III급 부정교합을 지닌 $6\sim14$세 아동의 중두개저각에 따른 안면두개골의 형태적 특징: 측두방사선규격사진을 이용한 횡단누년적 연구 (A CROSS-SECTIONAL LONGITUDINAL CEPHALOMETRIC STUDY ON CRANIOFACIAL SKELETAL CHARACTERISTICS IN KOREAN CHILDREN AGED 6 TO 14 WITH CLASS III MALOCCLUSIONS)

  • 부정교합의 분류가 치아의 위치적 관계에 의해 구분되면, 동일한 집단에 속하더라도 이들의 안면두개골은 서로 다른 구조적 관계를 지니고 있어 이들을 동일시 할 수 없으며 안면두개골의 형태적 특징에 의한 분류가 필요하다. 저자는 $6\sim14$세 사이의 III급 부정교합 아동 135명의 측두방사선규격사진을 7, 9, 11, 13세의 연령으로 구분한 후, 중두개저각 $40.3^{\circ}$를 기준으로 이보다 작은 집단(하악전돌효과군)과 큰 집단(하악후퇴효과군)으로 분류하여 각 집단의 안면두개골의 형태적 특징을 횡단 누년적으로 비교 평가하였다. 모집단의 39.3%가 하악전돌효과군으로, 60.7%가 하악후퇴효과군으로 나타났으며, 두 개저, 비상악복합체 상악골 및 하악골의 전후방적 길이 그리고 전안면높이와 후안면높이 등의 수직적 길이가 하악후퇴효과군에서 크게 나타난 것으로 미루어 III급 부정교합 아동의 많은 수가 좁고 긴 얼굴형 (leptoprosopic faceform)에서 찾아볼 수 있는 형태적 특징을 많이 지니고 있으며, 두 군간에는 형태적, 구조적인 차이와 함께 시기적으로도 각기 다른 성장양태를 보이는 것으로 가늠된다.

하악(下顎) 전돌증(前突症)에 관(關)한 방사선(放射線) 두부계측학적(頭部計測學的) 연구(硏究) (A ROENTGENOCEPHALOMETRIC STUDY ON MANDIBULAR PROGNATHISM)

  • This investigation was designed to compare the craniofacial and dental morphology of class III malocclusion with that of normal occlusin in children, and to determine the incidence of various class III craniofacial skeletal patterns. The material selected for this study consisted in standard lateral cephalograms of eighty two Korean children, forty one boys and forty one girls, aged 10 through 12 years, having class III malocclusion, and forty two Korean children, twenty boys and twenty two girls, with normal occlusion in the same age. Using the tracings of the standard lateral cephalograms, various angular and linear measurements were recorded, tabulated and statistically analyzed, and then the class III craniofacial skeletal morphology was divided into various patterns by the degree of SNA and SNB, which respectively were below, within or beyond the normal range of those of normal occlusion. The following characteristics of the craniofacial and dental morphology of class III malocclusion were observed. 1. The cranial base length of class III malocclusion was smaller than that of normal occlusion, and the small saddle angle was a characteristic figure of class III malocclucion. 2. Maxillary length of class III malocclusion was smaller than that of normal occlusion, and point A was retropositioned relative to cranial base but not PNS in class III malocclusion. Maxillary base inclination was not significantly different between the two, but occlusal plane to palatal plane was small in class III malocciusion. 3. The mandibular body length shown no difference between the two, but the mandibular body positioned anteriorly relative to cranial base in class III malocclusion. Ramus height, gonial angle, and mandibular effective length were large in class III malocclusion. Mandibular plane angle and joint angle had no difference between the two, and occlusal plane to mandibular plane angle was large in class III malocclusion. 4. Maxillary incisor inclination was not significantly different between class III malocclusion and normal occlusion, but mandibular incisors positioned and inclined lingually and consequently interincisal angle was large in class III malocclusion. 5. Class III malocclusion was divided into six categories of craniofacial skeletal pattern. The most common class III pattern was found to be one in which the maxilla was within the normal range of prognathism while the mandible extended beyond this range. The pattern in which the maxilla was below the normal range of prognathism while the mandible was within this range was approximately one fifth of the class III sample.

하악이부와 두개안면골격의 상관성에 관한 측모두부방사선 계측학적 연구 (A CEPHALOMETRIC STUDY ON CORRELATION BETWEEN MANDIBULAR SYMPHYSIS AND CRANIOFACIAL SKELETON)

  • 안모의 수직적 부조화에 부합되는 하악이부 형태에 관하여 보고되어 왔다. 이연구는 하악이부의 형태적 변이와 두개안면골격의 수직적 변이사이에 상관성을 관찰하기 위하여 시행되었다. 성인 남녀 부정 교합자 212명의 측모두부 방사선사진에서 이부의 후경과 고경을 계측하고 이부비를 산출한 후, 이부비 평균 -1 표준편차에 포함되는 즉모군(low symphysis군)과 이부비 평균 +1 표준편차에 포함되는 측모군(high symphysis군)으로 분류하고, 이들의 측모두부방사선사진을 계측하고 비교 분석하여 다음의 결론을 얻었다. 1. 이부비가 크면 hyperdivergent face 이고, 이부비가 작으면 hypodivergent face 가 되는 경향을 보였다. 2. 이부비가 크면 하악각도 컷으며, 이들사이의 상관성은 높았다. 3. 이부비와 수직안면 고경사이에 중등도의 상관성이 있었으며, 특히 이부 후경과 전안면고경사이 및 이부 고경과 후 안면 고경사이에도 중등도의 상관성이 있었다. 4. 이부의 형태적 변이와 설골의 위치사이에는 상관성이 없었다.

Clinical characteristics of pediatric nasal bone fractures by age group

  • Background: The clinical characteristics of pediatric nasal fractures can vary depending on the child's age, social activities, and environment. Therefore, this study aimed to analyze these characteristics in different age groups. Methods: We retrospectively reviewed of a series of patients aged under 12 years who received treatment between 2013 and 2021. The initial study design involved dividing the patients into four age groups, corresponding to different developmental ages, but there were no cases in infants aged 0 to 1 year. Therefore, the patients were divided into three groups: group I, between 2 and 5; group II, between 6 and 9; and group III, between 10 and 12 years of age. The following parameters were evaluated: sex, age, etiology, fracture type and severity, and the incidence of septal injuries. Results: In total, 98 patients were included in this study. In group III, the ratio of boys to girls was 3.88:1, exceeding the overall ratio of 1.97:1. The most common cause varied with age: slipping down in group I, bumping accidents in group II, and sports accidents in group III. Concomitant septal injuries were present in 4.17% of patients in group I, 5.71% of patients in group II, and 28.21% of patients in group III. Conclusion: Increasing age was accompanied by a greater tendency for male predominance and a higher prevalence of sports-related causes and septal injuries. Violence was infrequent but started to become a contributing factor during school age. These varying environmental factors across age groups can offer valuable insights into the epidemiology and clinical characteristics of pediatric nasal bone fractures.

상악전돌에 관한 방사선 두부계측학적 연구 (A ROENTGENOCEPHALOMETRIC STUDY ON MAXILLARY PROTRUSION)

  • This study was undertaken to compare the craniofacial morphology of Class II, Division 1 malocclusion with that of normal occlusion in children, and to investigate the incidence of various Class II, Division 1 craniofacial skeletal patterns. The subjects consist of thirty seven boys and fifty three girls with Class II, Division 1 malocclusion, and forty six boys and eighty one girls 10-15 years with normal occlusion. Measurements were recorded, tabulated and analyzed on the lateral cephalograms by the degree of SNA, SNB and ANB. The following characteristics of the Class II, Division 1 skeletal pattern were observed. 1. The anteroposterior relationship of the maxilla to the cranium in the Class II, Division 1 was very similar to that of normal occlusion. 2, Mandible of the Class II, Division 1 malocclusion was in the posterior position in relation to the cranial anatomy when compared to normal. 3. The chin point as measured by SN Pog and NS Gn showed distal positioning in relation to normal occlusion. 4. SN to mandibular plane angle was large in Class II, Division 1 malocclusion. 5. Mandibular incisor inclination was not significantly different between Class II, Division 1 malocclusion and normal occlusion, but maxillary incisors inclined and positioned labially and consequently overjet was large in Class II, Division 1 malocclusion. 6. Class II, Division 1 malocclusion was divided into four types of craniofacial skeletal pattern. The most common Class II, Division 1 pattern was found to be type C in which SN-Mand. Pl. was above mean range of normal occlusion. The next frequent pattern was found to be type A in which maxilla and mandible were within normal range of protrusion while upper incisors were severly labially inclined.

Hydroxyapatite Cement(MimixTM)와 격자틀을 이용한 전두부 재건술 (Forehead Reconstruction with Hydroxyapatite Cement(MimixTM) and the Check Framework)

  • Purpose: The purpose of this study is to develop hydroxyapatite cement simplified procedures for reconstruction of craniofacial deformities. Due to its expense and characteristics of quick hardening time, it may be inappropriate for forehead reconstruction or augmentation. Therefore we hear by introduce a more precise, easy and cheap method. The authors report forehead reconstruction with hydroxyapatite cement for a patient who suffered from craniofacial deformity. Methods: Case report and literature review. Results: A 35 year old man came to us with forehead and temporal area depression. He had a history of brain operations due to traumatic epidural hematoma. A physical exam showed an evidence of right side forehead weakness sign. Authors made RP model of his skull and applied check framework with Kirschner's wires for measuring accurate volume and contour on the depressed right side forehead area on the RP model. After complete exposure of defect area by bicoronary insicion, absorbable plate which applied on skull area was removed. Using three Kirschner's wires, authors made check framework on the right forehead lively and fixed with 2-hole miniplates on the boundary of the defect. After checking asymmetry, hydroxyapatite was applied on check shape framework just above Kirschner's wire. After removing Kirschner's wire, we corrected minimal unbalance and contour with bur. Conclusion: Check framework with Kirschner's wire was very convenient and cost saving methods for forehead reconstruction with hydroxyapatite cement.

수직적 분류에 의한 골격성 III급 부정교합자의 측모 특성에 관한 연구 (A ROENTGENOCEPHALOMETRIC STUDY OF CRANIOFACIAL CHARACTERISTICS OF THE SKELETAL CLASS III MALOCCLUSIONS CLASSIFIED BY MANDIBULAR PLANE (SN-MP) ANGLE)

  • This study was aimed to investigate the characteristics & the causative areas of the adult skeletal class III malocclusions with different facial divergency. The lateral cephalograms of 80 subjects with skeletal class III malocclusion from 17 to 29 years of age were classified into 3 groups according to SN-MP angle; hypodivergent group $(21.65{\pm}3.52^{\circ})$, neutrodivergent group $(30.50{\pm}2.29^{\circ})$ and hyperdivergent group $(40.02{\pm}3.98^{\circ})$. The data were gathered by digitizing of the traced cephalograms and were statistically analyzed. The results were as follows: 1. The anterior cranial base of the hyperdivergent group was shortest & tipped upwardly to the FH plane. 2. The maxilla of hyperdivergent group was shortest anteroposteriorly and positioned posteriorly to the anterior cranial base. 3. The degree of the mandibular prognathism in hyperdivergent group was less than the hypodivergent group. The hyperdivergent group showed the downward & backward rotated mandible. 4. The mandibular ramus & body was short & slender in the hyperdivergent group and the gonial angle was greatest in the hyperdivergent group. 5. The temporomandibular joint was positioned more superiorly to the anterior cranial base in the hyperdivergent group. 6. The cranial base, palatal plane, occlusal plane and mandibular plane were diverged in the hyperdivergent group. And this group had a great anterior total facial height, especially anterior lower facial height. 7. The craniofacial characteristics of skeletal class III malocclusion were critical in the vertical structure than the horizontal.

Influence of heritability on craniofacial soft tissue characteristics of monozygotic twins, dizygotic twins, and their siblings using Falconer's method and principal components analysis

  • Objective: The purpose of this study was to investigate the influence of heritability on the craniofacial soft tissue cephalometric characteristics of monozygotic (MZ) twins, dizygotic (DZ) twins, and their siblings (SIB). Methods: The samples comprised Korean adult twins and their siblings (mean age, 39.8 years; MZ group, n = 36 pairs; DZ group, n = 13 pairs of the same gender; and SIB group, n = 26 pairs of the same gender). Thirty cephalometric variables were measured to characterize facial profile, facial height, soft-tissue thickness, and projection of nose and lip. Falconer's method was used to calculate heritability (low heritability, $h^2$ < 0.2; high heritability, $h^2$ > 0.9). After principal components analysis (PCA) was performed to extract the models, we calculated the intraclass correlation coefficient (ICC) value and heritability of each component. Results: The MZ group exhibited higher ICC values for all cephalometric variables than DZ and SIB groups. Among cephalometric variables, the highest ${h^2}_{(MZ-DZ)}$ and ${h^2}_{(MZ-SIB)}$ values were observed for the nasolabial angle (NLA, 1.544 and 2.036), chin angle (1.342 and 1.112), soft tissue chin thickness (2.872 and 1.226), and upper lip thickness ratio (1.592 and 1.026). PCA derived eight components with 84.5% of a cumulative explanation. The components that exhibited higher values of ${h^2}_{(MZ-DZ)}$ and ${h^2}_{(MZ-SIB)}$ were PCA2, which includes facial convexity, NLA, and nose projection (1.026 and 0.972), and PCA7, which includes chin angle and soft tissue chin thickness (2.107 and 1.169). Conclusions: The nose and soft tissue chin were more influenced by genetic factors than other soft tissues.

Primary amelanotic melanoma of the mandibular gingiva

  • Oral mucosal melanoma is a very rare type of malignant melanoma, the characteristics of which differ from those of cutaneous melanoma. Primary amelanotic melanoma of the mandibular gingiva, which can invade the mandibular bone, is very rare worldwide. Here, we report a case in which we performed a reconstruction of the mandible and gingiva using the fibula osteocutaneous free flap procedure to treat a patient diagnosed with a primary amelanotic melanoma of the mandibular gingiva. The procedure was successful, and no recurrence was observed 10 months after surgery. Oral mucosal melanoma has a much poorer prognosis and a lower 5-year survival rate than cutaneous melanoma. However, recently, immunomodulatory therapies for mutations in melanocytic lesions have been used effectively to treat the increasing number of patients developing this type of melanoma, thus improving the prognosis of patients with oral mucosal melanoma.

Angle III급 부정교합자의 악안면골격 특성에 관한 연구 (STUDY ON THE CHARACTERISTICS OF CRANIOFACIAL SKELETON OF ANGLE'S CLASS III MALOCCLUSION CASES)

  • This investigation was designed to categorize Angle's class III malocclusion groups through analyzing horizontal and vertical components of craniofacial skeleton in Angle's class III malocclusion. The material selected for this study consisted in standard lateral cephalogram of one hundred and fifty five children, seventy four boys and eighty one girls, aged 6 through 15 years, having Angle's class III malocclusion. On the basis of findings of this study, the following results were obtained. 1. In horizontal skeletal classifications,16 groups were classified FMN-A-B, SE-FMN-A according to the Ba-SE-Me, Ba-SE/R. 2. The sequences that have relatively high frequency are as follow; a) Horizontal Group l b) Horizontal Group 3 c) Horizontal Group 5 d) Horizontal Group 9 e) Horizontal Group 4 3. In vertical skeletal classification, 8 groups were classified according to the PMV/PP, PMV/OP, PMV/MP. 4. The sequences that relatively high are as follows; a) Vertical Group 1 b) Vertical Group 3 c) Vertical Group 4 d) Vertical Group 8.
