Purpose: This thesis focuses on researching the burden of the Home Health Care that the Family Care-givers have. Method: This study had been conducted for the duration of 3 consecutive months from January 2006 to March 2006 and had investigated 120 person registered as the Home Health Care Clients to a University Hospital located in Incheon City. The thesis adopted the 'feeling of burden' measuring method jointly developed by Seo, Mi-Hye and Oh, Ga-Sil(1993), and FACES-III(Family Adaptability Cohesion Evaluation) developed by Olson(1985) etc. translated by Kim, Yun-Hee(1989) as the measuring method for Family Function. Result: The overall Health Status of the Home Health Care Clients was 2.18 point of average out of 3 point, where as 1.83 for Activities of Daily living and 1.98 for Vital sign, which are below the average. In the mean time, the Competence for Modified Barthel Index marked 30.88 point out of the full mark. The analyzed result of the burden that the family members have was 3.43 point of average out of 5 point which shows that the interviewee feel that they are considerable burden to their families. When it comes to 'the Burden to the family members' and 'the Function of the family' according to the characteristics of the patient, the result shows statistically significant differences, which are varied according to gender, the relationship between the care-givers and the patients. When it comes to the Family Cohesion, the difference was examined as 'statistically significant' according to the Academic background of the patients and the relationship between the patients and the interviewee.(p<0.05). The result also suggests that there exists 'Negative correlation' among the level of patients' health status, the Family Cohesion and the Family Burden. Conclusion: From the result of this study stated above, this thesis is strongly insisting that there is an urgent need for us to develop a health care mediation program, which could eventually reduce the burden of home health care that the patients' family have. At the same time, a follow-up research to prove the effect of the program is imminent.
Purpose: This study was aimed to investigate direct care stress and job satisfaction in home care nurses. Method: Subjects were 139 home care nurses of hospitals in urban cities. Data was collected by a self report questionnaire. Direct care stress was measured by Community Health Nurses Perceptions of Work-Related Stressors Questionnaire. Job satisfaction was measured by a visual analogue scale. Results: The average score of direct care stress was 60.4. There were significantly high direct care stress in those who had less than that 3 years experience than in their counterparts. The average score of job satisfaction was 79.5. There were significant high job satisfaction in those who willingly chose to work. Direct care stress negatively correlated with the length of office experience of the home care nurse. Conclusion: A direct care educational program should be developed for those who have less than 3 years experience to decrease their stress level.
Purpose : The purpose of this study was to analyze the nursing care needs which can properly cope with patients' needs in emergency room, and grasp the patients' satisfaction level which can assess the care quality. It was aimed to check the level of today's care and provide basic data for care performance and care standard. Method : Data were collected from 192 patients in general hospitals in Daegu. The results are as followed. Results : First, the result of analysis conducted on the degree of overall patients' need for nursing care revealed that the need for nursing information (4.21+57) was the highest, followed by the order of information of patient's condition (4.05+53), emotional support (3.98+47), and nursing participation (3.65+54). Secondly, our study revealed that there was a significant negative correlat interrelation between care need and care satisfaction (r=.516, p<.01), indicating the higher the overall emergency room patients' nursing care need, treatment and nursing information, patient's condition, emotional support and nursing participation were, the lower the actual feeling of their satisfaction turned out. Conclusion : The study shows that treatment and nursing related information is the most highly needed and the patients' satisfaction level relies on actual care participation.
Purpose : The person-centered care is essential for better patient outcome. This study was conducted to identify the factors affecting to the person-centered care among intensive care unit (ICU) nurses. Method : This study was cross sectional survey, and the participants included 107 ICU nurses who provided a written consent to participate in the study. Using a structured survey, nurse's compassion fatigue, compassion satisfaction, and person-centered care among ICU nurses were assessed. The collected data were analyzed using t-test, ANOVA, and Pearson correlation test with SPSS 23.0 program. The factors affecting patient-centered care were analyzed using multiple regression. Results : According to the multiple regression analysis, education level, perceived caregivers need priority and compassion satisfaction were positive affecting factors to the person-centeredness of ICU nurses. Conclusion : In conclusion, the compassion satisfaction was an affecting factor to the person-centered care, however compassion fatigue didn't affect to the person-centered care among ICU nurses. Therefore, the strategy enhancing compassion satisfaction among ICU nurses will be needed to increase person centeredness.
Purpose: This study focused on analysing costs per home health care nursing visit based on home health care nursing activities in medical institutes. Method: The data was collected in three stages. First, the cost elements of home health care nursing services were collected and 31 home care nurses participated. Second, the workload and caseload of home care nursing activities were measured by the Easley-Storfjell Instrument(1997). Third, the opinions on improving the home health care nursing reimbursement system were collected by a nation-wide mailing survey from a total of 125 home care agencies. Result: The cost of home health care nursing per visit was calculated as 50,626\. This was composed of a basic visiting fee of $35,090{\\}({\fallingdotseq}355$)$ and travel fee of $15,536{\\}({\fallingdotseq}15$)$. The major problems of the home care nursing payment system were the low level of the cost per visit, no distinction between first visit and revisits, and the limitations in health insurance coverage for home health care nursing services. Conclusion: This study's results will contribute as a baseline for establishing policies for improvement of the home health care nursing cost and for applying a community-based visiting nursing service cost.
Purpose: This study was done to identify the experience of health care services by foreign residents in Korea. Method: Participants were 12 foreigners from China, North America, and Japan. Qualitative data were collected using three focus group interviews and analyzed with qualitative content analysis. Results: The participants' experience of health services was categorized as 21 sub-categories, 11 categories and three main categories. The main categories were 'Quality of health care', 'Health care personnel', and 'Health care system and infrastructure'. The main category of 'Quality of health care' included three categories, 'Health care personnel' included another four categories, and 'Health care system and infrastructure' included the remaining four categories. The participants reported positive experiences such as favorable feelings and satisfaction particularly, with the high quality of health care but also negative experiences such as apprehension, distrust, difficulties, and inconvenience in the health care service in Korea. Conclusions: The results of this study can be helpful in the development of strategies to improve health care services for foreigners by providing fundamental information about the foreign residents' experience of health care services in Korea from their perspectives.
Purpose: This study was conducted to identify the status of health management and to evaluate the need for health professionals in day care centers in the South Korea. Method: Data were obtained from 360 child care providers in 16 provinces from May to August, 2007. Data was analyzed using SPSS WIN 14.0. Results: Safety education (traffic. 83.8% general safety. 71.6%) was provided more often than the other topic of education. All health related education was provided by child care providers. When minor health problems occurred, the most common management strategy was to send the child home (36.1%). During emergency conditions, majority of child care providers preferred taking the children to medical centers without giving any emergency care and 19.4% reported sending the child home without any immediate care. Child care providers recognized the need for health care professionals. Conclusion: The results of this study suggest the need for services of health professionals in day care centers to satisfy the needs and expectations for health management in these centers. As health professionals, public health nurses or pediatric nurse practitioners may play an important role in health management of children in day care centers.
Objectives; The purpose of this study was to develop a framework for home care and a Home Care Need Assessment Tool. Method 1. Identifying common domains in the provision of home care. 2. Charts of 253 home care clients were reviewed to obtain a classification of the nursing diagnoses. 3. A focus group methodology was used to develop the domains. 4. The tool was applied to 439 home care clients.(Kappa value=0.460-1.000, sensitivity, 0.444-1.000: specificity, 0.743-1.000). 5. Some refinements and extractions of the defining characteristics and related factors were made based on the results of the focus group. Results Home Care Need Assessment Tool consists of three parts; -Part I : factors related to basic conditions -Part II : a screening component that enables home care nurses to assess 30 multiple domains of 53 nursing diagnoses. -Part III : summative nursing diagnoses and nursing need intensity for the clients. Conclusion This tool provides a comprehensive assessment that helps the recognition of many strengths as well as problems of the clients. It will be usefully utilized in scheduling home care nursing plans and evaluating client outcomes.
Purpose: The purpose of this study was to analyze the differences in terms of services and cost between CVA without typical diseases (Group I), and CVA with typical diseases (Group II), in their Hospital-based home health care. Method: The subjects of this study were 308 CVA patients who used home care nursing during the second phase demonstration project of their hospital-based home health care. Results: The results of the study was as follows 1. Group II had more home visit (15.3/12.7) (p>0.05), and cases of death when home care (16.8/11.4) (p<0.05). 2. Group II needed more services than Group I such as bladder irrigation, skin care, bed sore care, glycerin enema, finger enema, lung care, urine sugar test, monitoring and surveillance of fluid infusion and R.O.M exercise (p<0.05). 3. The variables that showed statistical significance in the regression analysis were family style, OPD visit, level of consciousness, patient's state on termination of home care, and some extend of home health care services (R2=0.373, 0.205). Conclusion: Home nursing care needs to be planned by severity in Hospital-based home health care for CVA patients.
Purpose: This descriptive study was conducted to illuminate, from the nursing point of view, the infant care act, which was revised recently and to suggest a desirable direction of its enforcement decree and regulations. Method: This study was carried out through literature review and a collection of child care experts' opinions. Result: The most remarkable changes in infant care act and the suggestions for a desirable direction of its enforcement decree and regulations are as follows: It can be said that the law on 'health', 'nutrition' and 'safety' for a child care center was improved to the level of act from the level of enforcement regulation. In the enforcement regulation on the distribution of nurses in child care centers, it is desirable that nurse's aids are excluded. It is recommended that every child care center should have a nurse or a health care manager and/or an emergency caretaker. In the curriculum for child care, the subjects on child health care should be an essential subject, not elective. Conclusion: Child care act should be reformed under the discipline of first priority to child health. Every nurse and nursing professor should have interest and earnestness in child care and conduct related studies.
