Pulmonary and retroperitoneal benign metastasizing leiomyoma

  • Benign metastasizing leiomyoma (BML) is a rare disease, which usually occurs in women with a history of a prior hysterectomy or myomectomy for benign uterine leiomyoma, and has the potential to metastasize to distant sites, such as the lung, lymph nodes, muscular tissue, heart, or retroperitoneum. These lesions are slow-growing, asymptomatic, and usually found incidentally. The prognosis of BML is also excellent. However, there has been debate on the origin and the correct classification of BML, and there are no guidelines for the treatment of BML. We report here on a rare case of BML in both the retroperitoneal cavity and lung in a 48-year-old woman with a history of hysterectomy due to histologically benign uterine leiomyoma. The patient underwent retroperitoneal mass excision and bilateral salpingo-oophorectomy, and then wedge biopsy of two pulmonary nodules was performed additionally 9 days later. Until now, there has been no sign of recurrence and the patient remains asymptomatic. To our knowledge, pulmonary BML is rare and the co-existence of the retroperitoneal metastases after previous hysterectomy is even rarer.

Concurrent primary malignant tumors in mammary gland and uterus with pyometra and inguinal hernia

  • In this case study, we present a rare occurrence of simultaneous primary malignant neoplasms arising from the mammary gland and uterus in conjunction with pyometra and inguinal hernia. The subject, a 16-year-old intact mixed-breed dog, exhibited tubulopapillary carcinoma of the mammary gland with grade II malignancy and uterine leiomyosarcoma. Treatment involved partial mastectomy for the mammary carcinoma and ovariohysterectomy for the uterine leiomyosarcoma. Moreover, the inguinal hernia was concurrently repaired during the tumorectomies. The patient demonstrated positive postoperative progress over a span of two years, emphasizing the effectiveness of surgical intervention in managing the coinciding pathologies in an elderly canine. This unique co-occurrence of primary malignant neoplasms originating from distinct organ systems presents valuable insights into diagnostic approaches, disease characteristics, and surgical interventions in the realm of clinical oncology.

Postoperative Radiotherapy for the Early Stage Carcinoma of the Uterine Cervix (초기 자궁경부암의 수술후 방사선 치료)

  • This is a retrospective cohort analysis of 58 patients who treated with postoperative radiation therapy following radical hysterectomy and bilateral pelvic adenectomy for early stage carcinoma of uterine cervix between January 1988 and December 1990 at department of radiation oncology, Keimyoung University Hospital. Sixteen percent of patients (9/58) had chemotherapy. Most patients were FIGO I b (47 patients), and FIGO I a and II a were one and ten patients, respectively. The median follow-up periods were 48.5 months. The indications for radiation therapy included pelvic lymph node metastasis, large tumor size, deep stromal invasion, lymphovascular invasion, positive surgical margin, endometrial invasion and parametrial invasion. Eighty five percent of the patients had more than one risk factor. The actuarial overall five year survival rate (5 YSR) and five year disease free survival rate (5 YDFSR) were $89.5\%,\;and\;87.8\%,$ respectively. Their overall recurrence rate was $12.1\%,$(758). Distant metastasis was the most common cause of treatment failure $(71.4\%:5/7).$ The univariate analysis of prognostic factors affecting to five year survival rate disclosed pelvic lymph node status (negative: $95.5\%,\;positive:69.2\%,$ p=0.006) and hemoglobin level $(\le11 :75\%,>11g/dl:93.3\%,p=0.05)$ as significant factor. The age status was marginally significant $(\le40:96.0\%,\;>\;40:84.3\%p=0.15).$ Multivariate analysis clarified three independent prognostic factors: pelvic lymph node metastasis (p=p.006), hemoglobin level (p=0.015) and age (p=0.035). Multivariate analysis of prognostic factor affecting to five year disease free survival rate disclosed pelvic lymph node status (p=0.0078) and status of surgical margin (p=0.008). Complications relating to radiotherapy were $10.3\%,(6/58).$ There were no severe major complication requiring surgical intervention or a long hospital stay. It is our opinion that the benefit of postoperative pelvic radiotherapy may be gained in such a high risk patient population with acceptible morbidity.

Effects of Horse Bone Extracts on the Induced Postmenopausal Osteoporosis in Rats (백서의 폐경기 골다공증 모델에서 말뼈 추출물의 효과)

  • Osteoporosis is a metabolic bone disease associated with an imbalance of bone remodeling. Osteoporosis is characterized by decreased bone mass and increased bone fractures. In this study, we investigated the effects of horse bone extracts (HBEs) in vivo. Horse bone was extracted with 80% alcohol (HBE-A) at $100^{\circ}C$ or water (HBE-W) at $120^{\circ}C$. Animal model of postmenopausal osteoporosis was used, in which osteoporosis was induced by ovariectomy of female S.D. rats (female rats were divided into 5 groups), and HBEs were administered to ovariectomized rats every day for 8 weeks. After 8 weeks, the rats were sacrificed and the following osteoporotic factors were measured: body weight, bone mineral density (BMD), uterine/body weight ratio, serum estradiol (E2), and serum alkaline phosphatase (ALP). The results showed that the administration of HBE-W decreased the changes of body weight in ovariectomized rats. HBE-W increased the uterine/body weight ratio and BMD. In addition, HBEs decreased the ALP. Therefore, HBEs may be used for the prevention or treatment of bone disease.

  • Prenatal diagnosis (PND) such as amniocentesis or chorionic villi sampling has been widely used in order to prevent the birth of babies with defects especially in families with single gene disorderor chromosomal abnormalities. Preimplantation genetic diagnosis (PGD) has already become an alternative to traditional PND. Indications for PGD have expanded beyond those practices in PND (chromosomal abnormalities, single gene defects), such as late-onset diseases with genetic predisposition, and HLA typing for stem cell transplantation to affected sibling. After in vitro fertilization, the biopsied blastomere from the embryo is analyzed for single gene defect or chromosomal abnormality. The unaffected embryos are selected for transfer to the uterine cavity. Therefore, PGD has an advantage over PND as it can avoid the risk of pregnancy termination. In this review, PGD will be introduced and application of PGD in inborn error metabolic disorder will be discussed.

  • Adenomyosis is a benign gynecological disease frequently affecting women of reproductive age. It has a negative impact on the quality of life, causing bleeding disorders, dysmenorrhea, chronic pelvic pain, and infertility. However, the molecular mechanisms involved in adenomyosis development remain unclear. This paper summarizes the reports found in the MEDLINE database on the molecular mechanisms involved in the development and progression of uterine adenomyosis. The literature search included the following terms: "adenomyosis," "adenomyoma," "pathogenesis," "molecular mechanisms," and "gynecological disorders." Only peer-reviewed, English-language journal articles were included. This review focuses on the molecular genetics, epigenetic modifications, and pivotal signaling pathways associated with adenomyosis development and progression, which will provide insights into and a better understanding of its underlying pathophysiology.

A Pilot Study of the Difference between Gyejibongnyeong-hwan and Gyejibongnyeong-hwan combined Acupuncture Therapy on the Primary Dysmenorrhea (원발성 생리통에 대한 계지복령환(桂枝茯笭丸) 단독치료와 침(鍼) 치료 병행 차이 연구)

  • Purpose : The purpose of this study is to identify the difference of clinical effects of gyejibongnyeong-hwan and acupuncture therapy on primary dysmenorrhea. Methods : We studied with ten patients recruited from April, 2003 to Feburary, 2004. Women with organic disease as uterine myoma, ovarian cysts and pelvic inflammatory disease were excluded from this study. We treated them with gyejibongnyeong-hwan or gyejibongnyeong-hwan combined acupuncture therapy for 8weeks. The severity of dysmenorrhea was measured by VAS(Visual Analog Scale). Results : gyejibongnyeong-hwan significantly decreased the severity of dysmenorrhea. And Gyejibongnyeong-hwan combined Acupuncture therapy significantly decreased the severity of dysmenorrhea. The change of VAS of gyejibongnyeong-hwan group is significantly higher than gyejibongnyeong-hwan combined acupuncture. Both of Gyejibongnyeong-hwan and acupuncture did not show hepatic and renal virulence. Conclusion : This study shows that gyejibongnyeong-hwan has remarkable effects on dysmenorrhea patients. Obviously further researches concerning all these area still necessary.

Radiotherapy of Recurrent Uterine Cervical Cancer (수술후 재발된 자궁경부암의 방사선 치료)

  • Forty seven patients with locally recurrent uterine cerival cancer after surgery were treated with radiation during the 6 year period from 1979 through 1984 at the Department of Therapeutic Radiology of Seoul National University Hospital. In 30 out of the 47 patients, recurrence was diagnosed within 2 years after surgery. Site of recurrence was vagina in 19 patients, vagina and parametrium in 21 patients and parametrium only in 7 patients. Complete tumor control was achieved in 35 patients $(74.5\%)$; the complete response rates were $94.7\%(18/19)$ in vaginal recurrences, $57.1\%(12/21)$ in combined vaginal and parametrial recurrences and $71.4\%(5/7)$ in parametrial recurrences. Overall and disease free survival rates at 4 years were 55.2 and 50.1 percent, respectively, for entire group. Overall 4 year survival rates were $77.0\%$ for vaginal recurrences, $44.1\%$ for vaginal and parametrial recurrences and $42.9\%$ for parametrial recrrences. When the disease extent was classified in the same way as the staging system of FIGO, the 4 year survival was 80.4, 73.0, 25.0 and 0 percent for stage IIa, IIb, IIIb and IVa, respectively.

  • Objectives: This study was to investigate clinical usage of Guibi-tang based on actual application in obstetrics and gynecology. Methods: This study investigated the number of patients who were prescribed Guibi-tang-gami-bang and what herbal medicines were adjusted according to their chief complaint from January 1st, 2007 to August 31th, 2008. Results: The number of patients who were prescribed Guibi-tang for obstetrics and gynecological diseases at the Department of Obstetrics and Gynecology was 721. Guibi-tang has been used to treat menstrual irregularity, postpartum general weakness, postpartum pantalgia, perimenopausal symptoms and postmenopausal syndrome most frequently. And Guibi-tang has been widely used in obstetric and gynecologic diseases such as general weakness after abortion or gynecological surgery, pelvic pain, dysmenorrhea, uterine myoma, endometriosis, infertility, premenstrual syndrome, urinary incontinence and breast disease. Conclusions: Guibi-tang can be used in obstetric and gynecologic diseases as described in classical oriental medicine literatures, and it is necessary to research clinical usage of Guibi-tang through randomized controlled trial.

Prognostic analysis of uterine cervical cancer treated with postoperative radiotherapy: importance of positive or close parametrial resection margin

  • Purpose: To analyze prognostic factors for locoregional recurrence (LRR), distant metastasis (DM), and overall survival (OS) in cervical cancer patients who underwent radical hysterectomy followed by postoperative radiotherapy (PORT) in a single institute. Materials and Methods: Clinicopathologic data of 135 patients with clinical stage IA2 to IIA2 cervical cancer treated with PORT from 2001 to 2012 were reviewed, retrospectively. Postoperative parametrial resection margin (PRM) and vaginal resection margin (VRM) were investigated separately. The median treatment dosage of external beam radiotherapy (EBRT) to the whole pelvis was 50.4 Gy in 1.8 Gy/fraction. High-dose-rate vaginal brachytherapy after EBRT was given to patients with positive or close VRMs. Concurrent platinum-based chemoradiotherapy (CCRT) was administered to 73 patients with positive resection margin, lymph node (LN) metastasis, or direct extension of parametrium. Kaplan-Meier method and log-rank test were used for analyzing LRR, DM, and OS; Cox regression was applied to analyze prognostic factors. Results: The 5-year disease-free survival was 79% and 5-year OS was 91%. In univariate analysis, positive or close PRM, LN metastasis, direct extension of parametrium, lymphovascular invasion, histology of adenocarcinoma, and chemotherapy were related with more DM and poor OS. In multivariate analysis, PRM and LN metastasis remained independent prognostic factors for OS. Conclusion: PORT after radical hysterectomy in uterine cervical cancer showed excellent OS in this study. Positive or close PRM after radical hysterectomy in uterine cervical cancer correlates with poor prognosis even with CCRT. Therefore, additional treatments to improve local control such as radiation boosting need to be considered.
