Peripheral Facial Palsy with Fatigue Symptoms is Improved by Korean Medicine Treatment: A Case Report (피로증상을 동반한 말초성 안면마비 환자에게 익기보혈탕과 약침치료를 포함한 한의복합치료 증례보고 1례)

  • Objective: This study investigated the efficacy of using Korean medicine treatments on a patient with peripheral palsy with fatigue symptoms. Methods: A 58-year-old female patient with peripheral palsy and fatigue symptoms was treated with herbal medicine (Yikgeebohyul-tang), acupuncture, pharmacopuncture, Chuna manual therapy, and cupping therapy for 21 days. The treatment effects on peripheral palsy were evaluated using the Yanagihara and House-Brackmann grading systems. Results: Following treatment, the patient showed decreases in the House-Brackmann grading system and fatigue symptoms, as well as an improved Yanagihara grading system score. Conclusions: These results show that Korean medicine treatment improved the condition of a patient with peripheral palsy with fatigue symptoms.

A Case Report of Quadriparesis due to Subarachnoid Hemorrhage with Intraventricular Hemorrhage due to Cerebral Aneurysm Rupture Improved after Treated with Korean Medicine Treatment Including Tonggyuhwalhyeol-tang-gagambang (통규활혈탕가감방을 포함한 한의복합치료로 뇌동맥류 파열에 의한 뇌실내출혈을 동반한 지주막하출혈 환자의 사지마비에 개선을 보인 증례보고 1례)

  • This study reported the case of a patient with quadriparesis due to cerebral aneurysm rupture, subarachnoid hemorrhage, and intraventricular hemorrhage (IVH) treated with Korean medicine. The patient was treated with acupuncture, herbal medicine (mainly Tonggyuhwalhyeol-tang-gagam), Western medicine, moxibustion, cupping, and rehabilitative therapy for 75 days, and improved after administration. After treatment, the Manual Muscle Test grade improved from 4/4-/3+/3 to 4+/4+/4+/4+, the Korean version of the modified Barthel Index score improved from 9 to 100, the National Institute of Health's Stroke Scale score improved from 2 to 0, the Global Deterioration Scale score improved from 3 to 2, and the Korean version of the Mini-Mental State Examination score improved from 22 to 30. During administration, the patient did not show seizures, shock, or loss of consciousness, and the vital signs were stable in the normal range. We followed up the brain computed tomography findings for 2 times and found that there was no definite evidence of intracranial hemorrhage or IVH or re-rupture or rebleeding after Korean medicine treatment. This study suggests that Korean medicine treatment with blood-invigorating and stasis-removing herbs could be a safe and effective intervention option for improving quadriparesis due to cerebral aneurysm rupture and subarachnoid hemorrhage.

Effectiveness and Safety of Pharmacopuncture Therapy for Chronic Low Back Pain: A Study Protocol for a Pragmatic Randomized Controlled Trial

  • Background: Low back pain (LBP) is a common musculoskeletal disorder worldwide, with a lifetime prevalence of up to 80%. Among nonsurgical treatments for chronic LBP, Korean medicine treatments are highly preferred, and pharmacopuncture therapy combining acupuncture and herbal medicine is widely used. However, no evidence-based study has focused on the use of various types of pharmacopuncture. Methods: The pragmatic randomized controlled clinical trial will include 44 participants; recruitment will start in July 2023. All participants will receive integrated Korean medicine treatment including acupuncture, cupping, and infrared therapy, and the intervention group will also receive pharmacopuncture. After 16 treatment sessions, twice a week for 8 weeks, follow-up assessments will be performed at week 9. As a pragmatic randomized controlled clinical protocol, the type, dose, and acupoints of acupuncture and pharmacopuncture are not determined in advance but are selected and recorded according to the clinical judgment of the Korean medicine doctor. Results: The primary outcome will be measured using a visual analog scale score, and the secondary outcomes include the Oswestry disability index, patient global impression of change, no worse than mild pain, and range of motion. Safety will be assessed by examining participants' self-reported adverse events and vital signs and conducting blood tests before and after the test. Conclusion: This study aims to provide clinical evidence of the effectiveness and safety of pharmacopuncture for chronic LBP.

A Delphi Study of Korean Medicine for Anorexia in Children for Clinical Practice Guidelines (소아 식욕부진의 한의표준임상진료지침 개발을 위한 델파이 연구)

  • Objectives This study aimed to develop consensus-based recommendations for establishing standard clinical practice guidelines for pediatric anorexia through the utilization of a Delphi study. Methods We analyzed existing randomized controlled trials for pediatric anorexia treatment using the Delphi method-a structured process for achieving consensus among a panel of experts. A questionnaire was distributed among a select panel of nine specialists in the field. Results The initial Delphi round led to consensus on 30 distinct recommendations; however, consensus was not reached for 19 other recommendations, prompting a second Delphi round. In the subsequent round, adjustments were made based on feedback from the initial round, and deliberations were held on recommendations that previously lacked consensus. Following these adjustments, consensus was achieved on all recommendations. Additionally, a third Delphi iteration was conducted to address three specific queries that required amendment due to a reevaluation of the evidence levels of certain recommendations. In total, three Delphi rounds were carried out to produce informed recommendations related to the diagnosis, treatment, and general management of anorexia. Conclusions This investigation successfully generated evidence-based recommendations for the diagnosis and treatment of pediatric anorexia. The recommendations encompassed various practices, including herbal medicine, acupuncture, moxibustion, cupping, and Chuna manual therapy, which can be integrated into clinical settings.

Hip and Finger Degenerative Arthritis Case Series Protocol for Applying Korean Medicine Clinical Pathway (퇴행성관절염(고관절, 수지관절염) 한의표준임상경로 적용연구 프로토콜)

  • Objectives : The aim of this study is to set out case series study protocol to complete clinical pathway (CP) of hip and finger degenerative arthritis by applying CP, based on Korean medicine clinical practice guideline developed by clinical experts, to clinical field. Methods : The treatments included Manual acupuncture, acupoint injection, electroacupuncture, laser acupuncture, cupping, moxibustion, chuna, and physiotherapy. They were conducted in the 2nd week of admission and 4th week of out patient department (OPD) days. We carried out 10-point Likert scale questionnaires on the clinical usefulness and the satisfaction of patients and staff after applying CP. Appropriateness and improvement on patients were conducted using a 10-point Likert scale. An open-ended questionnaire was also conducted to ask if there was any requirement to be added. Results : In past research studies, there were no related studies about Korean medicine CP on hip and finger degenerative arthritis. Final version of CP is going to be completed based on the questionnaire. Conclusions : This evidence-based case series study protocol is expected to contribute development of hip and finger degenerative arthritis.

Excessive Sweating and Widespread Pain in Fibromyalgia Syndrome after Long-term Stress Improved by Complex Korean Medical Treatment (장기간의 스트레스 이후 전신 다한증 및 광범위한 통증을 호소하는 섬유근통증후군 환자에 대한 복합 한의치험 1례)

  • Objectives: This case report describes the effect of complex Korean medical treatment on the clinical symptoms of a patient with fibromyalgia syndrome (FMS). Methods: A 60-year-old female woman diagnosed with FMS in 2015 was prescribed Youngsunjaetong-em-hap-Sopunghwalhyeol-tang from 2 May 2023 to 20 June 2023 to reduce excessive sweating and widespread pain. With the same objective, acupuncture, moxibustion, and cupping treatment were administered from 23 May 2023 to 11 June 2023. Changes in excessive sweating and widespread pain were recorded 2 months post-treatment. Results: Treatment with Youngsunjaetong-em-hap-Sopunghwalhyeol-tang led to a considerable improvement in clinical symptoms after 2 months, especially excessive sweating and widespread pain compared to the baseline parameters of the first visit. In addition, no adverse effects, such as liver injury or decreased kidney function, were recorded during the herbal medicine treatment. Conclusion: This case report points to the use of complex Korean medical treatment as a therapeutic option for the management of FMS.

Density, Bonding Strength, Bending strength and Decay Resistance of Radiata Pine Laminated Veneer Lumber (라디에타소나무 단판적층재의 밀도·접착·강도성능 및 내부후성)

  • In this study, LVLs of radiata pine were fabricated with non-preservative treated veneers, CuAz treated veneers, and ACQ treated veneers, using aqueous vinyl urethane adhesive and phenol modified resorcinol resin adhesive. Then density gradient, bonding strength, bending properties and decay resistance of LVLs were evaluated. As results, the cone-shaped and higher density gradient pattern was found in layer close to glueline. After cyclic water boiled test, the LVL bonded with aqueous vinyl urethane resin adhesive was delaminated in all layers or partly delaminated including check, chasm in glueline layer. In the case of LVL bonded with phenol modified resorcinol resin adhesive, despite slight cupping due to great glueline stress and vertical check between glueline layers, it was observed that the bonding strength to delamination was higher, owing to most absence of delamination through overall glueline. On the other hand, in the decay test, mass loss by brown rot fungi was greater than white rot fungi in LVL bonded with aqueous vinyl urethane resin adhesive. However, in LVL bonded with phenol modified resorcinol resin adhesive, the mass loss by brown rot fungi was slight and non-preservative treated LVL was low. The mass loss of preservative-treated LVL was 0 (zero), showing the high decay resistance effect.

The Comparative Study of Effects between Acupuncture Treatment and Cotreatment with Mixed Pharmacopuncture of $Jungsongouhyul$ and $Cervi$ $Pantotrichum$ $Cornu$ on Low Back Pain Patients (단순 요통환자에 대한 침치료와 중성어혈-녹용 혼합약침 병행치료의 효과비교연구)

  • Objectives : The purpose of this study is to investigate the effect of mixed pharmacopuncture of $Jungsongouhyul$ and $Cervi$ $Pantotrichum$ $Cornu$ on low back pain patients. Methods : 83 patients with Low back pain who were treated in Dept. of Acupuncture and Moxibusion, Dongshin University Hospital in Suncheon from Sep. 1st, 2010 to August 31th, 2011, were divided into two groups. One group (52 patients) was treated with conservative treatments (acupuncture, herb med, cupping therapy and physical therapy) and mixed pharmacopuncture of $Jungsongouhyul$ and $Cervi$ $Pantotrichum$ $Cornu$ theraphy (PG) and the other group (31 patients) was treated with conservative treatments alone(NPG). To estimate the effects of treatments, visual analog scale(VAS) and Oswestry disability index(ODI) were checked. Results : 1. In VAS, both of PG and NPG shows statistically significant improvement rate after 3rd treatment and 5th treatment. 2. In ODI, both of PG and NPG shows statistically significant improvement rate after 3rd treatment and 5th treatment. 3. In comparing VAS of PG and NPG, there was more significant improvement in PG than NPG after 3rd treatment and 5th treatment. 4. In comparing ODI of PG and NPG, there was more significant improvement in PG than NPG after 3rd treatment but no significant improvement between PG and NPG after 5th treatment. Conclusions : According to the study, it is thought that mixed pharmacopuncture of $Jungsongouhyul$ and $Cervi$ $Pantotrichum$ $Cornu$ theraphy is effective to low back pain patients, and further study is needed for the confirmation of the effect of mixed pharmacopuncture of $Jungsongouhyul$ and $Cervi$ $Pantotrichum$ $Cornu$.

Characteristics of Patients' Self-Perceived Health in Traditional Korean Medical Facilities - Based on the Ministry of Health and Welfares Report on Usage and Consumption of Korean Medicine in 2011 - (주관적 건강인식수준에 따른 한방의료기관 이용환자의 특성 비교 - 2011년 한방의료이용 및 한약소비실태조사(보건복지부)를 중심으로 -)

  • Objective : The purpose of this study was to identify characteristics of patients' self-perceived health in traditional Korean medical facilities. Method : This research was conducted based on the survey on patients whom have visited traditional Korean medical facilities in 2011 by the Ministry of Health and Welfares and Korean Institute for Health and Social Affairs. Using a sample of 3,931 (1,180 male and 2,751 female) outpatients' self-perceived health based on the data from usage and consumption of Korean Medicine. 'Healthy', 'Fair', and 'Poor Health' were used to measure patients' self-perceived health status. The data was analyzed by frequency, t-test, cross correlation analysis and multiple logistics regression analysis using the SPSS program package. Results : Sex(P<0.001), age(P<0.001), marital status(P<0.001), education(P<0.001), employment status(P<0.001), occupation(P<0.001), health insurance(P<0.001) and income level(P<0.0001) showed statistical significance. Main Treatment Facilities(P<0.001), experience of taking Korean medicine(P=0.032), experience of receiving acupuncture treatment(P<0.001), number of visits(P<0.001), medical expense (P=0.005), and subjective health status after the treatments showed statistical significance for Korean herbal medicine(P=0.038), acupuncture (P=0.001), cupping therapy(P=0.006), oriental physiotherapy(P=0.003), and treatment satisfaction(P<0.001). For subjective health status based on suffering disorders in the past three months, the response of poor health was higher in the group suffering recent illnesses. Statistical significance was seen in hypertension (P=0.002), arthritis(P<0.001), lumbar pain(P<0.001), diabetes mellitus(P=0.001), stroke(P<0.001), hwa-byung (P=0.001), gastric disorders(P=0.021), common cold(P<0.001), ankle sprain(P<0.001), muscular injury(P<0.001), lumbar sprain(p=0.009) and fracture(P=0.03). Also the number of diseases during the past three months showed statistical significance(P<0.001). Statistical significance was also seen in Level of knowledge(P<0.001), route of information(P<0.001), reliability of Korean medicine(P=0.003), insurance coverage(P=0.005), medical costs(P<0.001), and future willingness to use Korean medicine(P<0.001). As a result of the multiple logistics regression analysis, risks of subjective poor health statistically increased in female population, elderlies, medicaid beneficiaries, less educated, higher medical expense, and more disorders during the past three months. Conclusion : Patients' self-perceived health status has significant differences with each variables such as sex, age, marital status, education, health insurance, medical expense, number of diseases.

A Survey on Satisfaction for Korean Medicine Treatments in 237 Traffic Injury Patients (교통사고 환자 237예에 대한 한의치료 만족도 조사)

  • Objectives The purpose of this study is to carry out a survey on satisfaction for Korean medicine treatments in traffic injury patients. Methods Satisfaction survey was carried out on the 237 traffic injury patients who had been treated by Korean medicine treatments in Department of Korean Rehabilitation Medicine, Wonkwang University Gwangju Medical Center from February, 2017 to August, 2017. The questionnaire form of preceding study was used to assess the satisfaction degree of patients. All data were coded by SPSS v20.0 (IBM Corp., Armonk, NY, USA) and analysed by descriptive maneuver, x2-test, T-test and correlation. Results Among the 237 cases, the first hospital to be selected for traffic injury treatment was Korean medicine hospital in 91 cases. And 97 patients were transferred from the western medicine hospital. The main reason for choosing Korean medicine hospital was that the patients prefer Korean medicine treatments than western medicine treatments. 85.2.% of patients answered that their symptoms were improved, and 75.1% of patients answered that they were satisfied with Korean medicine treatments. The satisfaction degree of cupping treatment and acupuncture was relatively high, but the degree of Herbal medicine and physical treatment was relatively low. There were no statistically significant differences in satisfaction degree with sex and age. The satisfaction degree, improvement of symptoms, intention to revisit and intention to recommend was significantly correlated each other. Conclusions The degree of improvement of symptoms and The satisfaction degree were revealed relatively high. Additional and repetitive studies are needed to understand the patient's position and improve the quality of Korean medicine treatments for traffic accident patients.
