분만방법 유형이 골반바닥근육 기능에 미치는 영향 (Effects of Pelvic Floor Muscle Function on Mode of Delivery)

  • Purpose: The purpose of this study was to investigate of the effects of the mode of delivery on pelvic floor muscle function by examining bladder base movement and urinary incontinence. Methods: This study was conducted on 100 females who had delivered in the previous 3-8 months. There were 2 groups: 56 vaginal deliveries and 44 cesarean section deliveries. Prior to the study, a survey was conducted on the subjects' general status, delivery mode, urinary incontinence, and physical activity. The groups could not be blinded due to scar tissue from the operations. The function of the pelvic floor muscle was observed with the bladder base movement using transabdominal ultrasound during pelvic floor contraction. To objectify and measure the subjective symptoms of subjects suffering from urinary incontinence, an incontinence quality of life (I-QoL) test was administered. Statistical analysis of the data was performed using SPSS version 20.0. An independent t-test was used to assess the statistical significance of pelvic floor muscle function between the 2 groups after delivery. Results: The movement of the bladder base in the pelvic floor muscle function was decreased in the vaginal delivery group, but the difference was not significant. Urinary incontinence was significantly increased in the vaginal delivery group. Conclusion: The function of the pelvic floor muscles was better in the cesarean section group and the incidence of urinary incontinence was relatively low in this group. Therefore, vaginal delivery requires more efforts to restore the function of the pelvic floor muscles than cesarean delivery.

방광의 소세포 신경내분비 암종의 요 세포학적 소견 - 1 예 보고 - (Urinary Cytologic Findings of Small Cell Neuroendocrine Carcinoma -A Case Report-)

  • We report the cytologic features of a case of primary small cell carcinoma of the urinary bladder with high grade transitional cell and signet ring cell carcinomatous components. A 64-year-old male presented with gross hematuria for one week. Computed tomography revealed an ill-defined mass in the left lateral wall of the urinary bladder. Urinary cytology showed hypercellularity with predominantly isolated single cells and clustered cells. They have scanty cytoplasm and naked hyperchromatic nuclei with finely granular nuclear chromatin and rare nucleoli. The tumor cells occurred predominantly singe cells, but a few in clusters. Nuclear molding was prominent. No glandular formation or nesting was noted. The second tumor cells had high nuclear/cytoplasmic ratio, irregular nuclear membrane, and coarse granular chromatin. The background was inflamed and necrotic. The histoiogic findings of transurethral resection were mainly composed of small cell carcinoma, and partly transitional cell and signet ring cell carcinomatous components. Small cell neuroendocrine carcinoma have distinctive cytologic features to make a proper diagnosis.

근치적자궁절제술을 받은 자궁경부암 여성의 배뇨장애 빈도, 강도 및 일상생활 불편감 (Frequency, Intensity and Daily Life Distress of Urinary Dysfunction in Women with Cervical Cancer after Radical Hysterectomy)

  • Purpose: This study was done to identify frequency, intensity of urinary dysfunction and daily life distress in women after a radical hysterectomy for cervical cancer. Methods: One hundred and fifty seven women who had undergone a radical hysterectomy and one hundred and sixty five women as healthy controls completed questionnaires on intensity of urinary dysfunction and daily life distress caused by urinary dysfunction. Results: Women with cervical cancer showed higher frequency of urinary dysfunction than healthy controls. Major urinary dysfunction for women with cervical cancer in order of frequency were night-time incontinence (odds ratio=10.39, p<.001), difficulty in starting urination, weak urine stream and sense of incomplete emptying of bladder. The highest score on intensity was difficulty in starting urination, followed by urgency, weak urine stream, daytime frequency and sense of incomplete emptying. Night-time incontinence was the urinary symptom causing the most daily life distress for cervical cancer women followed by difficulty in starting urination, urgency, sense of incomplete emptying, and night-time frequency. Conclusion: Results suggest that nurses should address the potential postoperative urinary complications and develop long term interventions to decrease urinary dysfunction and daily life distress for women who have had a radical hysterectomy for cervical cancer.

  • A retrospective analysis was performed with 30 cases of canine urolithiasis presented in the period between January and December 2001 The miniature schnauzer (6 cases), maltese (5 cases) and Shih Tzu (4 cases) had higher incidences than other breeds. Canine urolithiasis occurred from one year till 12 years, but the most prevalent age was 3 years. Urinary calculi occurred more often in males (63%) than females. The most prevalent anatomic locations of calculi were urinary bladder (8 of 22 cases) and urethra (13 of 22 cases) in males and urinary bladder (10 of 18 cases) in females. The major mineral component of calculi was struvite (72%). In male dogs, out of 15 cases, 10 cases were struvite and 5 cases were calcium oxalate uroliths. And, in female dogs, 8 of 10 cases were struvite. In some cases, mineral composition and urine pH was not matched. In conclusion, in the dogs with predisposing factors for urolithiasis, regular examination or proper diet should be added to prevent the disease. And the mineral composition of calculi should not be predicted solely by the pH or crystal component of urine.

  • Actinomycosis involving the female genital tract has increased since the advent of the intra-uterine contraceptive devices (IUD) : the incidence of actinomyces in IUD users with pelvic inflammatory disease (PID) was reported to be between 17% and 30%. However a definte relationship between actinomyces and PID has not been demonstrated as yet. We present a case of pelvic actinomycotic infection that was initially diagnosed by means of fine needle aspiration. A 57-year-old female presented with urinary frequency and difficulty for 100 days. Computerized tomographic findings showed a mixed heterogenous soft tissue mass in the left superior aspect of urinary bladder. Fine needle aspiration was done under the impression of urinary bladder cancer. Microscopically, smears disclosed many sulfur granules in necrotic background with many neutrophils, histiocytes, multinucleated giant cells and granulation tissue. These findings were confirmed by surgical removal of the mass.

Is Nitric Oxide Involved in Relaxation of Urinary Bladder\ulcorner

  • We investigated whether nitric oxide (NO) may serve a role in bladder function by immunohistochemical analysis of the distribution of intrinsic NADPH-diaphorase and functional study of isometric tension recordings via a photo-induced adequate nitric oxide (PIANO) generating system using rat bladder. Results suggest that a small number of NADPH-diaphorase-positive perikarya are present within the bladder wall and within adjacent small ganglia. Furthermore, NADPH-diaphorase-positive nerve fibers were observed in the adventitial and muscular layers, subjacent to the urothelium and perivascular fibers. Rat bladder strips precontracted with 3$\mu$M carbachol were reversibly relaxed upon NO generation by UV irradiation. PIANO-mediated relaxation was sensitive to oxygen free radicals. In addition, tissue cGMP levels were increased by the PIANO generating system and elevated cGMP levels were decreased by pretreatment of guanylate cyclase inhibitor, methylene blue. These results indicate that NO may serve a role in modulating bladder tone in the rat.

  • Percutaneous/intrathecal chemical neurolysis of sacral nerve with 12% phenol was performed on 13 cases of hyperreflexic bladder to augment bladder capacity and to reduce bladder pressure. Urodynamic evaluations were done before and after chemical neurolysis. Mean bladder capacity increased significantly after chemical neurolysis (from 171.4 ml to 375 ml). No significant changes in bowel or injection sites were noted. The result suggests that the chemolysis of sacral nerve was available modality for hyperreflexic bladder patients, who did not respond to anticholinergic medication, before decide the more aggressive alternatives such as augmentation cystoplasty or urinary diversion.

Significance of Suppressor of Cytokine Signaling-3 Expression in Bladder Urothelial Carcinoma in Relation to Proinflammatory Cytokines and Tumor Histopathological Grading

  • Background: Bladder cancer is among the five most common malignancies worldwide. Altered expression of suppressor of cytokine signaling -3 (SOCS-3) has been implicated in various types of human cancers; however, its role in bladder cancer is not well established. Aim: The present study was undertaken to investigate the mRNA expression of SOCS-3 in normal and cancerous bladder tissue and to explore its correlation with urinary levels of some proinflammatory cytokines, cytokeratin-18 (CK -18) and with tumor histopathological grading, in order to evaluate their role as potential diagnostic markers. Materials and Methods: SOCS3 mRNA expression levels were evaluated using quantitative real time PCR. Urinary levels of interleukins 6 and 8 were estimated by enzyme linked immunosorbent assay (ELISA). Cytokeratin-18 expression was analyzed by immuunohistochemistry then validated by ELISA. Results: SOC3 m RNA expression levels were significantly lower in high grade urothelial carcinoma ($0.36{\pm}0.12$) compared to low grade carcinoma ($1.22{\pm}0.38$) and controls ($4.08{\pm}0.88$), (p<0.001). However, in high grade urothelial carcinoma the urinary levels of IL-6, IL-8, total CK-18($221.33{\pm}22.84pg/ml$, $325.2{\pm}53.6pg/ml$, $466.7{\pm}57.40U/L$ respectively) were significantly higher than their levels in low grade carcinoma ($58.6{\pm}18.6pg/ml$, $58.3{\pm}50.2pg/ml$, $185.5{\pm}60.3U/L$ respectively) and controls ($50.9{\pm}23.0pg/ml$, $7.12{\pm}2.74pg/ml$, $106.7{\pm}47.3U/L$ respectively), (p<0.001). Conclusions: Advanced grade of urothelial bladder carcinoma is significantly associated with lowered mRNA expression of SOC3 as well as elevated urinary levels of proinflammatory cytokines and CK-18. Furthermore, our results suggested that urinary IL-8, IL-6 and CK-18 may benefit as noninvasive biomarkers for early detection as well as histopathological subtyping of urothelial carcinoma.

Risk Factors for Upper and Lower Urinary Tract Cancer Death in a Japanese Population: Findings from the Japan Collaborative Cohort Study for Evaluation of Cancer Risk (JACC Study)

  • Background: The incidence of bladder cancer is lower in Asian than in Western countries. However, the crude incidence and mortality of bladder cancer have recently increased in Japan because of the increased number of senior citizens. We have already reported risk factors for urothelial cancer in a large population-based cohort study in Japan (JACC study). However, we did not evaluate the cancer risk in the upper and lower urinary tract separately in our previous study. Materials and Methods: Here we evaluated the risk of cancer death in the upper and lower urinary tracts, separately, using the database of the JACC study. The analytic cohort included 46,395 males and 64,190 females aged 40 to 79 years old. The Cox proportional hazard model was used to determine hazard ratios and their 95% confidence intervals. Results: Current smoking increased the risk of both upper and lower urinary tract cancer deaths. A history of kidney disease was associated with an increased risk of bladder cancer death, even after controlling for age, sex and smoking status. Conclusions: The present study confirmed that current smoking increases the risk of both upper and lower urinary tract cancer deaths and indicated the possibility that a history of kidney disease may be a risk factor for bladder cancer death in the Japanese population.

도서지역 여성의 요실금 및 하부요로증상에 관한 연구 (A Study of Urinary Incontinence and Lower Urinary Tract Symptoms of Women on an Island)

  • The purpose of this study was to identify the frequency of Urinary Incontinence and lower urinary tract symptoms, to identify life style problems for UI of Women in anIsland. The sample consisted of 88 women in an island, a urinary symptom questionnaire developed by Jackson, et. al. and a demographic questionnaire were used to collect the data. The results of study were as following. 1. It was reported that the subjects have experienced UI and various lower urinary symptoms such as stress incontinence, nocturia, intermittency, abnormal stream, feeling of incomplete emptying and also they have experienced some degree of 'bother' 2. It was reported that the subjects have experienced various life style problems for UI 3. There was significant relationship between age and urgency (p=0.0262), bladder pain (p=0.0268), abnormal strength of stream (p=0.0074), inability to stop midstream (p=0.0026), and ability to perform daily tasks (p=0.0488), 4. There was a significant relationship between the number of discharges and urgency (p=0.0273), abnormal strength of stream (p=0.0307), and inability to stop midstream (p=0.0122). 5. There was a significant relationship between menopause and bladder pain (p=0.0020), abnormal strength of stream (p=0.0245), and inability to stop midstream (p=0.0002). 6. There was a significant relationship between the degree of experiences for lower urinary tract symptoms and life style behavior. Therefore, based on the results, a developing and carrying out of adequate UI care program is needed.
