산재환자의 의료재활서비스 활성화를 위한 산재보험시설과 민간시설간의 연계모형 개발 (Development of a Linking Model between Community Resourses and Industrial Injury Insurance Organs for the Activation of Medical Rehabilitation Services for Industrial Injury Patients)

  • Objectives: The purpose of this study is to develop a linking model between industrial injury insurance organs and local organizations providing social welfare health services, for the activation of medical rehabilitation services for industrial injury patients. Method: Research design for this study was a multi-step research through literature review, field research, and group interviews with persons in charge, to compare local society-oriented medical rehabilitation programs. The term of researches Sep.1 ~ Nov. 30, 2004. Results: 1. Home nurses from Workers Accident Medical Corporation have been taken to be adequate to case managers, who link industrial accident insurance institutions to local society services for the activation of medical rehabilitation services for industrial injury patients. They have been chosen for case managers because they have richer understanding and experiences of objects of industrial accidents than any other specialists, and because they have proved to be able to provide direct home services as a specialist. We have established the center for case management affairs within the workers accident general hospital, organized the committee for case management with doctors in charge, doctors in rehabilitation, rehabilitation consultants, social welfare workers, physical therapists, and nurses, determined objects of case managements from those of long-term recuperation, and constructed a course of case management containing from case management plans to evaluation. 2. We have made files of community resources, and organized the council of industrial accident administration to have it in charge of the adjustment and linking of services in case management affairs. 3. Because there are inequality of community resources between areas, differences in experiences in and bases for linking, and disparity of core organizations with active linking in the system of linking between public and civil sectors, we have taken a system of linking between parallel organs to be the core. In our linking model, workers accident general hospital, hospitals designated for industrial injury, and rehabilitation hospitals are linked in parallel, inadequate long-term recuperation managers are trusted to an workers accident medical corporations through examination by the examination committee in Korea Labor Welfare Corporation, and are dealt with by the committee for case management. Of the hospitals designated for industrial accidents, those running a home caring center provide home caring services for the handicapped at home from industrial injury. 4. Workers Accident Medical Corporation take part in medical rehabilitation, and Korea Labor Welfare Corporation in vocational/social rehabilitation. Furthermore, in the model, the latter should construct a system for job opportunities through Internet portals and provide cyber vocational consultation and introduction. Conclusion: Improvement of systems is needed to apply the linking model to practical affairs. Because this model is centered for practical affairs, it should be put under the analysis of effects, and evaluation of its adequacy to practical application, and its effects and efficiency through experimental running in the 8 workers accident general hospital in Korea.

도박중독의 재활: 타 정신장애와의 비교 (The Rehabilitation of Gambling Addiction: Comparison with the other psychiatric disorder)

  • 본 연구에서는 다른 정신장애(정신병적 장애 및 알코올 중독)와 비교하여 도박중독의 재활에 대한 국내외 현황과 실태, 차이점을 살펴보고 도박중독에 있어서의 재활의 필요성과 의미 및 도박중독에 고유한 재활 영역을 제시하고자 하였다. 먼저 국내외에서는 국가 혹은 사행 산업체가 운영하는 상담, 치료 센터에서 일부 주민등록과 의료보험회복지원, 법률 지원, 거주시설 및 직업 재활 등이 제공되고 있지만 도박중독 및 그 가족의 사회 복귀 및 적응에 필요한 재활 지원은 치료에 비해 매우 부족하거나 등한시되고 있는 실정이었다. 또한 타 정신장애와 달리 도박중독은 인지적 기능이나 일생생활 기능의 결함이 없거나 극소한 반면 사회적 역할의 손상이 심각한데 이는 도박의 고유한 본성, 도박중독으로 발전하면서 수반되는 성격적/정서적 변화, 병전의 성격적 결함 등에 기인하는 것으로 시사된다. 도박중독에서는 사회적 역할 손상과 이로 인한 신용 불량, 가족 해체, 사회 빈곤화 등 사회경제적 문제가 심각한 바, 도박중독의 재활에서 중요한 영역들은 사회복귀 서비스 지원, 신용회복 지원, 금전 관리 지원, 가족의 재활 지원, 직업 재활 서비스 등이 중심이 되어야 하며, 이러한 세부적 영역을 중심으로 그 의미와 기능을 논하였다. 마지막으로 본 논의의 시사점과 제한점에 대하여 논하였다.

  • Recently a new regime, called "back school" is advocated for the purpose of rapidly and economically returning a patient to full functional fitness and normal occupational activities. Assuming that this functional and vocational restoration is a goal of a regabilitation program, it is apparent that early recovery and higher success rate are major concern. In this study, a mixture model is developed and used to evaluate the recovery process of low back pain (LBP) patients in a rehabilitation program. The model provides (1) the probability of being recovered, and (2) the rate of recovery. Patients with gradual onset, with higher education, or of young age tend to show better prognosis.

직업적 역할은 정신장애인이 지각한 편견을 감소시키는가? (Effects of vocational roles on the perceived prejudice of people with psychiatric disabilities)

  • 본 연구는 정신장애인이 지각하는 사회의 편견에 영향을 미치는 요인은 무엇이며 이러한 요인의 영향은 어떠한 이론적 근거에서 설명될 수 있는지 규명하는데 목적이 있다. 상징적 상호작용주의이론에 근거하여 직업적 역할, 직업재활서비스욕구, 가족의 편견인식이 정신장애인이 지각하는 편견에 미치는 영향을 검정하였다. 본 연구의 분석에 활용된 자료는 2008년 국가인권위원회의 '전국 정신질환자 및 가족 생활실태조사'자료 중 충실히 응답된 229사례였고, 자료분석은 위계적 회귀분석을 활용하였다. 자료분석결과 정신장애인의 증상이 높을수록 편견인식이 높았고, 직업적 역할이 있으면 편견인식이 유의하게 감소하였다. 직업을 통해 재활을 도모하는 상태에 있으면 편견인식이 유의하게 증가하였고, 가족의 편견인식이 높으면 정신장애인의 편견인식도 높은 것으로 나타났다. 또한 가족의 편견인식은 직업적 역할이 정신장애인의 편견인식을 감소시키는 효과를 상쇄하는 것으로 나타났다. 이러한 연구결과에 근거하여 이론적, 실천적 함의를 논의하였다.

Occupational Performance of Hearing-Impaired and Normal-Hearing Workers in Korea

  • Background and Objectives: This study aimed to investigate the occupational performance of Korean workers with and without hearing loss and analyze the hearing-related difficulties in the working environment. Subjects and Methods: The Amsterdam checklist for hearing and work was used for the analyses and the occupational environments of the Korean workers were investigated. Out of 129 total participants, 86 workers experienced severe to profound hearing loss and 43 had the normal hearing ability. The hearing-impaired workers were recruited from two leading vocational centers and normal-hearing workers were their colleagues. Results: The hearing-impaired workers were found to take fewer sick leaves and exhibited higher rates of permanent job statuses compared to the normal-hearing workers. Workers with hearing loss rarely detected background sound; however, they could perceive reverberation more frequently. They felt more satisfied with their careers than the normal hearing workers as they received social support and needed to put their effort into hearing for most hearing activities. Furthermore, the effort in hearing increased with the increase in job demand, job control, social support, and career satisfaction. The working hours per week increased with the increase in age, education level, job demand, job control, and social support. Different trends were observed in 9 out of 12 variables while comparing the data from the present study with that obtained from the hearing-impaired workers of the Netherlands, indicating a large difference between countries. Conclusions: Although the hearing-impaired Korean workers operate diligently with good job positions, it is necessary to enhance their acoustic environment and provide them social support. Considering the cultural background of the hearing-impaired workers, the development of suitable vocational rehabilitation programs and specific questionnaires is strongly recommended worldwide.

정신장애인 직업재활 평가도구에 대한 고찰 (A Study on Vocational Rehabilitation Evaluation Tool for Mental Disorders)

  • 목적 : 본 연구는 정신장애인의 직업재활 평가도구를 확인하고 분석함으로써 작업치료 영역에서 임상적으로 유용하게 활용할 수 있는 정신장애인 직업재활 평가도구의 개발을 위한 기초자료로 활용하는 것이다. 연구방법 : 문헌 검색은 Pubmed 데이터베이스를 사용하였으며, 분석 내용은 개발연도, 평가방식, 문항 수, 척도, 평가 문항을 분석하였다. 분석 방법은 각 평가 항목을 기능, 내적요인, 환경, 정신증상의 4가지 범주로 구분하여 각 평가도구의 평가 요소에 대해 파악하였다. 결과 : 검색어를 통한 Pubmed 검색 시 161개의 문헌이 검색되었고, 선정 방법에 따라 Griffiths Work Behavior Scale (GWBS), Occupational Functioning Scale (OFS), Social and Occupational Functioning Assessment Scale (SOFAS), Work Ability Index (WAI), Work Behaviour Inventory (WBI), Work Environment Impact Scale (WEIS), Work and Social Adjustment Scale (WSAS)의 7개 평가도구가 선별되었다. 모든 평가도구의 평가항목은 직무와 관련된 기능평가를 포함하였으며, 각 평가도구의 목적에 따라 내적요인, 환경, 정신증상에 대한 측정이 이루어지는 것으로 나타났다. 결론 : 직업 기술은 신체, 인지, 사회기술, 대처능력 등 다양한 기능이 복합적으로 작용하는 기술이다. 따라서 본 연구에서 분석한 기능, 내적요인, 환경, 정신증상의 4가지 요소가 포함될 필요가 있으며, 평가과정에서 발생하는 물리적 제약을 보완하기 위해 ICT 융합 평가도구의 개발함으로써 보다 효율적인 정신장애인 직업재활 평가가 이루어져야 할 것이다.

지적 장애인의 직업능력 향상을 위한 보조공학 활용 연구 분석 : 단일대상 연구를 중심으로 (The Research Analysis About Assistive Technology Application for Vocational Ability Improvement of People with Intellectual Disabilities : Centered Single Subject Design)

  • 본 연구는 지적장애인의 직업능력 향상을 위한 보조공학의 활용에 대한 국외의 단일대상연구 논문을 분석하여 지적장애인의 직업능력 향상을 위한 보조공학의 활용 연구에 대한 기초자료를 제시하는데 목적이 있다. 본 연구는 문헌연구로써, 관련 단일대상연구 논문을 분석하여 연구의 특성, 연구방법의 내용, 연구 설계의 질적 수준을 알아보았다. 분석결과는 다음과 같았다: 첫째, 연구의 특성을 분석한 결과, 실험환경은 분리된 환경에서의 연구가 대부분이었으며 중다간헐 설계가 대부분이었다; 둘째, 연구 방법의 내용을 분석한 결과, 대상자의 선정기준에 대해서는 모든 논문에서 언급하였으나 대상자와 관련한 내용을 기술한 논문의 비율이 낮게 나타났다. 기초선 측정시의 행동통제방법과 외부변수 통제방법에 대한 기술의 빈도가 낮았다; 셋째, 연구 설계의 질적 수준을 분석한 결과, 대부분의 논문에서 독립변수를 측정하고 중재충실도를 제시하고 있으나 사회적 타당도를 제시한 논문의 비율은 낮게 나타났다. 직업재활 분야에서 보조공학의 효과적인 활용을 위해서는 직업재활에 있어 보조공학의 활용이 근거기반의 실제라는 것에 대한 결과의 축적이 필요하며, 이를 위해 앞으로의 연구에서는 연구방법의 내용이나 연구 설계의 질적 수준에 대한 보완이 필요할 것이라 생각된다.

취업한 정신장애인의 직업유지에 영향을 미치는 요인 (The Factors Influence upon Job Maintenance of the Mentally Disabled with Job Experience)

  • Purpose: This study tried to explore factors influencing on job maintenance of the mentally disabled who employed before. The subjects of this study were the mentally disabled who participated in community mental health center and social rehabilitation center, there had experienced a job in the past. Methods: The number of community centers were 27 places, there were located in different region. The number of participants were 221 persons agreed to this study. Data was analyzed by Mean, SD and logistic regression analyzation using SPSS/WIN 14.0. Results: General character such as age was significant factor. Monthly pay and working a day in the midst of job related factors were significant. Moreover, job rehabilitation program satisfaction rate and social support revealed significant factors. Conclusion: On the basis of results, Job rehabilitation programs for job maintenance of the mentally disabled should be development and establish mediate strategies to improve social support to them.
