CT를 이용한 지방측정에 ADMIRE 알고리즘이 미치는 영향 (Effects of ADMIRE Algorithms on Fat Measurements Using Computed Tomography (CT))

  • 본 연구는 CT를 이용한 지방측정(Fat measurement)에 반복 재구성법(IR)이 어떠한 영향을 미치는지 알아보기 위하여 기존의 필터보정 역투영법(FBP)과 비교하여 고급 모델링 반복 재구성법(ADMIRE)은 어떠한 차이가 있는지 알아보고자 한다. CT Fat measurement에 ADMIRE가 미치는 영향에 대해 알아본 결과 기존의 Filtered back projection과 비교하여 Visceral fat area, Subcutaneous fat area, V/S Ratio, Visceral fat area의 HU 그리고 Subcutaneous fat area의 HU는 통계적으로 유의한 차이가 없었다. 하지만 ADMIRE Strength가 커질수록 Visceral fat area의 Noise는 최대 12%, Subcutaneous fat area의 Noise는 최대 19% 감소하였다. 그리고 CT를 이용한 Fat measurement에서 내장지방과 피하지방 측정에 ADMIRE 알고리즘은 아무런 영향을 주지 않았다.

폐경전 한국인 비만여성에서 복부 지방의 분획별 특성에 대한 임상연구 (The Research about Distribution of Abdominal Fat in Obese Premenopausal Korean Women)

  • Objectives This study was performed to find out the characters about distribution of abdominal fat(especially superficial and deep subcutaneous fat) in obese premenopausal Korean women. Methods 39 obese premenopausal women were recruited in 2008. Anthropometry and body impedance analysis have been done and abdominal fat distribution had been assessed by computed tomography scan at the level of L4-5. Blood test and questionnaires about depression, eating attitude and physical activity were underwent. Result Abdominal total fat area, subcutaneous fat area including superficial and deep were significantly correlated with anthropometry and BIA result while visceral fat was correlated only with age and waist circumference. In blood profile, only visceral fat area was correlated with HDL cholesterol and triglyceride. And there were no correlation among questionnaires and abdominal fat. There were no difference between superficial and deep subcutaneous fat. Conclusion Abdominal subcutaneous fat including superficial and deep did not have any correlation with heart risk factor. superficial and deep subcutaneous fat had no differences with each other and they did not show any correlation with visceral fat in obese perimenopausal Korean women.

CT를 이용한 복부 지방측정에서 측정 위치에 대한 고찰 (무증상 한국인을 대상으로) (A Study on the Location of Measurements in the Abdominal Fat using CT (In asymptomatic Korean individual))

  • 본 연구는 비 조영 복부 CT 검사를 한 무증상 한국인을 대상으로 L1-S1 inter vertebral disc level에 따른 전체지방 면적, 피하지방 면적, 내장지방 면적 간의 차이를 비교 분석하여 최대지방면적(㎠)을 가지는 위치를 알아보았다. 전체지방 면적의 최대지방비를 보이는 위치는 남자 L2-3, L3-4, L4-5, 여자 L4-5, L5-S1에서 측정한 전체지방 면적이다. 피하지방 면적의 최대지방비를 보이는 위치는 남자 L4-5에서 측정한 피하지방 면적이며, 여자는 L4-5, L5-S1에서 측정한 피하지방 면적이다. 내장지방 면적의 최대지방비를 보이는 위치는 남자 L1-2, L2-3에서 측정한 내장지방 면적이며, 여자는 측정 위치에 따른 내장지방 면적의 차이는 없었다. L4-5 inter vertebral disc level에서 복부 지방측정을 하는 것이 일반적이지만, 넓게는 국적, 인종, 성별 등에 따라 좁게는 연구 대상자에 따라 지방 분포 특성이 다르다. 그러므로 연구목적에 따라 어떤 위치를 선택할 것인지 연구 자료를 통해 분석하고, 선택한 위치가 적절한 이유에 대해 언급할 필요가 있다고 생각된다.

복부비만 치료 후 피하 및 내장지방의 변화 (Changes in of Abdominal Subcutaneous and Visceral Fatfollowingafter Abdominal Obesity Treatment)

  • Objectives : This study was performed to assess the effect onf abdominal obesity treatment on changes in abdominal subcutaneous and visceral fat. Methods : The study was conducted on 61 abdominally obese patients (13 men and 48 women). Measures of body weight, waist circumference, abdominal subcutaneous and visceral fat area by CT scan, and V/S ratio (Viscero-subcutaneous fat ratio) were acquired before and after Kirindiet therapy. Paired t-test and Wilcoxon signed rank tests were used to est the effects of teatment. Results : Following a mean of 68 days of treatment, waist circumference (-15%), abdominal total fat (-40%), subcutaneous fat (-37.9%), visceral fat (-47.8%) and V/S ratio (-11.1%) were significantly reduced (p<0.05). The change in V/S ratio in female patients was not statistically significant (p=0.491) whereas the change in the V/S ratio in male patients was significant indicating a greater loss of visceral fat (p=0.017). Please check that my changes reflect what the study found Conclusions : The reduction in visceral fat was greater than for subcutaneous fat in male patients but not for female patients.

  • Visceral fat accumulation is known to be an evident clinical index for the insulin resistance related with obesity. Patients with excessive accumulation of visceral fat frequently suffered from metabolic disorder, such as hyperlipidemia, hypertension, and glucose intolerance. However, molecular mechanism for the pathogenesis of obesity-accompanied metabolic disorders has not been fully elucidated. It has been clarified that adipocytes in visceral fat area have different functions from subcutaneous fat area, and these differences might contribute the pathological significance of excessive accumulation of visceral fat for the accompanied insulin resistance and hyperinsulinemia.

복부지방면적과 비만지표, 대사증후군 구성요소와의 상관성 연구 (The Study on Correlation between Abdominal Fat Area and Obesity Index, Metabolic Syndrome Components in Obese Adult Women)

  • Objectives The waist circumference has been known as an obesity index reflecting the visceral fat. This study was conducted to investigate the obesity index of what can be predicted visceral fat obesity. Methods 121 test subjects who have $BMI{\geq}30$ or BMI 27~29.9 with having more than one high blood pressure, diabetes, and hyperlipidemia were gathered in the Gachon University Korean medical hospital. The relationship between obesity index, the abdominal fat area, and the metabolic syndrome component analyzed using Pearson correlation analysis. The obesity indices, that are largely used in clinics such as waist circumference (WC), HC (hip circumference), body mass index (BMI), waist-hip ratio (WHR) and waist-height ratio (WHtR). Total fat area (TFA), visceral fat area (VFA), subcutaneous area (SFA) and visceral fat/subcutaneous fat ratio (VSR) that was measured from CT. Results 1. Total fat area (TFA) was positively correlated WC, BMI, HC. 2. The subcutaneous fat area (SFA) was positively correlated HC, WC, BMI. 3. The visceral fat area (VFA) did not show a strong correlation with obesity indicators. Conclusions It was found indices of obesity and visceral fat area is a high correlation. BMI, WC was the obesity index showed a high correlation as the SFA, TFA.

기능적 전기 자극과 유산소 운동이 복부비만의 피하지방과 내장지방에 미치는 효과 (The Change of The Effect on The Subcutaneous Fat Area and Visceral Fat Area by The Functional Electrical Stimulation and Aerobic Exercise)

  • Back ground : Subcutaneous fat area is the main factor involved in replacement disease and arteriosclerosis. Simple weight control is the appropriate medical treatment. It's understood that weight reduction does not only reduce the fat concentrations in blood but also reduces blood pressure, improves glucose levels in diabetes patients and reduces incidents of heart disease. there are several methods for reducing fat in the abdominal region but their effectiveness is not folly understood. one method is electrical stimulation of the problem areas. Method : From May 1st 2002 to October 31st. The 15 subjects who received medical examination were aged between 25 and 53 and were of mixed gender. The subjects were divided into two groups one to received functional electrical stimulation and the other a control group. Using Broca's criterion for judging fat grades. I analysed the differences between the two groups before and after the treatment. Subjects received functional electrical stimulation on the abdominal muscle intensity 50Hz. They received this treatment 4 days a week for 40 minutes a day. In the case of aerobic exercise, at the Treadmill, we used it with the intensity of $75\%$ maximum heart rate (220-age). Result 1)After functional electrical stimulation in the case of male subjects, the weight was reduced 1.93kg, obesity $2.60\%$, fat mass 2.73kg, Percent body fat $4.40\%$, waist circumference 6.53cm, circumference of hips 5.53cm. On the other side, the quality of muscle was increased at the rate of 1.03kg, but it's not attentional level. The subcutaneous fat area was reduced by $26.63cm^2$, the visceral fat area was reduced by $43.00cm^2$, In the female subjects, we can see the reduction of fat grade by $26.63cm^2$, the quantity of body fat by 1.5kg, percent body fat by $1.77\%$, circumference of waist by 4.02cm, circumference of hips by 3.67cm, weight by 1.40kg but was increased 0.72kg at the quantity of muscles. We can see the reduction also in the subcutaneous fat area $24.03cm^2$, the visceral fat area by $25.36cm^2$. 2)After aerobic exercise, on the male subjects, we can see reduction of weight by 3.36kg, obesity by $4.00\%$, fat mass by 2.83kg and we can see increase at the soft lean mass by 2.96kg, but we can see reduction, the percent body fat by $3.03\%$, fat distribution by $0.023\%$, circumference of waist by 3.10cm, circumference of hips by 2.23cm. The female subjects show a reduction in the weight by 2.48kg, percent body fat by $2.20\%$, show an increase in the soft lean mass by 1.54kg. We can see a reduction in the quantity of fat mass by 2.32kg, the percent body fat by $2.80\%$, the circumference of waist by 2.16cm, the circumference of hips by 2.68cm, the fat distribution by $0.016\%$, the subcutaneous fat area by $15.25cm^2$ the visceral fat area by $11.52cm^2$. After aerobic exercise, we can't see the attentional change at the total cholesterol, triglyceride, high density lipoprotein cholesterol, low density lipoprotein cholesterol. 3)After the application of functional electrical stimulation and aerobic exercise, in result of measurement on the body ingredient, we could see the weight reduction and increase the quantity of muscle with the male group who exercised aerobic. We can see the attentional rate on the electrical stimulation about abdominal fat rate, circumference of waist, circumference of hips. The other hand, I couldn't see the attentional differences between the two groups in the rate of fatness and quantity of body fat and the rate of body fat. There isn't any attentional difference in the area of fat under skin, on the contrary, There is attentional difference in the fat in the internal organs area at the electrical stimulation site. We can't see the attentional change of total cholesterol, triglyceride, high density lipoprotein cholesterol, low density lipoprotein cholesterol between electrical stimulation and aerobic exercise. 4)After execution of functional electrical stimulation and aerobic exercise, in result of measurement on change of body ingredient among female objects, We could see weight reduction, increase at muscle quantity in the aerobic exercise group. We could see the attentional differences in the rate of fatness, the rate of abdominal region, the circumference which received electrical stimulation. But, we couldn't see the attentional differences between two groups in the quantity of body fatness, the circumference of hips. The subcutaneous fat area doesn't show the attentional differences. On the Contrary, we could see lots of differences in the visceral fat area of the electrical stimulation group. Conclusion The results show that functional electrical stimulation and aerobic exercise have insignificant differences when if comes to total cholesterol, triglyceride, high density lipoprotein cholesterol, low density lipoprotein cholesterol. Though there is affirmative change in body ingredient after both electrical stimulation and aerobic exercise. Functional electrical stimulation is more effective on the subcutaneous fat area and in changing visceral fat area. There fore. It is concluded that the physical therapy is more effective in the treatment of abdominal fatness.

Comparison of Computed Tomography-based Abdominal Adiposity Indexes as Predictors of Non-alcoholic Fatty Liver Disease Among Middle-aged Korean Men and Women

  • Objectives: We compared the associations of 3 computed tomography (CT)-based abdominal adiposity indexes with non-alcoholic fatty liver disease (NAFLD) among middle-aged Korean men and women. Methods: The participants were 1366 men and 2480 women community-dwellers aged 30-64 years. Three abdominal adiposity indexes-visceral fat area (VFA), subcutaneous fat area (SFA), and visceral-to-subcutaneous fat ratio (VSR)-were calculated from abdominal CT scans. NAFLD was determined by calculating the Liver Fat Score from comorbidities and blood tests. An NAFLD prediction model that included waist circumference (WC) as a measure of abdominal adiposity was designated as the base model, to which VFA, SFA, and VSR were added in turn. The area under the receiver operating characteristic curve (AUC), integrated discrimination improvement (IDI), and net reclassification improvement (NRI) were calculated to quantify the additional predictive value of VFA, SFA, and VSR relative to WC. Results: VFA and VSR were positively associated with NAFLD in both genders. SFA was not significantly associated with NAFLD in men, but it was negatively associated in women. When VFA, SFA, and VSR were added to the WC-based NAFLD prediction model, the AUC improved by 0.013 (p<0.001), 0.001 (p=0.434), and 0.009 (p=0.007) in men and by 0.044 (p<0.001), 0.017 (p<0.001), and 0.046 (p<0.001) in women, respectively. The IDI and NRI were increased the most by VFA in men and VSR in women. Conclusions: Using CT-based abdominal adiposity indexes in addition to WC may improve the detection of NAFLD. The best predictive indicators were VFA in men and VSR in women.

피하지방층과 등뼈 영역을 이용한 초음파 영상에서의 근육 영역 추출 (Extraction of Muscle Areas form Ultrasonographic Images using Subcutaneous Fat Areas and Thoracic Vertebra)

  • 본 논문에서는 요부 영상에서 근육을 추출하는 방법을 제안한다. 제안된 방법은 요부 초음파 영상에서 피하지방층과 등뼈 영역을 각각 분류하여 피하지방층의 하단 부분과 등뼈의 상단 부분의 경계선을 개선된 4방향 윤곽선 추적 알고리즘을 적용하여 추출하고 이 두 경계선 내에 있는 영역을 요부의 근육 영역으로 추출한다. 본 연구에서 제안한 방법을 392개의 요부 초음파 영상에 적용하여 근육 영역을 추출한 결과, 제안된 방법은 94% 이상의 추출률을 보였다.

Correlation between Abdominal Fat Distribution and Abdominal Temperature in Korean Premenopausal Obese Women

  • Objectives: Considering that homeothermy is a major component of metabolic rate, body temperature might play a role in the pathophysiology of obesity. This study aimed to determine the relationship between abdominal fat distribution and abdominal temperature in Korean, premenopausal, obese women. Methods: Weight and height were measured in 26 premenopausal, obese women to calculate body mass index (BMI). Obesity was defined as a $BMI{\geq}25kg/m^2$. Waist circumference (WC) was also measured as well as abdominal fat by computed tomography (CT) and abdominal temperature by digital infrared thermographic imaging (DITI). Results: Visceral abdominal fat area was found to have a significant negative correlation with the temperature of Guanyuan (CV4, lower abdomen acupoint). We also found the visceral-subcutaneous fat ratio had a significant negative correlation with the temperature of CV4 and Right Tianshu (RST25, lateral navel acupoint). Only visceral fat and its ratio to subcutaneous fat had a significant correlation with abdominal temperature. Subcutaneous fat area and total fat area were not correlated with abdominal temperature. Conclusions: This study suggests that abdominal visceral fat has a significant negative correlation with abdominal temperature. Further study is needed to uncover the relationship between abdominal fat distribution and temperature regulation in obese individuals and to define the role of body temperature in the pathogenesis of obesity.
