Frequency Analysis of Clinical Prescriptions in the Korean Medicine Hospital, Pusan National University based on Herb Weight Ratio(2) - Focusing on Prescriptions of Herbal Formula Study Textbook - (본초 중량비를 이용한 부산대학교 한방병원의 첩약 사용 빈도 분석(2) - 방제 교재 수록 처방을 중심으로 -)

  • Objectives : This Study aims to extract the actual prescriptions used frequently in the clinical settings and the frequently used prescription list of textbook on herbal formulae by comparing the prescriptions recorded in the textbook on herbal formulae, and to examine the range of the efficacies of the corresponding prescription on the basis of the records of the Electronic Medical Record (EMR). Methods : By making comparison of the herb weight ratios of the prescriptions recorded in the textbook on herbal formulae with those recorded in EMR, the frequency is measured on the basis of the textbook on herbal formulae prescription and the frequency indication is computed for the clinical prescriptions with lower level of differences. Results & Conclusions : On the basis of the details of the clinical prescriptions used at the P Hospital, Yugmijihwang-tang, Samlyeongbaegchul-san, Jugyeobseoggo-tang, Maegmundong-tang, Olyeong-san and Baegho-tang, among the similar prescriptions for which the title prescriptions of the textbook on herbal formulae and the herbal composition coincide by more than 80%, are not included in the list of prescriptions covered under the national health insurance system even though they are frequently used prescriptions.

A Trend Analysis of Case Reports and Case Series on Soeum Person in Journal of Sasang Constitutional Medicine from 2006 to 2015 (사상체질의학회지에 10년간 게재된 소음인 증례연구 논문 동향분석 - 2006년부터 2015년 까지 -)

  • Objectives This study aimed to provide a basis for treatment of soeum person by analysing published case reports and case series in Journal of Sasang Constitutional Medicine.Methods We have reviewed case reports and case series of soeum person in the Journal of Sasang Medicine from 2006 to 2015.Results Of the Twenty-four papers, reporting were reported in twenty-four papers. A total of 13 interventions were described, of which herbal medicine (100%) was most frequently used. Twenty-four papers reported herbal medicine, of which Palmulgunjatang (25%) was most frequently used. A total of 24 Sasang Constitutional Medicine pattern were used in 24 papers, for which soeum person kidney heat affection induced exterior heat disease depression frantic syndrome (25%) was most frequently used.Conclusions A sufficient number of clinical studies are warranted to provide a clinical basis for treatment of soeum person.

  • This study was performed on the basis of clinical consideration about patient who has hepatoma, cholangiocarcinoma and pancreatic cancer. Cancer has gradually increased morbidity and mortality all over the world including Korea. Western medicine has treated with surgical therapy, chemotherapy, radiotherapy and immunotherapy, but has not obtained outstanding results and has a difficulty due to side effects by those therapies. In this study, we recognized that symptom including pain by cancer are rapidly decreased with oriental medicine treatment. Oriental medicine treatment is applied throughly on the basis of the oriental medicine principle and the prescrptions were used Bulwhankumjunggisan(不換金正氣散), Sojukbaekchulsan(消積白朮散) Banhabackchulchunmatang(半夏白朮天麻湯). The communitial effect of those prescriptions are invigorating the spreen and benefiting vital energy(健脾益氣), supporting healthy energy to eliminate evils(扶正祛邪) and those treatments are corresponed with traditional oriental treatment principle. Also we can know this patient has a little improvement in the general test including biochemistry, blood test, urine analysis and etc. in spite of outstanding improvement of patient's symptom on administration of oriental medicine. According to the above results, though we has not got satisfaction in getting rid of cancer completely on administration of oriental medicine, it is helpful in decreasing symptoms including pain by cancer and in improving quality of life of patient and in improving quality of patient life.

  • This paper examines the relation between multidimensional linear interpolation ( MDI ) and regularization networks, and shows that and MDI is a special form of regularization networks. For this purpose we propose a triangular basis function ( TBF ) network. Also we verified the condition when our proposed TBF becomes a well-known radial basis function ( RBF ).

Distinction of Hot-Cold Using Fuzzy Inference (퍼지 추론에 의한 한열 판별)

  • Objectives Recently the fuzzy logic is widely used in the decision making, identification, pattern recognition, optimization in various fields. In this study, we propose the fuzzy logic as the objective method of distinguishing hot and cold, the basis of diagnosis in Korean medicine. Methods We developed fuzzy inference system to distinguish whether the subjects had hot or cold. The cold and hot questionnaire of Korean traditional university textbook, the pulse rate and the DITI value of face used in the system. These three kinds of information were defined as 'fuzzy sets,' and 54 fuzzy rules were established on the basis of clinical practitioners' knowledge. The fuzzy inference was performed by using the Mamdani's method. To evaluate the usefulness of the fuzzy inference system, 200 cases of data measured in the Woosuk university hospital of oriental medicine were used to compare the determining hot, normal, cold results obtained from the experts and from the proposed system. Results As a result, 100 cases of "cold", 54 cases of "normal", and 34 cases of "hot" were matched between the experts and the proposed system. This fuzzy system showed the conformity degree of 94%(${\kappa}=0.853$). Conclusions In this study, we could express the process of distinguishing hot-cold using the fuzzy logic for objectification and quantification of hot-cold identification. This is the first study that introduce a fuzzy logic for distinguish pattern identification. The degree of the heat characteristic of the patients inferred by this system could provide a more objective basis for diagnosing the hot-cold of patients.

A Clinical Study on the Modified RUCAM Application to Liver Injury Induced by Herbal Medicine (한약인성 간손상의 원인산정법으로 도입된 진단 척도(Modified RUCAM scale)에 대한 임상적 고찰)

  • Objectives : The purpose of this study is to confirm severe problem caused by using a modified RUCAM scale than that officially recognized to diagnose liver injury induced by herbal medicine. Materials and Methods : Cases with liver injury induced by herbal medicine received to a related institute from January 2004 to December 2007 were collected and evaluated by the original scale and modified scale. Results : 1. Based on this study, we couldn't clinicaly confirm that the case was not excluded by the relieved first item basis of modified RUCAM scale which has to be excluded by the first item basis of orginal RUCAM scale. 2. By modified 6th item, one score was added to the total score. 3. By adding 8th item, it gaind one point to the whole the total score. Conclusions : In this study, we can clinically confirm that the modified RUCAM application to case with liver injury induced by herbal medicine is a severe problem.

Immunologic Basis of Type 2 Biologics for Severe Asthma

  • Asthma is a chronic airway inflammatory disease characterized by reversible airway obstruction and airway hyperreactivity to various environmental stimuli, leading to recurrent cough, dyspnea, and wheezing episodes. Regarding inflammatory mechanisms, type 2/eosinophilic inflammation along with activated mast cells is the major one; however, diverse mechanisms, including structural cells-derived and non-type 2/neutrophilic inflammations are involved, presenting heterogenous phenotypes. Although most asthmatic patients could be properly controlled by the guided treatment, patients with severe asthma (SA; classified as a treatment-refractory group) suffer from uncontrolled symptoms with frequent asthma exacerbations even on regular anti-inflammatory medications, raising needs for additional controllers, including biologics that target specific molecules found in asthmatic airway, and achieving the precision medicine for asthma. This review summarizes the immunologic basis of airway inflammatory mechanisms and current biologics for SA in order to address unmet needs for future targets.

Traumatic Pseudoaneurysm of the Superficial Temporal Artery diagnosed with 3-Dimensional Computed Tomography Angiography: Two Cases Report (3차원 컴퓨터단층촬영 혈관조영술을 이용하여 진단한 외상성 천측두동맥 가성동맥류의 치험례)

  • Purpose: Superficial temporal artery(STA) aneurysms are very rare and mostly occur as pseudoaneurysms secondary to trauma. Clinical diagnosis of STA pseudoaneurysm is based on a history of trauma or surgery to frontotemporal region, which presents with pulsatile mass. To confirm diagnosis, many imaging strategies can be used such as digital subtraction angiography, sonography, CT and MRI. But, these imaging modalities are invasive or inaccurate or expensive. Thus, we used 3D CT angiography to confirm STA aneurysm and to get accurate information. Methods: We have experienced two cases of pulsatile mass on the temporal area, suspected as STA pseudoaneurysms. On the basis of clinical information, we performed 3D CT angiography to get more accurate information about this pulsatile mass and to confirm diagnosis. On the basis of information from 3D CT angiography, we performed operation. Results: The lesions were diagnosed as pseudoaneurysm of superficial temporal artery by 3D CT angiography, and surgically resected safely without any complication on the basis of information from 3D CT angiography. 3D CT angiography was excellent diagnostic method for detecting STA pseudoaneurysms, and effectively showed many information about pseudoanerysms such as relationship between the aneurysms and surrounding structures, and its size. Conclusion: We could effectively diagnose and treat on the basis of information from 3D CT angiography. We present our cases with a brief review of the literature related to STA traumatic pseudoaneurysms.

Systematic Review of Herbal Medicine Fumigation Treatment for Mycotic Vaginitis (Candida Vaginitis) (진균성 질염의 한약 훈증 치료에 대한 체계적 문헌 고찰)

  • Objectives: The purpose of this study is to identify the effectiveness of fumigation treatment, provide a clinical basis for fumigation therapy, and develop treatment protocols through consideration of treatment methods and herbal medicines. Methods: We searched for randomized controlled clinical trials using fumigation treatment on mycotic vaginitis in 8 electronic databases such as EMBASE, Pubmed, and CNKI. The results of the studies were analyzed and the risk of bias was assessed by using Cochrane risk of bias tool. A meta-analysis was performed to reveal the effectiveness of fumigation compared to control treatments. Results: We finally selected 5 studies among 54 articles according to inclusion criteria and exclusion criteria. In all selected studies, herbal fumigation was more effective than any control treatment and all the studies were statistically significant. Conclusions: The study provides a basis for applying fumigation treatment to patients with mycotic vaginitis and helps developing a treatment protocol of caring patients of mycotic vaginitis. But the limitation of this study is that the number of studies included is small and quantitative synthesis of all results has not been achieved.
