요양보호사의 직무스트레스, 우울과 피로가 수면의 질에 미치는 영향 (Effects of Job Stress, Depression and Fatigue on Sleep Quality of Care Workers in Long-term Care Facilities)

  • Purpose: The purpose of this study was to identify the effects of job stress, depression and fatigue on sleep quality of care workers employed in long-term care facilities. Methods: The participants were 213 care workers who worked in 11 long-term care facilities. The collected data was analyzed by descriptive statistics, t-test, ANOVA, Pearson's correlation coefficient and multiple linear regression with using the SPSS 23.0 program. Results: We found that patient-related job stress was the greatest stressor among care workers' job stresses. A significant difference in sleep quality was observed according to the self-perceived health status of the care workers. The care workers' sleep quality was negatively correlated with job stress (r=-.16, p=.018), depression (r=-.31, p<.001) and fatigue (r=-.29, p<.001). The influencing factors of sleep quality were depression (${\beta}=-0.22$, p=.001) and fatigue (${\beta}=-0.15$, p=.047). Conclusion: It is necessary to improve the health status and sleep quality of care workers by developing and applying health promotion programs to reduce the depression and fatigue of the care worker. This will ultimately improve the quality of service care for the patients.

요양병원 간호사의 간호역량 영향요인 (Influential Factors Associated with Nursing Competency of Nurses in Long-term Care Hospitals)

  • Purpose: This study aimed to investigate influencing factors on nursing competency of nurses in long-term care hospitals. Methods: A descriptive research design was used with a convenience sample of 150 nurses. Data were collected from Feb 15 to Mar 8, 2017 using self-reported questionnaires and analyzed by descriptive statistics, t-test, ANOVA, Pearson's correlation coefficient, and multiple regression analysis using the SPSS/WIN 18.0 program. Results: The mean scores of self-efficacy, organizational citizenship behavior and nursing competency were 3.84 out of 5, 3.77 out of 5 and 3.80 out of 5, respectively. Nursing competency was positively correlated with self-efficacy (r=.63, p<.001) and organizational citizenship behavior (r=.64, p<.001). Factors influencing on nursing competency were self-efficacy (${\beta}=.38$, p<.001), organizational citizenship behavior (${\beta}=.37$, p<.001) and Clinical career(${\beta}=.14$, p=.011), which explained 51% of the variance. Conclusion: Based on the finding of this study, it is necessary to develop education and intervention programs in order to increase the nursing competency for nurses in long-term care hospitals.

Effects of Awareness of Good Death and End-of-Life Care Attitudes on End-of-Life Care Performance in Long-Term Care Hospital Nurses

  • Purpose: The purpose of this study was to determine the effects of awareness of good death and end-of-life care attitudes on end-of-life care performance in long-term care hospital nurses. Methods: This study used a cross-sectional study design. The participants were 147 nurses working at six long-term care hospitals with more than 200 beds in B city, South Korea. Data were collected using self-reported questionnaires, and analyzed with descriptive statistics, the t-test, analysis of variance, Pearson correlation coefficients, and multiple stepwise regression analysis using IBM/SPSS 26.0 for Windows. Results: The participants' awareness of good death, end-of-life care attitudes, and end-of-life care performance were positively correlated. The factors affecting end-of-life care performance were age, education level, awareness of good death, and end-of-life care attitudes; these variables explained 19.0% of end-of-life care performance. Conclusion: In order to improve long term care hospital nurses' end-of-life care performance, continuing education and training should be provided regarding awareness of good death and end-of-life care attitudes.

치과위생사의 성취동기가 근속의사에 미치는 영향 (Effect of dental hygienist's achievement motivation on intention to stay)

  • Objectives: Previous research on turnover has been conducted from the organization's perspective. However, for the long-term retention of dental hygienists, it is necessary to conduct research from an individual's perspective. The purpose of this study is to examine the impact of achievement motivation on the intention to stay of dental hygienists. Methods: This study conducted an online survey targeting dental hygienists working in dental healthcare institutions, using convenience sampling from September, 2023. The analysis included 152 participants with long-term work experience and 121 participants with shorter work experience. Multiple regression analysis was conducted to identify factors influencing retention intentions (PASW Statistics ver. 23.0, p<0.05). Results: Factors influencing intention to stay for long-term dental hygienists included the number of dental hygienists and achievement motivation (responsibility, challenge, task orientation), with a significant impact (adj. R2=0.317). For shorter-term dental hygienists, achievement motivation (challenge, task orientation) demonstrated a significant impact (adj. R2=0.164). Conclusions: It was observed that intrinsic factors, particularly achievement motivation, had a significant impact on intention to stay. Consequently, exploring strategies to satisfy intrinsic motivation, such as achievement motivation, is deemed necessary.

중기예보를 이용한 태양광 일사량 예측 연구 (A study on solar radiation prediction using medium-range weather forecasts)

  • 급속적으로 비중이 증가하고 있는 태양광 에너지는 지속적인 개발 및 투자가 이루어지고 있다. 신재생에너지 정책인 그린뉴딜과 가정용 태양광 패널의 설치가 증가함에 따라 국내 태양광 에너지 보급이 점차 확대되어 그에 맞추어 발전량의 정확한 수요 예측 연구가 활발하게 진행되고 있는 시점이다. 또한, 일사량 예측이 발전량 수요 예측에 가장 영향을 미치는 요소로 작용하고 있다는 점에서 일사량 예측의 중요성을 파악하였다. 덧붙여, 본 연구는 선행 연구들에서 사용되지 않은 중기예보 기상 데이터를 활용하여 일사량 예측을 하고자 하였다는 점에서 가장 큰 차이점을 확인할 수 있다. 본 논문에서는 서울, 인천, 수원, 춘천, 대구, 대전의 총 여섯 지역의 태양광 일사량 예측을 위하여 다중선형회귀모형, KNN, Random Forest 그리고 SVR 모형과 클러스터링 기법인 K-means 기법을 결합한 후, 클러스터별 확률밀도함수를 계산하여 시간별 일사량 예측을 진행하고자 하였다. 중기예보 데이터를 사용하기 전, 모형 예측 결과를 비교하기 위한 지표로서 MAE (mean absolute error)와 RMSE (root mean squared error)를 사용하였다. 데이터는 2017년 3월 1일부터 2022년 2월 28일까지의 시간별 원 관측 데이터를 중기예보 데이터 양식에 맞추어 일별 데이터로 변환하였다. 모형의 예측 성능 비교 결과, Random Forest로 일별 일사량을 예측한 후, K-means 클러스터링으로 기후요인이 유사한 날짜들을 분류한 뒤 클러스터별 일사량의 확률밀도함수를 계산하여 시간별 일사량 예측값을 나타낸 방법이 가장 우수한 성능을 보였다. 또한 이 방법론을 이용하여 중기예보 데이터에 모형 적합 후, 예측 결과를 확인하였을 때, 일자별로 예측 오류가 상승하는 것을 확인할 수 있었다. 이는 중기예보 기상데이터의 예측 오류로 인한 것으로 보인다. 향후 연구에서는 중기예보 데이터에서 활용할 수 있는 기상요인 중, 강수 여부와 같은 외생 변수를 추가하거나 시계열 클러스터링 기법을 적용한 연구가 이루어져야할 것으로 보인다.

장기요양서비스 전.후 가족의 수발 부담감에 영향을 미치는 요인 (Caregiver Burden in Caring for Elders Before and After Long-term Care Service in Korea)

  • Purpose: Purposes of this study were: evaluation of family burden of caring for elders who receive long term care services, and examination of differences in burden before and after the introduction of long term care service in Korea. Methods: Data were collected by questionnaires from 416 caregivers of elders who were registered with the Long Term Care Insurance Corporation in six cities. Data were collected in September, 2010 and analyzed using descriptive statistics, paired t-test, and ANOVA with the Scheffe test, and stepwise multiple regression. Results: Family burden decreased significantly after longterm care service was initiated. Subjective burden decreased from 2.93 to 2.69 (t=11.78, $p$<.001), and objective burden, from 3.40 to 3.10 (t=12.73, $p$<.001). Stepwise multiple regression analysis revealed that factors affecting subjective burden were family relations (F=13.60, $p$=.003), age (F=5.47, $p$=.019), job (F=6.98, $p$=.008), and education (F=4.59, $p$=.032), and that factors affecting objective burden were living together (F=17.66, $p$<.001), job (F=13.34, $p$=.003), monthly income (F=6.61, $p$=.010), and type of service (F=6.62, $p$=.010). Conclusion: The results of this first study to investigate caregiver burden after the Korean Long-term Care Insurance System was begun provide positive information for the development of strategies to decrease family burden in long term care.

노인장기요양 인정조사원의 의사소통능력, 공감만족 및 공감피로가 소진에 미치는 영향 (The Effects of Communication Skills, Compassion Satisfaction, Compassion Fatigue on Burnout among Staff of Long-term Care Insurance for the Elderly in National Health Insurance Services in Korea)

  • Purpose: This study was a descriptive research to identify the effects of communication skills, compassion satisfaction, compassion fatigue on burnout among staff of long term care insurance for the elderly in National Health Insurance Services in Korea. Methods: A descriptive cross-sectional design was used. The participants were 191 staffs of long-term care insurance in National Health Insurance Services. Data were collected via mail from the 24 branch offices which were randomly selected among the total of 226 centers of National Health Insurance Corporation. Stamm's professional quality of life (ProQOL) and Communication Skills Test tool were included in questionnaire to detect. SPSS/WIN 20.0 was used to conduct the descriptive statistics, t-test, ANOVA, correlation analysis, and multiple regression. Results: Compassion fatigue had a significant positive effect on burnout, while Compassion satisfaction had a negative effect on burnout. The explained variance for burnout was 69% and compassion fatigue was the most significant factor in burnout among staff of long term care insurance services. Conclusion: The results indicate that the factors influencing staffs' burnout are compassion fatigue, compassion satisfaction, and education level. Therefore, strategies to decrease compassion fatigue and improve compassion satisfaction are needed to decrease burnout for staffs of long-term care insurance in National Health Insurance Services.

인자점수와 자기조직화지도를 이용한 희소한 문서데이터의 군집화 (Sparse Document Data Clustering Using Factor Score and Self Organizing Maps)

  • 통계학과 기계학습의 다양한 기법을 이용하여 문서집합을 군집화하기 위해서는 우선 군집화분석에 적합한 데이터구조로 대상 문서집합을 변환해야 한다. 문서군집화를 위한 대표적인 구조가 문서-단어행렬이다. 각 문서에서 발생한 특정단어의 빈도값을 갖는 문서-단어행렬은 상당부분의 빈도값이 0인 희소성문제를 갖는다. 이 문제는 문서군집화의 성능에 직접적인 영향을 주어 군집화결과의 성능감소를 초래한다. 본 논문에서는 문서-단어행렬의 희소성문제를 해결하기 위하여 인자분석을 통한 인자점수를 이용하였다. 즉, 문서-단어행렬을 문서-인자점수행렬로 바꾸어 문서군집화의 입력데이터로 사용하였다. 대표적인 문서군집화 알고리즘인 자기조직화지도에 적용하여 문서-단어행렬과 문서-인자점수행렬에 대한 문서군집화의 결과들을 비교하였다.

초고령화 사회를 대비한 노인장기요양보험제도에 관한 연구 (A Study on the Long-Term Care Insurance System prepare for the Super-Aged Society)

  • 우리나라는 경제발전, 출산율 저하, 보건의료의 발달로 65세 이상 노인 인구의 비율이 2018년 14% 고령사회, 2026년 20% 초고령사회로 예상되어 빠른 인구구조의 변화를 겪고 있다. 고령화에 따라 치매, 중풍 등 일상생활이 어려운 노인들의 수는 날로 증가하고, 핵가족화, 여성의 사회참여 증가 등으로 장기요양이 필요한 노인들을 가정에서 돌보는 것이 어려워졌다. 이에 정부는 이러한 사회적 구조 및 노인문제를 해결하기 위해 장기요양대상 노인의 삶의 질 향상과 가족부양 부담의 완화 등을 목적으로 노인장기요양보험법을 제정하여 시행하고 있다. 따라서 본 연구는 시설의 민영화와 난립으로 서비스의 공적 책임과 시설의 질 제고에 대한 요청이 확산되고 있는 부분을 살펴보고자 한다. 이 밖에도 저임금 요양인력의 양산, 재원조달 방식에 관한 문제 등의 대비가 다각도로 부족한 부분을 도출하고자 하였다. 본 연구를 통하여 노인장기요양보험제도가 초고령화 사회를 대비하여 장기 지속적으로 유지 발전하기 위해서는 법 제도적 측면의 개선방안의 모색과 더불어 노인복지서비스가 신체적 건강 뿐만 아니라 노후 생활의 안정을 위한 정신적 심리적 조치를 강구하는 방안을 검토하고자 한다.

장기요양시설 요양보호사를 위한 통합적 감염관리 교육프로그램의 효과 (Impact of an Integrated Infection Control Education Program on Caregivers in Long-Term Care Facilities)

  • Purpose: This study aimed to develop and assess the impact of an integrated infection control education program on the awareness, attitudes, and performance of infection control among caregivers in long-term care facilities. Methods: Participants were recruited from two long-term care facilities with 25 caregivers in both the experimental group and the control group. This study used non-equivalent control group quasi-experimental pre-post design. The effectiveness of a developed Integrated Infection Control Education Program was evaluated based on infection control awareness, attitudes, and performance. Data were analyzed using SPSS/WIN 28.0 through descriptive statistics, chi-square tests, Fisher's exact tests, and independent t-tests. Results: There was a statistically significant difference in the degree of awareness (t=-5.00, p<.001), attitude (t=-4.91, p<.001), and performance (t=-6.66, p<.001) of infection control between the two groups. Conclusion: Given these results, the integrated infection control education program significantly improved infection control awareness, attitudes, and performance among caregivers in long-term care facilities. This study is noteworthy because it provided comprehensive education on infection control practices to caregivers in environments that are particularly susceptible to infections, especially following COVID-19. This educational program is actively utilized and validated in practice, it would enhance the infection control performance of caregivers, thereby reducing infection rates within facilities, shortening the length of stay for elderly residents in long term care facilities, and contributing to the reduction of healthcare costs.
