강원도 춘천지역 유아의 이유실태에 관한 조사연구 (Study on Weaning Practice of Infants in Chunchon Area, Kangwon-Do)

  • The weaning practice of infants from 5 to 12 months of age in the area of Chunchon, Kangwon-do was studied. Subjects were divided into 2 groups depending on residential area, monthly family income and mother's educational level. Parents of group 1 (n=90) received higher education and more monthly family income than those of group 2(n=32). The family income was significantly higher in group 1 than in group 2. Mother's educational level has been markedly improved over 10 years, which affected the feeding methods before weaning in both groups(P<0.05). Group 1 appeared to be bottle-fed. Bottle-and mixed-feeding were mainly due to lack of breast milk. 95.6% of group 1 and 71.9% of group 2 started weaning before the age of 6 months and only 16.4% were completely finished regardless of the groups. Commercially prepared foods were used more than home-made for first given-supplementary food. 82.2% of group 1 received fruits and 84.4% of group 2 cookies/crackers as their main supplementary foods. As the motivation of onset of weaning, 'for baby's health and nutritional status' was the most common. These results demonstrate that there are distinct differences between these 2 groups in 1) the feeding methods prior to weaning, 2) time of onset of weaning, and 3) main supplementary foods, influenced by mother's educational level, economic purchasing power, or subjects birth order.

침치료가 산모의 모유량 변화에 미치는 영향에 대한 임상연구 (A Clinical Study on the Effect of Acupuncture Treatment for the Change of Milk Production)

  • Purpose: The aim of this study was to investigate the effect of Acupuncture Treatment on the change of milk production. Methods: The subjects were 43 women who admitted for postpartum treatment in Hospital of Woosuk University from 25th July 2005 to 25th May 2007. The Control group were 22 women and Acupunture treatment group were 21 women. In control group we measured only the breast milk amount and in acupunture treatment group we measured the breast milk amount after acupunture at Sot#aek(SI1), $Ch{\bar{o}}njung$(CV17), $Yug{\bar{u}}n$(ST18), Chok-Samni(ST36). We checked breast milk amount, VAS(satisfaction of breast milk amount), vital sign, wrong response everyday. We tested CBC, LFT(AST, ALT, ${\gamma}-GTP$), prolactin before and after clinical study. Results: VAS of acupunture treatment group was significantly decresed compared with that of control group. Conclusion: The results suggest that acupuncture treatment may be useful for breast feeding.

Effects of Dietary Supplementation of Aqueous Extracts of Liriopeplatyphylla and Akebiaquinata on Breast Meat Qualities of Broiler Chickens

  • The present study investigated the effects of Liriopeplatyphylla extract (LPE) and Akebiaquinata extract (AQE) on breast meat properties when used as dietary supplements of broiler chickens. First, the identification and quantification of phenolic acids and flavonoids were carried out by HPLC. As a result, the total amount of phenolic acids and flavonoids was higher in AQE than LPE. These extracts were added at a rate of 0.2% to the broiler diets, and a feeding trial was conducted in battery cages for 35 d. At the end of the experiment (d 35), six carcasses from each treatments were used for evaluating meat quality. The experimental results indicate that color shades, pH levels, volatile basic nitrogen, thiobarbituric acid reactive substance (TBARS), cooking loss and drip loss of breast meat fed with 2 extracts were not different as compared with the controls at d 0 and d 10 of storage. However, TBARS values of breast meat fed with either the control diet or the LPE supplementation was increased as the storage period increased (from d 0 to d 10) (p<0.05), while AQE-fed groups were not different between d 0 to d 10 of storage. In textural properties, the addition of LPE and AQE decreased shear force values at d 10 of storage (p<0.05). Cohesiveness, gumminess and chewiness of breast meat were increased in AQE-fed groups when compared with the control at d 0 of storage (p<0.05). Dietary additions of AQE and LPE only increased the linoleic acid contents of chicken breast meat (p<0.05). In conclusion, supplementation of these extracts in broiler diets may potentially influence meat qualities including the TBARS, textural properties and linoleic acid levels in broiler chicken meats.

모유영양아와 인공영양아의 칼슘과 인의 섭취상태에 관한 종단적 연구 (A Longitudinal Study of Calcium and Phosphorus Intakes of Korean Infants from 1 to 3 Months in Breast-Fed vs Formula-Fed Infants)

  • This study was carried out to evaluate the calcium and phosphorus intakes and correlation with growth performance of 25male breast and formula-fed infants from 1 to 3 months postpartum. There were four groups breast-fed(BF) and three formula-fed groups(FFM, FFN and FFP). There was no significant difference in the height and weight of infants according to feeding methods and formula brands. Milk intake and the concentration of calcium and phosphorus from human milk and the formulas were measured. The average calcium content of human milk was 26.6$\pm$4.1mg/dl and 13.5$\pm$2.0mg/dl, respectively. The average calcium intakes of the brest-fed and formula-fed infants were 205.5$\pm$29.3mg/day and the 460.5$\pm$70.6mg/day, respectively the average calcium intake of breast-fed infants was significantly lower than that of formula fed infant. The percent of RDA(41%) of breast-fed infants was lower than that(75.4%) of formula fed infant. The average phosphorus intake of the brest-fed and formula-fed infants were 105.1$\pm$20.1mg/day and 288.3$\pm$37.3mg/day, respectively. The precent of RDA(27%) for brest fed was significantly lower than that(75.4%) of formula fed. The Ca/P ratios were 2.1 in brest fed and 1.6 in formula fed infant. The average calcium and phosphorus intakes of the formula fed infants was higher than that of the brest-fed infants. This data suggests the calcium and phosphorus intakes form human milk sufficiently support the growth of infants. Therefore, the level of calcium and phosphorus recommended dietary allowances for infants should be reduced.

Association between breastfeeding and breast, thyroid, and cervical cancer among Korean adult women based on the Korean Genome and Epidemiology Study: a cohort study

  • Purpose: The purpose of this study was to explore the association between breastfeeding and the prevalence of breast, thyroid, and cervical cancer among Korean adult women. Methods: The study was a secondary analysis of data from the Korea Genome and Epidemiology Study. The final samples were 113,944 Korean women among 173,205 urban-based cohort participants collected between 2004 and 2013 for adults aged forty and over. To determine the association between female cancers and breastfeeding experience, the number of childbirth, and total breastfeeding duration, logistic regression analysis was done. The demographic characteristics, health behavior, and female history were adjusted. Results: The prevalence of breast cancer was 1.37 times higher in the non-breastfeeding group than in the breastfeeding group. Compared to having breastfed for more than 36 months, the prevalence of thyroid cancer was 1.68 times higher at breastfeeding for 13 to 36 months, 1.67 times higher at breastfeeding for 6 to 12 months, and 2.06 times higher at breastfeeding less than 6 months. Also, the prevalence of cervical cancer was 1.54 times higher at breastfeeding for 13 to 36 months, compared to breastfeeding for more than 36 months. Conclusion: The study found that breastfeeding experience and a longer breastfeeding duration are associated with reduced risk of breast, thyroid, and cervical cancer in Korean women. It can be used as a basis for encouraging breastfeeding, and suggests further research on modifiable factors that reduce cancer risks.

사료 내 단백질 함량 및 합성아미노산 첨가에 따른 육계의 사양성적 및 가슴육 성상에 미치는 영향 (Effects of Reduced Dietary Crude Protein Diet with Synthetic Amino Acid Supplements on the Growth Performance and Histochemical Characteristics of the Breast Meat of Male Broilers)

  • The present study was conducted to determine the effect of reduced dietary crude protein (CP) with a synthetic indispensable amino acids (AA) supplement on the growth performance, economics, and breast meat quality. A total of 450 male broilers (Ross 308) were used from the age of 7 to 28 days. On d 7, the birds were individually weighed and randomly assigned to three treatment diets with six replicate pens for each treatment in a randomized complete block design. The experimental diets were: (1) a control diet, (2) a diet with a reduction of 1% of the dietary CP with synthetic AA supplements to meet the indispensable AA requirement (OAA) of broiler chickens, and (3) a diet with a reduction of 1% of the dietary CP with synthetic Lys, Met, and Thr supplementation to exceed 5% of the indispensable AA requirement (HAA). There were no differences among the treatments on the final body weight, weight gain, and feed intake. However, the HAA diet impaired the feed conversion ratio at d 21 and during the overall feeding periods (P<0.05). The predicted total feed intake and feed price required to reached 1.5 kg BW was higher than for the birds fed the HAA diet (P<0.05). The breast meat muscle fiber cross sectional area and fiber density varied between the treatments (P<0.05). However, there were no differences in breast meat weight. In conclusion, reducing 1% of dietary CP had no adverse effects on the growth performance or breast meat yields.

수유 종류에 따른 위식도 역류의 빈도 (Study on Gastroesophageal Reflux according to Feeding Types)

  • 목 적: 위식도 역류는 위내용물이 불수의적으로 식도로 넘어가는 것으로, 신생아 및 영유아에서 하부식도 괄약근의 발달이 해부학적으로 미숙하기 때문에, 하부식도 괄약근의 수축력이 낮아 잘 일어난다. 식도는 15개월이 되어야 기능이 성숙하게 되므로 그 동안 위배출시간을 단축시킬 수 있다면 위식도 역류를 감소시킬 수 있을 것이며, 위배출시 간은 수유의 종류와 자세에 따라 영향을 받을 것이다. 방 법: 저자는 수유종류에 따른 위식도 역류 빈도의 차이를 알아보고자 1996년 8월부터 1999년 6월까지 순천향대학병원에 잦은 올려냄으로 내원한 6개월 미만의 영아 16명(모유 수유군 7례, 분유 수유군 9례)을 대상으로 위식도 역류증에 대한 검사를 시행하였다. 각 수유군을 24시간 식도 pH 검사를 통하여 역류 횟수, 역류율, 가장 긴 역류 시간, 5분 이상 지속된 역류 횟수, 직립위시 가장 긴 역류 시간, 앙와위시 가장 긴 역류 시간 등의 6가지 역류지수로 비교하였고 통계처리는 Mann- Whitney 검정을 통하여 분석하였다. 결 과: 1) 연구 대상은 모유 수유군 7례, 분유 수유군 9례 총 16례였으며 평균 월령은 모유 수유군이 2.1개월, 분유수유군이 2.6개월로 차이가 없었다. 2) 역류 횟수는 모유 수유군이 $244{\pm}151$회/일, 분유 수유군이 $275{\pm}155$회/일로 차이가 없었다. 3) 역류율은 모유 수유군이 $14{\pm}15%$, 분유 수유군이 $28{\pm}22%$로 모유수유군이 낮았다. 4) 가장 긴 역류 시간은 모유 수유군이 $20{\pm}28$분, 분유 수유군이 $58{\pm}66$분으로 분유 수유군이 길었다. 5) 5분 이상 지속된 역류 횟수는 각각 $5{\pm}6$회와 $9{\pm}3$회로 차이가 없었다. 6) 직립위시 가장 긴 역류 시간은 각각 $10{\pm}8$분과 $40{\pm}47$분으로 모유 수유군이 의미있게 짧았다. 7) 앙와위시 가장 긴 역류 시간은 각각 $18{\pm}29$분과 $52{\pm}66$분으로 모유 수유군이 의미있게 짧았다. 결 론: 이상의 결과로 보아 식도의 기능이 미숙하고 위의 연동운동이 아직 발달 중에 있는 영아들은 모유 수유를 하는 것이 위식도 역류증을 줄일 수 있는 방법이며 이는 모유 수유의 또 하나의 장점이라고 할 수 있겠다.

대구${\cdot}$경북지역 유방암 위험에 영향 미치는 생식적 요인 및 식품섭취 패턴 (Reproductive Factor and Food Intake Pattern Influencing on the Breast Cancer Risk in Daegu${\cdot}$Gyungbuk Area, Korea)

  • 본 연구에서는 대구${\cdot}$경북 지역의 103명의 유방암 환자 와 159명의 대조군을 대상으로 생식적 특성과 식품섭취패턴이 유방암 위험에 미치는 영향을 분석하였다. 조사는 설문지와 식품섭취빈도조사지, 개인 면담을 통하여 실시하였다. 식품섭취패턴에 따른 유방암의 상대적 위험도는 일반특성과 생식특성에서 환자군과 대조군간에 유의한 차이를 보인 요인들을 혼란변수로 통제한 후 산출되었다. 본 연구의 결과 유방암 환자군의 평균 BMI는 대조군에 비해 유의적으로 높았으며 이 차이는 특히 폐경 후 여성에서 현저하였다. 환자군은 유방암 가족력이 유의하게 높았으며 유산경험이 유의적으로 많았고, 모유수유 경험과 총 모유수유기간이 유의적으로 낮았다. 경구피임약과 호르몬 대체요법 등의 외인성 호르몬의 사용과 유방암 사이에는 유의적인 관련성이 나타나지 않았다. 식품섭취패턴과 관련하여서는 찜조리 선호군에 비해 튀김, 구이 조리 선호군에서 위험도가 유의적으로 높게 나타났다. 과일류와 해조류의 섭취 빈도가 높을수록 상대적 위험도가 유의적으로 낮았고, 녹황색 및 담색 채소류와 콩류는 섭취빈도가 많을수록 위험도가 낮은 경향을 보였지만 유의적인 결과는 아니었다. 생선류, 육류, 유지류, 유제품류의 섭취빈도에 따른 위험도의 관련성은 나타나지 않았으며, 녹차 커피의 경우는 일주에 2${\sim}$3회 섭취가 위험도를 낮추는 경향을 보였다. 본 연구의 결과 유방암의 위험과 관련된 요인으로는 높은 BMI, 유방암 가족력, 높은 유산 경험과 낮은 모유수유 경험, 짧은 모유수유 기간으로 나타났고, 식품섭취관련 인자로는 튀김 및 볶음, 구이 등의 조리법 선호와 과일과 해조류의 낮은 섭취 빈도가 위험요인으로 나타났다.

산욕기 산모와 신생아의 가정간호 알고리즘 개발 (Development of algorithms for the home care of postpartum mothers and infants)

  • The needs for the home care of postpartum mothers and their infants are increasing, but the quality control of home care nurses is not developed yet. The objective of this study is to develop assessment - intervention algorithms for the home care of postpartum mothers and their infants. We can use these algorithms when we assess the client's condition, and find appropriate nursing interventions. Also, these algorithms can offer guidelines for home care nurses, so that standardization of home care can be attained. Common problems for postpartum mothers are postpartum hemorrhage, abnormal vaginal discharge(endometritis), episiotomy pain, breast problems, breastfeeding difficulty, edema, urinary dysfunction and defecation difficulties. Also, commom problems for infants are abnormal body temperature, tarchycardia, respiratory problem, neonatal jaundice, cord problem, abnormal stool, breast feeding, and bathing. These algorithms can be used as a basis for the development of computerized infomation system for the home health care.

모유 미생물총에 대한 고찰 (Human Milk Microbiota: A Review)

  • A common belief is that human milk is sterile. However, the development of culture-independent molecular methods, especially Next Generation Sequencing, has revealed that human milk harbors diverse and rich bacterial communities. Although studies aimed at characterizing the microbiota of human milk have produced different findings, Staphylococcus and Streptococcus are presumed to be normal members of the microbiota. Factors that influence variation in the microbiota are unclear; however, the postpartum time, route of delivery, maternal obesity, and health status may be influential. The origin of the microbiota is a hotly debated topic. Human milk bacteria are thought to be introduced through bacterial exposure of the mammary duct during breast feeding and/or the entero-mammary pathway from the maternal gastrointestinal tract. Although the exact mechanism related to the entero-mammary pathway is unknown, it is presumed that bacteria penetrate the intestinal epithelium and then migrate to the mammary gland, dendritic cells, and macrophages. In this review, various relevant studies are introduced.
