한방 설진을 위한 컴퓨터 지원 진단 소프트웨어 구현 (Implementation of Computerized Assistant Diagnosis Software for Tongue Diagnosis in the Oriental Medicine)

  • 한방에서 설진은 초기 병증을 진단할 수 있는 방법으로 객관적인 진단 지표의 개발은 중요한 연구 분야 가운데 하나이다. 그러나 이전의 대부분의 설진 시스템은 한의사의 간섭을 배제한 자동 진단을 위한 전문가 시스템으로서 진료를 위한 객관화된 진단 지표의 생성에 있어서 어려움이 있었다. 따라서 본 논문에서는 한방 설진에 있어서 한의사의 진료 행위에 의해서 최적화된 설진을 위한 진단 지표를 계산하는 컴퓨터 지원 진단 소프트웨어를 제안한다. 개발한 소프트웨어는 반자동 방식으로 설질 추출을 위해서는 한의사의 매뉴얼 방식을 이용하고, 설태 영역을 자동 분할하여 비설태와 설태의 색상, WTCI 등에 대한 수치적 진단 지표를 한의사에게 실시간으로 제공한다. 또한, 설태 비율을 이용하여 소태, 박태, 후태의 분류 확률 추정 값과 진료 편의를 위한 전자의무기록 차트 기능도 제공한다. 구현된 컴퓨터 지원 진단 소프트웨어의 유효성 평가를 위해서는 60명의 피실험자를 대상으로 혀 영상을 구축하고, 설진 소프트웨어를 실험한 결과 피실험자의 95%가 소프트웨어 사용-유효성을 나타냈다.

영역 특징 학습을 이용한 혀의 자동 영역 분리 및 한의학적 설진 시스템 (Automatic segmentation of a tongue area and oriental medicine tongue diagnosis system using the learning of the area features)

  • 본 논문에서는 고가의 디지털 설진 장비와 특별한 장치 없이 누구나 손쉽게 사용할 수 있는 디지털 설진 시스템의 첫 단계로 미각 영역별 균열 유무를 판별하는 시스템을 제안한다. 훈련 DB는 한방 병원에서 수집한 사진 261장을 바탕으로 Haar-like feature, Adaboost 학습을 하였다. 학습된 결과를 통하여 입력영상으로부터 혀 후보영역을 검출하고, 검출된 혀 후보영역으로부터 혀 영역만을 분리하기 위하여 261장의 훈련 DB의 HSV 컬러모델의 Hue 성분 평균 값을 산출하였다. 검출된 혀 윤곽으로부터 Connected Component Labeling을 통하여 혀 영역을 분리 하였다. 분리된 혀 영역의 상대적 너비와 높이를 이용하여 미각 영역별 로 분할하였다. 분할된 미각 영역별 영상은 Gray영상으로 변환하고, 각각의 영역별 평균 밝기를 산출하여 이진화하였다. 이진화 영상에 Connected Component Labeling을 통하여 균열 유무를 판별하였다.

사상체질진단표(四象體質診斷表) 및 망진(望診), 설진(舌診)에 관한 연구(硏究) (A Study on the Sasang Constitutional Checkup List and Inspection Diagnosis of Face and Tongue)

  • 1. Objectives and Methods This study was performed to know the relationship between Sasang Constitution and characteristics of ordinary symptoms, inspection diagnosis of face and tongue. There were 666 participants(280 men and 386 women) who answered Sasang Constitutional Checkup list and were taken pictures of faces and tongues. Sasang Constitution was diagnosed by only PSSC(Phonetic System for Sasang Constitution). Distribution of Sasang Constitution and X2 test, kappa value for agreement were calculated using SPSS 10.0. 2. Results From characteristics of ordinary symptoms' point of view, Taeeumin had loud voices, big body shapes and easily sweated themselves. They felt good after sweating and had warm hands and feet. And their upper back, shoulders and chest were, they thought, well developed. They were apt to have meals quickly and overeat themselves. Soyangin and Soeumin had thin body shapes and were difficult to sweat themselves and felt powerless after sweating. They had cold hands and feet. Especially, Soeumin was apt to have meals slowly and a little amounts. They walked so fast. Soyangin was introvert and had developed their abdomen and waist. 'Looking heavy and steady' was higher in Taeeumin, 'delicate and modest' was higher in Soeumin(42.8%) and Soyangin(36.1 %) in first impression. It was thought to be difficult to discriminate Soyangin from Soeumin in first impression and make a notion of right Soyangin's first impression. Soyangin had pinky tongue color and little tongue fur whereas Soeumin had red tongue color and whitish tongue fur. Taeeumin had whitish tongue fur too. 3. Conclusions To diagnose Sasang Constitution much effectively, PSSC results and comprehensive Sasang Constitutional Checkup list extracted from effective Questionnaire items and Face and Tongue Inspection Diagnosis are thought to be needed.

설진과 종양의 관계에 대한 연구 (Study on relation between tongue diagnosis with cancer : a review of literature)

  • Recently, the occurrence and death rates of cancer have increased rapidly. In oriental medicine, the tongue gives some kind of special physiological information on human body. Oriental medical doctors have used information about the color, degree of wetness and shape of the patient's tongue to determine patient's disease and body condition. This fact leads us that the tongue image is one of the most important clinical data for helping doctor's decision making. It also has significant meaning to cancer patients related with traditional theory of oriental medicine. In diagnosis and treatment of cancer, tongue diagnosis can give some prognosis and change o cancer. It also detect the sign of early stage cancer, but it cannot diagnose the kind of cancer and/or benign or malignant. Nowadays BioTechnology(BT) has developed rapidly, but there are a lot of limits(economy, accuracy, clinical significance, etc). Tongue diagnosis is very economic and practical way of diagnose and has a lot of possibility of development. The need for doctors and it also helps the development of tongue diagnosis related with cancer.

체질량지수와 구강건강지표 사이의 상관성에 대한 연구 (A Correlation Study of the Body Mass Index and the Indicators of Oral Health)

  • Objectives: We aim to observe the relation of body mass index (BMI) and the indicators of oral health. Methods: 400 subjects participated in the study. The BMI values are calculated from the height and weight. For the tongue diagnosis, we used the tongue imaging device to analyze the color, tongue coating, and tooth marks. We measured the concentration of hydrogen sulfide (H2S) and methyl mercaptan (CH3SH) to evaluate the halitosis. The dry mouth was evaluated through the measurement of saliva secretion and with the questionnaire asking the frequency of dry mouth. Results: The BMI values were significantly higher in the group with light-white and blue-purple colored tongue, and significantly lower for lightly-coated tongue. However, the correlation of BMI and the amounts of saliva secretion was not significant as well as in the correlation of BMI and the concentration of H2S, CH3SH. In tongue diagnosis, the subjects who had blue-purple colored tongue also had significantly higher H2S and CH3SH, but tendency of lower saliva secretion. Conclusion: We obtain data showing that BMI value and the indicators of oral health including tongue diagnosis have meaningful correlation.

설태의 자외선 형광 반응을 이용한 설태 영역 추출 (Coated Tongue Region Extraction using the Fluorescence Response of the Tongue Coating by Ultraviolet Light Source)

  • 본 논문에서는 한방 의료의 설진에서 진단 지표로 활용될 수 있는 효과적인 설태 영역 추출 방법을 제안한다. 제안한 방법은 설태의 자외선 광원에 의한 형광 반응 특성을 이용하여 기존의 설태 추출 방법의 단점으로 지적되었던 진료 환경의 제약성 및 진료 결과의 객관성 부족에 대한 문제점을 해결할 수 있다. 처리 과정으로는 자외선 광원을 사용하여 설진 영상을 획득하고, 설질(Tongue body)과 설태(Tongue coating) 영역의 색차 크기에 상응하는 히스토그램(Histogram) 상의 골-포인트(Valley-points)를 임계 처리하여 이진화(Binarization)를 수행한다. 최종적으로 설진을 위하여 한의사에게 제공되는 진단 영상은 이진 영상에 케니-에지(Canny-Edge) 알고리즘을 사용하여 설태 윤곽 정보를 추출한 후에 환자의 원 혀 영상에 부과하여 제시한다. 제안한 방법의 성능 평가를 위해서는 다양한 혀 영상을 수집하고, 한의사가 수작업으로 설정한 설태 영역을 참영상(True image)으로 하여 제안한 방법으로 추출한 설태 영역과 비교하였다. 그 결과 제안한 방법은 87.87%의 추출률을 나타냈으며, 추출된 설태 영역의 형태 유사도도 높게 나타났다.

설진 시스템 특허동향 분석 (Analysis of patent trends of computerized tongue diagnosis systems)

  • Objectives Tongue diagnosis is an important diagnostic method in traditional Eastern medicine, and it has a high potential to be used in the future healthcare because of easy, quick, and non-contact measuring features. Recently, research and development efforts on computerized tongue diagnosis systems (CTDS) have been active that led to the technical advancements in the field of photographing techniques, image extraction and classification algorithms. In this study, we analyzed the trends in the CTDS patents. Using the WIPS search engine (www.wipsglobal.com), quantitative and qualitative patent analyses were performed through Korea, China, Japan, U.S.A and Europe. Methods For a systematic search and data analysis, we defined patent categories based on the application area and technical details. By applying thus-obtained categorical key words, we obtained 360 relevant patents on photographing techniques, image extraction and classification algorithms for the purpose of diagnosis or security. Results As a result, companies related to image acquisition, medical imaging and mobile devices and research groups of universities in East Asia were major patent applicants. In all the five countries, the number of patents have been increasing since 1980. In particular, technology related to color correction and image segmentation were most actively patented categories, and expected to continue a high application rate.

Defatting Vestibuloplasty for Functional and Esthetic Reconstruction of Tongue

  • The radial forearm free flap (RFFF) is a thin and pliable tissue with many advantages for tongue reconstruction. However, tongues reconstructed with RFFF occasionally need revision surgery because inadequate defect measurement at primary surgery can lead to bulkiness and limited movement of reconstructed tongue. In this case, the patient underwent partial glossectomy and RFFF reconstruction for treatment of tongue cancer five years prior. We could not make a lower denture for the patient, because the alveolo-lingual sulcus of tongue was almost lost. So we performed vestibuloplasty with a modified Kazanjian method on the lingual vestibule of the mandibular right posterior area, and defatting surgery to debulk the flap. After surgery, we observed that the color and texture of the revised tongue changed to become similar with adjacent tissue. The patient obtained a more functional and esthetic outcome. Accordingly, we present a case report with a review of relevant literature.

수면의 질과 심박변이도, 설진 및 한의학적 문진 결과의 상관성 (Correlations between Sleep Quality and Heart Rate Variability, Tongue Diagnosis, and Korean Medical Questionnaires)

  • Objectives We aimed to characterize the results obtained from Korean medical examinations and questionnaires on the quality of sleep. Methods The quality of sleep was measured using the Pittsburgh sleep quality index in 400 subjects. Afterwards, heart rate variability (HRV) measurements were done, along with the analyses for tongue color, teeth marks, and tongue coating through the tongue diagnosis system. A questionnaire about body's heat, cold and sweating conditions, and the perceived stress scale (PSS) were performed. Finally, correlations between all these indicators and the sleep quality were analyzed. Results As the quality of sleep decreased, the sympathetic nervous system was stimulated. The subjects who had the blue-purple colored tongue experienced decreased quality of sleep. The quality of sleep was also deteriorated in the subjects who usually feel cold and hot easily, sweat a lot, and feel a lot of discomfort. According to the PSS questionnaire, the higher the stress level is, the lower was the quality of sleep. Conclusions We obtained meaningful results by comprehensively analyzing the sleep quality, HRV, tongue diagnosis, heat and cold conditions, sweating, and stress conditions. In particular, the sleep quality had a significant correlation with the rest of the indicators.

디지털 설진 시스템의 색상 보정 (Colour Interpolation of Tongue Image in Digital Tongue Image System Blocking Out External Light)

  • Objectives The aim of this study is to propose an optimized tongue colour interpolation method to achieve accurate tongue image rendering. Methods We selected 60 colour chips in the chips of DIC color guide selector, and then divided randomly the colour chips into two groups. The colour chips of a group (Gr I) were used for finding the optimized colour correction factor of error and those of the other group (Gr II) were used for verifying the correction factor. We measured colour value of the Gr I colour chips with spectrophotometer, and took the colour chips image with a digital tongue image system (DTIS). We adjusted colour correction factor of error to equal the chip colour from each method. Through that process, we obtained the optimized colour correction factor. To verify the correction factor, we measured colour value of the Gr II colour chips with a spectrophotometer, and took the colour chips image with the DTIS in the two types of colour interpolation mode (auto white balance mode and optimized colour correction factor mode). And then we calculated the CIE-$L^*ab$ colour difference (${\Delta}E$) between colour values measured with the spectrophotometer and those from images taken with the DTIS. Results In auto white balance mode, The mean ${\Delta}E$ between colour values measured with the spectrophotometer and those from images taken with the DTIS was 13.95. On the other hand, in optimized colour correction factor mode, The mean ${\Delta}E$ was 9.55. The correction rate was over 30%. Conclusions In case of interpolating colour of images taken with the DTIS, we suggest that procedure to search the optimized colour correction factor of error should be done first.
