain, and fatigue in patients with SLE.

뇌졸중 전문치료실의 간호강도에 근거한 환자분류도구 개발 (Development of Patient Classification System based on Nursing Intensity in Stroke Unit)

  • Purpose: The purpose of this study was to develop a patient classification system based on nursing care intensity for patients with acute stroke-related symptoms and verify its validity and reliability. Methods: Data were collected between November, 2013 and February, 2014. The verification for content validity of the patient classification system was conducted by a group of seven professionals. Both interrater reliability and concurrent validity were verified at stroke units in tertiary hospitals. Results: The intensive nursing care for acute stroke patients consisted of 14 classified domains and 56 classified contents by adding 'neurological assessment and observation' and 'respiratory care': 'hygiene', 'nutrition', 'elimination', 'mobility and exercise', 'education or counselling', 'emotional support', 'communication', 'treatment and examination', 'medication', 'assessment and observation', 'neurological assessment and observation', 'respiratory care', 'coordination between departments', and 'discharge or transfer care'. Each domain was classified into four levels such as Class I, Class II, Class III, and Class IV. Conclusion: The results show that this patient classification system has satisfactory validity for content and concurrent and verified reliability and can be used to accurately estimate the demand for nursing care for patients in stroke units.

OBSERVATIONS OF TERRESTRIAL NIGHTGLOW (MEINEL BANDS) AT KING SEJONG STATION, ANTARCTICA

  • A Fourier Transform Spectrometer was used to study upper mesospheric thermodynamic by observing the hydroxyl(OH) emission. Rocket-born and satellited-born photometers place the peak emission near 87 km. The instrument was installed in February 1999 at King Sejong station ($62.22^{circ}S,301.25^{circ}E$), Antarctica and has been in routine operation since then. An intensive operational effort has resulted in a substantial data between April and June, 1999. A harmonic analysis was carried out to examine information on the tidal characteristics. The measured amplitudes of the 12-hour oscillation are in the range of 2.4-3.7 K, which are in resonable agreement with theoretical model outputs. The harmonic analysis also revealed 8-hour oscillation which is not expected from the traditional theoretical studies. In addition, the observed 8-hour oscillations are apparent and sometimes dominate the temperature variation in the upper mesosphere.

해안 광학영상 자료를 이용한 쇄파지역 연안류 측정기술 (Remote Sensing of Nearshore Currents using Coastal Optical Imagery)

  • In-situ measurements are labor-intensive, time-consuming, and limited in their ability to observe currents with spatial variations in the surf zone. This paper proposes an optical image-based method of measurement of currents in the surf zone. This method measures nearshore currents by tracking in time wave breaking-induced foam patches from sequential images. Foam patches in images tend to be arrayed with irregular pixel intensity values, which are likely to remain consistent for a short period of time. This irregular intensity feature of a foam patch is characterized and represented as a keypoint using an image-based object recognition method, i.e., Scale Invariant Feature Transform (SIFT). The keypoints identified by the SIFT method are traced from time sequential images to produce instantaneous velocity fields. In order to remove erroneous velocities, the instantaneous velocity fields are filtered by binding them within upper and lower limits, and averaging the velocity data in time and space with a certain interval. The measurements that are obtained by this method are comparable to the results estimated by an existing image-based method of observing currents, named the Optical Current Meter (OCM).

  • As mobile systems are performing various functionality in the IoT (Internet of Things) era, network-on-chip (NoC) plays a pivotal role to support communication between the tens and in the future potentially hundreds of interacting modules in system-on-chips (SoCs). Owing to intensive research efforts more than a decade, NoCs are now widely adopted in various SoC designs. Especially, studies on application-specific NoCs (ASNoCs) that consider the heterogeneous nature of modern SoCs contribute a significant share to use of NoCs in actual SoCs, i.e., ASNoC connects non-uniform processing units, memory, and other intellectual properties (IPs) using flexible router positions and communication paths. Although it is not difficult to find the prior works on ASNoC synthesis and optimization, little research has addressed the issues how to convert different protocols and data widths to make a NoC compatible with various IPs. Thus, in this paper, we address important issues on ASNoC implementation to support and convert multiple interfaces. Based on the in-depth discussions, we finally introduce our FPGA-proven full-custom ASNoC.

중환자실 간호사의 임종 환자 돌봄 경험 (Experiences of Critical Care Nurses Caring for Dying Patients)

  • Purpose : This study aimed to develop an in-depth and comprehensive understanding of the experiences of critical care nurses caring for dying patients. Method : Eleven critical care nurses with experience in caring for dying patients were recruited from four tertiary hospitals. Semi-structured face-to-face interviews were conducted between November 2016 and March 2017. The transcribed data were analyzed using qualitative content analysis to identify major themes and sub-themes that represented the experiences of critical care nurses. Results : The following six themes, and twelve sub-themes, were identified: (1) the gap between expectation and reality, (2) a distorted meaning of death, (3) repeated emotional pain and stress, (4) finding a solution alone, (5) sublimation into mission and calling, and (6) integration into one's own life. Conclusion : This study found that critical care nurses experience various psychological difficulties while caring for dying patients, and they made efforts on their own to overcome them. These findings are expected to inform the development of specialized programs to support critical care nurses to tackle these challenges, create guidelines on caring for dying patients, and help promote death education.

치매노인의 낙상위험요인에 관한 메타분석 (A Meta-analysis of the Risk Factors related to Falls among Elderly Patients with Dementia)

  • Purpose: The purpose of this study was to provide data about the risk factors related to falls among elderly patients with dementia using meta-analysis. Methods: Key words used for search through electronic database (CINAHL, PubMed, Ovid-MEDLINE, RISS, KISS, DBPIA, National Assembly Library) included 'dementia', 'Alzheimer', 'fall'. Twenty studies met the inclusion criteria for the meta-analysis and 'R' version 3.2.2 was used to analyze the correlated effect size. Results: Study results showed that risk factors related to falls were identified as the demographic (age, gender, education), dementia-related (disease duration, cognition), physical (body mass index, walking, balance, activity of daily living, use of walking aids, number of medications including psychotropic drugs, musculoskeletal problems, parkinsonism, comorbidity), psychological (neuropsychiatric symptom, depression), environmental (Physical environment), and fall-related (fall history, high risk group of fall) factors. The effect size of risk factors such as high risk group of fall (r=.35), use of walking aids (r=.33), depression (r=.31), psychotropic drugs (r=.27), Musculoskeletal problems (r=.25) were higher than the other risk factors. Conclusion: Based on the findings of this study, strategies to improve elderly patient's depression, intensive care for high risk group of fall, and adequate training with walking aids are needed for prevention of falls in elderly patients with dementia.
