좌심방내 점액종 치험예 (One Case Report of Surgical Treatment of Left Atrial Myxoma)

  • It is well known that the atrial myxoma is the most common intracardiac tumor which is hardly able to be diagnosed preoperatively and is completely curable if surgical treatment is appropriate. A case of the atrial myxoma, which was misdiagnosed as mitral valvular disease preoperatively and was successfully removed with the aid of the cardiopulmonary bypass, was reported. The common symptoms,signs,diagnostic measures and treatments of the intracardiac tumor were discussed with the review of literatures.

  • Congenital coronary arteriovenous fistula is a rare cardiac defect that causes coronary arterial flow to drain into the right cardiac chambers, the pulmonary artery, the coronary sinus, or the left cardiac chambers. The most frequently involved vessel is the right coronary artery. We experienced a case that had a coronary arteriovenous fistula associated with valvular heart disease. With the cardiopulmonary bypass done under hypothermia, mitral valve replacement was accomplished and the fistulas of both proximal and distal portions of the right coronary artery were closed with 3-0 prolene. Postoperative course was uneventful.

  • Although blunt traumatic cardiac rupture is an uncommon injury, it can be associated with a high mortality rate. Two cases of cardiac rupture in blunt trauma patients are described herein. In those cases, applications of mechanical support devices such as ECMO (extracorporeal membrane oxygenation) and early surgery for exploration under cardiopulmonary bypass may be helpful for treating blunt chest trauma patients.

  • Jehovah`s Witnesses who require cardiac operations represent a challenge to surgeon because of the patients` refusal to accept blood transfusion, We report a 6 year period, ending May, 1991 with a consecutive series of 25 Jehovah`s witnesses. Patients ranged in age from 11 months to 61 years. All operations were performed during cardiopulmonary bypass with bloodless priming, No patient received any blood or blood products during hospitalization. Perioperative mortality was 2 paients[8%]. We conclude that if we performed the cardiac surgery with careful operative technique and various blood conserving procedures, we can do the bloodless open heart surgery with relatively safety.

Plastic plates 를 이용한 흉골 봉합술 - 치험 1례 - (A New Technique of Sternal Closure with Plastic Plates)

  • A very early and effective technique of sternal closure by plastic plates was successfully applied to a 7-months-old infant of tetralogy of Fallot in whom the standard closure of sternum was not possible due to massive generalized edema after cardiopulmonary bypass for total correction. This technique provides not only solid but also permanent closure of the sternum so that any early reoperation for closure of sternum and its accompanying risk of infection in many other known methods can be avoided.

개심술 114례의 임상적 고찰 (Clinical Evaluation of Open Heart Surgery - Review of 114 Cases -)

  • Between March, 1988 and July, 1992, 114 patients underwent Open Heart Surgery under hypothermic cardiopulmonary bypass. There were 29 cases of congenital heart anomalies (25%), and 85 cases of acquired heart diseases (75%) consisting of 53 cases of valvular heart disease, 31 cases of ischemic heart disease, and a case of left atrial myxoma. The age distribution of 114 cases was 4 to 73 years, and mean age was 43 years old consisting of mean age of congenital heart disease 23, valvular heart disease 47, and ischemic heart disease 57 years old. Overall operative mortality was about 7.9%.

심장내 발생한 원발성 지방종 1례 보고 (Primary Lipoma of the Heart -A Case Report-)

  • Primary cardiac lipoma is an extremely rare condition. We reports a case of primary lipoma located mainly in the right atrium of the heart in a 58-year-old male patient. The initial presenting symptoms were dyspnea followed by rapidly progress!ve congestive heart failure. Echocardiogram revealed huge mass on right atrium with stalk arising from septum. Under cardiopulmonary bypass the mass was removed and revealed characteristic findings of lipoma on microscope. The patient was recovered without any problem. We would like to describe this case of rare tumor with the review of literatures.

  • This study was performed to determine respiratory manifestations of 105 male and 16 female dental technicians who were resident in seoul. The prevalence of respiratory symptoms was 19% on cough, 40% on sputum, 33% on coryza, and 36% on dyspnea without past cardiopulmonary disease. The number of subjects who was abnormal ventilatory function among % predicted value for FVC(<80%), FEV1(<80%) and $FEV_1$/FVC(<75%) was 18(22%). Indices FVC and $FEV_1$ among smokers were 3 to 6% lower than that of nonsmokers.

  • Advances in research and medical and supportive care have contributed to a growing population of adults formerly treated for childhood cancer. History of cancer and its therapy can have significant life-long health implications. Late effects of cancer therapy can be insidious on onset, occur outside the pediatric age, and contribute to premature morbidity and mortality. In this review, I have focused on the key long-term effects of pediatric cancer therapy, particularly on the metabolic syndrome, including cardiopulmonary complications, infertility, and secondary neoplasm.

  • A case of ventricular septal defect associated with aorticopulmonary septal defect, left superior vena cava, and pulmonary hypertension in 6 years old boy is presented. Teflon patch closure of ventricular septal defect through transtricuspital procedure and division and suture closure of aorticopulmonary window performed under cardiopulmonary bypass with cardioplegia. The postoperative course was uneventful and discharged with good general condition.
