Cross-sectional Study of Therapy Type and Pattern Identification in Lumbago Patients (요통 환자의 치료행태 및 변증에 관한 단면조사 연구)

  • Objectives Lumbago is very common symptoms and many lumbago patients visited Korean Medicine clinic to cure lumbago. But It is not studied therapy type and pattern identification in lumbago patients. This study aim to survey therapy type and pattern identification in lumbago patients. Methods We collected data of 755 patients treated to cure lumbago in Korean Medicine Hospital from 1, Jan. to 31, Dec., 2012. The therapy variable are acupuncture, herbal acupuncture, physical therapy, Chuna Tx, herbal medicine, cupping and moxibustion. Demographic variable are age, gender, district, medical insurance code, and etc. Results 1) We collected data of 755 lumbago patients in 5 Korean medical Hospitals. 2) Age, district, insurance code, diseases were statistically significant by pattern identification. 3) Therapy types of lumbago patients were different by pattern identification. Conclusions In lumbago patients, therapy type and herbal medicine were different by Pattern identification. In the future, it is needed prospective and large scale database for pattern identification in lumbago patients.

A Review of Chuna Manual Therapy for Tension Type Headache: Focusing on Pubmed and Korean Literature (긴장성 두통의 추나치료에 대한 임상 고찰: Pubmed와 국내문헌 중심으로)

  • Objectives : The purpose of this study was to review the effectiveness of Chuna manual therapy for the treatment of tension type headache(TTH). Methods : All processes were independently carried out by three investigators. Literature search was performed in 3 databases(pubmed, OASIS, NDSL) from their inception to May 2016. Searched reports was twice excluded for title, abstract and body. And then, data extract and analysis was done before assessing risk of bias by Cochrane Handbook. Results : 11 RCT were included. Generally, Fascia Chuna therapy and Chuna spine & joint manipulation therapy were used for TTH. Except for 1 report, Chuna manual therapy was shown to be effective in treating TTH. In assessing risk of bias, because of the characteristic of intervention, blinding of participants was high risk of bias in most reports. Conclusions : Chuna manual therapy was shown to be effective in treating TTH. In korea, better designed trials with high quality is needed from now on.

Compairison of Effect of Manual Myofascial Release and Self Myofascial Release Technique Using a Foam Roller on Pain Thresholds and Body Schema in Subjects with Chronic Tension-type Headache (도수근막이완기법과 폼롤러를 이용한 자가근막이완기법이 만성 긴장성 두통 환자의 통증문턱값 및 신체 도식에 미치는 효과 비교)

  • Purpose: Headache is a very common disease experienced at least once in daily life, and tension-type headaches have a high and increasing prevalence. Chronic headaches can cause functional damage and huge socioeconomic impacts. This study aimed to compare the effects of myofascial release technique with manual therapy and self-myofascial release technique using a foam roller on the pain threshold and body schema in patients with chronic tension-type headaches. Methods: The study was conducted on 20 patients living in Busan with chronic tension headaches. Myofascial release technique with manual therapy was performed on the suboccipital, sternocleidomastoid, scalene, and upper trapezius muscles. The self -myofascial release technique using a foam roller was applied to the cervical and thoracic muscles. A laterality test was performed using a recognized neck application developed to evaluate body schema ability. A pressure-pain threshold test was performed using an electronic pressure algometer to compare the results before and after the myofascial release technique. Results: After applying myofascial release with manual therapy and a foam roller, the pressure-pain threshold values showed significant changes in both groups (p<.05). As a result of the laterality test, myofascial release with manual therapy and a foam roller were applied to the painful area. The values showed significant changes in both groups (p<.05), but only the group using the foam roller showed a significant difference (p<.05) in painless areas. Conclusion: The myofascial release technique with manual therapy can be the primary treatment technique for pain control in painful areas. The self-muscle release technique using a foam roller can be an effective method when there is no pain or when maintenance is needed after pain control.

  • The period from .1959 to 1976, authors experienced 13 cases of thymoma at the Department of Thoracic and Cardiovascular Surgery in National Medical Center. Ten patients were male, and 3 patients were female.Main complaints were cough 76.9%, dyspnea, 69.2%, chest pain 61.6%, myasthenia gravis 23.1 % and S.V.S 23.1%. All of them located anterior mediastinum. 7 cases were benign and 6 cases were malignant. Lymphocytic type was 5 cases, epithelial type 3 case,spindle cell type 1 case, mixed type 2 cases, cystic type 1 case and seminomatous type 1 case. All cases were surgically approached but in malignant cases, only 1 case was palliatively removed and followed radiation therapy. 4 cases were made only biopsy, among them, obstained good response with radiation therapy in seminomatous type case. In benign cases, excellent result was obtained but 2 cases which were combined myasthenia gravis were relapsed the symptoms.

Approaching Chronic Tension-type Headache through Sasang Constitutional Therapy: A Series of 3 Cases who were classified as the Soeum Type and Taeeum Type (만성 긴장성 두통을 호소하는 소음인(少陰人), 태음인(太陰人) 환자 3례)

  • Objectives We report a series of 3 chronic tension-type headache cases who were constitutionally typed as the Soeum type, Taeeum type and treated with Dangguibaekhaoguanjung-tang, Chungshimyeonja-tang or Chunghyulganggui-tang. Methods In treating 1 patient who was constitutionally typed as the Soeum type, Case 1 fell into the category of the Taeeum symptomatology, 2 patients who were constitutionally typed as the Taeeum type, Case 2 and Case 3 fell into the category of Dry-Heat symptomatology. Each patient was given Sasang medication according to protocol: Dangguibaekhaoguanjung-tang to Case 1, Chungshimyeonja-tang to Case 2, Chunghyulganggui-tang to Case 3. On admission day and approximately 2 weeks later, patients evaluated their pain and quality of life using SF-36 and HIT-6. The patients symptoms were observed daily and NRS scores were recorded daily. Results and Conclusions 2 Patients(Case1, Case2) showed improvement in their NRS scores, SF-36 scores and HIT-6 scores. 1 patient(Case3) showed improvement in NRS scores, but worse SF-36 scores and no improvement in HIT-6 scores. Treating chronic tension-type headache based on Sasang Constitutional therapy was found to be effective for managing pain. The treatment protocol suggested in this investigation should be explored and verified in future studies.

Comparison of the effects of temporomandibular joint and cervical vertebra treatment on pain and functional improvement in persons with tension-type headaches

  • Objective: The purpose of this study was to evaluate the effects of temporomandibular joint and cervical vertebra treatment in persons with tension-type headaches on pain, tenderness, and functional improvement. Design: Three-group pretest-posttest design. Methods: Subjects with tension-type headaches were divided into the temporomandibular joint and cervical vertebra treatment group (n=11), temporomandibular joint treatment group (n=11), and cervical vertebra treatment group (n=11), and pre- and post-evaluation was performed. The temporomandibular joint treatment group underwent compression massage and joint ply of the muscles around the temporomandibular joint. The cervical vertebra group received deep tendon massage and Myofascial Release of the cervical muscles. The temporomandibular joint and cervical vertebra treatment group performed both types of treatment. Treatment was performed for 50 minutes, three times a week for 4 weeks. Measurement tools included the Korean version of the short form-McGill Pain Questionnaire (SF-MPQ, K), Headache Impact test-6 (HIT-6), Neck Disability Index (NDI), and the Digital Algometer FPX25. Results: The groups showed significant differences in SF-MPQ, HIT-6 test, NDI, and Alogometer FPX25 test scores before and after intervention (p<0.05). The differences between the groups were most significant in the group that received treatment of the temporomandibular joint and cervical vertebra (p<0.05). Conclusions: In this study, the treatment of the temporomandibular joint and cervical vertebra was shown to be effective for improving pain, quality of life, and cervical vertebra in persons with tension-type headaches. This data may be helpful in identifying treatment techniques for tension-type headaches in the future.

The Effects of Ankle Plantar flexion exercise and Stepper on the Balance Ability in Normal peoples (족저굴곡 운동과 스텝퍼가 정상인의 균형 능력에 미치는 영향)

  • The purpose of this study was to investigate the effects of plantar flexion exercise and stepper on the balance ability in normal peoples. Twenty subjects participated in this experiment were carried out the program on 6 weeks plantar flexion exercise and stepper. They were divided into two groups; plantar flexion group(plantar flexion exercise group, n=10) and stepper group(using stepper group, n=10). The effects of plantar flexion exercise and using stepper were evaluated by measurements of normal standing(NSEO, NSEC), and semitandom standing and dynamic type 1 and dynamic type 2, respectively. For each case, the experimental data were obtained about static balance in 3 items: mean X speed, mean Y speed and velocity moment, that of dynamic balance in 2 items: mean X speed, mean Y speed. The results of this study were as follows; 1. In plantar flexion exercise group, the statistically significants were shown on X, Y speed and velocity moment in the case of NSEO, NSEC, semitandom standing, and on X, Y speed in the case of dynamic type 1, dynamic type 2. 2. In using stepper group, the statistically significants were shown on X, Y speed and velocity moment in the case of NSEO, NSEC, semitandom and on only X, Y speed in the case of dynimic type 1, dynaimic type 2. 3. There was a statistically significant similar on all about case. however, the statistically difference were not shown with intergroup. The above results revealed that plantar flexion exercise and using stepper was effective for improving the balance ability.

The Effect of Forward Head Posture and Cervical ROM on Chronic and Episodic Tension-Type Headache in University Students (대학생에서의 만성 및 특발성 긴장성 두통이 두부전방자세와 경추가동범위에 미치는 영향)

  • Purpose: This study assessed the differences in the forward head posture (FHP), cervical range of motion, and headache clinical parameters in episodic tension-type headache (ETTH) subjects, chronic tension-type headache (CTTH) subjects, and healthy controls (university students). Methods: Fifteen CTTH subjects, 15 ETTH subjects and 15 controls without headache were examined. Side-view images of each group were taken in both the sitting and standing positions, in order to assess the FHP by measuring the craniovertebral angle. The CROM was used to measure the cervical range of motion. A headache diary was kept for 4 weeks to assess the headache intensity, frequency, and duration. Results: The craniovertebral angle was smaller, ie, there was a greater FHP, in the CTTH and ETTH subjects than in the healthy controls in both the sitting and standing positions (p<0.05). The CTTH and ETTH subjects showed a lower cervical range of motion than the healthy controls in the total range of motion as well as in the half-cycles (p<0.05). Conclusion: The increased FHP and decreased cervical range of motion might be a contributing factor in the initiation of tension-type headache.

The Analysis of Research Trend about Complementary and Alternative Therapy(CAT) in nursing of Korea (국내 보완·대체 치료 간호학 논문의 연구동향 분석)

  • Purpose: This study is to analyze the research trends centering on the theses related to CAT released in nursing of Korea. Method: The researcher collected the academic degrees and theses published on the book of the academic society from 1980 to 2001, and examined 88 domestic dissertations on nursing science of the CAT. Result: 1) By department, the results were: the nursing thesis 88(43.3%), medical field 45(22.1%), psychology counseling education 28(14%). 2) In the type of the CAT in nursing, the results were: acupressure 28(31.5%), Dan-Jeon Breathing 14(15.7%), aroma therapy 5(5.6%) in order. 3) The number of theses increased every year after 1997. 4) As for the type of study design, the results were: the experimental research had the most(47), and the survey research(30). 5) As for the subject, the results were: patients with disease(44), healthy adults(29), literature(9) and medical staff were only 6 theses. 6) In the theme of the theses, the results were: the effect of therapy(76), the experience of therapy(26), adaptation of nursing of therapy(11) and literature review were 7. Conclusion: More research needs to be encouraged in the unsatisfactory fields. Researches should be conducted to extend into the practical fields as Korean nursing mediation, project for the establishment of health policies should be conducted.

Anti-inflammation Effect of Low Intensity Laser Therapy in Collagen-induced Arthritis in Rats

  • Arthritis of the knee is the most common type of joint inflammatory disorder and it is associated with pain and inflammation of the joint capsule. The aim of present study was to investigate the endogenous effect of low intensity laser acupuncture on collagen-induced arthritis in rats. Forty Sprague-Dawley rats were randomly divided into normal group, arthritis group, low laser group with 10 rats in each group. Arthritis in rats was induced by subcutaneous injection of type II collagen combined with complete Freund's adjuvant. Here we investigated the effects of low intensity laser therapy in experimentally induced rat knee arthritis. To evaluate preventive and therapeutic effects of low intensity laser acupuncture on collagen-induced arthritis rats. In collagen induced arthritic rats, there was significant increase in rat paw volume and decrease in body weight increment, whereas low intensity laser therapy groups, showed significant reduction in paw volume and normal gain in body weight. The altered biochemical parameters(blood urea, serum creatinine, total proteins and acute phase proteins) in the arthritic rats were significantly brought back to near normal by the low intensity laser therapy. Therefore, low intensity laser acupuncture may be a useful treatment in the prevention and treatment of collagen-induced arthritis.
