Vulnerability Assessment of Human Health Sector due to Climate Change: Focus on Ozone (기후변화에 따른 보건 분야의 취약성 평가: O3을 중심으로)

  • Adaptation of climate change is necessary to avoid unexpected impacts of climate change caused by human activities. Vulnerability refers to the degree to which system cannot cope with impacts of climate change, encompassing physical, social and economic aspects. Therefore the quantification of climate change impacts and its vulnerability is needed to identify vulnerable regions and to setup the proper strategies for adaptation. In this study, climate change vulnerability is defined as a function of climate exposure, sensitivity, and adaptive capacity. Also, we identified regions vulnerable to ozone due to climate change in Korea using developed proxy variables of vulnerability of regional level. 18 proxy variables are selected through delphi survey to assess vulnerability over human health sector for ozone concentration change due to climate change. Also, we estimate the weighting score of proxy variables from delphi survey. The results showed that the local regions with higher vulnerability index in the sector of human health are Seoul and Daegu, whereas regions with lower one are Jeollanam-do, Gyeonggi-do, Gwangju, Busan, Daejeon, and Gangwon-do. The regions of high level vulnerability are mainly caused by their high ozone exposure. We also assessed future vulnerability according to the Intergovernmental Panel on Climate Change (IPCC) Special Report on Emissions Scenarios (SRES) A2, A1FI, A1T, A1B, B2, and B1 scenarios in 2020s, 2050s and 2100s. The results showed that vulnerability increased in all scenarios due to increased ozone concentrations. Especially vulnerability index is increased by approximately 2 times in A1FI scenarios in the 2020s. This study could support regionally adjusted adaptation polices and the quantitative background of policy priority as providing the information on the regional vulnerability of ozone due to climate change in Korea.

Meta-analysis of Associations between ATM Asp1853Asn and TP53 Arg72Pro Polymorphisms and Adverse Effects of Cancer Radiotherapy

  • Background: The ataxia telangiectasia mutated (ATM) protein and p53 play key roles in sensing and repairing radiation-induced DNA double strand breaks (DSBs). Accumulating epidemiological evidence indicates that functional genetic variants in ATM and TP53 genes may have an impact on the risk of radiotherapy-induced side effects. Here we performed a meta-analysis to investigate the potential interaction between ATM Asp1853Asn and TP53 polymorphisms and risk of radiotherapy-induced adverse effects quantitatively. Materials and Methods: Relevant articles were retrieved from PubMed, ISI Web of Science and the China National Knowledge Infrastructure (CNKI) databases. Eligible studies were selected according to specific inclusion and exclusion criteria. Odds ratios (ORs) and 95% confidence intervals (CIs) were pooled to estimate the association between ATM Asp1853Asn and TP53 Arg72Pro polymorphisms and risk of radiotherapy adverse effects. All analyses were performed using the Stata software. Results: A total of twenty articles were included in the present analysis. In the overall analysis, no significant associations between ATM Asp1853Asn and TP53 Arg72Pro polymorphisms and the risk of radiotherapy adverse effects were found. We conducted subgroup analysis stratified by type of cancer, region and time of appearance of side effects subsequently. No significant association between ATM Asp1853Asn and risk of radiotherapy adverse effects was found in any subgroup analysis. For TP53 Arg72Pro, variant C allele was associated with decreased radiotherapy adverse effects risk among Asian cancer patients in the stratified analysis by region (OR=0.71, 95%CI: 0.54-0.93, p=0.012). No significant results were found in the subgroup analysis of tumor type and time of appearance of side effects. Conclusions: The TP53 Arg72Pro C allele might be a protective factor of radiotherapy-induced adverse effects among cancer patients from Asia. Further studies that take into consideration treatment-related factors and patient lifestyle including environmental exposures are warranted.

Behavioural and Metabolic Risk Factors for Mortality from Colon and Rectum Cancer: Analysis of Data from the Asia-Pacific Cohort Studies Collaboration

  • Background: Colorectal cancer has several modifiable behavioural risk factors but their relationship to the risk of colon and rectum cancer separately and between countries with high and low incidence is not clear. Methods: Data from participants in the Asia Pacific Cohort Studies Collaboration (APCSC) were used to estimate mortality from colon (International Classification of Diseases, revision 9 (ICD-9) 153, ICD-10 C18) and rectum (ICD-9 154, ICD-10 C19-20) cancers. Data on age, body mass index (BMI), serum cholesterol, height, smoking, physical activity, alcohol and diabetes mellitus were entered into Cox proportional hazards models. Results: 600,427 adults contributed 4,281,239 person-years follow-up. The mean ages (SD) for Asian and Australia/New Zealand cohorts were 44.0 (9.5) and 53.4 (14.5) years, respectively. 455 colon and 158 rectum cancer deaths were observed. Increasing age, BMI and attained adult height were associated with increased hazards of death from colorectal cancer, and physical activity was associated with a reduced hazard. After multiple adjustment, any physical activity was associated with a 28% lower hazard of colon cancer mortality (HR 0.72, 95%CI 0.53-0.96) and lower rectum cancer mortality (HR 0.75, 95%CI 0.45-1.27). A 2cm increase in height increased colon and all colorectal cancer mortality by 7% and 6% respectively. Conclusions: Physical inactivity and greater BMI are modifiable risk factors for colon cancer in both Western and Asian populations. Further efforts are needed to promote physical activity and reduce obesity while biological research is needed to understand the mechanisms by which they act to cause cancer mortality.

Retrospective Appraisal of Cancer Patients from Vientiane Capital City, Lao People's Democratic Republic (PDR), Seeking Treatment in Thailand

  • Background: Recent estimates suggest that in the Lao People's Democratic Republic (Lao PDR) the burden of cancer in terms of DALYs lost is amongst the highest in South East Asia. As such, increasingly cancer is becoming an important public health concern in the country. Lao PDR however has no population-based cancer registry and only one hospital-based registry. Cancer treatment within the country is extremely limited. Patients who can, may travel to neighboring countries for treatment, but little information about this is available in the country. The aim of this study was to estimate some of the otherwise largely unknown parameters of the cancer burden in Lao PDR. Materials and Methods: This is a retrospective, descriptive study based on the records of 847 Lao cancer cases treated with surgery, radiation and chemotherapy at Srinagarind Hospital, Khon Kaen University, in Thailand between 1988 and 2010. Results: The annual rate of registration of Lao cancer cases fluctuated, but showed an increasing trend. Most cancers were diagnosed by histology (65.2%), and a combination of endoscopy and radiology (15.6%). In most cases (70.2%) the stage of cancer at diagnosis could not be determined. In those whose stage could be identified, 54.0% were at the final stage (Stage IV). Among males, the commonest cancer sites were the liver (16.1%), blood (12.3%) and nasopharynx (10.6%). Those in female patients were the cervix (22.2%), breast (14.6%) and blood (8.1%). Conclusions: This study indicates that despite some fluctuations, the number of Lao cancer patients presenting at Srinagarind Hospital, Khon Kaen, gradually increased between 1988 and 2010. The unfavorable pattern of late-stage cancer diagnosis among male and female patients suggests a need for cancer control interventions and the establishment of cancer registration and treatment facilities within Lao PDR.

Auto-Segmentation Algorithm For Liver-Vessel From Abdominal MDCT Image (복부 MDCT 영상으로부터 간혈관 자동 추출 알고리즘)

  • It is essential for living donor liver transplantation that surgeon must understand the hepatic vessel structure to improve the success rate of operation. In this paper, we extract the liver boundary without other surrounding structures such as heart, stomach, and spleen using the contrast enhanced MDCT liver image sequence. After that, we extract the major hepatic veins (left, middle, right hepatic vein) with morphological filter after review the basic structure of hepatic vessel which reside in segmented liver image region. The purpose of this study is provide the overall status of transplantation operation with size estimation of resection part which is dissected along with the middle hepatic vein. The method of liver extraction is as follows: firstly, we get rid of background and muscle layer with gray level distribution ratio from sampling process. secondly, the coincident images match with unit mesh image are unified with resulted image using the corse coordinate of liver and body. thirdly, we extract the final liver image after expanding and region filling. Using the segmented liver images, we extract the hepatic vessels with morphological filter and reversed the major hepatic vessels only with a results of ascending order of vessel size. The 3D reconstructed views of hepatic vessel are generated after applying the interpolation to provide the smooth view. These 3D view are used to estimate the dissection line after identify the middle hepatic vein. Finally, the volume of resection region is calculated and we can identify the possibility of successful transplantation operation.

A Study for 8 Constitution Medicine Diagnosis Expert System Development (8체질의학을 위한 진단 전문가 시스템 개발 및 고찰)

  • Background: There was seldom study about method that diagnose 8 Constitution beside method of pulse diagnosis in 8 Constitution Medicine. Objectives: This study is to make out 8 Constitution Medicine Diagnosis Expert System Development used CBR(Case based Reasoning). Methods: First, at case base construction process we constructed case base for CBR embodiment because gathering 925 cases all to patient who constitution is verified, and second, at study model establishment process superior expert system development by purpose CBR of reasoning process dividing fundamental type CBR that spend basis data value and expert type I II III CBR that reflect weight in basis data value according to advice expert opinion, and third, system embodiment process explained about way to give process and weight that diagnose constitution through Nearest Neighbor Method sampling process of CBR techniques, and fourth, at system estimation process we selected superior CBR model because comparing and estimate the diagnosis rate of expert system with fundamental type system (GECBR) model and expert type I II III CBR system (AVCBR, AACBR, AGCBR) model that reflect expert opinion in fundamental type system. GECBR and AGCBR chose on superior study model. Through such 4 study process, we developed 8 constitution diagnosis expert system lastly. Results: 1. When we select GECBR that is fundamental type by reasoning system, diagnosis rate 78.91% of 8 constitution diagnosis expert system is expected, and the constitution diagnosis rate Hepatonia 90.4%, Cholecystonia 63.0%, Pancreotonia 91.1%, Gastrotonia 0%, Pulmotonia 71.2%, Colonotonia 74.4%, Renotonia 37.5%, Vesicotonia 67.1% expect. 2. When we select AGCBR that is expert type III by reasoning system, diagnosis rate 77.51% of 8 constitution diagnosis expert system is expected, and the constitution diagnosis rate Hepatonia 93.4%, Cholecystonia 58.5%, Pancreotonia 91.1%, Gastrotonia 0%, Pulmotonia 73.1%, Colonotonia 64.4%, Renotonia 41.7%, Vesicotonia 72.2% expect. Conclusion: Based on this study, 8 constitution diagnosis expert system may give help to diagnose 8 constitution, and it is going to utilize as objective estimation tool of 8 constitution diagnosis, and further study for 8 Constitution Medicine Diagnosis Expert System Development used CBR(Case based Reasoning) is needed to supplement this study.

Consider the directional hole filling method for virtual view point synthesis (가상 시점 영상 합성을 위한 방향성 고려 홀 채움 방법)

  • Recently the depth-image-based rendering (DIBR) method is usually used in 3D image application filed. Virtual view image is created by using a known view with associated depth map to make a virtual view point which did not taken by the camera. But, disocclusion area occur because the virtual view point is created using a depth image based image 3D warping. To remove those kind of disocclusion region, many hole filling methods are proposed until now. Constant color region searching, horizontal interpolation, horizontal extrapolation, and variational inpainting techniques are proposed as a hole filling methods. But when using those hole filling method some problem occurred. The different types of annoying artifacts are appear in texture region hole filling procedure. In this paper to solve those problem, the multi-directional extrapolation method is newly proposed for efficiency of expanded hole filling performance. The proposed method is efficient when performing hole filling which complex texture background region. Consideration of directionality for hole filling method use the hole neighbor texture pixel value when estimate the hole pixel value. We can check the proposed hole filling method can more efficiently fill the hole region which generated by virtual view synthesis result.

The Significance of Serum Thyroid Peroxidase as a New Tumor Marker in Papillary Thyroid Carcinoma after Thyroidectomy (유두상 갑상선암의 수술후 재발예측인자로서 혈청 Thyroid Peroxidase의 의의)

  • Background: Total thyroidectomy and postoperative radiodiodine ablation therapy in differentiated thyroid carcinomas enhance the reliability of serum thyroglobulin(Tg) levels and radioiodine scan in detecting recurrence or distant metastasis. There have been, however, some limitations in using these methods under certain conditions. Recently, several reports have indicated that thyroid peroxidase(TPO) could be used as an alternative tumor marker. We aimed to estimate the significance of serum TPO levels in differentiated thyroid carcinoma. Materials and Methods: Forty-eight patients who had undergone total thyroidectomy due to papillary thyroid carcinomas and who had been followed-up for at least 3 years were classified into two groups: 27 patients without any evidence of recurrence in group 1; and 20 patients with recurrence or distant metastasis in group 2. All patients were examined by radioiodine scans. Serum Tg, TSH, antithyroglobulin antibody, and TPO were measured and the relationships were statistically analyzed. The sensitivity and specificity of $^{131}I$ scan, serum Tg, and serum TPO were evaluated. Results: Serum Tg levels were $3.81{\pm}5.16ng/mL$ in group 1 and $147.02{\pm}193.75ng/mL$ in group 2. Only 2 patients in group 1 showed Tg levels exceeding 10ng/mL. In contrast, 4 patients in group 2 were under 10ng/mL. Serum antithyroglobulin antibody and TSH levels showed no statistical difference between the two groups. In group 1, 16 patients showed negative serum TPO results, and 4 patients in group 2 showed negative results. There was no correlation among serum Tg levels, antithyroglobulin antibody titers, and serum TPO levels in each group. In group 2, 4 patients with negative serum Tg levels showed positive TPO results and positive whole body scans. Two cases with false negative $^{131}I$ scans showed positive serum TPO and Tg results. In 4 cases showing false negative serum TPO levels, serum Tg levels and $^{131}I$ scans were positive. Conclusion: Serum Tg levels, radioiodine scans, and serum TPO levels can be clinically used as complementary methods in the diagnosis of recurrent or metastatic thyroid carcinomas. Serum TPO levels may be helpful when other methods fail to detect recurrences or distant metastasis in highly suspected patients.

Changes in Acoustic Parameters According to Intensity Increase in Voice Assessment (음성질환자의 음성검사 시 강도 증가에 따른 음향학적 지표의 변화)

  • Background and Objectives : Clinically, as a tool for voice assessment before and after the operation or the voice treatment, acoustic analysis is widely used. However, in clinical situations, acoustic parameters vary according to how the assessment is made. Thus, with voice disease patients as subjects, we are to investigate what influence intensity increase exerts on acoustic parameters and how to reduce variation according to the way of assessing. Material and Method : At the voice clinic of the department of otorhinolaryngology in Gangnam Severance Hospital, with 30 female voice-disease patients (40.6 years old on the average) and 23 male voice-disease patients (40.1 years old on the average) as subjects, using the Dr Speech vocal-assessment program, we statistically tested the significance of the difference in each of acoustic parameters between when the "Ah" vowel is produced with a normal voice and when the "Ah" vowel is produced with a loud voice. Results : Acoustic parameters that showed a statistically significant difference according to intensity increase were Jitter, SD F0, and NNE for females, and Jitter, SD F0, HNR, SNR, and NNE for males. Voice quality estimates showed a statistically significant difference according to intensity increase in female hoarse voice, female breathy voice, and male breathy voice. Conclusion : In this research, acoustic analysis, which is generally used for voice assessment before and after the operation or the voice treatment, showed a tendency that acoustic parameters became better under the influence of intensity increase except for the cases where a voice disease was severe. Thus, to raise the reliability of voice assessment, the range of intensity needs to be set up. This should be the topic for the future research.

Characteristics of Greenhouse Gas Emissions with Different Combination Rates of Activated Rice Hull Biochar during Aerobic Digestion of Cow Manure (왕겨 활성 바이오차 혼합 비율에 따른 우분 호기소화 시 온실가스 발생 특성)

  • BACKGROUND: Among the biomass conversion techniques of livestock manure, composting process is a method of decomposing organic matter through microorganisms, and converting it into fertilizer in soil. The aerobic composting process is capable of treating cow manure in large quantities, and produces greenhouse gas as CO2 and N2O, although it has economical benefit. By using the activated rice hull biochar, which is a porous material, it was intended to mitigate the greenhouse gas emissions, and to produce the compost of which quality was high. Objective of this experiment was to estimate CO2 and N2O emissions through composting process of cow manure with different cooperated biochar contents. METHODS AND RESULTS: The treatments of activated rice hull biochar were set at 0%, 5%, 10% and 15%, respectively, during composting cow manure. The CO2 emission in the control was 534.7 L kg-1, but was 385.5 L kg-1 at 15% activated rice hull biochar. Reduction efficiency of CO2 emission was estimated to be 28%. N2O emission was 0.28 L kg-1 in the control, but was 0.03 L min-1 at 15% of activated rice hull biochar, estimating about 89% reduction efficiency. CONCLUSION: Greenhouse gas emissions during the composting process of cow manure can be reduced by mixing with 15% of activated rice hull biochar for eco-friendly compost production.
