  • Objective : Rett syndrome is a neurological disorder occurring primarily in girls. By process of treatment for a case which was diagnosed as Rett syndrome and out patient department treatment from the 12th, April, 1999 to November, 2001, the results are as follows. Method & Results : This patient was diagnosed as five-development disorders categories(五遲, 五軟), medicated with Yukmijihwang-tang(Liuweidihwang-tang, 六味地黃湯) and Kyejigayonggolmoryu-tang(Guizhijialonggu-tang, 柱枝加龍骨牡蠣湯). And acupunture therapy was taken on Shinjyungkyuk(shenchongge, 腎正格) and Paekoe(百會 GV 20), Taechu(大椎 GV 14), Renzhong(人中 GV 26). As a result, the patient's hypotonia and intermittent seizures were improved. Conclusion: Rett syndrome is most often misdiagnosed as autism, cerebral palsy or non-specific developmental delay. In oriental medicine, RS can be diagnosed as five development disorders categories (五遲, 五軟) and oriental medication with acupuncture treatment can improve RS patient's conditions.

  • This study in the treatment of autistic horse therapy of children with disabilities that affect the perception of parents is to some extent how well treatment for a sense of purpose and a horse therapy was to evaluate the expectations. As parents of autistic children with disabilities a questionnaire study of 100 people was used as a research tool. Analysis of the frequency analysis method, Chi-square Analysis were analyzed by taking advantage of each. Parents interested in horse therapy and children who do not have the experience of riding. In addition, Parents who have children age 10-20 riding in the response was that there would be effects Understanding mainly ride horses in the horse therapy is physical exercise. And therapy and exercise that combines specific area of expertise, as is the way to help. In addition, behavior modification, and it is used as a way to improve. Expectations in the therapeutic horse therapy should be major(occupational therapy, physical therapy, speech therapy). Horse therapy is suitable for individual therapy at least for 2-3 weeks and the Confederacy.

The Status of Comprehensive Dental Treatment and Type of Disabilities of the Patients Treated under Outpatient General Anesthesia at the Clinic for the Disabled in Seoul National University Dental Hospital (서울대학교치과병원 장애인진료실에서 전신마취 하 치과치료를 받은 환자의 장애 질환 및 협진 현황 조사)

  • Background: The number of patients with disabilities who require general anesthesia for dental treatment is increasing. Methods: We reviewed the cases of 538 patients with disabilities who underwent outpatient general anesthesia for dental treatment at the clinic for the disabled in Seoul national university dental hospital according to the types of disabilities and comprehensive treatments. Results: The patients mean age was 17.4 years and fifty-eight percent of the patients had epilepsy, autism, mental retardation and cerebral palsy. About twenty-five percent of the patients were medically compromised such as brain disease, heart disease and genetic problem. In 142 cases, more than two dentists from the clinical departments systemically co-worked to provide the patients with comprehensive treatment as accurately and quickly as possible under general anesthesia. Conclusions: The number of medically compromised patients was high and the system of comprehensive dental treatment was well run.

Efficacy of a Day-Center Treatment Program for Children with Developmental Disorders (발달장애아동을 대상으로 낮병동 치료 프로그램의 효과)

  • Objectives:This study was conducted in order to evaluate the effectiveness of a day-center treatment program to promote development of children with pervasive development disorder (PDD) and pervasive development disorder/mental retardation (PDD/MR). Methods:Twenty five children (14 in the PDD group and 11 in the PDD/MR group) participated in a day-center treatment program. They had been enrolled in the whole program for 2-3 years. Their performance was evaluated according to the Preschool Language Scale (PRES), Social Maturity Scale (SMS), and Korean version of the Childhood Autism Rating Scale (CARS). They were grouped by diagnosis at the beginning of the program and the treatment effect was compared. Results:Children who participated in the day-center treatment program showed a significant increase in their PRES and SMS scores and a decrease in their CARS scores. Conclusions:A day-center treatment program is effective for development of children with PDD and PDD/MR.

Survey on the Status and Perceptions, Needs of Non-verbal Autism Spectrum Disorders Intervention of Speech-Language Pathologists (무발화 자폐스펙트럼장애 중재에 대한 언어재활사의 현황과 인식, 요구 조사)

  • The purpose of this study is to examine the status and perceptions, needs of speech language pathologists(SLPs) for the intervention of non-verbal autism spectrum disorders(ASD) through a survey. Among SLPs registered in the Korean Association of Speech-Language Pathologists (KSLP), 116 SLPs participated in this survey. The result is as follows. First, 96.6% of SLP reported that they had been referred for non-verbal ASD, and it was found that SELSI was the most used evaluation tool, and communication ability and social interaction were the most frequently used intervention goals. Second, 86.2% of the SLP said that speech therapy had difficulties, and the reason for the burden was the lack of speech therapy methods. Also, the level of knowledge of speech therapy for non-verbal ASD was low in the treatment area, and the level of confidence responded positively only in communication ability and social interaction. Third, education that was considered necessary within the curriculum was education on treatment methods, and it was found that the improvement points of education other than regular courses were increased education such as expert courses and workshops and activation of supervision. From the results of this study, it is expected that the related curriculum will be expanded and improved in the future.

A Comparison Between Music and Non-music Conditions in Reciprocal Attention Intervention for Improving Joint Attention Behaviors of Children With Autism Spectrum Disorder (자폐범주성장애 아동의 공동주의 기술 향상을 위한 단계적 상호 주의 중재효과: 음악 조건과 비음악 조건 비교)

  • This study aimed to examine the effects of a reciprocal attention intervention on the joint attention behaviors of children with autism spectrum disorder (ASD) by comparing the music and nonmusic conditions. An alternating treatment design was applied as one of the single subject designs and the reciprocal attention intervention included music and nonmusic conditions implemented alternately within a session. The participants were four children between the ages 4 and 5 years and each participant who participated in 23 intervention sessions that followed the sequence of baseline (3 sessions), treatment (15 sessions), and follow-up (5 sessions). The music condition consisted of structured joint instrument playing, trials for attentional shift, and interactive instrument playing. The nonmusic condition consisted of joint toy play, trials for attentional shift, and turn taking-based play. The occurrence of target behaviors (i. e., joint attention behavior, eye gaze, and joint action) was analyzed across sessions. At pretest and posttest, the Early Social Communication Scale was administered. All participants showed increasing tendency in all target behaviors, but such occurrence was greater in the music condition than in the nonmusic condition. The findings support the use of a reciprocal attention intervention with musical stimuli to effectively improve joint attention in this population.

SATISFACTION OF HANDICAPPED PATIENT TREATED UNDER OUTPATIENT ANESTHESIA (외래 전신마취를 이용한 치과치료에 대한 만족도 조사 -장애인환자 보호자를 대상으로-)

  • 90 handicapped patients with dental problem treated under outpatient general anesthesia at one day operation room of kyungpook National University Hospital during 2000-2005 were analyzed. Following are the conclusions. 1. For those 90patients treated under general anesthesia, 62(68.9%)patients had autism or mental retardation. 2. The most difficult work for the preparation of general anesthesia was preoperative examination(53.3%). 3. The parent's satisfaction level after treatment under general anesthesia was high(75.6%). 4. 36(40%) of the patients responded to periodic recall check till now.

Effects of Semi-structured DIRFloortime® Therapy Using Board Games on Verbal Comprehension and Processing Speed Index in Children With High Functioning Autism Spectrum Disorders (보드 게임을 활용한 반 구조화된 DIRFloortime® 치료가 고기능 자폐스펙트럼 장애 아동의 언어 이해 및 정보처리 지표 기능향상에 미치는 효과)

  • This study was conducted to explore the effects of semi-structured $DIRFloortime^{(R)}$ treatment on the enhancement of language comprehension and information processing of children with high-performance autistic spectrum disabilities. We measured the general characteristics of the test subjects, which are level of autism, total intelligence, language comprehension and information processing indicators. The intervention method used was a semi-structured $DIRFloortime^{(R)}$ therapy using board game intervention program after revising and supplementing the expert content validity. A pre/post-test for a group was designed as a similar experiment and the pre/post test was initiated with the t certification at .05 of significance level. After initiating the program, the post test has shown that the language comprehension indicators showed statistically significant levels of difference (p<.001) and the information processing indicator also had a statistically significant effect (p<.001). There was a statistically significant difference (p<.001) in the level of verbal comprehension index after the program implementation, and statistically significant differences in the information processing index (p<.001). The semi-structured $DIRFloortime^{(R)}$ treatment using boardgames for enhancing the language comprehension and information processing indicators of children with high performance autistic spectrum disorder had a significant effect.

A SURVEY OF DENTAL TREATMENT UNDER GENERAL ANESTHESIA (조선대학교 치과병원 소아치과에서 시행한 전신마취에 대한 연구)

  • This study describes the characteristics of patient receiving comprehensive dental treatment under general anesthesia at the pediatric dentistry of Chosun University dental hospital in Gwangju, between August 2004 and March 2008. The conclusion runs as follows : 1. Most patients(64.8%) were young (under 10 years of age) and mean age was 8.6 years. 2. The primary reason(31.1%) why they chose general anesthesia was lack of child's cooperation (normal but unmanageable). Following in order are mental retardation, Autism and cerebral palsy etc. 3. Resin restorations were performed most frequently(87.8%) followed by pulp treatment, preformed crown, extraction and sealant. 4. It takes 3 hours 10 minutes for general anesthesia on the average. The mean treatment time under general anesthesia was 2 hours 52 minutes. 5. 29 children(39.7%) visited again after general anesthesia for recall check.

  • Objectives : The purpose of this study was to investigate the effectiveness of a day-hospital treatment program designed to help development of children with pervasive developmental disorder (PDD) and mental retardation (MR). Methods : Retrospective review of the charts of 32 children (28 with PDD, 4 with MR), who participated in a day-hospital treatment program of Seoul Metropolitan Eunpyeong Hospital, from October 2008 to February 2012, was conducted. Development level of each patient was evaluated according to the Psycho-Educational Profile-Revised (PEP-R), Social Maturity Scale (SMS), and Childhood Autism Rating Scale (CARS). Evaluation was done at two points, before participation and after 1 year participation. Results : Children who participated in the day-hospital treatment program showed significant improvement in all categories of PEP-R, SMS, and CARS. Conclusion : Day-hospital treatment program is effective for helping development of children with PDD and MR.
