Quality of Life in Malay and Chinese Women Newly Diagnosed with Breast Cancer in Kelantan, Malaysia

  • Background: Breast cancer is the leading cause of cancer-related death among women in Malaysia. A diagnosis is very stressful for women, affecting all aspects of their being and quality of life. As such, there is little information on quality of life of women with breast cancer across the different ethnic groups in Malaysia. The purpose of this study was to examine the quality of life in Malay and Chinese women newly diagnosed with breast cancer in Kelantan. Materials and Methods: A descriptive study involved 58 Malays and 15 Chinese women newly diagnosed with breast cancer prior to treatment. Quality of life was measured using the Malay version of the European Organization for Research and Treatment of Cancer Quality of Life Questionnaire (EORTC QLQ-C30) and its breast-specific module (QLQ-BR23). Socio-demographic and clinical data were also collected. All the data were analyzed using SPSS version 20.0. Results: Most of the women were married with at least a secondary education and were in late stages of breast cancer. The Malay women had lower incomes (p=0.046) and more children (p=0.001) when compared to the Chinese women. Generally, both the Malay and Chinese women had good functioning quality-of-life scores [mean score range: 60.3-84.8 (Malays); 65.0-91.1 (Chinese)] and global quality of life [mean score 60.3, SD 22.2 (Malays); mean score 65.0, SD 26.6 (Chinese)]. The Malay women experienced more symptoms such as nausea and vomiting (p=0.002), dyspnoea (p=0.004), constipation (p<0.001) and breast-specific symptoms (p=0.041) when compared to the Chinese. Conclusions: Quality of life was satisfactory in both Malays and Chinese women newly diagnosed with breast cancer in Kelantan. However, Malay women had a lower quality of life due to high general as well as breast-specific symptoms. This study finding underlined the importance of measuring quality of life in the newly diagnosed breast cancer patient, as it will provide a broader picture on how a cancer diagnosis impacts multi-ethnic patients. Once health care professionals understand this, they might then be able to determine how to best support and improve the quality of life of these women during the difficult times of their disease and on-going cancer treatments.

A Study on the Development of GIS Based Water Quality Simulation System using HSPF in Basin of Yeong-san River (HSPF 모델을 적용한 GIS기반의 영산강 유역 수질모의 시스템 개발에 관한 연구)

  • The basin environment has been seriously damaged by reckless development during the past half century. The demand for management in the basin has increased, but the system for prediction and management is not sufficient. Therefore, the aim of this study is to design a GIS-based water quality linkage system using the most suitable simulation, HSPF (Hydrological Simulation Program-Fortran) in this basin of South Korea. To achieve this, data of HSPF model for simulation and GIS data for spatial analysis is collected. And the system applied linkages of the water quality model and GIS such as Loose coupling. Also, the major function of the system was designed as a modular unit. Ultimately, the system is developed using development language of VB.NET from Microsoft and ArcObjects component from ESRI based on design for a module unit. The water quality simulation system can be supported to prediction and management for basin environment of Yeong-San River. In the future study, scenario will be established using the result of HSPF model And will be expected to support to situation of future basin and policy making.

Study on the Performance Verification of PRB Isolation Device using Simulation and Experiment (PRB 지진격리장치의 성능 검증을 위한 해석 및 실험적 연구)

  • This study introduces a technique for improving the elastomeric-isolator performance using modular devices. The modular devices are shear resistance block, polymer spring, displacement acceptance guide, and anti-falling block. They are installed on the elastomeric isolator as a supplementary device. Each modularized device improves the isolator performance by performing step-by-step actions according to the seismic intensity and displacement. The PRB isolation device works in four stages, depending on the seismic magnitude, to satisfy the target performance. It is designed to accommodate design displacement in the first stage and large magnitude of earthquakes in the second and third stages. This design prevents superstructures from falling in the fourth stage due to large-magnitude earthquakes by increasing the capacity limit of the elastomeric isolator. In this study, the PRB isolation device is analyzed using finite element analysis to verify that the PRB isolation device works as intended and it can withstand loads corresponding to large-magnitude earthquakes. The performance of the PRB isolation device is validated by the analysis, which is further corroborated by actual experiments.

A study on monitoring for process time and process properties by measuring vibration signals transmitted to the mold during injection molding (사출성형공정에서 금형에 전달되는 진동 신호 측정을 이용한 성형 단계별 공정시간과 공정특성의 모니터링에 대한 연구)

  • In this study, the vibration signal of the mold was measured and analyzed to monitoring the process time and characteristics during injection molding. A 5 inch light guide plate mold was used to injection molding and the vibration signal was measured by MPU6050 acceleration sensor module attached the surface of fixed mold base. Conditions except for injection speed and packing pressure were set to the same value and the change of the vibration signal of the mold according to injection speed and packing pressure was analyzed. As a result, the vibration signal had a large change at three points: "Injection start", "V/P switchover", and "Packing end". The time difference between "injection start" and "V/P switchover" means the injection time in the injection molding process, and the time difference between "V/P switchover" and "Packing end" means the packing time. When the injection time and packing time obtained from the vibration signal of the mold are compared with the time recorded in the injection molding machine, the error of the injection time was 2.19±0.69% and the error of the packing time was 1.39±0.83%, which was the same level as the actual value. Additionally, the amplitude at the time of "injection start" increased as the injection speed increased. In "V/P switchover", the amplitude tended to be proportional to the pressure difference between the maximum injection pressure and the packing pressure and the amplitude at the "packing end" tended to the pressure difference between the packing pressure and the back pressure. Therefore, based on the result of this study, the injection time and packing time of each cycle can be monitored by measuring the vibration signal of the mold. Also, it was confirmed that the level and trend of process variables such as the injection speed, maximum injection pressure, and packing pressure can be evaluated as the change of the mold vibration during injection molding.

Pharmacophore Mapping and Virtual Screening for SIRT1 Activators

  • Silent information regulator 2 (Sir2) or sirtuins are NAD(+)-dependent deacetylases, which hydrolyze the acetyllysine residues. In mammals, sirtuins are classified into seven different classes (SIRT1-7). SIRT1 was reported to be involved in age related disorders like obesity, metabolic syndrome, type II diabetes mellitus and Parkinson’s disease. Activation of SIRT1 is one of the promising approaches to treat these age related diseases. In this study, we have used HipHop module of CATALYST to identify a series of pharmacophore models to screen SIRT1 enhancing molecules. Three molecules from Sirtris Pharmaceuticals were selected as training set and 607 sirtuin activator molecules were used as test set. Five different hypotheses were developed and then validated using the training set and the test set. The results showed that the best pharmacophore model has four features, ring aromatic, positive ionization and two hydrogen-bond acceptors. The best hypothesis from our study, Hypo2, screened high number of active molecules from the test set. Thus, we suggest that this four feature pharmacophore model could be helpful to screen novel SIRT1 activator molecules. Hypo2-virtual screening against Maybridge database reveals seven molecules, which contains all the critical features. Moreover, two new scaffolds were identified from this study. These scaffolds may be a potent lead for the SIRT1 activation.

The Development and Implementation of Problem-Based Learning Package in Physical Therapy (물리치료학에서의 PBL 학습교재 개발 및 적용)

  • Within physical therapy education, there has been increased attention to curricula and course that emphasize problem solving, clinical reasoning, and synthesis of information across traditional discipline-specific boundaries. This article describes the development implementation, and outcomes of a problem-based learning course in Physical therapy. The course was designed to help students to integrate the various elements of a physical therapy curriculum and to enhance their abilities to respond to an ever-changing health care environment. An evaluation of the course by the first 50 students who completed it revealed both strengths and weaknesses. Students responded that the course enhanced their professional behavior, including interpersonal communication skills, team work, and follow-through with professional responsibilities. The learning package was developed by the authors and implemented to a college students during three weeks of the first semester of 2001. Most studies which conducted PBL module development were short period or temporary PBL package application and evaluation rather than a whole semester's. While, this study carried on partial integrated PBL curriculum development and application with recomposing content of the two subjects to one subject Physical therapy which includes four PBL packages. This package was developed from a simple concept to complex and partial integrated PBL curriculum application systematically variable learning methods such as discussion, practice, lecture, video. There are 2 classes, each class has 25 students, in the college. Each class has 5 small groups consisting 5 students. Two tutors proceeded discussion charging each class also, they used multiple methods and materials like tutorials, self-directed learning, lecture, and video. The package is 5 grades and 5 hours per week and the rate of discussion, lecture is 4, 1 respectively. One of the most change is the increase of interaction between students and tutors. Whenever students need information and suggestion, they can visit tutors who provide reading materials and guide for the direction of self learning. Therefore, this study describes the PBL package development process and application during one semester recomposing contents of two subjects to Physical therapy concepts. Besides, it will contribute to active application of existing each subject to tutors who intend to convert as PBL methods. The study has significant meaning to show potentiality of partially integrated PBL application, using systematic PBL package development from two subjects contents. However, when students' need of yearning is over the extent of Introduction of Physical therapy and Rehabilitation medicine, tutors should set learning extent. So, there is limitation to attain completely integrated PBL education within one subject, therefore, it is high lighted to proceed development of integrated curriculum to maximize learning effects of PBL. It is exected that partial integrated PBL package development and application will distribute to prosper excellent physiotherapist in practice.

The Effects of Simulation-Based Practice on Critical Thinking Disposition, Communication Skill, and Clinical Performance for Nursing Students (시뮬레이션 기반 실습 교육이 간호대학생의 비판적 사고성향, 의사소통능력, 임상수행능력에 미치는 효과)

  • The purpose of this study was to develop a simulation-based practice that enhances the core competencies of nursing students and to investigate the effects on critical thinking disposition, communication skill, and clinical performance for nursing students. This study used a one group, pre-post test design. There were 53 nursing students from G city enrolled in this study, and self-reported questionnaires were collected between June 10 and August 19, 2016. The simulation-based practice was applied for 6-8 hours/day for 60 hours, using a 4-practice module. The data were analyzed by frequencies, paired t-test, and Pearson's correlation using SPSS Win 18.0. The results showed that there was a significant difference in the clinical performance, however, there was no significant difference in the critical thinking disposition and communication skill. After the simulation-based practice, clinical performance was found to be positively correlated with critical thinking disposition (r= .628, p<.001) and communication (r=.716, p<.001). Critical thinking disposition and communication (r=.650, p<.001) also showed a positive correlation. In order to improve clinical competence, it is necessary to develop a simulation-based practice that enhances critical thinking disposition and communication skill.

Ways to Restructure Science Convergence Elective Courses in Preparation for the High School Credit System and the 2022 Revised Curriculum (고교학점제와 2022 개정 교육과정에 대비한 과학과 융합선택과목 재구조화 방안 탐색)

  • The goal of this study is to explore ways to restructure Convergence Elective Courses in science in preparation for the high school credit system, ahead of the 2022 revised science curriculum. This study started from the problem that the 2015 revised science curriculum has not guaranteed science subject choice for students with non-science/engineering career aptitudes. To this end, a survey was conducted by randomly sampling high schools across the country. A total of 1,738 students responded to the questionnaire of 3 science elective courses such as Science History, Life & Science, Convergence Science. In addition, in-depth interviews with 12 science teachers were conducted to examine the field operation of these three courses, which will be classified and revised as Convergence Elective subjects in the 2022 revised curriculum. According to the results of the study, high school students perceive these three courses as science literacy courses, and find these difficult to learn due to lack of personal interest, and difficulties in content itself. The reason students choose these three courses is mainly because they have aptitude for science, or these courses have connection with their desired career path. Teachers explained that students mainly choose Life & Science, and both teachers and students avoid Science History because the course content is difficult. Based on the research results, we suggested ways to restructure Convergence Electives for the 2022 revised curriculum including developing convergence electives composed of interdisciplinary convergence core concepts with high content accessibility, developing convergence electives with core concepts related to AI or advanced science, developing module-based courses, and supporting professional development of teachers who will teach interdisciplinary convergence electives.

Analysis of Streamflow Characteristics of Boryeong-dam Watershed using Global Optimization Technique by Infiltraion Methods of CAT (CAT 모형의 침투해석방법별 전역최적화기법을 이용한 보령댐 유역의 유출 특성 변화 분석)

  • In this study, the changes of the streamflow characteristics of the watershed were analysed depending on the infiltration methods of CAT. The study area, Boryeong-dam watershed located in Chungcheongnam-do area, has been suffered from severe drought in recent years and stabilized regarding on the storage rate through efforts such as constructing a channel connecting the upstream of Boryeong-dam from the downstream of the Geum river. In this study, the effects of soil infiltration parameters on the watershed streamflow characteristics were analyzed by the infiltration methods of CAT such as Rainfall Excess, Green&Ampt and Horton. And the parameter calibrations were conducted by SCEUA-P, a global optimization technique module of the PEST, the package for parameter optimization and uncertainty analysis, to compare the yearly variations of soil parameters for infiltration methods of CAT. In addition, the streamflow characteristics were analyzed for three infiltration methods by applying three different scenarios, such as applying calibrated parameters for every years to simulate the model for each years, applying calibrated parameters for the entire period to simulate the model for entire period, and applying the average value of yearly calibrated parameters to simulate the model for entire period.

A Study on the Korean Patent Registration Trend of Outdoor Exercise Equipment for the Elderly (노인 관련 야외운동기구의 국내 특허 등록 동향에 관한 연구)

  • This study analyzed the patent status of the outdoor exercise equipment used primarily by the elderly. The purpose is to utilize the basic data obtained to promote the health of the elderly. The information on the patent was collected from KIPRIS, an information search service provided by the Korean Intellectual Property Office. The search term used was 'outdoor exercise equipment', directly related patents were selected, and a final 157 were analyzed. As a result of the analysis, first, patent registration began in 2007, and 2-3 patents were registered on average every year. Second, patents from the perspective of sports convergence that provide an exercise prescription system using wireless communication, such as the ability to generate electricity by operating a power generation module, providing information on the user's exercise amount, or preventing the loss and theft of weights and safety accidents due to their characteristics, were searched for. Lastly, patents related to exercise equipment that can provide user convenience and increase the frequency of use of exercise equipment were searched. The results of this study confirmed that outdoor exercise equipment is being developed more for the elderly and their convenience, and that companies and public institutions are showing increased interest in outdoor exercise equipment for the elderly. In addition to patent trends analysis, follow-up research in connection with exercise programs using outdoor exercise equipment is needed to develop practical and convenient outdoor exercise equipment in the future.
