Improvements in various health indices have been ushered in, along with soaring economic growth in the last few decades in Korea. Life span of Koreans at present stands at somewhere around age of seventies. It seems of worth to elucidate some facets of health profile of historic Koreans during Yi dynasty encompassing about five hundreds years between the 14th and 19th century. Datas on health of historic Koreans are meager with exception for sovereigns for whom royal archives have been preserved intact through centuries. Though the health environments for monarchs are, no doubt, incomparable to that of the grassroot levels during the same period, health profile of monarchs would facilitate to assess health indices for historic Koreans during Yi dynasty. Arithmetic average of life span of 25 consecutive Kings(omitted the last two Kings under Japanese control) is 44.6 years of age 81 year old for King Yongjo and 16 year old for King Danjong, representing two extremes. The principal causes of death are septicemia, subsequent to infections of soft tissue and ether organs(8 in number, one-thirds of all). Secondly, both epidemic disease and vascular accidents caused deaths of three Kings each. Two kings succumbed to diabets and one suicide. The causes for the remainder, eight kings are hardly ciarified. The monarchs who contributed in establishment of new Yi dynasty with highspirited and disciplined survied beyond their fifties and sixties. In contrast, those who reigned during mid - era of the dynasty at its culmination succumbed to death in their young adulthood, most likely due to their indulgence in court life.
Sleep is a vital, highly organized process regulated by complex systems of neuronal networks and neurotransmitters. Normal sleep comprises non-rapid eye movement (NREM) and REM periods that alternate through the night. Sleep usually begins in NREM and progresses through deeper NREM stages (2, 3, and 4 stages), but newborns enter REM sleep (active sleep) first before NREM (quiet sleep). A period of NREM and REM sleep cycle is approximately 90 minutes, but newborn have a shorter sleep cycle (50 minutes). As children mature, sleep changes as an adult pattern: shorter sleep duration, longer sleep cycles and less daytime sleep. REM sleep is approximately 50% of total sleep in newborn and dramatically decreases over the first 2 years into adulthood (20% to 25%). An initial predominant of slow wave sleep (stage 3 and 4) that peaks in early childhood, drops off abruptly after adolescence by 40% from preteen years, and then declines over the life span. The hypothalamus is recognized as a key area of brain involved in regulation of sleep and wakefulness. The basic function of sleep largely remains elusive, but it is clear that sleep plays an important role in the regulation of CNS and body physiologic processes. Understanding of the architecture of sleep and basic mechanisms that regulate sleep and wake cycle are essential to evaluate normal or abnormal development of sleep pattern changes with age. Reduction or disruption of sleep can have a significant impact on daytime functioning and development, including learning, growth, behavior, and emotional regulation.
Purpose : This study aimed at uncovering the disabled adult's way of thinking and behavior due to their changed life style. Method : In depth interviews with 11 participants were held and analysed based on the Grounded Theory Methodology by Strauss and Corbin (1990). Result : This study showed us that the disabled are getting independent by selfconfidence through developing coping strategies for their physical, emotional, social, sexual problems. And in the process of creating these strategies, support structures, satisfaction for performance effect, financial burdens, and mobility had an effect on the intervening factors. Adults who become disabled go through many changes. First, they have vague hopes of improving. Second, they become discouraged. Third, they accept their disability. Next, they learn coping strategies and finally, they integrate the changes into their new life style. Conclusion : This study can be utilized in rehabilitation nursing so that the disabled can adapt to their changed life style quickly and lead independent and self-confident lives.
Purpose: The reproductive health of women in early adulthood can affect pregnancy, childbirth, and menopause in later life. Menstrual symptoms not only affect daily life, but are also a reflection of a woman's reproductive health. This study was conducted to explore the factors affecting menstrual symptoms among university students. Methods: The general characteristics, life style, menstrual characteristics, stress, and menstrual symptom of 177 female students were assessed through an online survey at one university. An independent sample t-test, one-way variance analysis, correlation analysis, and multiple regression analysis were performed using the SPSS 22.0 program. Results: The menstrual symptom score was 91.68±32.11 points, and the score of 'mood change' was the highest. Stress (=.38, p<.001), amount of menstruation (=.20, p=.001), menarche age (=-.18, p=.003), health problems (=.16, p=.010), and age (=.15, p=.016) were found to have a significant effect on menstrual symptom, and the explanatory power of the regression model was 40%. Conclusion: This study investigated the degree and influencing factors of menstrual symptom using the Korean Menstrual Symptom Scale (KMSS) developed for Korean university students. Among the factors influencing menstrual symptom, stress appeared to play a significant role. Stress management, observation of menstrual characteristics, and general health care are necessary to maintain a woman's reproductive health.
Patients with gastrointestinal disease (GI) are at risk for osteopenia or osteoporosis, which can lead to fractures. Although these patients may be at risk from a young age, gastroenterologists often overlook this fact in practice. There are well-known GI diseases associated with osteopenia and osteoporosis, such as the post-gastrectomy state, inflammatory bowel disease (IBD), and celiac disease. As there is an increase in the prevalence of IBD patients, newly diagnosed celiac disease in adulthood, and gastric cancer survivors following gastrectomy, bone disease in these patients becomes an important issue. Here, we have discussed osteoporosis and fractures in GI disease, especially in the postgastrectomy state, IBD, and celiac disease. Although the pathogenesis of bone loss in each disease has not been fully identified, we have confirmed that the prevalence of osteoporosis and fractures in each of these diseases is high. There are scarce studies comparing the prevalence of osteoporosis or osteoporotic fractures in GI disease patients with studies in postmenopausal women, and specific guidelines for their management in each disease have not been established. Intensive surveillance and management are needed to ensure that these patients attain peak bone mass for age and sex to prevent fractures.
Over the last few decades, the rates of pediatric obesity have more than doubled regardless of sociodemographic categorization, and despite these rates plateauing in recent years there continues to be an increase in the severity of obesity in children and adolescents. This review will discuss the pediatric obesity mediated cardiovascular disease (CVD) risk factors such as attenuated levels of satiety and energy metabolism hormones, insulin resistance, vascular endothelial dysfunction, and arterial stiffness. Additionally, early intervention to combat pediatric obesity is critical as obesity has been suggested to track into adulthood, and these obese children and adolescents are at an increased risk of early mortality. Current suggested strategies to combat pediatric obesity are modifying diet, limiting sedentary behavior, and increasing physical activity. The effects of exercise intervention on metabolic hormones such as leptin and adiponectin, insulin sensitivity/resistance, and body fat in obese children and adolescents will be discussed along with the exercise modality, intensity, and duration. Specifically, this review will focus on the differential effects of aerobic exercise, resistance training, and combined exercise on the cardiovascular risks in pediatric obesity. This review outlines the evidence that exercise intervention is a beneficial therapeutic strategy to reduce the risk factors for CVD and the ideal exercise prescription to combat pediatric obesity should contain both muscle strengthening and aerobic components with an emphasis on fat mass reduction and long-term adherence.
Purpose: Based on the health belief model and theory of planned behavior, this study investigated how age group (adolescence and emerging adulthood) moderated the relative effects of perceived susceptibility, perceived severity, and subjective norms on preventive behavior against coronavirus disease 2019 (COVID-19). Methods: This secondary data analysis utilized data from adolescents (n=272) and emerging adults (n=239). Hierarchical multiple regression analysis was performed to test the moderating effect of age group on the relationships among variables. Results: Higher perceived susceptibility (β=.21, p<.001), perceived severity (β=.14, p=.002), subjective norms (friends) (β=.26, p<.001), subjective norms (parents) (β=.44, p<.001), and subjective norms (schools) (β=.28, p<.001) enhanced COVID-19 preventive behaviors. Moderated regression analysis showed that subjective norms (friends and school) impacted preventive behavior in adolescents more than in emerging adults. Conclusion: Given the need to increase perceived susceptibility and severity among adolescents and emerging adults, these findings provide baseline data for designing effective COVID-19 prevention interventions that consider the developmental characteristics of different age groups. Interventions by health centers at universities can strengthen COVID-19 preventive behavior among emerging adults. As adolescents are influenced by friends, their peer roles must be strengthened to enhance adherence to COVID-19 preventive guidelines.
Purpose: The aim of the present study was to evaluate the characteristics of electrophysiologic studies (EPS) and radiofrequency ablation (RFA) performed in subjects aged less than 30 years with Wolff-Parkinson-White (WPW) syndrome, particularly pediatric patients under 18 years of age, based on our experience. Methods: Two hundred and one consecutive patients with WPW syndrome were recruited and divided to 3 groups according to age: group 1, 6 to 17 years; group 2, 18 to 29 years; and group 3, 30 to 60 years. The clinical, electrophysiological, and therapeutic data for these patients were evaluated by a retrospective medical record review. Results: A total of 73 (36%) of these patients were <30 years of age. Although there were more males than females in group 2 (male:female, 31:11), there was no sex difference in group 1 (male:female, 16:15). Left accessory pathway was detected less frequently in group 1 (32%, 10/31) than in group 2 (57%, 24/42) and group 3 (63%, 81/128) (P=0.023 and P=0.002, respectively). Conclusion: The present study describes several different electrophysiological characteristics in children and adolescents with WPW syndrome. Therefore, when EPS and RFA are performed in children and adolescence with WPW syndrome, we recommend that these characteristics be considered.
Objectives : It is well-known that Korean people show distinctive drinking behaviors depending on the gene polymorphisms of alcohol metabolizing enzymes. This study examined the gene polymorphisms of ALDH2 and ADH1B and their combination on the drinking behaviors of Korean young adults. Methods : Through a follow-up survey performed for a cohort consisting of 551 university freshmen for six years, the authors attempted to identify genetic factors affecting drinking behaviors. In 2000, drinking behaviors and scores of CAGE questionnaires were assessed and ALDH2 gene polymorphism was determined with PCR-RFLP. In 2006(n= 150), AUDIT-K was assessed in addition to the above and gene polymorphism of ADH1B was determined through SNaPshot$^{TM}$ method. Results : While ALDH2*2 allele was associated with increased degree of drinking in 2000 and 2006. When both enzymes were active, the possibility to be classified into the risk group for alcohol dependence such as AUDIT-K(>12), and CAGE(>2) was high. Conclusion : The ALDH2 genotype had a significant effect on drinking behavior and degree of drinking during early adulthood. However, the combination of the active form of ADH1B and the active form of ALDH2 can be risk factor for problem drinking.
본 연구에서는 청년패널 자료를 이용하여 주거특성(주거형태 및 주택종류)과 다양한 가구배경 변인들이 청년니트(NEET:Not in Employment, Education or Training) 이행에 미치는 영향을 진단해 보고, 적절한 정책적 방향성을 제시하고자 하였다. 분석 결과, 주거환경이 전월세, 다가구, 빌라, 오피스텔인 청년층은 등 거주환경이 열악한 청년층의 경우 청년 니트로 이행할 가능성이 높게 나타났다. 또한 부모와 동거하고 있는 청년일수록 니트로 이행할 가능성이 매우 높은 것으로 확인되며, 부모로부터 경제적 지원을 받는 청년일수록 니트 상태로 이행할 가능성이 매우 높은 것으로 확인된다. 그러므로, 청년 니트의 특성에 따른 맞춤형 주거 및 일자리 정책 등 종합적인 청년니트 지원 정책이 마련되어야 한다. 특히, 성인이 되었지만, 경제적 독립을 하지 못하고 부모와 동거하고 있는 청년층이거나 부모로부터 경제적 지원을 받는 청년일수록 니트 상태로 이행할 가능성이 매우 높은 것으로 나타나고 있다. 이들 청년층의 니트 이행을 완화하고 방지하기 위해서는 학교-노동시장 이행 과정이 원활하게 이루어지도록 지원해야 하겠다.
