성인 쿠싱병의 치료성적과 예후인자 (Treatment Outcome and Prognostic Factors of Cushing's Disease in Adults)

  • Objectives : The authors analyzed the surgical series of Cushing's disease to evaluate the proper treatment policy and to verify the possible prognostic factors. Material and Methods : Of 50 patients diagnosed as Cushing's disease and operated at Department of Neurosurgery of our institute between 1988 and 1999, 48 patients with available medical records were analyzed retrospectively. Mean follow-up period was 48 months(3 to 109 months). Preoperative diagnosis was made after evaluating the patients with multiple-stage endocrinological studies and 31 selective patients were evaluated with inferior petrosal sinus sampling(IPSS). Magnetic resonance imaging(MRI) and/or high resolution computerized tomography(CT) was done in all patients. A total of 51 transsphenoidal adenomectomy(TSA) were performed including 3 revision for initial surgical failure cases. Remission was decided on the basis of both endocrinological criteria and clinical status. Radiation and/or ketoconazole therapy were applied to failed cases. For the verification of prognostic factors, the authors evaluated the statistical significance of multiple variables over remission rate by chi-square test. Result : Sensitivity of IPSS for central localization was 93.5% which was better than that of MRI(87.5%). But for lateralization, it was 72.4% for IPSS versus MRI 90.5%. Success rate of TSA was 82%(42/51) and recurrence rate was 9%(4/48). When including adjuvant treatments for surgically failed cases, overall success rate was 89.6% and all of 3 reoperated cases(TSA) due to recurrence were successful. Significant complication occurred in 7.8%(4/51) after TSA including hypopituitarism, diabetes insipidus, and visual loss. Non-existence of tumor in MRI and prolonged symptom duration(>3 years) were significant prognostic factors. Conclusion : TSA can be considered as initial treatment for Cushing's disease. In surgically failed cases, multiple treatment modality may improve the overall outcome and repeated TSA for recurrent cases seem to provide similar success.

통증어휘를 이용한 통증비율척도의 개발연구 (A Study for Development of Ratio Beale Measuring Pain Using Korean Pain Tersm)

  • The main purpose of this study is to develop a ratio scale measuring level of pain using Korean pain terms. The specific purposes of this study are to identify the degree of pain of each pain term in each subclass: to classify each subclass in terms of dimensions of pain; and to analyze factors of the Korean pain ratio scale clustering together. One hundred an4 fifty eight pain terms which were originally identified as representative terms and their synonyms were used for data collection. Fifty eight nursing professors ana sixty one medical doctors who have contacted with patients having pain were asked to rate the weight of each pain term on a visual analogue scale. Subclasses in which ranks of pain terms were same f s findings in two previous studies were 1) thermal 3 am 2) cavity pressure, 3) single stimulating pain, 4) radiation pain. and 5) chemical pain. Subclasses in which ranks of pain terms were confused were 1) incisive pressure, and 2) cold pain. Subclasses in which one new pain term was added were 1) inflammatory-repeated pain, 2) punctuate pressure, 3) constrictive pressure, 4) fatigue-related pressure, and 5) suffering-relate4 pain. Subclasses in which two new pain terms were added were 1) traction pressure, 2) peripheral nerve pain, 3) dull pain, 4) pulsation-related pain, 5) digestion-related pain, 6) tract pain, and 7) punishment-related pain. Subclass in which 3 new pain terms were included was fear-related pain. Rating scores of 5 words in 4 subclasses were significantly different between the normal group and the extreme group of subjects in terms of subjective rating. Only one word among 6 words was that newly added to the scale. Rating scores of 12 words in 9 subclasses were significantly different between doctor group and nursing professor group. Among these 12 words, only 3 were those newly added to the scale. In comparison of these 12 words, mean scores of the nursing professors were always 7 to 16 points higher than those of the medical doctors. In the analysis of judgement of subjects in terms of dimensions of pain terms, subclasses of dull pain, cavity pressure, tract pain and cold pain were suggested to be included in the miscellaneous dimension. As a result of factor analysis of the ratings given to 96 pain words using principal components analysis without iteration and with varimax rotation limiting the number of factors to 4, factors of severe pain (factor I) mild-moderate pain (factor II) , causative pain (factor III) and temperature-related pain(factor IV) were extracted with the factor loading above 0.388. When the pain words were re-arranged on the bases of factor loading above 0.368, number of factors decreased to only first two factors. Maximum score of pain word in factor II was 46.17 and the minimum score of the factor I was 45.36. Further studies are needed to identify the validity, reliability, sensitivity and practicability of this ratio scale using patients having various sources of pain.

Value of FDG PET/Contrast-Enhanced CT in Initial Staging of Colorectal Cancer - Comparison with Contrast-Enhanced CT

  • Background: FDG PET/CT is at an equivocal stage to recommend for staging of colorectal cancer as compared to contrast-enhanced CT (ceCT). This study was intended to evaluate the value of FDG PET/ceCT in colorectal cancer staging as compared to ceCT alone. Materials and Methods: PET/ceCT was performed for 61 colorectal cancer patients who were prospectively enrolled in the study. Three patients were excluded due to loss to follow-up. PET/ceCT findings and ceCT results alone were read separately. The treatment planning was then determined by tumor board consensus. The criteria for T staging were determined by the findings of ceCT. Nodal positive by PET/ceCT imaging was determined by visual analysis of FDG uptake greater than regional background blood pool activity. The diagnostic accuracy of T and N staging was determined only in patients who received surgery without any neoadjuvant treatment. Results: Of 58 patients, there were 40 with colon cancers including sigmoid cancers and 18 with rectal cancers. PET/ceCT in pre-operative staging detected bone metastasis and metastatic inguinal lymph nodes (M1a) that were undepicted on CT in 2 patients (3%), clearly defined 19 equivocal lesions on ceCT in 18 patients (31%) and excluded 6 metastatic lesions diagnosed by ceCT in 6 patients (10%). These resulted in alteration of management plan in 15 out of the 58 cases (26%) i.e. changing from chemotherapy to surgery (4), changing extent of surgery (9) and avoidance of futile surgery (2). Forty four patients underwent surgery within 45 days after PET/CT. The diagnostic accuracy for N staging with PET/ceCT and ceCT alone was 66% and 48% with false positive rates of 24% (6/25) and 76% (19/25) and false negative rates of 47% (9/19) and 21% (4/19), respectively. All of the false negative lymph nodes from PET/ceCT were less than a centimeter in size and located in peri-lesional regions. The diagnostic accuracy for T staging was 82%. The sensitivity of the peri-lesional fat stranding sign in determining T3 stage was 94% and the specificity was 54%. Conclusions: Our study suggested promising roles of PET/ceCT in initial staging of colorectal cancer with better diagnostic accuracy facilitating management planning.

컬러 영상의 압축센싱을 위한 평활 그룹-희소성 기반 반복적 경성 임계 복원 (Smoothed Group-Sparsity Iterative Hard Thresholding Recovery for Compressive Sensing of Color Image)

  • 압축센싱은 성긴(Sparse) 또는 압축가능한(Compressible) 신호에 대해 Nyquist rate 미만의 샘플링으로도 신호 복원이 가능하다는 것을 수학적으로 증명한 새로운 패러다임의 신호 획득 방법이다. 단순한 신호 획득 과정을 이용하면서도, 동시에 우수한 압축센싱 복원 영상을 얻기 위한 많은 연구들이 수행되고 있다. 그러나, 에너지 분포 및 인간 시각 시스템 등 컬러 영상에 대한 기본적인 특성을 복원 과정에 활용한 기존 압축센싱 관련 연구는 많이 부족하다. 이러한 문제를 해결하기 위해, 본 논문에서는 컬러영상의 압축센싱 복원을 위한 평활 그룹-희소성 기반 반복적 경성 임계 알고리즘을 제안한다. 제안하는 방법은 그룹-희소성에 기반한 경성 임계치 적용과 프레임 기반 필터의 사용을 통해 영상의 변환 영역에 대한 희소성을 증대시키는 동시에 화소 영역의 평활 정도를 복원 과정에 활용할 수 있도록 한다. 또한, 그룹-희소화 경성 임계 과정은 자연 영상의 에너지 분포 및 인간 시각시스템 특성에 따라 중요하다고 판단되는 RGB-그룹 계수들을 보전하도록 설계하였다. 실험 결과 객관적 화질 측면에서 제안방법이 대표적인 그룹-희소화 평활 복원 기법 보다 평균 PSNR이 최대 2.7dB 높은 것을 확인하였다.

신축사무실 내 식물 적용 후 재실자 안구 증상 및 실내공기질 평가 (Assessment of Indoor Air Quality and the Eye Symptom of Occupants in Newly-built Office Building after Planting Indoor Plants)

  • 목적: 본 연구에서는 신축사무실 건물 및 재실자를 대상으로 식물적용에 따른 안구건조 증상 및 실내공기질 등에 대한 변화를 평가하고자 하였다. 방법: 세종시 정부청사 신축건물 및 사무실 재실자를 대상으로 하였다. 식물적용에 따라 실내공기질을 측정하였으며, 휘발성유기화합물류와 알데하이드류 및 온 습도를 측정하여 평가하였다. 안구건조 증상은 Ocular surface disease index(OSDI)를 이용하여 정상, 경도, 중등도, 및 중증으로 분류하여 측정하였다. 결과: 식물적용사무실에서 휘발성유기화합물류의 감소율이 다소 높게 나타나 식물적용으로 인한 저감효과가 있는 것으로 조사되었다. 식물적용 재실자에서 새건물증후군 증상 점수가 점차적으로 감소하였고, 식물미적용사무실 재실자에서는 새건물증후군 증상 점수가 상승하였다. 재실자의 안구건조설문결과에서 통계적으로 유의한 차이는 관찰되지 않았다. 결론: 식물적용 사무실 내 유해물질의 감소효과가 있었고, 재실자를 통한 설문조사결과 안구건조 및 새건물증후군 증상에 긍정적인 효과는 있었다. 개인의 민감도 등에 의한 차이 및 현장조사로 인한 연구의 제한점 등을 보완한 장기 연구가 필요하다.

소나무 풍매차대묘의 오존 내성 및 민감성 가계간 생장, SOD 활성 및 MDA 함량 차이 (Difference in Growth, SOD Activity and MDA Content Between Ozone Tolerant and Sensitive Families of Open-Pollinated Pinus densiflora)

  • 국내 주요 경제수종인 소나무의 내성 가계와 민감성 가계간의 오존에 대한 피해반응 및 내성반응을 알아보고자 본 연구를 수행하였다. 내성 가계와 민감성 가계를 각각 5가계씩 선발하여 오존 100 ppb 농도에서 90일간 처리를 하여 근원경 생장, SOD 활성, MDA 함량을 측정하였다. 근원경 생장에서 오존 처리 초기에 내성 가계가 민감성 가계에 비하여 약 3배 높은 생장율을 보였으며, 처리 기간 동안 내성 가계의 생장이 더 우수한 것으로 나타났다. 대조구에 대한 처리구의 SOD 활성비는 민감성 가계의 경우 오존 처리 초기에 증가하고 처리 기간이 길어짐에 따라서 지속적으로 감소한 반면, 내성 가계는 60일까지 증가한 후 감소하였다. 오존 처리구의 MDA 함량은 대조구에 비하여 전체적으로 높게 나타났으며, 민감성 가계가 내성 가계보다 높은 MDA 함량을 나타냈다. 따라서 내성 가계는 민감성 가계에 비하여 항산화 능력이 우수하기 때문에 지질과산화 작용을 억제하는 등 생리적 장애를 극복하는 능력이 높아 생장감소가 적게 나타나는 것으로 판단된다.

구제역바이러스 신속진단을 위한 pan-serotype reverse transcription loop-mediated isothermal amplification (RT-LAMP) 진단법 (Pan-serotype reverse transcription loop-mediated isothermal amplification (RT-LAMP) for the rapid detection of foot-and-mouth disease virus)

  • In this study, we developed a sensitive and specific reverse transcription loop-mediated isothermal amplification (RT-LAMP) assay for rapid visual detection of foot-and-mouth disease virus (FMDV) circulated in Korea. The RT-LAMP was completed in 40 min at $62^{\circ}C$ and the results of the assay were directly detected by naked eye without any detection process. The assay specifically amplified all 7 serotypes of FMDV RNAs but not amplified other viral and cellular nucleic acids. The sensitivity of the RT-LAMP was $10^2$, $10^3$ and $10^3TCID_{50}/mL$ for serotype O, A and Asia 1 FMDV, respectively, which was comparable to conventional reverse transcription polymerase chain reaction (RT-PCR) and relatively lower than that of real time quantitative RT-PCR (qRT-PCR). Clinical evaluation of the RT-LAMP using different serotypes of Korean and foreign FMDV strains showed a 100% (35/35) agreement with the results of the RT-PCR and qRT-PCR. These results indicated that RT-LAMP assay developed in this study could be a valuable diagnostic method for FMDV monitoring and surveillance.

ECG Gated Blood Pool Scan을 이용한 심실벽 역행성 운동의 평가 - Cine Loop, Phase Analysis, Paradox Image의 비교 - (Identification and Assessment of Paradoxical Ventricular Wall Motion Using ECG Gated Blood Pool Scan - Comparison of Cine Loop, Phase Analysis and Paradox Image -)

  • Sixty-four patients with paradoxical ventricular wall motion noticed both in angiocardiography or 2-dimensional echocardiography were assessed by ECG gated blood pool scan (GBPS). Endless cine loop image, phase and amplitude images and paradox image obtained by visual inspection of each cardiac beat or Fourier transformation of acquired raw data were investigated to determine the incremental value of GBPS with these processing methods for identification of paradoxical ventricular wall motion. The results were as follows: 1) Paradoxical wall motions were observed on interventricular septum in 34 cases, left ventricular free wall in 26 and right ventricular wall in 24. Underlying heart diseases were ischemic (23 cases) valvular(9), congenital heart disease (12), cardiomyopathy (5). pericardial effusion(5), post cardiac surgery(3), col pulmonale (2), endocarditis(1) and right ventricular tumor(1). 2) Left ventricular ejection fractions of patients with paradoxical left ventricular wall motion were significantly lower than those with paradoxical septal motion(p<0.005). 3) The sensitivity of each processing methods for detecting paradoxical wall motion was 76.9% by phase analysis, 74.6% by endless cine loop mapping and 68.4% by paradox image manipultion respectively. Paradoxial motions visualized only in phase, paradox or both images were appeared as hypokinesia or akinesia in cine loop image. 4) All events could be identified by at least one of above three processing methods, however only 34 cases (48.4%) showed the paradoxical molies in all of the three images. By these findings, we concluded that simultaneous inspection of all above three processing methods-endless cine loop, phase analysis and paradox image-is necessary for accurate identification and assessment of paradoxical ventricular wall motion when performing GBPS.

Value of Bone Scintigraphy and Single Photon Emission Computed Tomography (SPECT) in Lumbar Facet Disease and Prediction of Short-term Outcome of Ultrasound Guided Medial Branch Block with Bone SPECT

  • Background: Facet joint disease plays a major role in axial low-back pain. Few diagnostic tests and imaging methods for identifying this condition exist. Single photon emission computed tomography (SPECT) is reported that it has a high sensitivity and specificity in diagnosing facet disease. We prospectively evaluated the use of bone scintigraphy with SPECT for the identification of patients with low back pain who would benefit from medial branch block. Methods: SPECT was performed on 33 patients clinically suspected of facet joint disease. After SPECT, an ultrasound guided medial branch block was performed on all patients. On 28 SPECT-positive patients, medial branch block was performed based on the SPECT findings. On 5 negative patients, medial branch block was performed based on clinical findings. For one month, we evaluated the patients using the visual analogue scale (VAS) and Oswestry disability index. SigmaStat and paired t-tests were used to analyze patient data and compare results. Results: Of the 33 patients, the ones who showed more than 50% reduction in VAS score were assigned 'responders'. SPECT positive patients showed a better response to medial branch blocks than negative patients, but no changes in the Oswestry disability index were seen. Conclusions: SPECT is a sensitive tool for the identification of facet joint disease and predicting the response to medial branch block.

웨어러블 가속도 기기 측정에 의한 수면/비수면 동적 분류 (Sleep/Wake Dynamic Classifier based on Wearable Accelerometer Device Measurement)

  • 최근 현대인들의 스트레스 증가로 인하여 수면 장애에 대한 문제가 부각되면서 수면의 질에 대한 관심이 급증하고 있다. 이러한 수면 장애는 정밀한 진단을 위해서 긴 시간과 높은 비용을 요구하는 수면다원검사를 수행하여야 한다. 따라서 실생활에서 낮은 비용으로 손쉽게 수면의 효율을 측정하는 가속도 센서가 내장된 손목 착용형 장비가 활용 가능한 방안으로 부각되고 있다. 하지만 가속도 데이터를 이용한 기존의 수면/비수면 구간 분류 방식은 개별 시점의 가속도 값만을 이용해 수면/비수면 상태를 구분하기 때문에 수면 중 간헐적 움직임과 비수면 중 미세한 움직임이 발생한 구간에서 오분류율이 높은 약점이 있다. 본 논문에서는 분류과정에서 인근 시점간의 SVM 분류기 스코어의 유사성을 고려하는 동적 분류기를 이용함으로써 앞에서 언급한 문제에 효과적으로 대처할 수 있음을 보인다. 제안하는 알고리즘의 실생활 적용가능 여부 판단을 위하여 50개 데이터 세트에 대해 구간 구분을 수행하여 정확도 88.9%, 민감도 88.9%, 특이도 88.5%의 결과를 얻었다.
