Case of seropositive allergic bronchopulmonary aspergillosis in a 10-year-old girl without previously documented asthma

  • Allergic bronchopulmonary aspergillosis (ABPA) is a hypersensitivity lung disease due to bronchial colonization of Aspergillus fumigatus that occurs in susceptible patients with asthma or cystic fibrosis. A 10-year-old girl was referred to the Department of Pediatric Pulmonology for persistent consolidations on chest radiography. Pulmonary consolidations were observed in the right upper and left lower lobes and were not resolved with a 4-week prescription of broad-spectrum antibiotics. The patient had a history of atopic dermatitis and allergic rhinitis but no history of asthma. She had no fever but produced thick and greenish sputum. Her breathing sounds were clear. On laboratory testing, her total blood eosinophil count was $1,412/mm^3$ and total serum IgE level was 2,200 kU/L. Aspergillus was isolated in the sputum culture. The A. fumigatus-specific IgE level was 15.4 kU/L, and the Aspergillus antibody test was also positive. A chest computed tomography scan demonstrated bronchial wall thickening and consolidation without bronchiectasis. An antifungal agent was added but resulted in no improvement of pulmonary consolidations after 3 weeks. Pulmonary function test was normal. Methacholine provocation test was performed, revealing bronchial hyperreactivity ($PC_{20}=5.31mg/mL$). Although the patient had no history of asthma or bronchiectasis, ABPA-seropositivity was suspected. Oral prednisolone (1 mg/kg/day) combined with antifungal therapy was started. Pulmonary consolidations began decreasing after 1 week of treatment and completely resolved after 1 month. This is the first observed and treated case of seropositive ABPA in Korean children without previously documented asthma.

  • A complex interplay between genetic and environmental factors partially contributes to the development of allergic diseases by affecting development during prenatal and early life. To explain the dramatic increase in the prevalence of allergic diseases, the hygiene hypothesis proposed that early exposure to infection prevented allergic diseases. The hygiene hypothesis has changed to the microbial hypothesis, in which exposure to microbes is closely linked to the development of the early immune system and allergic diseases. The intestinal flora may contribute to allergic disease through its substantial effect on mucosal immunity. Based on findings that exposure to microbial flora early in life can change the Th1/Th2 balance, thus favoring a Th1 cell response, probiotics may be beneficial in preventing allergic diseases. However, evidence from clinical and basic research to prove the efficacy of probiotics in preventing allergy is lacking. To date, studies have yielded inconsistent findings on the usefulness of probiotics in allergic diseases. It is difficult to demonstrate an exact effect of probiotics on asthma, allergic rhinitis, and food allergy because of study limitations, such as different first supplementation period, duration, different strains, short follow-up period, and host factors. However, many studies have demonstrated a significant clinical improvement in atopic dermatitis with the use of probiotics. An accurate understanding of the development of human immunity, intestinal barrier function, intestinal microbiota, and systemic immunity is required to comprehend the effects of probiotics on allergic diseases.

Effect of 1-palmitoyl-2-linoleoyl-3-acetyl-rac-glycerol on Immune Functions in Healthy Adults in a Randomized Controlled Trial

  • We previously reported that 1-palmitoyl-2-linoleoyl-3-acetyl-rac-glycerol (PLAG) accelerates hematopoiesis and has an improving effect on animal disease models such as sepsis and asthma. The effects of PLAG supplementation on immune modulation were assessed in healthy men and women. The objective was to evaluate the effects of PLAG supplementation on immune regulatory functions such as activities of immune cells and cytokine production. A randomized double blind placebo-controlled trial was conducted. Seventy-five participants were assigned to one of two groups; all participants had an appropriate number of white blood cells on the testing day. The PLAG group (n=27) received oral PLAG supplements and the control group (n=22) received oral soybean oil supplements. IL-4 and IL-6 production by peripheral blood mononuclear cells (PBMC) were lower (p<0.001 and p<0.001, respectively) with PLAG than with soybean oil. However, the production of IL-2 and IFN-$\gamma$ by PBMC was unaltered with PLAG supplementation. The B cell proliferation decreased significantly in the PLAG group compared to the soybean oil control (p<0.05). The intake of PLAG in healthy adults for 4 weeks was deemed safe. These data suggest that PLAG has an immunomodulatory function that inhibits the excessive immune activity of immunological disorders such as atopic and autoimmune diseases. PLAG could improve the condition of these diseases safely as a health food supplement.

  • The common cold is the most common pediatric infectious disease, and the incidence is higher in early childhood than in any other period of life. Treatment of the common cold is largely symptomatic. Thus treatment of oriental medicine for the common cold is very effective and safe. We studied 81 children suffering from common cold Oriental Medicine Hospital in Kyunghee University, and analyzed age, sex, season, clinical manifestation, complications, prescription. The results were as follows: 1. Age and sex distribution of children from 1 to 3 was 37.1%, 3 to 6 was 34.6%, over 6 was 24.7%, male to female ratio was 1.31 : 1 2. Frequency of affection per year, from 3 to 5 was 30.9%, 6 to 9 was 29.6%, over 10 was 27.2%, below 3 was 4.9% 3. Frequency of seasons giving rising to common cold, winter was 40.7%, spring and autumn were 16%, summer was 1.2%. 4. Frequency of complications, empyema and rhinitis were 38.3%, asthma was 21%, pneumonia was 16%, atopic dermatitis was 7.4%. 5. Frequency of Clinical manifestation, cough and sputum were 45.7%, high fever and sore throat were 27.2%, complex symptom was 25.9%. 6. Frequency of prescription, BoAh-Tang-gami(補兒湯加味)was 27.2% BangPungHaeDoc-Tang(防風解毒湯)was 11.1 %, GamiGinHae-Tang (加味鎭咳湯)was 11.1%.

  • The purpose of this research is that infant artificial feeding products is used in clonic with the study on characteristic, ingredients and indication of geneal and special modified milks. The result is as follows. 1. The main ingredients of four company products-Maeil , Namyang, Pasteur, Aebout is similar but the functional is different 2. General infant formula is divided into 100days, 5-6months, 12months, 24months and 36months out of consideration for growth and development of infant. 3. The indication and sorts of the special infant formula used at a hospital is as follows. PKU-1, PKU-2 formula is available for phenylketonuria. A Study of the Chief Complaint of Pediatric Outpatients in the Kyung Hee Oriental Medicine Hospital (경희의료원 한방소아.청소년과 외래 환자의 주소증에 대한 연구(2009년 3월~2010년 2월))

  • Objectives: The purpose of this study is investigate about chief complaints of pediatric outpatients in the oriental medicine hospital and search for a latest of patients and their symptom. Methods: The study was composed of 3024 new patients aged between 0 and 20 who had been visited pediatrics in Kyung Hee oriental medicine hospital from 2009 March to 2010 February. Results: In age distribution, 1 year old child have most visited, and the number of the patients have fallen sharply as age have increased. The most visited month is August, and the least is November. In systemic division of chief complaints, in order of respiratory part, growth part, restorative herb medicine part, digestive part, neurologic & psychologic part, dermatologic part visited in patients number, and in single chief complaints, in order of growth, restorative herb medicine, anorexia, rhinosinusitis, cold, atopic dermatitis, seizure & epilepsy have visited. As for age and systemic division, between 1 and 6 years old, the patients of respiratory division have most visited, more than 7 years old, growth division. As for age and single chief complaints, in 0 year old, seizure & epilepsy have most visited, between 4 and 6 years old, rhinosinusitis, more than 7 years old, growth. Growth and respiratory division have increased the most in August, restorative herb medicine and digestive division have increased the most in September. In single chief complaints, patients of growth have most visited in most of time, but in September and November patients of restorative herb medicine have visited the most. Conclusions: The most visited division was respiratory part, and the most visited chief complaints was growth. More data of patients should be accumulated for further study, and we expect more specific study would be done about many kinds of disease in basis of this study.

Inhibitory Effect of Gamisaenghyeolyunbueum on Mast Cell-Mediated Allergic Inflammatory Reactions

  • Gamisaenghyeolyunbueum (GSYE) is a traditional Oriental herbal medicine prescription, which has been used for the treatment of various allergic disorders, atopic dermatitis, extravasated bleeding from skin, especially skin related disease. The author investigated the effects of GSYE on mast cell-mediated allergic inflammatory reactions. GSYE dose-dependently (0.01-1 g/kg) inhibited compound 48180-induced systemic anaphylactic shock and ear swelling response. The inhibitory effect of GSYE on the histamine release from rat peritoneal mast cells induced by compound 48f80 reveals significantly (p<0.05) at concentrations ranging from 0.01 to 1 mg/ml in a dose-dependent manner. GSYE also inhibited the passive cutaneous anaphylaxis(PCA) by oral administration at 1 g/kg. In addition, GSYE dose-dependently (0.01-1 g/kg) inhibited the phorbol 12-myristate 13-acetate(PMA) and A23187-induced tumor necrosis $factor-{\alpha}$ secretion from human mast cell line HMC-1 cells. These results indicate that GSYE may be a beneficial applicability in the allergic-related diseases.

Effects of Sesami Semen on Anti-Allergic Inflammation Mechanism related with Atopic Dermatitis (흑지마(黑芝麻)가 알러지성 염증 반응에 미치는 영향)

  • This study aimed to investigate the effects of Sesami Semen on the itching and anti-allergic inflammation mechanism related with cytokine, chemokine, histamine, $\beta$-hexosaminidase, NF-$\kappa$B, and free radical, and it was concluded as follows : 1. Sesami Semen did not show any cytotoxicity at the range of con-centration (1-250 ${\mu}g/m\ell$) on the human fibroblast cell (hFCs). 2. Sesami Semen reduced the gene expressions of IL-8, TNF-$\alpha$, IL-6 mRNA comparing with control. 3. Sesami Semen reduced the levels of IL-6, IL-8, MCP-1 within THP-1 cell depending on the concentration, and especially significantly reduced the the levels of IL-6, MCP-1 at all the concentration. 4. Sesami Semen significantly decreased the histamine secretion on HMC-1 at all the concentration. 5. Sesami Semen decreased the $\beta$-Hexosaminidase secretion 6.2% at 10 ${\mu}g$/ml conc., 58.3% at 100 ${\mu}g$/ml conc. and 63.2% at 200 ${\mu}g$/ml conc., respectively. And IC50 (${\mu}g$/ml) was 158.25 ${\mu}g$/ml. 6. Sesami Semen significantly suppressed the activity of NF-$\kappa$B promoter depending on the concentration. 7. Sesami Semen decreased the production of reactive oxygen species (ROS) and DPPH generation depending on the concentration. As judged with above results, the effects of Sesami Semen on the anti-allergic inflamation would be recognized, and it could be applied on the medicinal sources for prevention or treatment of several allergy disease. And more studies are needed furthermore with the seperation of effective materials.

Association of Thymic Stromal Lymphopoietin Receptor (TSLPR) Polymorphisms with the Susceptibility of Rheumatoid Arthritis in a Korean Population (한국인 류마티스 관절염의 감수성과 TSLPR 유전자 다형성의 연관성)

  • Human thymic stromal lymphopoietin receptor (TSLPR) might play an important role in the development of inflammatory and allergic responses. We previously identified eleven single nucleotide polymorphisms (SNPs) and two variation sites in the TSLPR gene and showed that all the SNPs of the TSLPR gene are associated with susceptibility to atopic asthma. The present study aimed to investigate whether the TSLPR gene SNPs are associated with susceptibility to rheumatoid arthritis (RA). We compared the genotype and the allele frequencies of the TSLPR SNPs in 457 RA patients and 570 healthy controls. The genotype and the allele frequencies of the TSLPR gene SNPs in the RA patients were not significantly different from the respective frequencies of the healthy controls. Additional analysis showed that the genotype and the allele frequencies of the TSLPR gene SNPs did not appear to be associated with RA in female RA patients. The TSLPR gene SNPs in the RA patients did not affect the production of rheumatoid factor (RF) and antisynthetic cyclic citrullinated peptide (CCP). Our results suggest that the TSLPR gene SNPs are not associated with susceptibility to RA in the Korean population.

A Study of the Chief Complaint of Pediatric Outpatients in the Oriental Hospital (모 한방병원 소아과 외래 환자의 주소증에 대한 연구 ('01-'04년))

  • Objective : The purpose of this study was to investigate chief complaints of pediatric outpatients .Fin the oriental hospital and search for a trend of patients and their symtoms. Method : The study was composed of 2,915 new patients aged between 0 and 20 who had been visited pediatrics in ${\bigcirc}{\bigcirc}$ university oriental hospital from 2001 to 2004. Results : The percentage of new patients decreased every year with 30.7% in 2001, 25.5% in 2002, 24.1% in 2003, 17.8% in 2004. The age distribution showed 9.9% in infancy patients, 62.6% in early childhood, 16.8% in late childhood, 10.6% in adolescence. The percentage of infants decreased every year with 13.3% in 2001, 9.4% in 2002, 8.8% in 2003, 7.1% in 2004. The percentage of adolescents increased every year with 8.0% in 2001, 9.6% in 2002, 12.8% in 2003, 13.5% in 2004. Systemic division of chief complaints was respiratory symtoms(37.6%), general symtoms(26.1%), digestive symtoms(19.2%), skin symtoms(6.8%), nervous and mental symtoms (6.1%), urinary symtoms(2.2%), musculoskeletal symtoms(1.3%) and others(0.7%) in order of frequency. The main chief complaints were nasal mucus/obstruction(17.5%), weakness(16.9%), cough(12.8%), loss of appetite(9.8%), inappropriate growth(4.3%), atopic skin problem(3.4%), repeated common cold(2.4%), abdominal pain(2.4%), vomiting (2.2%), diarrhea(2.0%) and fever (1.9%). Loss of appetite, inappropriate growth and repeated common cold increased every year. Diarrhea and convulsion decreased every year. Conclusions : The two main causes of visit of patients were respiratory problems and improvement of general condition. More data of patients should be accumulated for further study. Not only analysis according to chief complaint, but also analysis according to diagnosis will accomplish more aaccurate information about disease of childrens in the future.
